Exogenous insulin autoimmune syndrome:A case report and review of literature

BACKGROUND Insulin autoimmune syndrome(IAS)is a severe manifestation of spontaneous *** is characterized by elevated levels of immune-reactive insulin and highly potent insulin autoantibodies(IAAs),which are induced by endogenous insulin circulating in the *** is distinguished by recurring instances of spontaneous hypoglycemia,the presence of IAA within the body,a substantial elevation in serum insulin levels,and an absence of prior exogenous insulin ***,recent studies show that both conventional insulin and its analogs can induce IAS episodes,giving rise to the notion of nonclassical ***,more attention should be paid to these *** SUMMARY In this case report,we present a rare case of non-classical IAS in an 83-year-old male patient who present with symptoms of a psychiatric *** symptom onset,the patient exhibited Whipple's triad(including hypoglycemia,blood glucose level less than 2.8 mmol/L during onset,and rapid relief of hypoglycemic symptoms after glucose administration).Concurrently,his serum insulin level was significantly elevated,which contradicted his C-peptide *** a comprehensive examination,the patient was diagnosed with exogenous insulin autoimmune *** that the patient had type 2 diabetes mellitus and a history of exogenous insulin use before disease onset,it was presumed that non classical IAS was induced by this *** PubMed database was used to search for previous cases of IAS and non-classical IAS to analyze their characteristics and treatment *** The occurrence of non-classical IAS is associated with exogenous insulin or its analogs,as well as with sulfhydryl *** can be effectively alleviated through the discontinuation of relevant medications,administration of hormones or immunosuppressants,plasma exchange,and lifestyle adjustments.

The Sb^(3+) doping strategy has been proven to be an effective way to regulate the band gap and improve the photophysical properties of organic-inorganic hybrid metal halides(OIHMHs).However,the emission of Sb^(3+) ions in OIHMHs is primarily confined to the low energy region,resulting in yellow or red *** date,there are few reports about green emission of Sb^(3+)-doped ***,we present a novel approach for regulating the luminescence of Sb^(3+) ions in 0D C_(10)H_(2)_(2)N_(6)InCl_(7)·H_(2)O via hydrogen bond network,in which water molecules act as agents for hydrogen ***^(3+)-doped C_(10)H_(2)2N_(6)InCl_(7)·H_(2)O shows a broadband green emission peaking at 540 nm and a high photoluminescence quantum yield(PLQY)of 80%.It is found that the intense green emission stems from the radiative recombination of the self-trapped excitons(STEs).Upon removal of water molecules with heat,C_(10)H_(2)_(2)N_(6)In_(1-x)Sb_(x)Cl_(7) generates yellow emis-sion,attributed to the breaking of the hydrogen bond network and large structural distortions of excited *** water molecules are adsorbed by C_(10)H_(2)_(2)N_(6)In_(1-x)Sb_(x)Cl_(7),it can subsequently emit green *** water-induced reversible emission switching is successfully used for optical security and information *** findings expand the under-standing of how the local coordination structure influences the photophysical mechanism in Sb^(3+)-doped metal halides and provide a novel method to control the STEs emission.

Configuring single-layer MXene nanosheet onto natural wood fiber via C-Ti-C covalent bonds for high-stability Li-S batteries

Lithium-sulfur batteries(LSBs)are considered promising candidates for next-generation battery technologies owing to their outstanding theoretical energy density and ***,the low conductivity and polysulfide shuttling effect of S cathodes severely hamper the practical performance of ***,in situ-generated single layer MXene nanosheet/hierarchical porous carbonized wood fiber(MX/PCWF)composites are prepared via a nonhazardous eutectic activation strategy coupled with pyrolysis-induced gas *** unique architecture,wherein single layer MXene nanosheets are constructed on carbonized wood fiber walls,ensures rapid polysulfide conversion and continuous electron transfer for redox *** C-Ti-C bonds formed between MXene and PCWF can considerably expedite the conversion of polysulfides,effectively suppressing the shuttle *** impressive capacity of 1301.1 m A h g^(-1)at 0.5 C accompanied by remarkable stability is attained with the MX/PCWF host,as evidenced by the capacity maintenance of 722.6 m A h g^(-1)after 500 ***,the MX/PCWF/S cathode can still deliver a high capacity of 886.8 m A h g^(-1)at a high S loading of 5.6 mg cm^(-2).The construction of two-dimensional MXenes on natural wood fiber walls offers a competitive edge over S-based cathode materials and demonstrates a novel strategy for developing high-performance batteries.

Prospective study of reconstructing pelvic floor with GORE-TEX Dual Mesh in abdominoperineal resection

Background Mesh reconstruction has been proved to be an effective method in incisional hernia repairment. This study was designed to evaluate the effect of reconstructing the pelvic floor with the high-inlay expanded polytetrafluoroethylene （ePTFE） GORE-TEX Dual Mesh （WLGore And Associates, Flagstuff, USA） in abdominoperineal resection. Methods Sixty patients who underwent abdominopedneal resection for rectal cancer were assigned to 2 groups. The pelvic peritoneum was closed by routine sutures in group 1 and reconstructed with ePTFE in group 2. Postoperative complications and related items were evaluated and the patients were followed up. Results Time of confining to bed, bowel function recovery, fasting, and detaining drainage were significantly different between two groups （P 〈0.05）. In group 1, three patients developed bowel obstruction （10%）, while no bowel obstruction was observed in group 2. Conclusion Reconstruction of the pelvic floor using ePTFE results in quicker postoperative recovery and could decrease the risk of postoperative intestinal obstruction.

Structure regulating of metal clusters in carbonized metallic organic frameworks for high-efficient microwave absorption via tuning interaction strength between metals and ligands

Carbonized metallic organic frameworks(CMOF)have been attracting attention in microwave absorption(MA)research area because of their diverse structures,tunable compositions,and rich ***,structure regulation on metal clusters in CMOF is achieved by tuning the interaction strength between metals and ligands to enhance microwave absorption *** to relatively weak interaction among copper cations and ligands,copper nanoclusters(CuNC)can be uniformly formed and embedded within the cobalt/zinc(Co/Zn)***,copper cations are added to the Co/Zn bimetallic zeolitic imidazolate frameworks(ZIFs).Secondly,the CMOF composite particles with CuNCs(CuNCs/CoZn-CMOF)were developed by a pyrolysis *** CuNCs/CoZn-CMOF with an appropriate amount of CuNCs can harmonize both dielectric and magnetic *** a result,the minimum reflection loss(RLmin)reaches–45.1 dB at a matching thickness of 2.30 mm and the effective absorption bandwidth(EAB)is 8.80 GHz at a thickness of 3.10 *** broadband response to electromagnetic waves is attributed to interfacial polarization at CuNCs surface and heterogeneous interfaces,impedance matching and multiple scattering of electromagnetic *** study provides a feasible method to develop CMOF microwave absorption materials with high EAB values.

HI in High Gas-phase Metallicity Dwarf Galaxy WISEA J230615.06+143927.9

We present resolved Giant Metrewave Radio Telescope H I observations of the high gas-phase metallicity dwarf galaxy WISEA J230615.06+143927.9(z = 0.005)(hereafter J2306) and investigate whether it could be a Tidal Dwarf Galaxy(TDG) candidate. TDGs are observed to have higher metallicities than normal dwarfs. J2306 has an unusual combination of a blue g-r color of 0.23 mag, irregular optical morphology and high-metallicity(12 +log(O/H) = 8.68 ± 0.14), making it an interesting galaxy to study in more detail. We find J2306 to be an H I rich galaxy with a large extended, unperturbed rotating H I disk. Using our H I data we estimated its dynamical mass and found the galaxy to be dark matter(DM) dominated within its H I radius. The quantity of DM, inferred from its dynamical mass, appears to rule out J2306 as an evolved TDG. A wide area environment search reveals J2306 to be isolated from any larger galaxies which could have been the source of its high gas metallicity. Additionally, the H I morphology and kinematics of the galaxy show no indication of a recent merger to explain the *** detailed optical spectroscopic observations of J2306 might provide an answer to how a seemingly ordinary irregular dwarf galaxy achieved such a high level of metal enrichment.

Detection of a mud volcano in the Weitan Banks area of the northern South China Sea

Situated between the petroliferous Cenozoic Zhujiang(Pearl)River Mouth Basin and the mud volcano-rich Mesozoic Dongsha Basin in the middle sector of the northern South China Sea,the Weitan Banks area has been previously mapped as a basement high that is composed of Mesozoic magmatic *** this study,we present several favorable indicators for petroleum geology that were detected from geophysical profiling and benthic sampling in the area.A conspicuous hill was discovered,named“Zhongwei Hill”,~80 m high above the~340 m deep seafloor and~1 km broad,in a depression with more than 7 km thick sedimentary *** Zhongwei Hill was seismically imaged with a mushroom-shaped structure and containing a cake-like crown,fluid flow pipes,and an~10 km broad anticline at ***,the hill represents a source-plumbing-eruption *** gas zones linked to deep fracture were found at or near the *** correlation indicates that the deep strata comprise the Jurassic and Paleogene strata,the major hosts of hydrocarbon source *** addition to the hill,there are number of mounds from which three bottom water samples were collected and the samples are rich in dissolved methane with concentrations high up to~900 nmol/L,much higher than the background level(0.5–2 nmol/L).The benthic samples are rich in coarse sediment clastics,authigenic carbonate nodules,and deep-water habitats likely feeding on methanotrophic *** these observations and the context,we propose that the Zhongwei Hill represents a mud volcano,likely thermally driven,that seeps methane from Jurassic and Paleogene source layers,thus poses a favorable clue for significant hydrocarbon generation capacity in transitional zone of the Zhujiang River Mouth Basin and the Dongsha Basin.

Minimally invasive surgery is a kind of surgical operation,which is performed by using professional surgical instruments and equipment to inactivate,resect,repair or reconstruct the pathological changes,deformities and wounds in human body through micro-trauma or micro-approach,in order to achieve the goal of treatment,its surgical effect is equivalent to the traditional open surgery,while avoiding the morbidity of conventional surgical *** addition,it also has the advantages of less trauma,less blood loss during operation,less psychological burden and quick recovery on patients,and these minimally invasive techniques provide unique value for the examination and treatment of gastric cancer *** minimally invasive surgical techniques have developed rapidly and offer numerous options for the treatment of early gastric cancer(EGC):endoscopic mucosal resection(EMR),underwater EMR(UEMR),endoscopic submucosal dissection(ESD),endoscopic full-thickness resection(EFTR),endoscopic submu-cosal excavation(ESE),submucosal tunnel endoscopic resection,laparoscopic and endoscopic cooperative surgery(LECS);Among them,EMR,EFTR and LECS technologies have a wide range of applications and different modific-ations have been derived from their respective surgical operations,such as band-assisted EMR(BA-EMR),conven-tional EMR(CEMR),over-the-scope clip-assisted EFTR,no-touch EFTR,the inverted LECS,closed LECS,and so *** new and improved minimally invasive surgeries are more precise,specific and effective in treating different types of EGC.
