Mechanical properties and energy evolution of Beishan shallow-layer granite under different unloading paths

Rock has mechanical characteristics and a fracture damage mechanism that are closely related to its loading history and loading path. The mechanical properties, fracture damage features, acoustic emission(AE) characteristics, and strain energy evolution of the Beishan shallow-layer granite used in triaxial unloading tests were investigated in this study. Three groups of triaxial tests, namely, conventional triaxial compression test(Group Ⅰ), maintaining deviatoric stress synchronously unloading confining pressure test(Group Ⅱ), and loading axial pressure synchronously unloading confining pressure test(Group Ⅲ), were carried out for the cylindrical granite specimens. AE monitoring device was utilized in these tests to determine the degree to which the AE waves and AE events reflected the degree of rock damage. In addition, the crack stress thresholds of the specimens were determined by volumetric strain method and AE parameter method, and strain energy evolution of the rock was explored in different damage stages. The results show that the shallow-layer granite experiences brittle failure during the triaxial loading test and unloading test, and the rock has a greater damage degree during the unloading test. The crack stress thresholds of these samples vary greatly between tests, but the threshold ratios of all samples are similar in the same crack damage stage. The Mogi-Coulomb strength criterion can better describe the unloading failure strength of the rock. The evolution of the AE parameter characteristics and strain energy differs between the specimens used in different stress path tests. The dissipative strain energy is the largest in Group Ⅱ and the smallest in Group Ⅰ.

Efficacy and Safety of Bufei Jiedu Granules in Treating Multidrug-Resistant Pulmonary Tuberculosis: A Multi-center, Double-Blinded and Randomized Controlled Trial

Objective: To assess the efficacy and safety of Bufei Jiedu(BFJD) Granules as adjuvant therapy for patients with multidrug-resistant pulmonary tuberculosis(MDR-PTB). Methods: A large-scale, multi-center, double-blinded, and randomized controlled trial was conducted in 18 sentinel hospitals in China from December 2012 to December 2016. A total of 312 MDR-PTB patients were randomly assigned to BFJD Granules or placebo groups(1:1) using a stratified randomization method, which both received the long-course chemotherapy regimen for 18 months(6 Am-Lfx-P-Z-Pto, 12 Lfx-P-Z-Pto). Meanwhile, patients in both groups also received BFJD Granules or placebo twice a day for a total of 18 months, respectively. The primary outcome was cure rate. The secondary outcomes included time to sputum-culture conversion, changes in lung cavities and quality of life(QoL) of patients. Adverse reactions were monitored during and after the trial. Results: A total of 216 cases completed the trial, 111 in the BFJD Granules group and 105 in the placebo group. BFJD Granules, as adjuvant treatment, increased the cure rate by 13.6% at the end of treatment, compared with the placebo(58.4% vs. 44.8%, P=0.02), and accelerated the median time to sputum-culture conversion(5 months vs. 11 months). The cavity closure rate of the BFJD Granules group(50.6%, 43/85) was higher than that of the placebo group(32.1%, 26/81; P=0.02) in patients who completed the treatment. At the end of the intensive treatment, according to the 36-item Short Form, the BFJD Granules significantly improved physical functioning, general health, and vitality of patients relative to the placebo group(all Plusions: Supplementing BFJD Granules with the long-course chemotherapy regimen significantly increased the cure rate and cavity closure rate, and rapidly improved QoL of patients with MDR-PTB(Registration No. ChiCTR-TRC-

Background:Immunoglobulin G4-related disease(IgG4-RD)is a recently recognized immune-mediated disorder that can affect almost any organ in the human body.IgG4-RD can be categorized into proliferative and fibrotic subtypes based on patients’clinicopathological characteristics.This study aimed to compare the clinical manifestations,laboratory findings,and treatment outcomes of IgG4-RD among different subtypes.Methods:We prospectively enrolled 622 patients with newly diagnosed IgG4-RD at Peking Union Medical College Hospital from March 2011 to August 2021.The patients were divided into three groups according to their clinicopathological characteristics:proliferative,fibrotic,and mixed subtypes.We compared demographic features,clinical manifestations,organ involvement,laboratory tests,and treatment agents across three subtypes.We then assessed the differences in treatment outcomes among 448 patients receiving glucocorticoids alone or in combination with immunosuppressants.Moreover,risk factors of relapse were revealed by applying the univariate and multivariate Cox regression analysis.Results:We classified the 622 patients into three groups consisting of 470 proliferative patients,55 fibrotic patients,and 97 mixed patients,respectively.We found that gender distribution,age,disease duration,and frequency of allergy history were significantly different among subgroups.In terms of organ involvement,submandibular and lacrimal glands were frequently involved in the proliferative subtype,while retroperitoneum was the most commonly involved site in both fibrotic subtype and mixed subtype.The comparison of laboratory tests revealed that eosinophils(P=0.010),total IgE(P=0.006),high-sensitivity C-reactive protein(Plus

Impact of specialized nursing outpatient case management on postcoronary artery bypass grafting patients

BACKGROUND Some patients with coronary atherosclerotic heart disease experience major adverse cardiac events(MACE)and require readmission after Coronary Artery Bypass Grafting(CABG)surgery.This is often attributed to patients'unhealthy lifestyles and dietary habits,inadequate understanding of the disease,and poor disease management compliance.Thus,searching for more targeted nursing intervention models that can enhance patients'self-management abilities and reduce the risk of readmission after CABG surgery is significant.AIM To observe the impact of specialized nursing outpatient case management on patients after CABG surgery.METHODS A total of 103 patients who underwent CABG surgery in our hospital between April 2021 and April 2022 comprised the study sample.The patients were divided into two groups using an odd-even number grouping method.The control group received routine nursing care,while the case management group received specialized nursing outpatient case management.The differences in psychological status,adherence to medical treatment,self-care ability,knowledge mastery,quality of life scores,and the occurrence rate of MACE were compared between the two groups.RESULTS After the intervention,the case management group had lower scores on the selfrating depression scale and self-rating anxiety scale and lower MACE rate,as well as higher scores for adherence to a healthy diet,medication adherence,good lifestyle habits,regular exercise,and timely follow-up,higher scores on the Coronary Heart Disease Self-Management Scale,higher scores for managing adverse habits,symptoms,emotional cognition,emergency response,disease knowledge,general lifestyle,and treatment adherence,higher scores for understanding coronary heart disease,recognizing the importance of medication adherence,understanding selfcare points after CABG surgery,and being aware of post-CABG precautions,higher scores for physical well-being,disease condition,general health,social-psychological well-being,and work-related aspects(PluSION Spec

Acral melanoma(AM)is the most common histologic subtype of melanoma in dark-skinned patients and is associated with a worse prognosis and a high mortality rate,largely due to the inconspicuous nature of early-stage lesions,which can lead to late diagnosis.Because of the overlapping clinical and histopathological features of AM with other forms of cutaneous melanomas,early detection of AM requires a multidisciplinary approach that integrates various diagnostic modalities,including clinical examination,dermoscopy,histopathology,molecular testing,radiological imaging,and blood tests.While surgery is the preferred method of treatment for AM,other therapeutic options may be employed based on the stage and underlying etiology of the disease.Immune checkpoint inhibitors,molecular targeted therapy,radiotherapy,chemotherapy,and oncolytic virotherapy represent promising advanced treatment options for AM.In this review,we provide an overview of the latest advancements in diagnostic and therapeutic methods for AM,highlighting the importance of early detection and the prompt,individualized management of this challenging disease.

目的:探讨血清游离轻链(sFLC)在单克隆免疫球蛋白血症(MG)患者中的表达水平及其临床意义。方法:收集98例MG患者和30例健康志愿者外周血,采用免疫比浊法检测sFLC水平,并分析sFLC对MG患者的诊断、疾病严重程度以及疗效评估的价值。结果:98例MG患者中,男性58例,女性40例;具有肾脏意义的单克隆球蛋白血症(MGRS)45例,意义未明的单克隆球蛋白血症(MGUS)33例,血液系统肿瘤20例;Ig G型58例,Ig A型26例,Ig M型7例,轻链型5例,不分泌型2例;κ型35例,λ型63例;肾功能异常53例,肾功能正常45例。MG患者外周血中sFLC-κ和sFLC-λ表达水平相比对照组均显著升高(P<0.01)。血液系统肿瘤患者sFLC-κ和sFLC-λ表达水平显著高于MGRS和MGUS患者(P<0.05),而MGRS患者又显著高于MGUS患者(P<0.05)。肾功能异常者sFLC-κ和sFLC-λ表达水平显著高于肾功能正常者(P<0.01)。sFLC-κ和sFLC-λ与MG患者的球蛋白(r=0.392,r=0.435)、β2-微球蛋白水平呈正相关(r=0.403,r=0.468),在伴肾功能异常的患者中,与血肌酐水平亦呈正相关(r=0.586,r=0.631),与年龄、性别、白蛋白、乳酸脱氢酶、血钙等指标均未见明显相关性。经过治疗,患者sFLC-κ和sFLC-λ的表达水平均显著下降(P<0.01)。结论:sFLC水平在MG患者中显著升高,是一个可以快速检测、敏感度高,有助于疾病类型诊断、病情严重程度判定和疗效评估的新指标。

飞机通信寻址与报告系统(ACARS)允许飞机和地面站之间发送和接收报文,在保障飞行安全、实现空中交通管制等方面具有重要意义。通信链路的噪声和频偏是影响ACARS信号接收性能的关键因素。针对ACARS信号接收过程中的失真等问题,深入研究频偏、噪声等因素与ACARS报文接收性能之间的关系;在充分考虑噪声与多普勒频移等因素的基础上,提出基于过零点斜率加窗解调的ACARS信号接收方法,并从理论和仿真分析2方面探讨不同方法的抗频偏抗噪声能力;设计并搭建基于软件无线电平台的ACARS信号接收系统。通过对ACARS报文接收性能的仿真分析和真实报文接收性能的对比分析,表明所提方法在提升抗噪声和频偏能力、降低误码率等方面均具有优势。

Although land surface water only covers a small portion(~3.7%) of our planet,it plays many essential roles:e.g.,freshwater storage,industrial consumption,agricultural irrigation,and biodiversity maintenance [1,2].Water storage variability is associated with various natural processes and human activities on the Earth [3,4] and it is well known that reservoir regulation is the most widely distributed and fundamental human activity capable of altering the global water cycle.In recent years,through reservoir regulation,humans have been able to obtain more freshwater from the Earth’s surface,with great benefits to human wellbeing.Currently,however,the extent to which human activities affect the global water cycle remains unknown.As a result,there is an urgent need to quantify and understand the role of human activities in the global water cycle to ensure that freshwater resources are managed sustainably at the global scale.
