Immediate Effect of Electro-acupuncture on Endometrial Blood Flow in Patients with Recurrent Implantation Failure:A Randomized Controlled Trial

Objective To investigate the immediate effects of electro-acupuncture(EA)on endometrial blood flow among recurrent implantation failure(RIF)*** Eighty RIF patients,enrolled from March 2022 to December 2022,were randomly allocated into either the EA group(40 cases)or the waiting-list(WL)group(40 cases)by using a random number *** EA group underwent acupuncture at points of Shenting(GV 24),Baihui(GV 4),Benshen(GB 13),bilateral Zigong(EX-CA 1),Huangshu(KI 16),Sanyinjiao(SP 6)and Xuehai(SP10),and electric acupuncture apparatus was connected to EX-CA 1,KI 16,SP 6,and SP 10 with disperse-dense waves at 4/20 Hz frequencies for 30 min after transvaginal ultrasound,while the WL group received no *** primary outcome measured was the endometrial volume blood *** secondary outcomes included the bilateral uterine artery index,endometrial volume,endometrial blood flow type,vascular distribution index(VI^(MV))for endometrial and ovary,clinical pregnancy rate,and embryo implantation *** In the EA group,there was a notable decrease in the bilateral pulsatility index and a significant improvement in the endometrial blood flow type post-EA(Plinical pregnancy rate and embryo implantation rate between the two groups after treatment(P>0.05).Besides,no adverse events related to EA were *** EA can promptly ameliorate VI^(MV) for the endometrial and right ovary,and endometrial blood flow *** randomized controlled trials are warranted to investigate the long-term effects of EA on blood flow of RIF patients and its implications for pregnancy outcomes.(Trial registration ***2200057377

Kinetic Analysis of the Interaction between Nonsteroidal Anti-inflammatory Drugs and Cyclooxygenase-2 Using Wavelength Modulation Surface Plasmon Resonance

A surface plasmon resonance （SPR） biosensor based on wavelength modulation technology was developed and validated for the kinetic analysis of the interactions between two nonsteroidal anti-inflammatory drugs （NSAIDs） and cyclooxygenase-2 （COX-2）. After the effect of different concentration COX-2 on the binding capacity of the SPR biosensor surface was studied, the COX-2 was immobilized covalently onto the biosensor surface using a standard amine coupling method. The affinity constants for indomethacin, ketoprofen binding to COX-2 are 7.5× 10^3 L/tool and 9.25 × 10^3 L/mol, respectively. The biosensor surface can be regenerated after being rinsed with 0.01 mol/L NaOH, and the biosensor can be used repeatedly. These indicated that the wavelength modulation SPR biosensor has the potential application in the fields of pharmacokinetics, pharmacodynamics and drug discovery.

目的:分析我国自主研发的第二代酪氨酸激酶抑制剂(TKI)氟马替尼对一、二线治疗失败的慢性髓系白血病慢性期(CML-CP)患者的疗效及安全性。方法:回顾性收集2020年1月至2022年9月连云港市第一人民医院采用氟马替尼治疗的30例CML-CP患者的临床资料,其中15例曾接受伊马替尼一线治疗且治疗失败的患者作为二线组,另外15例采用尼洛替尼或达沙替尼二线治疗失败的患者作为三线组;统计分析两组患者治疗3、6和12个月时的血液学、分子学反应,以及至随访终点患者的无事件生存(EFS)和不良反应发生情况。结果:二线组治疗3、6和12个月时获得主要分子学反应(MMR)分别有10、11、12例患者,均高于三线组的3、4、5例患者(P=0.010,P=0.011,P=0.010)。二线组患者治疗3个月时获完全血液学反应(CHR)、早期分子学反应(EMR)分别有12、13例,高于三线组患者的9、13例,但两组差异无统计学意义(P=0.232,P=1.000);治疗6和12个月时二线组获得MR4.5的患者分别有6、7例,高于三线组的3、2例,但差异无统计学意义(P=0.427,P=0.713)。二线组患者治疗期间出现的血液学不良反应主要为1-2级血小板减少和贫血,未出现3-4级不良反应;三线组中1-2级血小板减少2例,1-2级贫血及白细胞减少各3例,3-4级贫血1例、中性粒细胞减少2例。二线组非血液学不良反应为皮疹(2例)、头痛(1例)、腹泻(1例)、疲乏(1例)、四肢疼痛(1例),三线组腹泻、恶心、水肿各1例。两组患者在血液学不良反应及非血液学不良反应方面均无统计学意义(P>0.05)。截止随访时,二线组患者的EFS率高于三线组(100%vs 93.3%,P=0.317)。结论:我国自主研发的二代TKI氟马替尼对一、二线治疗失败的CML-CP患者具有良好的疗效及安全性。

Influence of Ar/N_2 Flow Rate on Structure and Property for TaN/NbN Multilayered Coatings

TaN/NbN multilayered coatings with nanoscale bilayer periods were synthesized at different Ar/N2 flow rates by RF （radio frequency） magnetron sputtering. XRD （X-ray diffraction） and Nano Indenter System were employed to investigate the influence of Ar/N2 flow rate （FAr：FN2） on microstructure and mechanical properties of the coatings. The low-angle XRD pattern indicated a well-defined composition modulation and layer structure of the multilayered coating. All multilayered coatings almost revealed higher hardness than the rule-of-mixtures value of monolithic TaN and NbN coatings. At FAr：FN2=10, the multilayered coating possessed desirable hardness, elastic modulus, internal stress, and fracture resistance, compared with ones synthesized at other Ar/N2 flow rates. The layered structure with strong mixture of TaN （110）, （111）, （200） and Nb2N （101）textures should be related to the enhanced mechanical properties.

Heparanase expression,degradation of basement membrane and low degree of infi ltration by immunocytes correlate with invasion and progression of human gastric cancer

AIM： To disclose the mechanisms that accelerate or limit tumor invasion and metastasis in gastric cancer patients. METHODS： The heparanase expression, continuity of basement, degree of infiltration by dendritic cells and lymphocytes in gastric cancer tissues from 33 the early and late stage patients were examined by immunohistochemistry, in situ hybridization and transmission electron microscopy. RESULTS： Heparanase mRNA expression in the late stage patients with gastric cancer was stronger than that in the early stage gastric cancer patients. In the early stage gastric cancer tissues, basement membrane （BM） appeared intact, whereas in the late stage, discontinuous BM was often present. The density of Sl00 protein positive tumor infiltrating dendritic cells （TIDC） in the early stage gastric cancer tissues was higher than that in the late stage. The infiltrating degree of tumor infiltrating lymphocytes （TIL） in the early stage patients whose tumor tissues contained a high density of TIDC was significantly higher than that in the late stage gastric cancer tissues patients with a low density of TIDC. There were few cancer cells penetrated through the continuous BM of cancer nests in the early stage gastric cancers, but many cancer cells were found outside of the defective BM of cancer nests in the late stage. CONCLUSION： Our results suggest that strongheparanase expression is related with the degradation of BM which allows or accelerates tumor invasion and metastasis. However, high density of TIDC and degree of infiltration by TIL are associated with tumor progression in human gastric cancers.

Simulated enantiomeric excess of glutamic acid was determined by a lab-made sixteen-channel capillary array electrophoresis with confocal fluorescent rotary scanner. The experimental results indicated that the capillary array electrophoresis method can accurately determine the enanfiomefic excess of glutamic acid and can be used for high-throughput screening system for combinatorial asymmetric catalysis.

Arterioles in cerebral amyloid angiopathy and vascular dementia

Background Small cerebrovascular lesions are one of the most important factors in cerebral amyloid angiopathy （CAA） and vascular dementia （VaD）. We analyzed the difference of arteriolar pathology between CAA patients （CAAs） and vascular dementia patients without CAA （VaDs）. Methods Ten deceased CAAs and twelve deceased VaDs were available for this study. Five deceased patients without known cerebrovascular diseases served as controls. These patients were all autopsy cases. All transversely cut arterioles in the gray matter and white matter with an external diameter equal to or larger than 30 um and with a maximum of 300 um were examined. The internal and external diameters of arterioles were measured. Results The external diameter of gray matter arterioles in the CAAs was significantly greater than in controls. In gray matter arterioles, the diameter of the lumen in VaDs was markedly smaller than in the CAAs, whereas there was no significant difference between CAAs and controls. CAAs and VaDs may cause remarkable thickening of the arteriolar walls in either white matter or gray matter. The sclerotic index of arterioles in VaDs was significantly greater than in CAAs and controls. Conclusions Stenosis of arterioles occurred in both CAA and VaD, but the tendency was greater in VaD. Arterioles of CAA were also expanded in gray matter, which may be related to lobar hemorrhage. The loss and/or degeneration of vascular smooth muscle cells was predominant in CAA, while the over-proliferation of vascular smooth muscle cells was areater in VaD.

为提高聚四氟乙烯(PTFE)作为聚结材料时的除油效果,构建了脱脂棉(Cot)和超疏水PTFE分层叠卷(Cot+超疏水PTFE)的规整异质聚结材料。结果表明,PTFE作为聚结床层时,出水油浓度为220 mg/L,超疏水PTFE的出水油浓度为155 mg/L,PTFE的超疏水改性提高了其聚结性能;Cot+超疏水PTFE的出水油浓度为55 mg/L,进一步提升了除油效果;Cot+超疏水PTFE在最佳床层长度100 mm、孔隙率0.883时,品质因子可达39 kPa^(-1),出水中几乎不存在粒径大于10μm的油滴,床层有效处理了分散油及乳化油,且在12 h长周期运行中稳定保持94.5%的除油率。

Coronary plaque characterization of nonculprit or nontarget lesions assessed by analysis of in vivo intracoronary ultrasound radio-frequency data

Background Unheralded sudden death and acute myocardial infarction are common manifestations of coronary atherosclerosis. Such events are related to thrombotic occlusion at the site of non-flow limiting atherosclerotic plaques in epicardial coronary arteries. This study aimed to assess plaque characterization of nonculprit lesions in patients with acute coronary syndrome （ACS） compared with those with stable angina pectoris （SAP） determined by analysis of intravascular ultrasound （IVUS） radiofrequency （RF） data. Methods In 81 patients, nonculprit vessels with 〈50% diameter stenosis and nontarget segment of culprit vessels with 〈50% diameter stenosis were studied with IVUS. Tissue maps were reconstructed from RF data using IVUS-Virtual Histology software. Results Mean lipid core percentage was significantly higher in patients with ACS than in those with SAP （（25.78±6.30）% vs （9.11±4.90）%, P 〈0.001）. In addition, patients with SAP showed more fibrotic vessels （（59.66±16.87）% vs （49.07±10.20）%, P 〈0.001）. There was no significant difference in either mean calcium （（4.37±2.40）% vs （5.12±3.00）%, P=-0.225） or fibrolipid （（24.94±9.40）% vs （25.82±13.60）%, P=0.731） percentages in nonculprit vessels, but the mean calcium percentage was significantly higher in nontarget lesions of culprit vessels （（5.51±3.29）% vs （3.57±2.10）%, P=0.003）. In addition, there was a positive correlation between lipid core and remodeling index （RI）（r=0.847, P〈0.001） and a negative correlation between fibrous tissue and RI （r= -0.946, P〈0.001）. Conclusions In this study, in both nonculprit vessels and nontarget lesion of culprit vessels, plaque characterization of nonculprit lesions determined by spectral analysis of IVUS RF data was significantly different in patients with ACS. The percentage of lipid core was significantly higher in patients with ACS than in those with SAP. Conversely, SAP patients showed more fibrotic content.
