Pattern Classification of Enterovirus 71-Associated Hand, Foot, and Mouth Disease in Chinese Medicine: A Retrospective Study in 433 Cases

Objective： To determine whether patterns of enterovirus 71（EV71）-associated hand, foot, and mouth disease（HFMD） were classified based on symptoms and signs, and explore whether individual characteristics were correlated with membership in particular pattern. Methods： Symptom-based latent class analysis（LCA） was used to determine whether patterns of EV71-HFMD existed in a sample of 433 cases from a clinical data warehouse system. Logistic regression was then performed to explore whether demographic, and laboratory data were associated with pattern membership. Results： LCA demonstrated a two-subgroup solution with an optimal fit, deduced according to the Bayesian Information Criterion minima. Hot pattern（59.1% of all patients） was characterized by a very high fever and high endorsement rates for classical HFMD symptoms（i.e., rash on the extremities, blisters, and oral mucosa lesions）. Non-hot pattern（40.9% of all patients） was characterized by classical HFMD symptoms. The multiple logistic regression results suggest that white blood cell counts and aspartate transaminase were positively correlated with the hot pattern（adjust odds ratio=1.07, 95% confidence interval： 1.006–1.115; adjust odds ratio=1.051, 95% confidence interval： 1.019–1.084; respectively）. Conclusions： LCA on reported symptoms and signs in a retrospective study allowed different subgroups with meaningful clinical correlates to be defined. These findings provide evidence for targeted prevention and treatment interventions.

To improve the precision and reliability in predicting methane hazard in working face of coal mine, we have proposed a forecasting and forewarning model for methane hazard based on the least square support vector (LS-SVM) multi-classifier and regression machine. For the forecasting model, the methane concentration can be considered as a nonlinear time series and the time series analysis method is adopted to predict the change in methane concentration using LS-SVM regression. For the forewarning model, which is based on the forecasting results, by the multi-classification method of LS-SVM, the methane hazard was identified to four grades: normal, attention, warning and danger. According to the forewarning results, corresponding measures are taken. The model was used to forecast and forewarn the K9 working face. The results obtained by LS-SVM regression show that the forecast- ing have a high precision and forewarning results based on a LS-SVM multi-classifier are credible. Therefore, it is an effective model building method for continuous prediction of methane concentration and hazard forewarning in working face.

Placebo Response among Different Types of Sham Acupuncture for Low Back Pain:A Systematic Review and Meta-Analysis of Randomized Controlled Trials

Background: Low back pain(LBP) is a prevalent and debilitating condition that poses a significant burden on healthcare systems. Acupuncture has been proposed as a promising intervention for LBP, but the evidence supporting its specific effect is insufficient, and the use of sham acupuncture as a control in clinical trials presents challenges due to variations in sham acupuncture techniques and the magnitude of the placebo ***: To investigate the magnitude of the placebo response of sham acupuncture in trials of acupuncture for nonspecific LBP, and to assess whether different types of sham acupuncture are associated with different responses. Methods: Four databases including PubMed, EMBASE, MEDLINE, and the Cochrane Library were searched through April 15, 2023, and randomized controlled trials(RCTs) were included if they randomized patients with LBP to receive acupuncture or sham acupuncture intervention. The main outcomes included the placebo response in pain intensity, back-specific function and quality of life. Placebo response was defined as the change in these outcome measures from baseline to the end of treatment. Random-effects models were used to synthesize the results, standardized mean differences(SMDs, Hedges’ g) were applied to estimate the effect size. Results: A total of 18 RCTs with 3,321 patients were included. Sham acupuncture showed a noteworthy pooled placebo response in pain intensity in patients with LBP [SMD –1.43, 95% confidence interval(CI) –1.95 to –0.91, I2=89%]. A significant placebo response was also shown in back-specific functional status(SMD –0.49, 95% CI –0.70 to –0.29, I2=73%),but not in quality of life(SMD 0.34, 95% CI –0.20 to 0.88, I2=84%). Trials in which the sham acupuncture penetrated the skin or performed with regular needles had a significantly higher placebo response in pain intensity reduction, but other factors such as the location of sham acupuncture did not have a significant impact on the placebo ***

The instability of slope blocks occurred frequently along traffic corridor in Southeastern Tibet(TCST),which was primarily controlled by the rock mass structures.A rapid method evaluating the control effects of rock mass structures was proposed through field statistics of the slopes and rock mass structures along TCST,which combined the stereographic projection method,modified M-JCS model,and limit equilibrium *** instabilities of slope blocks along TCST were then evaluated rapidly,and the different control factors of instability were *** showed that the probabilities of toppling(5.31%),planar(16.15%),and wedge(35.37%)failure of slope blocks along TCST increased *** instability modes were respectively controlled by the anti-dip joint,the joint parallel to slope surface with a dip angle smaller than the slope angle(singlejoint),and two groups of joints inclined out of the slope(double-joints).Regarding the control effects on slope block instability,the stabilization ability of doublejoints(72.7%),anti-dip joint(67.4%),and single-joint(57.6%)decreased sequentially,resulting in different probabilities of slope block ***,nearby regional faults significantly influenced the joints,leading to spatial heterogeneity and segmental clustering in the stabilization ability provided by joints to the slope ***,the stability of slope blocks gradually weakened as they approached the fault *** paper can provide guidance and assistance for investigating the development characteristics of rock mass structures and the stability of slope blocks.

Recommendations for Acupuncture in Clinical Practice Guidelines of the National Guideline Clearinghouse

Objective： To organize the clinical practice guidelines（CPGs） related to acupuncture included in the National Guideline Clearinghouse（NGC） to systematically summarize the diseases and disorders most commonly treated with acupuncture, the strength of recommendations for acupuncture and the quality of evidence. Methods： The NGC database was systematically searched for guidelines that included acupuncture as an intervention. Two independent reviewers studied the summaries and the full texts of the guidelines and included guidelines based on the inclusion and exclusion criteria. Thirty-nine guidelines were collected with 80 recommendations. The Appraisal of Guidelines for Research and Evaluation（AGREE） Ⅱ instrument was used to assess the quality of these guidelines. Results： Of the 80 recommendations on acupuncture, 49 recommendations were clearly for acupuncture, 25 recommendations were against acupuncture and 6 recommendations did not indicate any clear recommendations, 37 recommendations were for painful diseases/disorders, and 12 recommendations were for non-painful diseases/disorders. Locomotor system disorders were the most common in the painful diseases/disorders category. Out of all the recommendations for acupuncture, most recommendations（87.76%） were weak in strength, and most of the evidence（40.84%） was of low quality. Conclusion： In the National Guideline Clearinghouse, the recommendations for acupuncture focus on painful diseases/disorders. The recommendations in the guidelines are not high in strength, and most of the evidence is moderate or low in quality.

Background： With the worldwide use and modern development of acupuncture-moxibustion, there is disagreement on its basic definition. There has been an obstacle for clinical practice, research, and legislation. Considering this challenge, the World Federation of Acupuncture-Moxibustion Societies proposes to develop the definition and basic scope of acupuncture-moxibustion with a Delphi study. Methods： A reactive Delphi method will be used to ask a group of experts to rate the relevance and clarity of a predesigned initial item list. This list was generated by the steering committee through a literature review and group discussion. Forty experts will be selected from many disciplines and continents to ensure appropriate representation. Predefined rules will be applied to modify the items until a consensus is achieved. After achieving the consensus on all items through the survey round, the steering committee will form the definition and basic scope of acupuncture-moxibustion by organizing a literal statement with a detailed explanation.
