Review of controlled clinical trials on acupuncture versus sham acupuncture in Germany

OBJECTIVE: To examine German controlled clinical trials on the therapeutic effects of acupuncture vs sham acupuncture, and to find whether there are problems with the conclusion that sham acupuncture has no significant deviation from acupuncture. METHODS: We focused on literature from the last ten years (2002-2011) included in PubMed about controlled clinical trials on acupuncture vs sham acupuncture carried out in Germany. The methods applied in sham acupuncture are summarized, and the difference between the acupuncture and sham groups were analyzed. We measured effects based on the following criteria: acupuncture is effective and superior to sham, acupuncture is effective but similar to sham, both of them have uncertainty regarding treatment effect, or no significant effect. Finally, we reviewed the hypotheses of different scholars on sham acupuncture and analyzed their ***: Four types of controlled clinical trials including sham acupuncture on non-Traditional Chinese Medicine acupoints, minimal acupuncture on non-acupoints, placebo needle and sham laser acupuncture had varying results in the 57 articles analyzed. Some showed that acupuncture had a better effect than sham, while some suggest acupuncture and sham had similar effects. In all studies using sham acupuncture on non-therapeutic points, sham electrodes, and sham electro-acupuncture, the therapeutic effect was better than sham. Of the trials, 37 demonstrated that acupuncture had a better effect than sham acupuncture. Only nine trials found no significant difference between acupuncture and sham. Two controlled trials for the same condition (neck pain) conducted by two different German research institutes used the same control method, but reached contradictory conclusions. CONCLUSION: We found problems in conclusions based on results of controlled clinical trials of sham acupuncture in Germany. 目的探讨超声造影(CEUS)时间-强度曲线(TIC)及应变式弹性成像(RTE)在乳腺影像报告和数据系统(BI-RADS)4类乳腺病变鉴别诊断中的应用价值。方法选取该院2019年6月至2021年2月收治的118例BI-RADS 4类病变患者进行回顾性研究,以病理诊断为金标准,恶性病变组46例,良性病变组72例,比较两组CEUS图像特征、TIC形态及其定量参数[良恶性病变部位的CEUS达峰时间(TTP)、峰值强度(PI)],以及弹性成像应变率比值(B/A);评估两种超声诊断技术单独应用和联合应用对BI-RADS 4类病变诊断的灵敏度、特异度、准确率,绘制受试者工作特征(ROC)曲线,获得曲线下面积(AUC)。结果乳腺恶性病变组CEUS的TIC形态较陡直,TTP短于良性病变组[(13.87±3.89)s vs.(22.70±9.10)s],PI大于良性病变组[(21.25±7.32)dB vs.(11.77±4.10)dB],差异均有统计学意义(P<0.05);乳腺恶性病变组B/A大于良性病变组,差异有统计学意义(4.47±1.63 vs.2.81±1.04,P<0.05)。CEUS的TIC与RTE单独应用的诊断灵敏度、准确率有明显差异(P<0.05),RTE与CEUS+RTE的灵敏度、准确率亦有明显差异(P<0.05);ROC曲线提示CEUS+RTE诊断的AUC为0.967,CEUS诊断的AUC为0.955,RTE诊断的AUC为0.826,差异有统计学意义(P<0.05)。结论CEUS的TIC联合RTE及单独应用CEUS对BI-RADS 4类乳腺病变的鉴别诊断均有重要的临床意义,两种技术的联合应用更具参考价值。

Cluster analysis for syndromes of real-world coronary heartdisease with angina pectoris

Syndromes of coronary heart disease with angina pectoris were analyzed to provide guidance for clinical practice and to improve accuracy of traditional Chinese medicine (TCM) diagnoses and efficacy of TCM treatment. A total of 860 cases with coronary heart disease with angina pectoris were selected from TCM Clinical Research Information Sharing System for TCM clinics and research. Syndromes were automatically extracted with the cluster method and were analyzed to provide objective evidence for clinical studies. Final syndrome classifications were recognized and confirmed by clinical experts. Popular syndromes included Qi and blood deficiency, blood stasis and obstruction collaterals, liver depression and spleen deficiency, and Qi stagnation and blood stasis. Syndromes Qi and blood deficiency and blood stasis and obstruction collaterals accounted for 28.61% of total syndromes, whereas liver depression and spleen deficiency and Qi stagnation and blood stasis accounted for 26.44%. The main syndrome elements comprised Qi deficiency, blood deficiency, blood stasis, and Qi stagnation.

li_(2)O-Al_(2)O_(3)-SiO_(2)(LAS)glass-ceramics were prepared by a melting *** of different Al_(2)O_(3)content on the structure,crystallization,transmittance and fracture toughness of LAS glassceramics were investigated by means of XRD,FESEM and other methods as *** results showed that the glass transition temperature and crystallization temperature of samples increased as the content of Al_(2)O_(3)increased from 4.1 wt%to 13.1 wt%,which restrained the precipitation of lithium disilicate *** main crystalline phase of glass-ceramics transformed from lithium disilicate and petalite to silicon dioxide,which reduced the fracture toughness of *** the Al_(2)O_(3)content was 7.1 wt%,the specimen had outstanding transmittance and fracture *** transmittance was 90.32%.The fracture toughness was 1.13 MPa•m^(1/2).Compared with high-alumina glass,the fracture toughness of the glass-ceramic was greatly improved,and it could be used as a new type of protective material for mobile devices.

A novel classification method for aid decision of traditional Chinese patent medicines for stroke treatment

Traditional Chinese patent medicines are widely used to treat stroke because it has good efficacy in the clinical environment. However, because of the lack of knowledge on traditional Chinese patent medicines, many Western physicians, who are accountable for the majority of clinical prescriptions for such medicine, are confused with the use of traditional Chinese patent medicines. Therefore, the aid-decision method is critical and necessary to help Western physicians rationally use traditional Chinese patent medicines. In this paper, Manifold Ranking is employed to develop the aid-decision model of traditional Chinese patent medicines for stroke treatment. First, 115 stroke patients from three hospitals are recruited in the cross-sectional survey. Simultaneously, traditional Chinese physicians determine the traditional Chinese patent medicines appropriate for each patient. Second, particular indicators are explored to characterize the population feature of traditional Chinese patent medicines for stroke treatment. Moreover, these particular indicators can be easily obtained by Western physicians and are feasible for widespread clinical application in the future. Third, the aid-decision model of traditional Chinese patent medicines for stroke treatment is constructed based on Manifold Ranking. Experimental results reveal that traditional Chinese patent medicines can be differentiated. Moreover, the proposed model can obtain high accuracy of aid decision.

A review was undertaken of the operation process and development of transcutaneous electrical acupoint stimulation(TEAS)and related devices for TEAS,with the aim to offer a reference for developing an international standard for the basic safety and essential performance of the *** articles related to TEAS and instruction of devices for TEAS were searched using the EMBASE,MEDliNE,and Web of Science databases with the time period from inception to July 18,*** the absence of a parameter description of the stimulators,a multimeter was used to measure the output voltage,resistance,and ***-two related devices for TEAS were *** safety parameters ofmost devices were neither clearly defined,nor standardized,and in some cases *** was a noticeable disparity in the upper safety limits of the output current among the *** sizes of the skin electrode pads as well as the lengths of the electrode connecting wires of most devices were not clearly *** on different parts of the human body,including the upper limbs,head,auricle,chest,abdomen,trunk,and lower limbs,required different maximum tolerable current intensities and current *** is important to indicate comprehensive output/safety parameters and essential performance for devices for TEAS to meet the need of global distribution,achieve precise stimulation parameters at different acupoints across the human body,and allay any safety concern of national therapeutic device authorities,the regulators,manufacturers,and end users.
