Comparison of tirofiban combined with dalteparin or unfractionated heparin in primary percutaneous coronary intervention of acute ST-segment elevation myocardial infarction patients

Background Primary percutaneous coronary intervention （PCI） is the best treatment of choice for acute ST segment elevation myocardial infarction （STEMI）. This study aimed to determine the clinical outcomes of tirofiban combined with the low molecular weight heparin （LMWH）, dalteparin, in primary PCI patients with acute STEMI. Methods From February 2006 to July 2006, a total of 120 patients with STEMI treated with primary PCI were randomised to 2 groups： unfractionated heparin （UFH） with tirofiban （group I： 60 patients, （61.2±9.5） years）, and dalteparin with tirofiban （group il： 60 patients, （60.5±10.1） years）. Major adverse cardiac events （MACE） during hospitalization and at 4 years after PCI were examined. Bleeding complications during hospitalization were also examined. Results There were no significant differences in sex, mean age, risk factors, past history, inflammatory marker, or echocardiography between the 2 groups. In terms of the target vessel and vascular complexity, there were no significant differences between the 2 groups. During the first 7 days, emergent revascularization occurred only in 1 patient （1.7%） in group I. Acute myocardial infarction （AMI） occurred in 1 （1.7%） patient in group I and in 1 （1.7%） in group II. Three （5.0%） patients in group I and 1 （1.7%） in group II died. Total in-hospital MACE during the first 7 days was 4 （6.7%） in group I and 2 （3.3%） in group Ⅱ. Bleeding complications were observed in 10 patients （16.7%） in group I and in 4 patients （6.7%） in group II, however, the difference was not statistically significant. No significant intracranial bleeding was observed in either group. Four years after PCI, death occurred in 5 （8.3%） patients in group I and in 4 （6.7%） in group Ⅱ. MACE occurred in 12 （20.0%） patients in group I and in 10 （16,7%） patients in group Ⅱ. gender differences in efficacy of primary percutaneous coronary intervention in patients with ST-elevation myocardial infarction

Background The clinical outcome of percutaneous coronary intervention （PCI） is poorer in women than that in men. This study aimed at comparing the impact of gender difference on the strategy of primary PCI in patients with acute ST-segment elevation myocardial infarction （STEMI）. Methods Two hundred and fifty-nine patients with STEMI who underwent primary PCI within 12 hours of symptom onset were enrolled. The male group consisted of 143 men aged 〉55 years, and a female group included 116 women without age limitation. Procedural success was defined as residual stenosis 〈20% with thrombolysis in myocardial infarction flow grade 〉2 and without death, emergency bypass surgery or disabling cerebral events during the hospitalization. The rate of major adverse cardiac events （MACE）, including death, nonfatal myocardial infarction and target vessel revascularization during follow-up, was recorded. Results Female patients were more hypertensive and diabetic and with fewer cigarette smokers than male counterparts. The prevalence of angiographic 3-vessel disease was higher in the female group, but the procedural success rate was comparable between the two groups （94.4% vs 92.2%）. The occurrence rate of MACE did not differ during the hospitalization （4.2% vs 6.0%, P=0.50）, but was significantly higher in the female group during follow-up （mean （16.0±11.2） months） than that in the male group （5.4% vs 0.7%, P=0.02）. Conclusion Despite a similar success rate of primary PCI and in-hospital outcomes in both genders, female patients with acute STEMI still have a worse prognosis during the long-term follow-up.

There is considerable potential for integrating transarterial chemoembolization(TACE),programmed death-(ligand)1(PD-[L]1)inhibitors,and molecular targeted treatments(MTT)in hepatocellular carcinoma(HCC).It is necessary to investigate the therapeutic efficacy and safety of TACE combined with PD-(L)1 inhibitors and MTT in real-world *** this nationwide,retrospective,cohort study,826 HCC patients receiving either TACE plus PD-(L)1 blockades and MTT(combination group,n=376)or TACE monotherapy(monotherapy group,n=450)were included from January 2018 to May *** primary endpoint was progression-free survival(PFS)according to modified *** secondary outcomes included overall survival(OS),objective response rate(ORR),and *** performed propensity score matching approaches to reduce bias between two *** matching,228 pairs were included with a predominantly advanced disease *** PFS in combination group was 9.5 months(95%confidence interval[CI],8.4-11.0)versus 8.0 months(95%CI,6.6-9.5)(adjusted hazard ratio[HR],0.70,P=0.002).OS and ORR were also significantly higher in combination group(median OS,19.2[16.1-27.3]vs.15.7 months[13.0-20.2];adjusted HR,0.63,P=0.001;ORR,60.1%vs.32.0%;Pgest that TACE plus PD-(L)1 blockades and MTT could significantly improve PFS,OS,and ORR versus TACE monotherapy for Chinese patients with predominantly advanced HCC in real-world practice,with an acceptable safety profile.

We investigate high time resolution data obtained by the Gravitational wave high-energy Electromagnetic Counterpart All-sky Monitor(geCAM)during the flare event on 2022 April 21 at 01:52 *** subpeaks with durations of 4-6 s have been detected in the hard X-ray precursor phase,and the key feature is that they appear in pairs and seem like double-peak *** subpeaks are rarely observed in hard X-ray band and confirmed by the microwave obtained by Nobeyama Radio Polarimeters(NoRP)and Radio Solar Telescope Network(RSTN).While an exponential function can describe the continuum component of the time profile from the precursor to part of the impulsive *** periods of quasi-periodic pulsations(QPPs)are detected to be about 7.3 and 12.8 s for the precursor and impulsive phase,respectively,with at least 95%confidence *** paired QPPs are assumed to be double-peak QPPs and then the scenario of current loop coalescence model is found to be in good agreement with our *** precursor phase can be interpreted as the oscillating coalescence of two islands,while the impulsive phase can be interpreted as more islands to coalesce one by one to form larger islands.

The Large sky Area Multi-Object Fiber Spectroscopic Telescope（LAMOST） general survey is a spectroscopic survey that will eventually cover approximately half of the celestial sphere and collect 10 million spectra of stars, galaxies and QSOs. Objects in both the pilot survey and the first year regular survey are included in the LAMOST DR1. The pilot survey started in October 2011 and ended in June 2012, and the data have been released to the public as the LAMOST Pilot data Release in August 2012. The regular survey started in September 2012, and completed its first year of operation in June 2013. The LAMOST DR1 includes a total of 1202 plates containing 2 955 336 spectra, of which 1 790 879 spectra have observed signalto-noise ratio（SNR） ≥ 10. All data with SNR ≥ 2 are formally released as LAMOST DR1 under the LAMOST data policy. This data release contains a total of 2 204 696 spectra, of which 1 944 329 are stellar spectra, 12 082 are galaxy spectra and 5017 are quasars. The DR1 not only includes spectra, but also three stellar catalogs with measured parameters： late A,FGK-type stars with high quality spectra（1 061 918 entries）, A-type stars（100 073 entries）, and M-type stars（121 522 entries）. This paper introduces the survey design, the observational and instrumental limitations, data reduction and analysis, and some caveats. A description of the FITS structure of spectral files and parameter catalogs is also provided.

This paper describes the data release of the LAMOST pilot survey, which includes data reduction, calibration, spectral analysis, data products and data access. The accuracy of the released data and the information about the FITS headers of spectra are also introduced. The released data set includes 319 000 spectra and a catalog of these objects.

Objective The relationship between serum uric acid(SUA)levels and glycemic indices,including plasma glucose(FPG),2-hour postload glucose(2 h-PG),and glycated hemoglobin(HbA1 c),remains *** aimed to explore the associations between glycemic indices and SUA levels in the general Chinese *** The current study was a cross-sectional analysis using the first follow-up survey data from The China Cardiometabolic Disease and Cancer Cohort Study.A total of 105,922 community-dwelling adults aged≥40 years underwent the oral glucose tolerance test and uric acid *** nonlinear relationships between glycemic indices and SUA levels were explored using generalized additive *** A total of 30,941 men and 62,361 women were eligible for the current *** additive models verified the inverted U-shaped association between glycemic indices and SUA levels,but with different inflection points in men and *** thresholds for FPG,2 h-PG,and HbA1 c for men and women were 6.5/8.0 mmol/L,11.0/14.0 mmol/L,and 6.1/6.5,respectively(SUA levels increased with increasing glycemic indices before the inflection points and then eventually decreased with further increases in the glycemic indices).Conclusion An inverted U-shaped association was observed between major glycemic indices and uric acid levels in both sexes,while the inflection points were reached earlier in men than in women.

We present the observational results from a detailed timing analysis of the black hole candidate EXO 1846-031 during its outburst in 2019 with the observations of Insight-HXMT,niCER and *** outburst can be classified roughly into four different ***-C quasi-periodic oscillations(QPOs)observed by niCER(about 0.1-6 Hz)and Insight-HXMT(about 0.7-8 Hz)are also reported in this ***,we study various physical quantities related to QPO *** QPO rms-frequency relationship in the energy band 1-10 keV indicates that there is a turning pointing in frequency around2 Hz,which is similar to that of GRS 1915+105.A possible hypothesis for the relationship above may be related to the inclination of the source,which may require a high inclination to explain *** relationships between QPO frequency and QPO rms,hardness,total fractional rms and count rate have also been found in other transient sources,which can indicate that the origin of type-C QPOs is non-thermal.

目的探讨与金属裸支架（BMS）相比，Cypher支架（Cordis公司，美国）在急性ST段抬高心肌梗死（STEMI）急诊经皮冠状动脉介入治疗（PCI）中应用的安全性和长期有效性。方法连续选择2002年12月至2005年3月间STEMI患者407例，均于发病12h内行急诊PCI治疗，于梗死相关血管靶病变置入Cypher支架或BMS。对所有病例随访1．5～4．0年（平均28．7±11．7个月），比较两类支架院内及出院后严重心脏不良事件（MACE，包括死亡、再发心肌梗死、靶病变重建等）发生的差异。结果407例患者中置入Cypher支架者131例，置入BMS者276例。Cypher支架组所置入支架的直径明显小于BMS组（3．0mm与3．2mm，P=0．00）。两组在随访期间心因性死亡（1．5％与6．9％，P=0．02）以及随访期间总的主要不良心脏事件（MACE）（6．1％与12．7%，P=0.04）差异均有统计学意义。应用Cox回归比例风险模型计算Cypher支架组在随访期间发生总MACE的相对危险度为0．45（P〈0．05）。两组的支架内血栓形成发生率差异无统计学意义，支架内再狭窄发生率以及靶病变重建率差异亦无统计学意义，但Cypher支架较之BMS有明显降低的趋势。结论在STEMI的急诊PCI治疗时，置入Cypher支架具有较好的安全性和长期有效性，与BMS相比可以降低远期复合MACE以及心因性死亡的发生率。
