Standardizing mI-TlIF, a proposal for a Reproducible Technique

Background: minimally invasive transforaminal lumbar interbody fusion (mI TlIF) is a widely known and performed technique, however its versatility among different physicians continues to hinder its replication and results. Therefore, this study aimed to provide a step-by-step surgical guide to perform a safe mI-TlIF, based on the results obtained in patients operated on by a single surgeon over a period of 12 years. patients and methods: A retrospective, single center, longitudinal, and observational cohort study was conducted with 931 patients who underwent mI TlIF by a single surgeon between 2010 and 2022 using the technique described on this paper, each with a minimum follow-up of 12 months. Criteria included Schizas classification, listhesis according to meyerding classification, number of levels treated, cage size, and complications (screw repositioning or cerebrospinal fluid leak). patient clinical outcomes were assessed using the Oswestry Disability Index (ODI), Visual Analog Scale (VAS) for pre- and postoperative radicular pain. Thin slice CT scans were used to assess the progression of the fusion using the Bridwell classification. In the statistical analysis, percentages, median, and interquartile range (IQR) were calculated. Results: Nine hundred and thirty one patients underwent mI TlIF using the technique described, eight hundred and eighty (94.5%) had a single level treated and fifty one (5.5%) had a 2 level procedure (982 levels), an 8mm cage was placed on five hundred and seventeenlevels (52.7%), six hundred and sixty three levels(67.6%) achieved grade I fusion, two hundred and sixty six levels (27.1%) achieved grade II fusion, 52 levels (5.3) achieved grade III fusion and one level (0.1) achieved a grade IV fusion or non-union. Revision surgery was performed on 3 patients (0.3%) for screw repositioning, cerebrospinal fluid leak was present on 2 patients during surgery and treated before closure. Epidemiology of insertional and midportion Achilles tendinopathy in runners:A prospective cohort study

Industry decarbonization requires the development of highly efficient and flexible technologies relying on renewable energy resources,especially biomass and solar/wind *** the case of pure oxygen production,oxygen transport membranes(OTms)appear as an alternative technology for the cryogenic distillation of air,the industrially-established process of producing ***,OTms could provide oxygen from different sources(air,water,CO_(2),etc.),and they are more flexible in adapting to current processes,producing oxygen at 700^(-1)000℃.Furthermore,OTms can be integrated into catalytic membrane reactors,providing new pathways for different *** first part of this study was focused on electrification on a traditional OTm material(Ba_(0.5)Sr_(0.5)Co_(0.8)Fe_(0.2)O_(3-δ)),imposing different electric currents/voltages along a capillary *** to the emerging Joule effect,the membrane-surface temperature and the associated O_(2) permeation flux could be ***,the OTm is electrically and locally heated and reaches 900℃on the surface,whereas the surrounding of the membrane was maintained at 650℃.The O_(2)permeation flux reached for the electrified membranes was~3.7 Nml min^(-1)cm^(-2),corresponding to the flux obtained with an OTm non-electrified at 900℃.The influence of depositing a porous Ce_(0.8)Tb_(0.2)O_(2-δ) catalytic/protective layer on the outer membrane surface revealed that lower surface temperatures(830℃)were detected at the same imposed electric ***,the electrification concept was demonstrated in a catalytic membrane reactor(CmR)where the oxidative dehydrogenation of ethane(ODHE)was carried *** reaction is very sensitive to temperature,and here,we demonstrate an improvement of the ethylene yield by reaching moderate temperatures in the reaction chamber while the O_(2) injection into the reaction can be easily fine-tuned.

High Strain-Rate and Shock Response of Carbon SupercompositeTm

This study focuses on assessing the dynamic behaviors of carbon SupercompositeTm laminates when subjected to high strain-rates and air blast loads, using a shock tube for testing. The investigation aims to understand the response of these advanced materials under extreme conditions, which is crucial for applications in aerospace, military, and other high-performance industries. SupercompositeTm (CZE) prepreg, made up of a 3K plain weave carbon fabric with milled carbon fibers as interlaminar reinforcements impregnated with epoxy, is used to create SupercompositeTm (CZE) laminates. A woven carbon composite (CBE) laminate was also created using 3K plain weave Carbon/Epoxy (CBE) prepreg. Both types of laminates were designed and fabricated using the autoclave process. The dynamic behaviors of CZE and CBE laminate under transverse compression loads were evaluated using a modified Split Hopkinson pressure Bar (SHpB). The study found that the 3D reinforcement with milled carbon fibers significantly affected the dynamic behavior of the CZE laminate. Stereo imaging videos, captured using two SHImADZU high-speed video cameras in shock tube experiments, recorded the time history of back surface deflection. The plate specimens exhibited low deflections without any visible damage. The experimentally observed center point deflections of the CZE plates decayed sooner than those of the CBE laminates, indicating an improvement in damping due to the presence of 3D reinforced milled carbon fibers. This research shows that optimized utilization of milled carbon fibers as 3D reinforcement can withstand high stress in the thickness direction and higher energy absorption when subjected to impact and high strain-rate loading.

This study aims to predict the undrained shear strength of remolded soil samples using non-linear regression analyses,fuzzy logic,and artificial neural network modeling.A total of 1306 undrained shear strength results from 230 different remolded soil test settings reported in 21 publications were collected,utilizing six different measurement *** water content,plastic limit,and liquid limit were used as input parameters for fuzzy logic and artificial neural network modeling,liquidity index or water content ratio was considered as an input parameter for non-linear regression *** non-linear regression analyses,12 different regression equations were derived for the prediction of undrained shear strength of remolded ***-Forward backpropagation and the TANSIG transfer function were used for artificial neural network modeling,while the mamdani inference system was preferred with trapezoidal and triangular membership functions for fuzzy logic *** experimental results of 914 tests were used for training of the artificial neural network models,196 for validation and 196 for *** was observed that the accuracy of the artificial neural network and fuzzy logic modeling was higher than that of the non-linear regression ***,a simple and reliable regression equation was proposed for assessments of undrained shear strength values with higher coefficients of determination.

Resistance to cancer immunotherapy is mainly attributed to poor tumor immunogenicity as well as the immunosuppressive tumor microenvironment(TmE)leading to failure of immune *** therapeutic strategies including chemotherapy,radiotherapy,photodynamic,photothermal,magnetic,chemodynamic,sonodynamic and oncolytic therapy,have been developed to induce immunogenic cell death(ICD)of cancer cells and thereby elicit immunogenicity and boost the antitumor immune ***,many challenges hamper the clinical application of ICD inducers resulting in modest immunogenic ***,we outline the current state of using nanomedicines for boosting ICD of cancer ***,synergistic approaches used in combination with ICD inducing nanomedicines for remodeling the TmE via targeting immune checkpoints,phagocytosis,macrophage polarization,tumor hypoxia,autophagy and stromal modulation to enhance immunogenicity of dying cancer cells were *** further highlight the emerging trends of using nanomaterials for triggering amplified ICD-mediated antitumor immune *** reticulum localized ICD,focused ultrasound hyperthermia,cell membrane camouflaged nanomedicines,amplified reactive oxygen species(ROS)generation,metallo-immunotherapy,ion modulators and engineered bacteria are among the most innovative *** challenges,merits and demerits of ICD inducer nanomedicines were also discussed with shedding light on the future role of this technology in improving the outcomes of cancer immunotherapy.

Desmodus rotundus and Diphylla ecaudata, both of which are mammals of the order Chiroptera, Desmodontidae family, their diet consisting exclusively of blood. D. rotundus is the main vector and transmitter of the rabies virus, which affects human beings as well as several livestock species so the study of this bat species is of high importance within the fields of animal agriculture and public health. The present study describes and compares the histologic characteristics of the urinary system of two hematophagous bat species. A total of 5 bats from each species were captured in the municipalities of progreso de Obregón, Hidalgo (D. rotundus), and Huayacocotla, Veracruz (D. ecaudata). Organs belonging to the urinary system were extracted: kidneys, ureters, urinary bladder, and urethra;samples were fixed using 10% formalin and processed by the paraffin embedding technique, obtaining sections of 5 µm thickness, which in turn were stained using hematoxylin-eosin (H-E) and Gomori trichrome (GT) stains. From the obtained histologic preparations, a descriptive and comparative analysis of the structural organography of the urinary system of both species was made, and no noteworthy histological differences between samples were noted. The present research is intended to provide a framework for future studies of these species’ currently understudied microscopic anatomy.

Academic Stress Assessment Using Virtual Reality as an Educational Tool in Spine Surgery

Introduction: The evaluation of academic stress in medical students and residents is a topic of significant interest, given the considerable challenges they face during their learning process with traditional teaching methods. The use of technologies like virtual reality presents an opportunity to enhance their skills through simulations and training. The main objective of this study is to qualitatively assess the stress levels experienced by medical students and residents by integrating virtual reality into their current learning methods, aiming to improve their ability to manage stressors in their practice. material and methods: A questionnaire was conducted with 12 medical students and 12 Traumatology and orthopedics residents. The purpose of the questionnaire was to evaluate the levels of academic stress using the SISCO inventory. The stress levels were calculated by transforming average values into percentages, and the following criteria were assigned: 0 to 33% for mild Stress, 34 to 66% for moderate Stress, and 67 to 100% for Deep Stress. Then, a virtual reality class focused on spine surgery was provided. Both medical students and residents were trained using the Non Nocere SharpSurgeon software platform and Oculus Quest 2 virtual reality glasses. At the end of the session, a second questionnaire related to the practice with virtual reality was conducted with the same evaluation criteria and a comparative analysis was carried out. Results: 12 undergraduate students from Hospital Angeles mexico, CDmX and 12 traumatology and orthopedics residents at Hospital Santa Fe, Bogota were evaluated. The students in CDmX reported an average qualitative stress of 28.50% during habitual practices, which decreased to an average of 14.67% after virtual reality practice. Residents in Bogotá experienced an average qualitative stress of 30.50% with their current learning methods but this reduced to an average of 13.92% after using virtual reality. The tumor microenvironment is composed of a complex mixture of different cell types interacting under conditions of nutrient deprivation,but the metabolism therein is not fully understood due to difficulties in measuring metabolic fluxes and exchange of metabolites between different cell types in ***-scale metabolic modeling enables estimation of such exchange fluxes as well as an opportunity to gain insight into the metabolic behavior of individual cell ***,we estimated the availability of nutrients and oxygen within the tumor microenvironment using concentration measurements from blood together with a metabolite diffusion *** addition,we developed an approach to efficiently apply enzyme usage constraints in a comprehensive metabolic model of human *** combined modeling reproduced severe hypoxic conditions and the Warburg effect,and we found that limitations in enzymatic capacity contribute to cancer cells’preferential use of glutamine as a substrate to the citric acid ***,we investigated the common hypothesis that some stromal cells are exploited by cancer cells to produce metabolites useful for the cancer *** identified over 200 potential metabolites that could support collaboration between cancer cells and cancer-associated fibroblasts,but when limiting to metabolites previously identified to participate in such collaboration,no growth advantage was *** work highlights the importance of enzymatic capacity limitations for cell behaviors and exemplifies the utility of enzyme-constrained models for accurate prediction of metabolism in cells and tumor microenvironments.
