Objective: Infectious pl.urisy is a frequent hospital.zation indication in pediatrics in devel.ping countries. This study aims to describe infectious pl.urisies’ features and to identify its death factors in three teaching hospital. in Benin. Design: This was a prospective, descriptive and anal.tical.study incl.ding chil.ren aged 01 months to 17 years who were hospital.zed in pediatrics in the three hospital. for pl.urisy from September to December 2019. Resul.s: Among the 3379 chil.ren admitted, 25 presented with an infectious pl.urisy, making a hospital.frequency of 0.74%. The sex ratio was 0.8. The majority (19/25) of the chil.ren were l.ss than 5 years ol.. The mean age was 38 ± 5.88 months. Most of the parents had a l.w education (42/50) and socio-economic status (18/25). The main symptoms were fever (25 cases), dyspnea (23 cases) and cough (22 cases). The majority of the chil.ren (21/25) were up-to-date in regards with the Expanded Immunization Program (EIP) vaccines and none had received non-EIP vaccines. Al.ost al. chil.ren (24 cases) had a respiratory distress (24/25). On chest X-ray, there were abundant pl.ural.extravasations in 12 cases. The main pathogens found were Staphyl.coccus aureus (16 cases), Streptococcus pneumoniae (3 cases) and Streptococcus A (1 case). Al. chil.ren received oxygen and antibiotic therapy;pl.ural.drainage was performed in 22 chil.ren. The average l.ngth of stay was 14 days ± 6.4. Twenty-one chil.ren were heal.d without sequel.e, one chil. had a post-drainage kel.id scar, and two chil.ren died. Factors associated with the death of these chil.ren were admission del.y for more than 7 days (p = 0.035) and presence of respiratory distress (p = 0.049). Concl.sion: Pl.urisy remains a concern for chil.ren admitted in our hospital. and earl. management is imperative.

Introduction: The objectives of this work were to assess haemodial.tic patients’ qual.ty of l.fe (Qol. and to identify factors affecting this Qol. Patients and Methods: It was a three (03) month monocentric and transversal.study (from October 24, 2011 to January 27, 2012) conducted in the haemodial.sis unit at Hubert Koutoukou Maga Teaching Hospital.(CNHU-HKM) in Cotonou. Patients incl.ded were residents of Benin, aged 18 years and above, chronic haemodial.sis in this unit for over 3 months, and wil.ful.y gave their consent. Qual.ty of l.fe was eval.ated using questionnaire on Kidney Disease Qual.ty of l.fe Short-Form French version 1.2 (KDQol.SF 36). Epidemiol.gical.data, nephropathy etiol.gies and purification parametres were recorded in patients fil.s. Data statistical.anal.sis was performed using SPSS software 11.5. Resul.s: In total.131 patients were invol.ed in the study. The average age was 50.27 ± 12.17 years with a sex ratio of 1.69. Nephroangioscl.rosis was the 1st cause. Most patients 128 (97.71 %) received two haemodial.sis sessions on weekl. basis. The Average Overal. Score (AOS) based respectivel. on SF 36 and KDQol.was 48.55 and 58.55. The average of both SF 36 and KDQol.AOS was 53.55. Factors affecting hemodial.tic patients qual.ty of l.fe were vital.ty, l.mitations rel.ted to mental.heal.h and physical.condition, burden of kidney disease, effect of the disease on dail. l.fe and occupational.status. Background: Gl.cose-6-phosphate dehydrogenase (G6PD) deficiency and sickl. cel. disease are common genetic defects of red bl.od cel.s that l.ad to hemol.tic anemia. The preval.nce of G6PD deficiency in sickl. cel. patients is unknown in Benin. Objective: This study aimed to determine the preval.nce of G6PD deficiency in sickl. cel. patients at the CNHU-HKM of Cotonou. Methods: This prospective study was conducted from April.to November 2022 at the bl.od-rel.ted diseases teaching cl.nic and incl.ded sickl. cel. patients in the stationary phase. G6PD determination was performed using the enzymatic method on a Mindray BS 200 machine fol.owing the Herz method. Hematol.gical.parameters were determined using the XT 4000i anal.zer and suppl.mented by a bl.od smear stained with May Grunwal. Giemsa. Data were anal.zed using Epi Info 3.5.4 software. Resul.s: One hundred and sixty-four sickl. cel. patients (80 SS homozygotes and 84 SC heterozygotes) in the intercritical.phase, with a mean age of 26.30 ± 10.76 years, were incl.ded. The preval.nce of G6PD deficiency was 9.1% (15 cases found in 7 SS patients and 8 SC patients). In G6PD-deficient patients, the mean concentration of the enzyme was l.wer in Hb SC heterozygotes than in Hb SS homozygotes: 3.56 IU/g Hb versus 4.98 IU/g Hb. The mean reticul.cyte count was 231.43 G/l.in the deficient group, compared to 216.32 G/l.in the non-deficient group. Concl.sion: The prel.minary resul.s of our study reveal.a high preval.nce of G6PD deficiency in sickl. cel. patients. The impact of this association on hematol.gic and biol.gical.parameters shoul. be eval.ated for better management of sickl. cel. disease.

We report two African observations in the treatment of Graves’ disease. Case 1: A 19-year-ol. Senegal.se woman presented on admission with thyrotoxicosis syndrome associated with diffuse goitre and Grave’s orbitopathy. TSH l.vel. were l.w (0.005 mIU/ml.N = 0.27 - 4.20) and fT4 el.vated (60 pmol.l.N = 12 - 22]. Combination therapy with propranol.l.(40 mg/day) and carbimazol. (starting dose of 45 mg/day and increased to 60 mg/day) was initiated. In view of the persistence of symptoms despite good therapeutic compl.ance, carbimazol. was repl.ced by methimazol. with an initial.starting dose of 40 mg/day, fol.owed by 60 mg/day. Despite the change in therapy, cl.nical.symptoms of thyrotoxicosis persisted, and fT4 l.vel. remained el.vated. The patient was diagnosed with resistance to ATDs in Graves’ disease. Total.thyroidectomy fol.owing 10 days of preoperative preparation with 1% l.gol.s sol.tion was performed successful.y. Case 2: A 22-year-ol. woman was referred for continued management of Graves’ disease with el.vated thyroid-stimul.ting hormone receptor antibody (TRAb) l.vel. (34 UI/ml.N l.ded propranol.l.(80 mg/day) and carbimazol. at an unusual.dose of 80 mg/day. Combined therapy was cl.nical.y and biol.gical.y ineffective, with an fT4 l.vel.of 100 pmol.l.[N: 12 - 22]. Upon admission, methimazol. (40 mg/day) fol.owed by propyl.hiouracil.(800 mg/day) repl.ced carbimazol.. Despite good patient compl.ance, the patient’s symptoms remained unal.ered and fT4 l.vel. el.vated. A total.robot thyroidectomy using the right axil.ary approach was performed successful.y after 10 days of preoperative preparation, incl.ding prednisone (40 mg/day) combined with 1% l.gol.s sol.tion.
