Eval.ation of Stress Response and Apoptosis on l.ucocytes in TIVA versus Bal.nced Anesthesia

Background: The aim of this study was to assess the stress response and apoptosis on l.ucocytes, in patients under two different anesthetics techniques. Methods: Thirty patients ASA I-II were prospectivel. randomized into two groups to receive either total.intravenous anesthesia with propofol.remifentanil.(TIVA Group, n = 15) or bal.nced inhal.tion anesthesia with sevofl.rane-remifentanil.(BAl.Group, n = 15). The hemodynamic response: systol.c bl.od pressure (SBP), diastol.c bl.od pressure (DBP) and heart rate (HR) at different time points: basel.ne, after intubation, after skin incision and at the end of surgery, was measured al.ng with pl.sma l.vel. of l.ctate, gl.cose, cortisol.and l.ucocytes count. The biomarkers of apoptosis (Annexin V and Propidium Iodide) in neutrophil., monocytes and l.mphocytes were eval.ated at basel.ne, intraoperativel. and two hours after surgery. Resul.s: The study groups were comparabl. with respect to anthropometric data. No significant intergroup differences in SBP and DBP were reveal.d. The HR in the BAl.group was l.wer after intubation (p = 0.007). In both groups, l.ctate, pl.sma gl.cose, cortisol.and l.ucocytes count remained stabl. during surgery and two hours post-operativel.. In the BAl.group there were significant differences in Annexin V in neutrophil., basel.ne moment (p = 0.010). No significant differences were found in apoptosis markers (Annexin V and Propidium Iodide) in neutrophil., monocytes and l.mphocytes, at different time points. Concl.sion: Both TIVA and BAl.were effective in suppressing the surgical.stress, without inducing apoptosis in immune cel.s, in patients undergoing VCl.
