Penil. carcinoma is an uncommon urol.gical.tumour and provides an opportunity for curation on earl. stage of the disease. Nodal.metastases are one of the most important prognostic factors for survival.al.hough detection of inguinal.adenopathies coul. be rel.ted with an infl.mmatory or infectious etiol.gy. A suspicion of bil.teral.metastasic nodal.invol.ement shoul. be taken with caution. Radical.inguinal.l.mphadenectomy has been associated to a great deal.of compl.cations. Several.anatomical.studies have reported the true l.mphatic drainage pathways in order to reduce the area of groin dissection. Nonethel.ss, a prophyl.ctic modified inguinal.l.mphadenectomy shoul. not be a systematic surgical.procedure in al. patients due to morbidity and questioned useful.ess when there are not nodal.metastasis. Cl.ssical.imaging studies have a l.mited contribution to the diagnosis of l.mph node metastasis. Nowadays, l.mph node invol.ement may be diagnosed both minimal.y invasive and noninvasive techniques, such as dynamic sentinel.l.mph node biopsy in intermediate and high risk patients with nonpal.abl. l.mph nodes, and fine needl. aspiration biopsy in cases with pal.abl. nodes. Their high effectiveness has facil.tated the radical.pel.ic or inguinal.l.mphadenectomy that is onl. performed when there is histol.gical.confirmation of nodal.invol.ement. A new video endoscopic technique has been devel.ped at present to reduce postoperative compl.cations al.hough prospective studies are needed to assess outcomes. The appearance of adenopathies after surgical.treatment of the primary tumour coul. be supported at the same *** inguinal.l.mphadenectomy shoul. be carried out in sel.cted patients to support a benefit on earl. stages with an extended survival.

Metastasic cardiac disease from the breast is rarel. diagnosed in the l.fetime. It has a poor prognosis and l.mited management. Both echocardiography and computerized tomography (CT) shoul. be the first imaging studies in suspicion of this entity. Other diagnostic methods shoul. be based on the possibil.ties of treatment al.hough a histopathol.gical.anal.sis of the metastasic mass is needed to confirm the diagnosis. Magnetic resonance imaging (MRI) coul. be useful.to compl.te a morphol.gical.and functional.eval.ation in case of surgical.removal.

Introduction: Presacral.tumours are extremel. rare entities as the asymptomatic retrorectal.mass, al.hough its cl.nical.presentation incl.des infectious compl.cations and signs of mal.gnant degeneration. Magnetic resonance imaging is the most efficient imaging study for its diagnosis. The treatment of choice is compl.te surgical.excision and traditional.approaches are mainl. through abdominal. posterior and perineal.approach, depending on anatomical.characteristics of the l.sion. l.paroscopic excision of these retrorectal.l.sions has been reported in a few cases. Patients and methods: We report two cases of 38 and 24-year-ol. women who compl.ined of anorectal.symptoms and were diagnosed as retrorectal.tumour by imaging studies. One of them was infected. Both cases were resected by means of l.paroscopic techniques. There was no surgical.compl.cation and they were discharged on the 3rd and 4th postoperative day, respectivel.. Histopathol.gic findings reveal.d benign cystic teratoma in both cases. A fol.ow-up after 36 months showed no recurrence. Our surgical.endoscopic technique and a brief review of perioperative cares are presented and discussed. Discussion: l.paroscopic excision coul. be indicated in sel.cted retrorectal.tumours and a great surgeon’s resol.e is al.ays required. A meticul.us dissection must be performed in order to identify and preserve vital.structures. It’s onl. absol.te contraindications seemed to be the suspicion of mal.gnancy and operative inexperience. Concl.sions: The compl.te l.paroscopic removal.of presacral.l.sions is a responsibl. surgical.procedure which offers advantages from the sanitary and aesthetic point of view. This new endoscopic indication coul. be considered probabl. as a better technique than open approach in sel.cted patients.

Fractional.initial.val.e probl.ms(IVPs) and time-fractional.initial.boundary val.e probl.ms(IBVPs), each with a Caputo temporal.derivative of order α ∈(0, 1), are considered. An averaged variant of the wel.-known l. scheme is proved to be O(N^(-2)) convergent for IVPs on suitabl. graded meshes with N points, thereby improving the O(N^(-(2-α))) convergence rate of the standard l. scheme. The anal.sis rel.es on a del.cate decomposition of the temporal.truncation error that yiel.s a sharp dependence of the order of convergence on the degree of mesh grading used. This averaged l. scheme can be combined with a finite difference or piecewise l.near finite el.ment discretization in space for IBVPs, and under a restriction on the temporal.mesh width, one gets again O(N^(-2)) convergence in time, together with O(h^(2)) convergence in space,where h is the spatial.mesh width. Numerical.experiments support our resul.s.

Purpose:To examine the effects of a school.based karate intervention on academic achievement,psychosocial.functioning,and physical.fitness in chil.ren aged 7-8 ***:Twenty school. in 5 different European countries(2 second-grade cl.ssrooms per school.participated in a cl.ster randomized control.ed trial.Sport at School.trial..Participants were assigned to either a control.group,which continued with their habitual.physical.education l.ssons,or to an intervention group,which repl.ced these l.ssons with a 1-year karate intervention(Karate Mind and Movement program).A total.of 721 chil.ren(344 girl. and 377 boys,7.4±0.5 years ol.,mean±SD)compl.ted the study,of which 333 and 388 were assigned to the control.group and intervention group,*** incl.ded academic performance(average grade),psychosocial.functioning(Strengths and Difficul.ies Questionnaire for parents),and different markers of physical.fitness(cardiorespiratory fitness,bal.nce,and fl.xibil.ty).Resul.s:The intervention provided smal. but significant benefits compared to the control.group for academic achievement(d=0.16;p=0.003),conduct probl.ms(d=-0.28;p=0.003),cardiorespiratory fitness(d=0.36;pl.nce(d=0.24;p=0.015).There was a trend towards significant benefits for fl.xibil.ty(d=0.24;p=0.056).No significant benefits were observed for other variabl.s,incl.ding psychosocial.difficul.ies,emotional.symptoms,hyperactivity/inattention,peer probl.ms,or prosocial.behaviour(al. p>0.05).Concl.sion:A 1-year school.based karate intervention was effective in improving academic achievement,conduct probl.ms,and physical.fitness in primary school.*** resul.s support the incl.sion of karate during physical.education l.ssons.
