Darier disease (DD), also known as follicular keratosis, is a rare autosomal dominant hereditary acantholytic dermatosis caused by a mutation in the ATP2A2 gene. Skin lesions are characterized by persistent confluent papules, preferentially located on the seborrheic areas of the face, chest and back. The hands, feet and nails are also commonly affected. DD has a chronic course with frequent relapses. Their treatment is often difficult and unsatisfactory. We present the case of a 54-year-old male with a history of keratotic papules with a brownish-black color, with a foul-smelling, pruritic and ardent appearance on the back and trunk, since the age of 19, which worsen in the summer. His 27-year-old daughter has a similar picture with her skin lesions started at age of 17 in the region below the breast, back, trunk, feet and hands. After the skin biopsy, the diagnosis was confirmed and acitretin 30 mg/day was instituted. The importance of this case report is to alert health professionals that the diagnosis of DD should be considered in family members of patients, especially those in the first degree, and also in patients with follicular keratolytic dermatoses, being an important differential diagnosis of this group of patients diseases.

Sorbents for CO2 capture have been prepared by wet impregnation of a commercial active carbon （Ketjen-black, Akzo Nobel） with two CO2-philic compounds, polyethylenimine （PEI） and tetraethylenepentamine （TEPA）, respectively. The effects of amine amount （from 10 to 70 wt.%）, CO2 concentration in the feed, sorption temperature and gas hourly space velocity on the CO2 capture performance have been investigated. The sorption capacity has been evaluated using the breakthrough method, with a fixed bed reactor equipped with on line gas chromatograph. The samples have been characterized by N2 adsorption-desorption, scanning electron microscopy and energy dispersive X-ray （SEM/EDX）. A promising CO2 sorption capacity of 6.90 mmo1/gsorbent has been obtained with 70 wt.% of supported TEPA at 70℃ under a stream containing 80 vol% of CO2. Sorption tests, carried out with simulated biogas compositions （CH4/CO2 mixtures）, have revealed an appreciable CO2 separation selectivity; stable performance was maintained for 20 adsorption-desorption cycles.

Brazil has a wide variety of fruits that remain insufficiently *** encourage the use of one such fruit,the bacupari((Garcinia brasiliensis(Mart.)),the current study produced flours from its by-products(the peel and seed)at two maturation stages and subsequently evaluated their proximate composition,physical,antioxidant,bioactive,microstructural and thermal *** results revealed that the flours have a high in fiber content(>15%)and are a potential source of protein(>5%).Seed flour has an energy value similar to that of wheat flour and other conventional *** flour micrographs indicate the presence of starch,a fact confirmed by viscosity *** flours contain phenolic compounds and exhibit antioxidant activity,especially the unripe peel *** the compounds identified by chromatography,those present in the greatest concentrations were quinine(≅111μg/g),p-coumaric acid(≅25μg/g),m-coumaric acid(≅15μg/g),and formononetin(≅21μg/g),all of which are known to have several human health *** flour crystallinity diminished as the fruit *** flour exhibited similar thermal transitions but less thermal stability than seed *** summary,this study revealed that flours produced from the peel and seeds of bacupari,which are generated in large quantities as agro-industrial by-products,can be used as alternative ingredients in various sectors of the food industry.

The B1 dam of Co rrego do Feij～ao Mine, owned by Vale, S.A. mining company and located on the FerroCarv～ao stream, collapsed and injected 2.8 Mm3of clayey, silty, and sandy iron-and manganese-rich tailings into the Paraopeba River(Minas Gerais state, Brazil). The accident occurred on 25 January2019 and the tailings have been co-transported with coarser natural sediment since then, being partly trapped in the Igarape Weir reservoir located on the Paraopeba River nearly 45 km downstream the injection point. The general purpose of the current study was to model suspended sediment transport in the vicinity of the Igarape Weir aiming to assess the concomitant barrier effect imposed by this ***, the spatial distributions of suspended clay, silt, and very fine-grained sand fractions(CSS) of sediment were mapped around the Igarape Weir under low-flow(16 m3/s) and high-flow(5 to 10,000years return period stream discharge; 699—2,699 m3/s) regimes, using RiverFlow2D as the modelling tool. The concentrations of the various grain materials in the upstream and downstream sectors were quantified linking the barrier effect to concentration reductions in the direction of stream flow. It was also a study goal to calculate differences of iron and manganese concentrations in the sediment + tailings mixtures along the Paraopeba River. The study results showed reductions in the CSS between 6.6% and18%, from upstream to downstream of the Igarape Weir, related with backwater effects, free and submerged hydraulic jumps, bank sedimentation in periods of high flow, and streambed sedimentation controlled by channel sinuosity and tailings density. These reductions were accompanied by drops in the concentrations of iron and manganese present in the clay and silt fractions, which varied between 6% and42% under low flows and between 16% and 44% under high flows. Background:Children with congenital heart disease(CHD),even after surgical approaches,and especially those who undergo staged procedures in theﬁrst months of life,remain vulnerable to readmissions and complications,requiring very close monitoring and differentiated intervention ***:descriptive and exploratory study,of the experience report type,which presents the process of building the high-risk outpatient clinic for complex congenital heart diseases(AAR)at the Instituto do Coração(InCor).Results:Report of the path taken to structure the AAR,demonstrating the organization,interface with the multidisciplinary team,admission and discharge criteria,training,and patient proﬁ*** theseﬁve years of care,275 patients were treated,59.65%with biventricular interstage physiology,followed by univentricular interstage physiology(34.55%),residual defects after surgical procedures(3.63%),tumors with risk of mechanical obstruction(1.45%)and patients with an intrauterine approach(0.72%).The signiﬁcant number of critical patients who were successfully discharged from hospital(44.72%),the low mortality rate due to sudden deaths at home and the high adherence to follow-up cor-roborate the impact of this specialized ***:despite the limitations of the study,this experi-ence report showed that with few resources,there is the possibility of organizing an AAR with differentiated care,with the objective of early detection and treatment of residual injuries,identiﬁcation of early interventions,edu-cation of parents for follow-up of their children,resulting in individualized treatment,promoting a better quality of life for this population.

depending on the magnitude and nature of a disaster,identifying the victims can be a complex task that requires coordinated work by disaster victim identification(DVI)teams based on pre-established ***,the analysis of fingerprints has been presented as a method to establish,when possible,the identity of the victims during the DVI *** study discusses the importance of this primary method of identification and the results obtained in four different disasters in which Brazilian DVI teams were involved:the Air France Flight AF447 plane crash in the Atlantic Ocean,floods and mudslides in the State of Rio de Janeiro,Brazil,the laMia Flight 2933 plane crash in Colombia,and the tailings dam collapse in Brumadinho,***,we also report the use of the automatic fingerprint capture and identification system,called Alethia,developed by the Federal Police of Brazil and used in the victim identification process in the two latter events mentioned above.

AIM: To establish whether virulence factor genes vacA and cagA are present in Helicobacter pylori (H. pylori) retrieved from gastric mucosa and dental plaque in pa-tients with dyspepsia. METHODS: Cumulative dental plaque specimens and gastric biopsies were submitted to histological exami-nation, rapid urease test and polymerase chain reac-tion (PCR) assays to detect the presence of cagA and vacA ***: detection of H. pylori from dental plaque and gastric biopsy samples was greater by PCR com-pared to histological examination and the rapid ure-ase test. DNA from H. pylori was detected in 96% of gastric mucosa samples and in 72% of dental plaque samples. Sixty-three (89%) of 71 dental plaque sam-ples that were H. pylori-positive also exhibited identical vacA and cagA genotypes in gastric mucosa. The most common genotype was vacAs1bm1 and cagA positive, either in dental plaque or gastric mucosa. These viru-lent H. pylori isolates were involved in the severity of clinical ***: These pathogenic strains were found simultaneously in dental plaque and gastric mucosa, which suggests that gastric infection is correlated with the presence of H. pylori in the mouth.

AIM： To study the association between Interleukin-1 （Il-1） and tumor necrosis factor （TNF）-α polymorphisms, infection by Helicobacter pylori （H pylori） and the development of gastrointestinal diseases.METHODS： Genomic DNA was extracted from the peripheral blood of 177 patients with various gastrointestinal diseases and from 100 healthy volunteers. The polymorphisms in Il-1β and TNF-α genes were analyzed using the polymerase chain reactionrestriction fragment length polymorphism method （PCRRFlP） and those from Il-1RN with PCR. The presence of infection due to H pylori and the presence of the CagA toxin were detected by serology. The histopathological parameters in the gastric biopsies of the patients were according to the Sydney classification.RESUlTS： A comparison of the frequencies of the different polymorphisms studied among the patients and the control group demonstrated that the allele Il- 1RN＊2 was more frequent among patients with gastric ulcers and adenocarcinoma. Carriers of the allele Il- RN＊2 and those with reactive serology for anti-CagA IgG had a greater risk of developing peptic ulcer and gastric adenocarcinoma, as well as a higher degree of inflammation and neutrophilic activity in the gastricCONClUSION： Our results indicate a positive association between Il-1RN gene polymorphism and infection by positive H pylori CagA strains and the development of gastric ulcers and adenocarcinoma.

Topotecan Use for Second-line Treatment in Patients with Recurrent or Metastatic Cervical Cancer at Brazilian National Cancer Institute (INCA)

Objective: Cervical cancer represents the third most commonly diagnosed cancer and is an important cause of death for women suffering with malignancies. Patients who are refractory or progressed after first-line palliative treatment have a dismal prognosis and no second-line chemotherapy is considered standard so far. Several agents have been investigated in this setting and topotecan is one of the most characterized. The objective of this study was to evaluate response rate (RR), progression-free survival (PFS), overall survival (OS) and toxicity of topotecan in second palliative line for cervical cancer. Methods: An analysis was performed of all patients with recurrent or metastatic cervical cancer treated with topotecan in second palliative line at Brazilian National Cancer Institute, between 2008 and 2010. Results: A total of 73 courses of topotecan were given in the current study (median: 3.5 cycles;range 1 - 6). Anemia was the most frequent adverse event (grade 2:35%;grade 3:30%). Of the 20 patients evaluable, there were 2 partial responders to the treatment. The overall response rate (ORR) was 10%;3 patients (15%) had stable disease as maximum response. The median PFS for the entire group was 2.93 months (95% CI 2.41 - 3.45) and OS was 4.66 months (95% CI 1.21 - 8.11). Conclusion: The limited activity of topotecan schemas in second-line treatment of cervical cancer and the associated overall toxicity may not justify their use in this setting. Patients who progress after first-line treatment may be offered participation in clinical trials, other second-line agents or best supportive care measures.
