As the consumption of fructose and saturated fatty acids(FAs) has greatly increased in western diets and is linked with an increased risk of metabolic syndrome,the aim of this study was to investigate the effects of a mod-erate(10 weeks) and a prolonged(30 weeks) high fructose and saturated fatty acid(HFS) diet on plasma FA com-position in *** effects of a few weeks of HFS diet had already been described,but in this paper we tried to es-tablish whether these effects persist or if they are modified after 10 or 30 *** hypothesized that the plasma FA profile would be altered between 10 and 30 weeks of the HFS *** fed with either the HFS or a standard diet were tested after 10 weeks and again after 30 *** 10 weeks of feeding,HFS-fed rats developed the metabolic syndrome,as manifested by an increase in fasting insulinemia,total cholesterol and triglyceride levels,as well as by impaired glucose ***,the plasma FA profile of the HFS group showed higher proportions of monounsaturated FAs like palmitoleic acid [16:1(n-7)] and oleic acid [18:1(n-9)],whereas the proportions of some polyunsaturated n-6 FAs,such as linoleic acid [18:2(n-6)] and arachidonic acid [20:4(n-6)],were lower than those in the control *** 30 weeks of the HFS diet,we observed changes mainly in the levels of 16:1(n-7)(decreased) and 20:4(n-6)(increased).Together,our results suggest that an HFS diet could lead to an adaptive response of the plasma FA profile over time,in association with the development of the metabolic syndrome.

Prise en Charge des Pleuresies Infectieuses de l’Enfant Dans Trois Hopitaux du Sud du Benin Management of Infectious Pleurisies in Three Hospitals in the South of Benin

Objective: Infectious pleurisy is a frequent hospitalization indication in pediatrics in developing countries. This study aims to describe infectious pleurisies’ features and to identify its death factors in three teaching hospitals in Benin. Design: This was a prospective, descriptive and analytical study including children aged 01 months to 17 years who were hospitalized in pediatrics in the three hospitals for pleurisy from September to December 2019. Results: Among the 3379 children admitted, 25 presented with an infectious pleurisy, making a hospital frequency of 0.74%. The sex ratio was 0.8. The majority (19/25) of the children were less than 5 years old. The mean age was 38 ± 5.88 months. Most of the parents had a low education (42/50) and socio-economic status (18/25). The main symptoms were fever (25 cases), dyspnea (23 cases) and cough (22 cases). The majority of the children (21/25) were up-to-date in regards with the Expanded Immunization Program (EIP) vaccines and none had received non-EIP vaccines. Almost all children (24 cases) had a respiratory distress (24/25). On chest X-ray, there were abundant pleural extravasations in 12 cases. The main pathogens found were Staphylococcus aureus (16 cases), Streptococcus pneumoniae (3 cases) and Streptococcus A (1 case). All children received oxygen and antibiotic therapy;pleural drainage was performed in 22 children. The average length of stay was 14 days ± 6.4. Twenty-one children were healed without sequelae, one child had a post-drainage keloid scar, and two children died. Factors associated with the death of these children were admission delay for more than 7 days (p = 0.035) and presence of respiratory distress (p = 0.049). Conclusion: Pleurisy remains a concern for children admitted in our hospitals and early management is imperative.

Oeis Syndrome (Cloacal Exstrophy): About Two Cases Treated at the Mother and Child Teaching Hospital in Cotonou (Benin)

Introduction: OEIS syndrome, the most severe form of exstrophy-epispadias complex, is characterized by the existence of an omphalocele, a bladder exstrophy, an anal imperforation and a spina bifida. Case Presentation: Two clinical cases admitted at the first day of life in neonatology department are reported;one was born by caesarean section, weighing 2500 g and the other one vaginally with a weight at 1890 g. The first child’s mother was a primigravid, primiparous and the second mother was multiparous. Prenatal ultrasound was performed in one case. The clinical examination found two exstrophied hemi-bladders with two productive ureteral meatus, separated by a double intestinal orifice, a manifestation of anal imperforation, omphalocele, spina bifida and lumbar vertebral malformations;which corresponds to a typical form of OEIS syndrome. One of the babies had an associated clubfoot. The karyotype could not be performed. Both were dead;the first in the early postoperative period, the second one at 16 days of life without surgery. Conclusion: OEIS syndrome is a rare condition and represents a therapeutic challenge in developing countries. Promoting prenatal diagnosis is essential.
