Usefulness of duodenal biopsy during routine upper gastrointestinal endoscopy for diagnosis of celiac disease

AIM： To describe the trend in duodenal biopsy performance during routine upper gastrointestinal endoscopy in an adult Spanish population, and to analyze its value for the diagnosis of celiac disease in clinical practice. METHODS： A 15 year-trend （1990 to 2004） in duodenal biopsy performed when undertaking upper gastrointestinal endoscopy was studied. We analysed the prevalence of celiac disease in the overall group, and in the subgroups with anaemia and/or chronic diarrhoea. RESUlTS： Duodenal biopsy was performed in 1033 of 13 678 upper gastrointestinal endoscopies （7.6%）; an increase in the use of such was observed over the study period （1.9% in 1990-1994, 5% in 1995-1999 and 12.8% in 2000-2004）. Celiac disease was diagnosed in 22 patients （2.2%）, this being more frequent in women than in men （3% and 1% respectively）. Fourteen out of 514 （2.7%） patients with anaemia, 12 out of 141 （8.5%） with chronic diarrhoea and 8 out of 42 （19%） with anaemia plus chronic diarrhoea had celiac disease. A classical clinical presentation was observed in 55% of the cases, 23% of the patients had associated dermatitis herpetiformis and 64% presented anaemia; 9% were diagnosed by familial screening and 5% by cryptogenetic hypertransaminasaemia. CONClUSION： Duodenal biopsy undertaken during routine upper gastrointestinal endoscopy in adults, has been gradually incorporated into clinical practice, and is a useful tool for the diagnosis of celiac disease in high risk groups such as those with anaemia and/or chronic diarrhoea.

AIM: To evaluate the factors involved in the impairment of health-related quality of life (HRQOl) in patients with celiac ***: A multicenter, cross-sectional prospective study was performed in patients with celiac disease who completed two HRQOl questionnaires: the gastrointestinal quality of life index (GIQlI) and the EuroQol-5D (EQ).RESUlTS: Three hundred and forty patients (163 controlled with a gluten-free diet, and 177 newly diagnosed with a normal diet) were included. The GIQlI score was significantly better in patients on a gluten-free diet (GFD) than in non-treated patients on their usual diet, both in terms of the overall score (3.3 vs 2.7, respectively; P ll as on the individual questionnaire dimensions. Both the preference value of the EQ as the visual analogue scale were significantly better in treated than in non-treated patients (0.93 vs 0.72 P ly). Variables significantly associated with a worse HRQOl score were female gender, failure to adhere to a GFD, and symptomatic ***: In untreated celiac disease, the most important factors that influence patient perception of health are the presence of symptoms and a normal diet. HRQOl improves to levels similar to those described in the general population in celiac disease patients well controlled with a GFD.

AIM: To evaluate the tissular expression of Androgen (A), Estrogen (E) and Progesterone (Pg) receptors, and Apolipoprotein D (ApoD), in liver tumors from resected hepatocellular carcinoma (HCC) cases in order to assess their possible relationship to prognosis. METHODS: We performed an immunohistochemical study using tissue microarrays (containing more than 260 cancer specimens, from 31 HCC patients and controls) to determine the presence of specif ic antibodies against AR, ER, PgR and ApoD, correlating their findings with several clinico-pathological and biological variables. The staining results were categorized using a semi-quantitive score based on their intensity, and the percentage of immunostained cells was measured. RESUlTS: A total of 21 liver tumors (67.7%) were positive for AR; 16 (51.6%) for ER; 26 (83.9%) for PgR and 12 (38.7%) stained for ApoD. We have found a wide variability in the immunostaining score values for each protein, with a median (range) of 11.5 (11.5-229.5) for AR; 11.1 (8.5-65) for ER; 14.2 (4-61) for PgR; and 37.7 (13.8-81.1) for ApoD. A history of heavy ethanol consumption, correlated positively with AR and PgR and negatively with ER status. HCV chronic infection also correlated positively with AR and PgR status. However, the presence of ApoD immunostaining did not correlate with any of these variables. Tumors with a positive immuno-staining for PgR showed a better prognosis. CONClUSION: Our results indicate a moderate clinical value of the steroid receptor status in HCC, emphasizing the need to perform further studies in order to evaluate the possible role of new hormonal-based therapies.

Metabolic complications in liver transplant recipients

The metabolic syndrome(MS), which includes obesity,dyslipidaemia, hypertension and hyperglycaemia according to the most widely accepted definitions now used, is one of the most common post-transplant complications, with a prevalence of 44%-58%. The MS, together with the immunosuppression, is considered the main risk factor for the development of cardiovascular disease(CVD) in transplant recipients, which in turn accounts for 19%-42% of all deaths unrelated to the graft. The presence of MS represents a relative risk for the development of CVD and death of 1.78. On the other hand, non-alcoholic fatty liver disease(NAFlD), considered as the manifestation of the MS in the liver, is now the second leading reason for liver transplantation in the United States after hepatitis C and alcohol. NAFlD has a high rate of recurrence in the liver graft and a direct relation with the worsening of other metabolic disorders, such as insulin resistance or diabetes mellitus. Consequently, it is vitally important to identify and treat as soon as possible such modifiable factors as hypertension, overweight, hyperlipidaemia or diabetes in transplanted patients to thus minimise the impact on patient survival. Additionally, steroid-free regimens are favoured, with minimal immunosuppression to limit the possible effects on the development of the MS.

Ergonovine stress echocardiography: Recent experience and safety in our centre

AIM:To study recent experience and safety of ergonovine stress echocardiography in our ***:In this study we collected the clinical variables of patients referred since 2002 for ergonovine stress echocardiography,in addition to indications,the results of this test,complications,blood pressure and heart rate values during the test and the number and results of tests requested before this ***:We performed 40 tests in 38 patients,2 tests were carried out to verify therapy *** prevalence of classic cardiovascular risk factors was low and the most frequent indication was chest pain (57.5%).Coronary angiography was performed in 32 patients,and showed normal coronary arteries in 27 patients and non-significant stenosis in 5 *** 16 patients,coronary angiography was carried out after a positive or inconclusive ischemia *** 6 patients had a normal stress test(5 exercise electrocardiography tests and 1 nuclear imaging test).Of the 40 ergonovine stress echocardiography tests,6 were positive(4 in the right coronary artery territory and 2 in the circumflex coronary artery territory),all of them by echocardiographic criteria,and by electrocardiographic criteria in only 3(50%).The presence of non-significant coronary artery stenosis was more frequent in patients with positive ergonovine stress echocardiography(50%vs 6%,P =0.038),and were related to ischemic *** the maximum stress stage,there was a higher systolic(130.26±19.17 mmHg vs 136.58±27.27 mmHg,95%CI:-12.77 to 0.14 mmHg,P=0.055)and diastolic blood pressure(77.89±13.49 mmHg vs 83.95± 15.73 mmHg,95%CI:-10.41 to-1.69 mmHg,P=0.008) than at the baseline stage,and the same was registered with heart rate(73±10.96 beats/minvs 79.79± 11.72 beats/min,95%CI:-9.46 to-4.11 beats/min,P less,there were only 2 hypertensive reactions during the last stage,which did not force a premature end to the test,without sustained tachy or bradyarrhythmias,and the technique was

Post-transplant plasma cell hepatitis in a liver transplant patient treated with pegylated interferon plus ribavirin

The recurrence of hepatitis C after a liver transplant remains an important cause of graft loss and retransplant. Antiviral therapy with peginterferon plus ribavirin (PEG-INF/RBV) can achieve a sustained viral response and histological improvement in a high percentage of cases. However, this treatment is not exempt from important side effects or from the possibility of precipitating rejection, with the resulting graft loss. We report the case of a liver transplant patient who received treatment with PEG-INF/RBV and developed plasma cell hepatitis as the presenting form of rejection.

Hummingbird-plant interactions in Chile:An ecological review of the available evidence

Hummingbird species have closely evolved with the plants they feed on,which is confirmed by their often tight ecological relationships in natural ***-plant interactions are of interest for research areas such as ecology,evolution,and even agriculture,as they usually inform on the conservation status of interacting species assemblages,and its disappearance may precede the population extinction of the species involved in the *** Chile,there are nine hummingbird species,which interact with a large and diverse number of angiosperm species in forest,desert,and mountain range *** motivation to perform this review is to systematize the available information regarding the flowering plants visited by hummingbird species in Chile,to present some basic plant-hummingbird pollination network metrics,and on this basis to identify the components of the plant-hummingbird relationships in need of further research.A plant-hummingbird metanetwork revealed a low connectance value,low niche overlap,and strong modularity among ***,the fact that most species present a strong allopatric distribution across Chile,suggests that network structure results mostly from the history of colonization rather than from ecological ***,the main threats to Chilean hummingbirds are anthropogenic disturbance and climate change,which disrupt hummingbird-plant interactions,leading to unpredictable ecological consequences at the community *** periods of drought may reduce the resource base for hummingbirds,with dramatic consequences for the maintenance of bird and plant populations.

Experimental hybridization and reproductive isolation between two sympatric species of tephritid fruit flies in the Anastrepha fraterculus species group

Among tephritid fruit flies,hybridization has been found to produce local adaptation and speciation,and in the case of pest species,induce behavioral and ecological alterations that can adversely impact efficient pest *** species group within Anastrepha (Diptera:Tephritidae),is a rapidly radiating aggregate,which includes cryptic species complexes,numerous sister species,and several pest species. Molecular studies have highlighted the possibility of introgression between *** and *** isolation has been studied among morphotypes of the A. fraterculus species complex as a tool for species *** we examined the exis- tence and strength of prezygotic and postzygotic isolation between sympatric populations of two closely related species within the highly derivedfraterculus group (*** and ***),coexisting in *** adults of both species showed a strong tendency for assortative mating,a small proportion of hybrid pairings in both directions were *** also observed asymmetric postzygotic isolation,with one hybrid cross displaying a strong reduction in fecundity and F1 egg *** was greater for the progeny of homotypic and hybrid crosses in the maternal *** was a marked female biased sex ratio distortion for both F1 hybrid *** between *** and *** in nature may be difficult but possible;these two species display stronger reproductive isolation than all pairs of species previously examined in the *** species *** postzygotie isolation is suggestive of Wolbachia mediated cytoplasmic incompatibilities that may be exploited in area-wide pest management.
