Retrospective Cohort Study to Investigate Pregnancy Outcomes in a Population of Advanced Maternal Age Congolese Women of Kinshasa: A Study Protocol

Introduction: Pregnancies at advanced maternal age (AMA) are those occurring after the age of 35 years old. they carry a high risk of maternal-fetal morbidity and mortality, thus constituting a public health problem. Several African countries have reported an upward trend in both the age of childbirth and the frequency of women with AMA over the past 20 years. In the Democratic Republic of Congo (DRC), where maternal and neonatal morbidity and mortality remain very high, data on AMA pregnancies go back more than 20 years. Objective: We propose evaluating obstetrical outcomes among women in AMA in our setting and the associated factors. Methods: this retrospective cohort study will be conducted in two healthcare facilities (ESS) in Kinshasa. the study population will consist of all women who delivered a single fetus after 28 weeks of gestation between January 2012 and December 2022 (10 years) in the selected ESS. the data collected will be analyzed using R software version 4.2.0. Quantitative variables will be summarized as means with standard deviation or medians with interquartile range. Qualitative variables will be presented as proportions (%). Multivariate logistic regression will be used to determine the main maternal-fetal complications associated with AMA and predictors of obstetric outcomes. P Discussion: the high maternal and infant mortality rates in DRC are among the highest in the world. the context of maternal age has become a topic of growing interest due to its potential implications for the health of women and newborns, it is crucial to identify the risk factors associated with obstetric outcomes by identifying obstetrical outcomes associated with advanced maternal age in the DRC. Cervical meningoceles are the least common forms of spina bifida, representing between 4% to 8% of all spinal dysraphisms. they are not accompanied by neurological deficit in childhood but attached cord syndrome can occur with growth. Surgical treatment, in addition to avoiding rupture and correcting the aesthetic damage, will preserve neurological functions. We report 2 cases that we took care of. they are a 5-month-old infant and a 3-year-old girl. the first presented a 7 cm rounded meningocele pedunculated through the C1 lamina defect. the second presented an 8.6 cm polylobed meningocele pedunculated through the C2 lamina defect. they both underwent surgery with good progress.

Using mixed integer programming and airborne laser scanning to generate forest management units

Airborne laser scanning(AlS)has been widely applied to estimate tree and forest attributes,but it can also drive the segmentation of forest *** algorithms are the dominant technique in segmentation but spatial optimization using exact methods remains *** study presents a novel approach to segmentation based on mixed integer programming to create forest management units(FMUs).this investigation focuses on using raster information derived from AlS *** mainstream clustering algorithms were compared to the new MIP formula that simultaneously accounts for area and adjacency restrictions,FMUs size and homogeneity in terms of vegetation *** optimal problem solution was found when using less than 150 cells,showing the problem formulation is *** results for MIP were better than for the clustering algorithms;FMUs were more compact based on the intravariation of canopy height and the variability in size was *** MIP model allows the user to strictly control the size of FMUs,which is not possible in heuristic optimization and in the clustering algorithms *** definition of forest management units based on remote sensing data is an important operation and our study pioneers the use of MIP AlS-based optimal segmentation.

leveraging the high resolution,sensitivity,and wide frequency coverage of the Atacama large Millimeter/submillimeter Array(AlMA),the QUARKS survey,standing for“Querying Underlying mechanisms of massive star formation with AlMA-Resolved gas Kinematics and Structures”,is observing 139 massive starforming clumps at AlMA Band 6(λ~1.3 mm).this paper introduces the Atacama Compact Array(ACA)7 m data of the QUARKS survey,describing the ACA observations and data *** multiwavelength data,we provide the first edition of QUARKS atlas,offering insights into the multiscale and multiphase interstellar medium in high-mass star *** ACA 1.3 mm catalog includes 207 continuum sources that are called ACA *** gas kinetic temperatures are estimated using three formaldehyde transitions with a non-ltE radiation transfer model,and the mass and density are derived from a dust emission *** ACA sources are massive(16–84 percentile values of 6–160 M_(⊙)),gravity-dominated(M∝R^(1.1))fragments within massive clumps,with supersonic turbulence(M>1)and embedded star-forming *** find a linear correlation between the masses of the fragments and the massive clumps,with a ratio of 6%between the *** considering fragments as representative of dense gas,the ratio indicates a dense gas fraction(DGF)of 6%,although with a wide scatter ranging from 1%to 10%.If we consider the QUARKS massive clumps to be what is observed at various scales,then the size-independent DGF indicates a self-similar fragmentation or collapsing mode in protocluster *** the ACA data over four orders of magnitude of luminosity-to-mass ratio(l/M),we find that the DGF increases significantly with l/M,which indicates clump evolutionary *** observed a limited fragmentation at the subclump scale,which can be explained by a dynamic global collapse process.

Introduction: Microbiology of effusion fluids in children in Burkina Faso is characterized by the scarcity of data. this work aimed to study the bacteriological and antibiotics susceptibility profile of bacteria involved in effusion fluid infections in paediatrics in order to improve the choice of probabilistic antibiotics therapy. Methods: A cross-sectional, descriptive study was used in children aged 0 to 15 years from 2017 to 2020 at the Charles De Gaulle Pediatric University Hospital Center (CHUP-CDG) in Ouagadougou. Classical bacteriology methods such as macroscopy, Gram staining, identification galleries and antibiotics susceptibility testing were used. Results: Of 231 samples, 64 bacteria were isolated. the most common bacterial strains of pleural fluid were Staphylococcus aureus (25%) and 40% for Enterobacteriaceae. Of the peritoneal fluid, 77% were Enterobacteriaceae with 57% Escherichia coli;and from joint fluid, 33% were S. aureus and 22% for P. aeruginosa. the overall susceptibility profile showed 29% extended-spectrum beta-lactamase-producing Enterobacteriaceae (ESBl), 10% methicillin-resistant S. aureus (MRSA), and 8% carbapenemases. Conclusion: Bacteriological profile is characterized by ESBl-producing Enterobacteriaceae and MRSA. the most active antibiotics were macrolides, aminoglycosides, and cefoxitin (methicillin) for Gram-positive cocci, carbapenems, and aminoglycosides for Gram-negative bacilli. then, the monitoring of antibiotics resistance must be permanent.

Determination of Surface Precipitation type Based on the Data Fusion Approach

Hazardous events related to atmospheric precipitation depend not only on the intensity of surface precipitation,but also on its *** related to determination of the precipitation type(Pt)leads to financial losses in many areas of human activity,such as the power industry,agriculture,transportation,and many *** this study,we use machine learning(Ml)algorithms with the data fusion approach to more accurately determine surface *** on surface synoptic observations,ERA5 reanalysis,and radar data,we distinguish between liquid,mixed,and solid precipitation *** study domain considers the entire area of Poland and a period from 2015 to *** purpose of this work is to address the question:“How can Ml techniques applied in observational and NWP data help to improve the recognition of the surface Pt?”Despite testing 33 parameters,it was found that a combination of the near-surface air temperature and the depth of the warm layer in the 0-1000 m above ground level(AGl)layer contains most of the signal needed to determine surface *** accrued probability of detection for liquid,solid,and mixed Pts according to the developed Random Forest model is 98.0%,98.8%,and 67.3%,*** application of the Ml technique and data fusion approach allows to significantly improve the robustness of Pt prediction compared to commonly used baseline models and provides promising results for operational forecasters.

Introduction: Seasonal Malaria Chemoprevention (SMC) aims at preventing malaria in children during the high transmission season. It has been recommended by the WHO since 2013 for children from the age of 3-59 months. However, despite the impact of this intervention, a peak in the prevalence and incidence of malaria is observed in children from the age of 5-9 years. the aim of this study is to determine from the current literature the feasibility, impact and cost-effectiveness of extending SMC to five cycles and to older children. Methods: A litterature search of PubMed/Medline, NCBI and Google scholar identified 1333 articles. After reading the titles and abstracts by two authors, 24 articles were selected and submitted for full reading. Random control studies on the extension of SMC, malaria, feasibility of SMC, impact of SMC and cost-effectiveness of SMC were selected. A total of 16 articles were included for the qualitative synthesis after excluding 8 studies. Results: Following the summary of the evidence, we conclude that the extension is feasible but will be confronted with the unavailability of older children. the intervention period coincides with field work. SMC is effective in reducing the prevalence and incidence of malaria and the parasite density in children. the financial cost of administering SMC is lower than that of treating a child suffering from malaria. Conclusion: After analysing the information, it was found that the majority of the African population supports the extension of the SMC to the number of cycles and the age group in order to alleviate the high mortality and morbidity rates among children due to malaria.
