Biliary strictures are characterized by the narrowing of the bile duct lumen,usually caused by surgical biliary injury,cancer,inflammation,and scarring from *** stent placement is a well-established method for the management of biliary ***,maintaining optimal mechanical properties of stents and designing surfaces that can prevent stent-induced tissue hyperplasia and biofilm formation are challenges in the fabrication of biodegradable biliary stents(BBSs)for customized *** study proposes a novel approach to fabricating functionalized polymer BBSs with nanoengineered surfaces using 3D *** 3D printed stents,fabricated from bioactive silica poly(ε-carprolactone)(PCL)via a sol-gel method,exhibited tunable mechanical properties suitable for supporting the bile duct while ensuring ***,a nanoengineered surface layer was successfully created on a sirolimus(SRL)-coated functionalized PCL(fPCL)stent using Zn ion sputtering-based plasma immersion ion implantation(S-PIII)treatment to enhance the performance of the *** nanoengineered surface of the SRL-coated fPCL stent effectively reduced bacterial responses and remarkably inhibited fibroblast proliferation and initial burst release of SRL in vitro *** physicochemical properties and biological behaviors,including in vitro biocompatibility and in vivo therapeutic efficacy in the rabbit bile duct,of the Zn-SRL@fPCL stent demonstrated its potential as a versatile platform for clinical applications in bile duct tissue engineering.

Dear Editor,We report our experience using a continuous intraoperative optical coherence tomography(OCT)-enabled microscope system for real-time anatomical evaluation of the optic pit during surgery in an 8-year-old girl who presented with symptomatic optic disc pit(ODP) maculopathy. ODP is an extremely rare congenital anomaly secondary to colobomatous malformation of the optic nerve head, with a reported incidence of 0.00009%[1]. Although patients are frequently asymptomatic, approximately 40%-50% of patients experience visual symptoms because of subretinal fluid(SRF) accumulation beneath the macula.

Purpose: We investigated the effect of menstrual cycle on the cardiovascular responses during recovery period from exercise. 10 healthy women were participated in the early follicular phase (EP: low estrogen and low progesterone: 1 - 4 days) and the late follicular phase (LP: high estrogen and low progesterone: 10 - 13 days) during menstrual cycle. Methods: All subjects completed a graded cycling exercise testing to determine the relative exercise intensity. All subjects were assessed systolic blood pressure (SBP), diastolic blood pressure (DBP), mean arterial pressure (MAP), heart rate (HR), stroke volume (SV), cardiac output (CO), and total vascular conductance (TVC) at resting and during recovery period from moderated cycling exercise during EP and LP. Results: During EP, compared with LP, resting SBP and MAP were higher and TVC was lower (p p Conclusions: This study could be increased the peripheral vasoconstriction in low estrogen levels. And during early follicular phase with low estrogen level, it may maintain an increased blood pressure during recovery period following cycling. We suggest that the timing of the menstrual cycle should be considered when measuring blood pressure during clinical exercise testing.

We investigate the localized surface plasmon(LSP) effect by Al nanorings on the AlGaN/GaN multiple quantum well(MQW) structure emitting at 365 nm. For this experiment, first, the size of Al nanorings is optimized to maximize the energy transfer(or coupling) between the LSP and MQW using the silica nanospheres. Then, the Al nanorings with an outer diameter of 385 nm, which exhibit a strong absorption peak in the near-ultraviolet region, are applied to the top surface of the AlGaN/GaN MQW. The photoluminescence(PL) intensity of the MQW structure with Al nanorings increased by 227% at 365 nm compared to that without Al *** improvement is mainly attributed to an enhanced radiative recombination rate in the MQWs through the energy-matched LSPs by the temperature-dependent PL and time-resolved PL analyses. The radiative lifetime was about two times shorter than that of the structure without Al nanorings at room temperature. In addition, the measured PL efficiency at room temperature of the structure with Al nanorings was 33%, while that of the structure without Al nanorings was 19%, implying that LSP-QW coupling together with the nanoring array pattern itself played important roles in the enhancement.

Detection and treatment of synchronous lesions in colorectal cancer: The clinical implication of perioperative colonoscopy

AIM： To evaluate the clinical significance of preand intra-operative colonoscopy for the detection of synchronous lesions in colon cancer.METHODS： Two hundred and sixty-five pre-operative and 51 intra-operative colonoscopic evaluations were performed in 316 colorectal cancer patients who underwent curative resection from January 2001 to June 2006. The incidence and characteristics of synchronous lesions and their influence on surgery were evaluated.RESULTS： Two hundred and eighty-two synchronous lesions were detected in 124 （39.2%） of 316 patients including all lesions regardless of their histologic type. True adenomatous polyps were found in 91 （28.8%） of 326 patients, and 27 （5.4% of all patients） patients had synchronous colon cancers. The preoperative identification of synchronous lesions altered the planned surgery in 37 （14.0%） of 265 patients. In 18 patients among the surgically removed cases, the lesions were removed by extending the resection range. Further segmental resection or polypectomy through enterotomy was necessary in 29 patients. Nineteen （37.2%） of 52 intraoperative colonoscopy cases had synchronous lesions. Additional surgical procedures including segmental bowel resection and polypectomy with enterotomy were necessary in 7 （23.7%） of 52 intraoperative colonoscopy cases to remove the lesions.CONCLUSION： Synchronous colorectal polyps or cancer are frequent and their preoperative detection is important for optimal surgical planning and treatment. Intraoperative colonoscopy is a useful option in cases where a preoperative colonoscopy is not feasible.

The aim of this study was to optimize a tablet formulation of dutasteride that is bioequivalent to a commercially available soft gelatin capsule(Avodart~?). The effect of cyclodextrin on enhancing the aqueous solubility of dutasteride was investigated, after which the formulation was further optimized with solubilizing polymer and surfactant. Among the cyclodextrins tested, the highest solubility was observed when dutasteride was complexed with γ-cyclodextrin. Moreover, the addition of polyvinylpyrrolidone and Gelucire/TPGS further enhanced the solubility of dutasteride. Differential scanning calorimetry(DSC) and powder X-ray diffraction(pX RD) studies demonstrated that dutasteride existed in the amorphous form in the complex. Optimized dutasteride complexes were selected after a pharmacokinetic study in rats, and film-coated tablets were prepared by the direct compression method. In vitro dissolution profiles for the tablets of dutasteride complexes were similar to those of the reference. Moreover, pharmacokinetic parameters including the C max and AUC values after oral administration in beagle dogs were not significantly different from those of the reference with a relative bioavailability of 92.4%. These results suggest the feasibility of developing a tablet formulation of dutasteride using cyclodextrin complex in addition to a solubilizing polymer and surfactant.

AIM:To investigate the efficacy of hepatic arterial infusion chemotherapy(HAIC) using floxuridine(FUDR) in patients with advanced hepatocellular carcinoma(HCC) confined to the ***:Thirty-four patients who had advanced HCC with unresectability or unsuccessful previous therapy in the absence of extrahepatic metastasis were treated with intra-arterial FUDR chemotherapy at ourhospital between March 2005 and May *** the 34 patients,9 patients were classified as Child class C,and 18 patients had portal vein tumor thrombus(PVTT).One course of chemotherapy consisted of continuous infusion of FUDR(0.3 mg/kg during day 1-14) and dexamethasone(10 mg on day 1,4,7 and 11),and this treatment was repeated every 28 ***:Two patients(5.9%) displayed a complete response,and 12 patients(35.3%) had a partial *** tumor control rate was 61.8%.The median overall survival times were 15.3 mo,12.4 mo and 4.3 mo for the patients who were classified as Child class A,Child class B and Child class C,respectively(P = 0.0392).The progression-free survival was 12.9 mo,7.7 mo and 2.6 mo for the patients who were classified as Child class A,Child class B and Child class C,respectively(P = 0.0443).The cumulative survival differed significantly according to the Child-Pugh classification and the presence of *** addition to hepatic reserve capacity and PVTT,the extent of HCC was an independent factor in determining a poor *** most common adverse reactions to HAIC were mucositis,diarrhea and peptic ulcer disease,but most of these complications were improved by medical treatment and/or a delay of ***:The present study demonstrates that intra-arterial FUDR chemotherapy is a safe and effective treatment for advanced HCC that is recalcitrant to other therapeutic modalities,even in patients with advanced cirrhosis.

Objective: To investigate the clinical significance of MET gene amplification in patients with gastric cancer in the palliative ***: MET amplification was assessed using fluorescence in situ hybridization(FISH) in 50 patients and quantitative polymerase chain reaction(qPCR) in 326 patients;259 patients treated with first-line fluoropyrimidine and platinum were included for survival ***: The results of FISH and qPCR indicated that the c-MET/CEP7 ratio was correlated with gene copy number. The optimal cutoff value for the copy number using qPCR to detect MET gene amplification with FISH was 5(κ=0.778, P5 detected by qPCR. MET-amplified gastric cancer was associated with an Eastern Cooperative Oncology Group(ECOG) performance status(PS) score of ≥2(33.3% vs. 10.5% P=0.007), peritoneal metastasis(76.2% vs. 46.2%, P=0.008), and elevated bilirubin levels(28.6% vs. 7.3%, P=0.006). The median overall survival(OS) and progression-free survival(PFS) were 11.9 and 5.6 months, respectively. MET-amplified gastric cancer was not associated with survival outcomes [hazard ratio(HR)=0.68, 95% confidence interval(95% CI): 0.35-1.32,P=0.254 for PFS;HR=0.68, 95% CI: 0.35-1.32, P=0.251 for OS].Conclusions: qPCR can be used to detect MET gene amplification. MET amplification was not a predictor of poor prognosis in patients with metastatic or unresectable gastric cancer.
