Follow-up strategy for early detection of delayed pseudoaneurysms in patients with blunt traumatic spleen injury: A single-center retrospective study

BACKGROUND The spleen is the most commonly injured solid organ in blunt abdominal trauma,and splenic pseudoaneurysm rupture is associated with a high risk of *** management has become the standard treatment for hemodynam-ically stable patients with splenic *** the other hand,delayed splenic pseudoaneurysms can develop in any patient,and at present,there are no known risk factors that may reliably predict their ***,there is a lack of consensus regarding the most appropriate strategies for monitoring and mana-ging splenic injuries,especially lower-grade(I-III).AIM To determine the predictors of pseudo-aneurysm formation following splenic injury and develop follow-up strategies for early detection of *** We retrospectively analyzed patients who visited the Level I Trauma Center bet-ween January 2013 and December 2022 and were diagnosed with spleen injuries after blunt abdominal *** Using the American Association for the Surgery of Trauma spleen injury scale,the splenic injuries were categorized into the following order based on severity:Grade I(n=57,17.6%),grade II(n=114,35.3%),grade III(n=89,27.6%),grade IV(n=50,15.5%),and grade V(n=13,4.0%).Of a total of 323 patients,35 underwent splenectomy and 126 underwent angioembolization.19 underwent delayed angioembolization,and 5 under-went both initial and delayed *** 14 patients who had undergone delayed angioembolization,no extravasation or pseudoaneurysm was observed on the initial computed tomography *** are no particular patient-related risk factors for the formation of a delayed splenic pseudoaneurysm,which can occur even in a grade I spleen injury or even 21 days after the *** mean detection time for a delayed pseudoaneurysm was 6.26±5.4(1-21,median:6,inter-quartile range:2-9)*** We recommend regular follow-up computed tomography scans,including an arterial and portal venous phase,at least 1 week an

Hydroxyapatite(HA)is a representative substance that induces bone *** research team extracted nanohydroxyapatite(EH)from natural resources,especially equine bones,and developed it as a molecular biological ***(PEI)was used to coat the EH to develop a gene *** verify that PEI is well coated in the EH,we first observed the morphology and dispersity of PEI-coated EH(pEH)by electron *** pEH particles were well distributed,while only the EH particles were not distributed and ***,the existence of nitrogen elements of PEI on the surface of the pEH was confirmed by EDS,calcium concentration measurement and fourier transform infrared spectroscopy(FT-IR).Additionally,the pEH was confirmed to have a more positive charge than the 25 kD PEI by comparing the zeta *** a result of pGL3 transfection,pEH was better able to transport genes to cells than 25 kD *** verification as a gene carrier for pEH,we induced osteogenic differentiation of DPSCs by loading the BMP-2 gene in pEH(BMP-2/pEH)and delivering it to the *** a result,it was confirmed that osteogenic differentiation was promoted by showing that the expression of osteopontin(OPN),osteocalcin(OCN),and runt-related transcription factor 2(RUNX2)was significantly increased in the group treated with BMP-2/*** conclusion,we have not only developed a novel nonviral gene carrier that is better performing and less toxic than 25 kD PEI by modifying natural HA(the agricultural byproduct)but also proved that bone differentiation can be effectively promoted by delivering BMP-2 with pEH to stem cells.

Clinical outcomes of radiation therapy for early-stage gastric mucosa-associated lymphoid tissue lymphoma

Analysis of demographic characteristics in 3242 young age gastric cancer patients in Korea

AIM:To evaluate the epidemiologic features of young age gastric cancer(GC). METHODS:Retrospectively,a total of 3242 patients with GC between 18 and 45 years of age and 3000 sex-and age-matched controls were *** subjects were stratified into 3 groups based on age (A,18-30 years;B,31-40 years;C,41-45 years). Epidemiologic characteristics and risk factors were investigated with reference to their age and gender. RESULTS:Compared to controls,more frequent intake of high risk diet(P=0.00075),history of heavy smoking(P=0.00087),intake of heavy alcohol(P= 0.00091),lower social economic status(P=0.00083), body mass index>30(P=0.00097),urban residence(P=0.00065),and more frequent exposure to harmful occupational environments(P=0.00072)were observed in all age groups and both genders in young age *** relationships were weaker in females compared to males of the same age,and were stronger as the age of patients ***,in group C of young age GC patients,environmental factors played important roles in females and males with a similar body *** females,older age at first delivery(> 35 years),lack of lactation history,nulliparity,and poor nutritional status during pregnancy were significantly associated with an increased risk of GC(P=0.00034). In this study,252 patients(7.8%)had a family history of GC with high odds ratio(OR)(3.22-4.21).In particular, family history was more closely associated with GC in males(OR,4.21 in male vs 3.46 in female)and more advanced cases(P=0.00051). CONCLUSION:Hormonal associated factors were more commonly associated with females whereas environmental factors were more commonly associated with males in young age GC patients.

Clinical significance and usefulness of soluble heparin binding-epidermal growth factor in gastric cancer

AIM:To evaluate the clinical usefulness of solubleheparin-binding epidermal growth factor(s HB-EGF)as a serum biomarker for gastric cancer(GC).METHODS:Serum s HB-EGF levels were measured by a commercially available human HB-EGF ELISA Kit and compared among 60 normal controls,30 highrisk patients,37 early gastric cancer(EGC),and30 advanced gastric cancer(AGC)through ANOVA *** between serum s HB-EGF and clinicopathological features of GC were analyzed through Spearman’s *** diagnostic performance of serum s HB-EGF for GC was evaluated through receiver operating characteristic(ROC)curve and logistic regression ***:Serum s HB-EGF levels were significantly higher in AGC group(314.4±127.5 pg/m L)than EGC(165.3±123.2 pg/m L),high-risk(98.7±67.3 pg/m L),and control(94.7±83.6 pg/m L)groups(post-hoc Bonferroni,all P<0.001),*** s HB-EGF levels were also significantly higher in EGC group than high-risk(P=0.049)and control(P=0.006)***,serum s HB-EGF levels closely correlated with depth of invasion(T-stage,γs=0.669,P<0.001),lymph node metastasis(N-stage,γs=0.407,P=0.001),and distant metastasis(M-stage,γs=0.261,P=0.030).ROC curve and logistic regression analysis demonstrated a remarkable diagnostic potential of serum s ***:Serum s HB-EGF is closely correlated with advanced stage GC and can be a promising serological biomarker for GC.

An 80-year-old woman presenting with chest pain was found to have a large,lobulated soft tissue mass in the liver and nearby tissues on abdominal computed tomography(CT).The tumor had invaded the common hepatic artery and main portal *** developed 4 wk later,at which point,a pancreas and biliary CT scan revealed a large mass in the right lobe of the liver and a hilar duct obstruction,which was found to be a small cell *** its rarity,liver and bile duct small cell carcinoma should be considered in the differential diagnosis of atypical chest pain without jaundice.

Intramedullary spinal cord metastasis from pancreatic neuroendocrine tumor

Intramedullary spinal cord metastasis(ISCM) is very rare and its optimal treatment remains controversial. Pancreatic neuroendocrine tumor(pNET) is a rare tumor that usually presents with hepatic metastasis. Hepatic failure due to tumor progression is the major cause of death in cases of pNET. To date,no report has described a case of ISCM from pNET. Although spinal cord metastasis of a solid tumor is uncommon,it is a critical condition that can cause a potentially irreversible loss of neurologic function. Here,we report the case of a 45-year-old man who presented with leg weakness and voiding difficulty,and was found to have ISCM from pNET. Surgical treatment prevented further neurological deterioration. This is the first case report of ISCM from pNET.

Single-step Preparation of Nano-homogeneous NiO/YSZ Composite Anode for Solid Oxide Fuel Cells

Homogeneous co-precipitation and hydrothermal treatment were used to prepare nano- and highly dispersed Ni O/YSZ(yttria-stabilized zirconia) composite powders. Composite powders of size less than 100 nm were successfully prepared. This process did not require separate sintering of the YSZ and Ni O to be used as the raw materials for solid oxide fuel cells. The performance of a cell fabricated using the new powders(*** density ~0.87 W/cm^2) was higher than that of a cell fabricated using conventional powders(max. power density ~0.73 W/cm^2). Co-precipitation and hydrothermal treatment proved to be very effective processes for reducing cell production costs as well as improving cell performance.
