Simultaneous laparoscopic multi-organ resection combined with colorectal cancer:Comparison with non-combined surgery

AIM: To access the short-term outcomes of simultaneous laparoscopic surgery combined with resection for synchronous lesions in patients with colorectal cancer. METHODS: Between March 1996 and April 2010 prospectively collected data were reviewed from 93 consecutive patients who had colorectal cancer and underwent simultaneous multiple organ resection (combined group) and 1090 patients who underwent conventional laparoscopic right hemicolectomy or laparoscopic low/ anterior resection for colorectal cancer (non-combined group). In the combined group, there were nine gastric resections, three nephrectomies, nine adrenalectomies, 56 cholecystectomies, and 21 gynecologic resections. In addition, f ive patients underwent simultaneous laparoscopic resection for three organs. The patient demographics, intra-operative outcomes, surgical morbidity, and short-term outcomes were compared between thetwo groups (the combined and non-combined groups). RESULTS: There were no signifi cant differences in the clinicopathological variables between the two groups. The operating time was signifi cantly longer in the combined group than in the non-combined group, regardless of tumor location (laparoscopic right hemicolectomy and laparoscopic low/anterior resection groups; P = 0.048 and P < 0.001, respectively). The other intraoperative outcomes, such as the complications and open conversion rate, were similar in both groups. The rate of post-operative morbidity in the combined group was similar to the non-combined group (combined vs non-combined, 15.1% vs 13.5%, P = 0.667). Oncological safety for the colon and synchronous lesions were obtained in the combined group. CONCLUSION: Simultaneous laparoscopic multiple organ resection combined with colorectal cancer is a safe and feasible option in selected patients.

Bioactive hydrogel microcapsules for guiding stem cell fate decisions by release and reloading of growth factors

Human pluripotent stem cells(hPSC)hold considerable promise as a source of adult cells for treatment of diseases ranging from diabetes to liver *** of the challenges that limit the clinical/translational impact of hPSCs are high cost and difficulty in scaling-up of existing differentiation *** this paper,we sought to address these challenges through the development of bioactive microcapsules.A co-axial flow focusing microfluidic device was used to encapsulate hPSCs in microcapsules comprised of an aqueous core and a hydrogel ***,the shell contained heparin moieties for growth factor(GF)binding and *** aqueous core enabled rapid aggregation of hPSCs into 3D spheroids while the bioactive hydrogel shell was used to load inductive cues driving pluripotency maintenance and endodermal ***,we demonstrated that one-time,1 h long loading of pluripotency signals,fibroblast growth factor(FGF)-2 and transforming growth factor(TGF)-β1,into bioactive microcapsules was sufficient to induce and maintain pluripotency of hPSCs over the course of 5 days at levels similar to or better than a standard protocol with soluble ***,stem cell-carrying microcapsules that previously contained pluripotency signals could be reloaded with an endodermal cue,Nodal,resulting in higher levels of endodermal markers compared to stem cells differentiated in a standard ***,bioactive heparin-containing core-shell microcapsules decreased GF usage five-fold while improving stem cell phenotype and are well suited for 3D cultivation of hPSCs.

Locally recurrent penile apocrine carcinoma initially diagnosed as metastatic adenocarcinoma of colon

Apocrine carcinoma is a rare malignant adnexal neoplasm. The differential diagnosis between apocrine carcinoma and cutaneous metastasis is often difficult. Here, we report a case of locally recurrent penile apocrine carcinoma initially diagnosed as metastatic adenocarcinoma of the colon. A 75-year-old man with a history of surgical resection due to sigmoid colon cancer and penile metastasis two years prior to this study presented with a nodule at the left penile base. He underwent a wide local resection of the penile mass under a suggested preoperative diagnosis of extra-mammary Paget＇s disease （EMPD） associated with previous sigmoid colon cancer. However, the previously and currently resected penile masses were identified as primary apocrine carcinoma upon hematoxylin and eosin （H＆E） staining and immunohistochemical staining. Although the incidence is extremely rare, both clinicians and pathologists should be alert to the possibility of synchronous double primary apocrine carcinoma in cancer patients with malignant cutaneous lesions.

*** Teachers Learning With Multilingual Learners in Korea Through Educational Technology:Bringing Heteroglossic and Global Approaches to TESOL Teacher Education

Purpose This study aimed to explore how educational technology influenced a preparatory teacher education program using heteroglossic and global ***/Approach/Methods The researchers drew upon the theoretical framework of multilingual digital storytelling(MDST),which emphasizes the intercultural awareness attributes of multilingual learners(MLs)and takes a heteroglossic perspective in linguistic *** qualitative case study examined the experiences of 11 U.S.-based preservice teachers(PSTs)and 12 MLs elementary students in Korea in the MDST project of a TESOL methods *** The findings showed that PSTs and MLs enhanced their appreciation for educational technology,multilingualism,and intercultural *** the project aimed to decenter English as a hegemonic language,both PSTs and MLs maintained traditional discourses that privileged English over MLs’home language and targeted literacy correctness in written English only,moving away from the heterogeneous goals of the course *** and MLs also faced challenges in navigating technological tools,which negatively affected their perception of the ***/Value This study contributes to heteroglossic approaches in preparatory TESOL teacher education programs and improves the understanding of challenges in educational technology use for global multilingual exchanges to promote global citizenship.

Among many biomaterials,gelatin methacrylate(GelMA),a photocurable protein,has been widely used in 3D bioprinting process owing to its excellent cellular responses,biocompatibility and ***,GelMA still shows a low processability due to the severe temperature dependence of *** overcome this obstacle,we propose a two-stage temperature control system to effectively control the viscosity of *** optimize the process conditions,we evaluated the temperature of the cooling system(jacket and stage).Using the established system,three GelMA scaffolds were fabricated in which different concentrations(0,3 and 10 wt%)of silanated silica particles were *** evaluate the performances of the prepared scaffolds suitable for hard tissue regeneration,we analyzed the physical(viscoelasticity,surface roughness,compressive modulus and wettability)and biological(human mesenchymal stem cells growth,western blotting and osteogenic differentiation)***,the composite scaffold with greater silica contents(10 wt%)showed enhanced physical and biological performances including mechanical strength,cell initial attachment,cell proliferation and osteogenic differentiation compared with those of the *** results indicate that the GelMA/silanated silica composite scaffold can be potentially used for hard tissue regeneration.

Inhibition of histone deacetylase 2 mitigates profibrotic TGF-β1 responses in fibroblasts derived from Peyronie＇s plaque

Epigenetic modifications, such as histone acetylation/deacetylation, have been shown to play a role in the pathogenesis of fibrotic disease. Peyronie＇s disease （PD） is a localized fibrotic process of the tunica albuginea, which leads to penile deformity. This study was undertaken to determine the anti-fibrotic effect of small interfering RNA （siRNA）-mediated silencing of histone deacetylase 2 （HDAC2） in primary fibroblasts derived from human PD plaque. PD fibroblasts were pre-treated with HDAC2 siRNA and then stimulated with transforming growth factor-p1 （TGF-β1）. Protein was extracted from treated fibroblasts for Western blotting and the membranes were probed with antibody to phospho-Smad2/Smad2, phospho-Smad3/Smad3, smooth muscle α-actin and extracellular matrix proteins, including plasminogen activator inhibitor-β 1, fibronectin, collagen I and collagen IV. We also performed immunocytochemistry to detect the expression of extracellular matrix proteins and to examine the effect of HDAC2 siRNA on the TGF-β1-induced nuclear translocation of Smad2/3 in fibroblasts. Knockdown of HDAC2 in PD fibroblasts abrogated TGF-β1-induced extracellular matrix production by blocking TGF-β1-induced phosphorylation and nuclear translocation of Smad2 and Smad3, and by inhibiting TGF-β1-induced transdifferentiation of fibroblasts into myofibroblasts. Decoding the individual function of the HDAC isoforms by use of siRNA technology, preferably siRNA for HDAC2, may lead to the development of specific and safe epigenetic therapies for PD.

Reduction of gastrointestinal motility by unilateral thyroparathyroidectomy plus subdiaphragmatic vagotomy in rats

AIM: To investigate whether the combined methods of unilateral thyroparathyroidectomy (TPX) and subdiaphragmatic vagotomy (VAX) can be adapted for rats and used as a reliable method to produce a rat model of long-term reduction of gastrointestinal (GI) motor function. METHODS: Male Sprague-Dawley rats were randomly divided into 3 groups, normal, sham-operated and unilateral TPX plus VAX. The TPX plus VAX rats received VAX 7 d after application of TPX, and dietary intake and fecal output were then measured daily for 1 *** completion of the experiments, gastric emptying and small bowel transit were measured in vivo, and the contractile responses of colonic strips to excitatory and inhibitory neurotransmitters were estimated using isometric force transducers in vitro. RESULTS: In comparison with normal and sham-operated rats, rats which received unilateral TPX plus VAX showed a significant decrease in body weight and in fecal pellet number and weight throughout the entire week. Application of TPX plus VAX to rats markedly delayed gastric emptying and small bowel transit. In TPX plus VAX rats, the longitudinal muscles of the proximal colon showed a significant reduction in contractile responses to acetylcholine (5 × 10-6 mol/L), and a dramatic attenuation of contractile responses was also observed in both the longitudinal and circular muscles of the distal colon. However, the spontaneous contractility of the colonic strips from TPX plus VAX rats was not significantly affected by treatment with N-nitro-Larginine-methyl ester (0.1 mol/L). CONCLUSION: The results indicate that unilateral TPX plus VAX reduced the motor function of the GI tract in rats, and the reduced gut motility is likely mediated, at least in part, by inhibition of the excitatory neurotransmitter system.

Medial Patella Subluxation after Minor Trauma

Most cases of medial patella subluxation occur under iatrogenic conditions that are often associated with a prior excessive lateral retinacular release procedure. We report a case of medial subluxation following minor trauma with no history of previous lateral release. The abnormality was identified on magnetic resonance imaging and was successfully treated by direct repair of the lateral retinaculum. The recognition and treatment of medial subluxation of the patella without retinacular release can be difficult. Therefore, consideration of multiple causes through careful medical history taking, physical examination, and radiologic examination is needed for accurate diagnosis.

Effects of High CO_2 Pretreatment on the Quality of Fresh-Cut Potatoes

Effects of short-term high CO_2 pretreatment on quality of fresh-cut potatoes were studied. Potatoes were treated under 90% CO_2 for 18 h, peeled, cut, dipped in 1% ascorbic acid or distilled water for 3 min, packed in polyethylene bags and stored for 12 d at 4 ℃. Non-treated potatoes were used as the control. High CO2 pretreatments ensured maintenance of a high level of CO_2 in headspace of packages containing fresh-cut potatoes. Both the growth of total aerobic bacteria in initial stages of post-cut storage and browning index of the cut surface were lower in high CO_2 treated cut potato. Anti-browning effect of high CO_2 pretreatment was decreased when applied in combination with ascorbic acid. Soluble solids, pH, titratable acidity and flesh firmness of fresh-cut potatoes were not affected by high CO_2 treatment. Therefore, high CO_2 treatment before slicing could be effective method to maintain quality of flesh-cut potatoes.
