Role of nitric oxide in liver transplantation:Should it be routinely used?

Ischemia-reperfusion injury(IRI) continues to be a major contributor to graft dysfunction, thus supporting the need for therapeutic strategies focused on minimizing organ damage especially with growing numbers of extended criteria grafts being utilized which are more vulnerable to cold and warm ischemia. Nitric oxide(NO·) is highly reactive gaseous molecule found in air and regarded as a pollutant. Not surprising, it is extremely bioactive, and has been demonstrated to play major roles in vascular homeostasis, neurotransmission, and host defense inflammatory reactions. Under conditions of ischemia, NO· has consistently been demonstrated to enhance microcirculatory vasorelaxation and mitigate pro-inflammatory responses, making it an excellent strategy for patients undergoing organ transplantation. Clinical studies designed to test this hypothesis have yielded very promising results that includes reduced hepatocellular injury and enhanced graft recovery without any identifiable complications. By what means NO· facilitates extra-pulmonary actions is up for debate and speculation. The general premise is that they are NO· containing intermediates in the circulation, that ultimately mediate either direct or indirect effects. A plethora of data exists explaining how NO·-containing intermediate molecules form in the plasma as S-nitrosothiols(e.g., S-nitrosoalbumin), whereas other compelling data suggest nitrite to be a protective mediator. In this article, we discuss the use of inhaled NO· as a way to protect the donor liver graft against IRI in patients undergoing liver transplantation.

The critical role of bulk density of graphene oxide in tuning its defect concentration through microwave-driven annealing

Controlling the concentration of defects in reduced graphene oxide （rGO） to tailor its electrical and physicochemical properties has remained a significant challenge. We report that extent of microwave （MW）-driven annealing of rGO is influenced significantly by its bulk density, which allows us to vary its defect density and crystallite size over wide ranges by controlling this parameter. Extent of anneal- ing was investigated by multiple techniques including Raman and X-ray photoelectron spectroscopies, and electrical conductivity measurements. Improved corrosion resistance of rGOs upon annealing was ex- amined by cyclic voltammetry in H_2SO_4 electrolyte and temperature-programmed oxidation of rGO. Our results indicate that a low bulk density of rGO facilitates defect annealing, yielding high-quality carbon with 99.3 wt% purity, oxidative resistance as high as graphite powder, and an electrical conductivity of 36,000 S m-1 in the compressed powder form. These results demonstrate a prospective synthesis route for tailor-made nanocarbons from rGO through MW-driven annealing.

Objective Unilateral L5 spinal nerve ligature can induce neuropathic pain and NMDA receptor-independent synaptic potentiation in neurons in the latero-capsular part of the central nucleus of amygdale(CeLC).CeLC receives predominantly nociceptive information from the spinal dorsal horn via the lateral parabrachial nucleus(LPB)(spinoparabrachio -amygdaloid pathway) and is called the "nociceptive amygdala".However,it remains unknown about the morphological mechanisms underlying such synaptic plasticity at the ultrastructural *** To address this issue,one week after the left L5 spinal nerve ligation,neuropathic pain rats which received GFP-expressing lentivirus injection into LPB were used for electron microscopic and reconstruction *** operation rats with the lentivirus injection were used as a control *** LPB terminals in the right CeLC were shown as GFP-positive presynaptic buttons and made asymmetrical synapses mainly with dendritic *** spinal nerve ligature induced a significant difference in PSD length when compared to the control[(366.33±151.30) nm vs(284.43±105.12) nm,respectively;P<0.001, Kolmogrov-Smirnov test].And the results taken from serial ultrathin sections showed that PSD areas in right CeLC of neuropathic pain group had a significant increase compared to the sham group[(129725.9±4892.34) nm vs(89218.24±7147.75) nm,respectively;P<0.002).To approve this result,another marker of projecting fiber terminals in CeLC from LPB,CGRP immunoreactivity was used in both ligated and sham *** with the above results,spinal nerve ligature induced a significant enlargement of CGRP positive synaptic PSD lengths[(356.56±138.71) nm vs(269.18±92.61) nm, respectively,P<0.001]and areas[(115351.6±7302.79) nm vs(83801.82±7177.07) nm,respectively,P<0.01]in the right *** addition,the ratio of non-macular PSDs counted from either GFP-positive or CGRP-positive synapses had significant differences between operation and sham group

Detecting Design Patterns in Object-Oriented Program Source Code by Using Metrics and Machine Learning

Detecting well-known design patterns in object-oriented program source code can help maintainers understand the design of a program. Through the detection, the understandability, maintainability, and reusability of object-oriented programs can be improved. There are automated detection techniques;however, many existing techniques are based on static analysis and use strict conditions composed on class structure data. Hence, it is difficult for them to detect and distinguish design patterns in which the class structures are similar. Moreover, it is difficult for them to deal with diversity in design pattern applications. To solve these problems in existing techniques, we propose a design pattern detection technique using source code metrics and machine learning. Our technique judges candidates for the roles that compose design patterns by using machine learning and measurements of several metrics, and it detects design patterns by analyzing the relations between candidates. It suppresses false negatives and distinguishes patterns in which the class structures are similar. As a result of experimental evaluations with a set of programs, we confirmed that our technique is more accurate than two conventional techniques.

目的本文报告在锥束CT下接受内脏神经阻滞术(SNB)治疗的上腹部恶性肿瘤疼痛患者24例的疼痛缓解效果,并讨论CT成像在两例患者中的应用.方法癌症相关的上腹部疼痛患者24例,从2015年6月起6个月内接受SNB治疗.分别对他们在治疗前和治疗后1周的疼痛数字评分量表(NRS)变化、阿片类药物使用剂量变化(等效剂量吗啡转换)及治疗引起的并发症发生率进行回顾性研究.SNB经T12～L1或L1-2椎间盘入路进行,应用阻力消失法将造影剂注入膈脚后间隙,并使用锥束CT以及透视法确认造影剂的扩散.神经阻滞剂使用99.5％脱水乙醇10～20 ml.结果受试者24例,男15例,女9例,平均年龄63.8岁.NRS从治疗前的5/10下降至治疗后1周的0.86/10(P＜0.01),阿片类药物使用剂量(等效剂量吗啡转换)从治疗前86.6 mg/d降至治疗后1周的58 mg/d(P＜0.01).仅观察到以下轻度不良反应:一过性低血压(8例)、恶心(5例)和腹泻(3例),未观察到严重不良反应.结论 SNB是治疗癌症诱发的腹部疼痛的有效方法,联合应用锥束CT可提高疼痛缓解效果.

Peritoneal bleeding due to percutaneous transhepatic gallbladder drainage:An autopsy report

A 77-year-old man underwent percutaneous transhepatic gallbladder drainage(PTGBD) for acute cholecystitis as a preoperative procedure;however,he suddenly suffered cardiopulmonary arrest 4 h after the PTGBD and *** were three centesis scars for the PTGBD,and only one pathway from the most dorsal centesis scar reached the ***,the PTGBD pathway crossed and injured the intrahepatic arterial wall,and hepatic parenchymal bleeding extended along the PTGBD pathway to the inferior surface of the *** flowed to the peritoneal cavity through a small gap between the liver and ***,the PTGBD caused lethal *** the percutaneous transhepatic cholangio drainage/PTGBD pathway runs close to vessels near the liver surface,it might be necessary to deal with the possibility of rapid and lethal peritoneal bleeding.

＜正＞ Our previous study has demonstrated that electroacupuncture （ EA） produces antinociceptive effect,whereas the antinociception of intrathecal. but not intrac-erebroventricular, morphine is paradoxically attenuated after EA stimulation,indicating that EA activates two opposing systems （i.e.. opioid and anti - opioid systems）. In this study,we examined the involvement of cholecystokinin （CCK） in the anti - opioid property following EA stimulation in the spinal cord. EA was applied to ST - 36 acupoints. and pain thresholds were assessed by the hind -paw pressure test in male Sprague - Dawley rats. The amount of mRNA expression of CCK and its receptors （CCK - 1 and CCK - 2） in the spinal cord were determined by reverse transcriptase- polymerase chain reaction. The attenuation of morphine - induced antinociception after EA was significantly reversed by proglumide, CCK receptor antagonist. And the expression of CCK and CCK - 2 receptor mRNA in the spinal cord was markedly increased after EA stimulation. These results suggest that CCK mediated - neural systems in the spinal cord may be involved in the attenuation of antinociceptive effect of morphine after EA.

Development of Online Machining and Measurement System in Die and Mold Manufacturing： Concept and Fundamental Experiments

The corrective machining to compensate the resulting errors is usually tried at plural times based on the trial-and-error process when the machining is realized in die and mold manufacturing. This corrective machining has an influence on accuracy and efficiency and is an important factor. The measurement system for measuring the forms of die and mold at all times is essential in order to machine the die and mold with high accuracy and efficiency. However, the problems of management and operation errors are found in compensating process of die and mold machining. In this paper, an online machining and measurement system in die and mold manufacturing is developed in order to overcome these problems. In this online system, 2-axis control system is added to a surface roughness measuring instrument, and both NC machining program and measured data are linked and controlled using a same computer. Therefore, the machining and measurement can be recognized for consistent process, and can be realized on the machine. This system has the advantages such as the high accuracy, low-price, and online convenience and so on. The possibility of practical use of this online system was investigated by fundamental experiments.

从大豆（品种Williams）基因组文库中筛选出大豆！11S球蛋白Gy4（A5A4B3）染色体基因，对其基因及基因上游启动区和基因下游区域进行了测序，全长3680bp。基因编码区长度为2597bp，由四个外显子和三个内含子组成。四个外显子的长度为：289bp，266bp，747bp和387bp；三个内含子的长度为：332bp，75bp和501bp。基因上游除具有CAAT盒和TATA盒外，还具有Legumin盒和类似Glycinin盒的核苷酸序列。基因下游具有三个多聚腺苷酸加尾信号（AATAAA）。与大豆品种Raiden，Dare及Forrest的Gy4基因比较，与前两者碱基顺序基本相同，只是个别碱基有差异，但与品种Forrest差异较大。该基因已被收入基因数据库（DDBJ）中，其注册号是AB004062。
