Background:The mobilization and redistribution of severe acute respiratory syndrome coronavirus 2(SArS-CoV-2)specific T-cells and neutralizing antibodies(nAbs)during exercise is purported to increase immune surveillance and protect against severe coronavirus disease 2019(COVID-19).We sought to determine if COVID-19 vaccination would elicit exercise-responsive SArS-CoV-2 T-cells and transiently alter nAb ***:Eighteen healthy participants completed a 20-min bout of graded cycling exercise before and/or after receiving a COVID-19 *** major leukocyte subtypes were enumerated before,during,and after exercise by flow cytometry,and immune responses to SArS-CoV-2 were determined using whole blood peptide stimulation assays,T-cell receptor(TCr)-βsequencing,and SArS-CoV-2 nAb ***:COVID-19 vaccination had no effect on the mobilization or egress of major leukocyte subsets in response to intensity-controlled graded ***,non-infected participants had a significantly reduced mobilization of CD4+and CD8+naive T-cells,as well as CD4+central memory T-cells,after vaccination(synthetic immunity group);this was not seen after vaccination in those with prior SArS-CoV-2 infection(hybrid immunity group).Acute exercise after vaccination robustly mobilized SArS-CoV-2 specific T-cells to blood in an intensity-dependent *** groups mobilized T-cells that reacted to spike protein;however,only the hybrid immunity group mobilized T-cells that reacted to membrane and nucleocapsid *** increased significantly during exercise only in the hybrid immunity ***:These data indicate that acute exercise mobilizes SArS-CoV-2 specific T-cells that recognize spike protein and increases the redistribution of nAbs in individuals with hybrid immunity.

This editorial commented on an article in the World Journal of Gastroenterology titled“risks of reactivation of Hepatitis B Virus in Oncological Patients Using Tyrosine Kinase-Inhibitors:Case report and Literature Analysis”by Colapietro et *** this editorial,we focused on providing a more comprehensive exploration of hepatitis B virus reactivation(HBVr)associated with the usage of tyrosine kinase inhibitors(TKIs).It includes insights into the mechanisms underlying HBV reactivation,the temporal relationship between TKIs and HBV reactivation,and preventive *** aim is to understand the need for nucleos(t)ide analogs(NAT)and serial blood tests for early recognition of reactivation and acute liver injury,along with management *** are considered to be an intermediate(1%-10%)of *** guidelines stipulate that patients receiving therapy with high or moderate risks of reactivation or recent cancer diagnosis must have at least tested hepatitis B surface antigen,anti-hepatitis B core antigen(HBc),and anti-hepatitis B surface ***-HBc screening in highly endemic areas means people with negative tests should be vaccinated against *** or nucleotide analogs(NAs)like entecavir(ETV),tenofovir disoproxil fumarate(TDF),and tenofovir alafenamide(TAF)form the basis of HBV reactivation prophylaxis and treatment during ***,lamivudine,telbivudine,and adefovir are generally discouraged due to their reduced antiviral efficacy and higher risk of fostering drug-resistant viral ***,these less effective NAs may still be utilized in cases where ETV,TDF,and TAF are not feasible treatment options.

流星进入地球大气层时会发生空中解体和爆炸,威胁地球局部区域乃至全球安全针对流星进入大气演化的超高速动力学仿真研究具有重要意义.为提高预报的精确性,应关注流星体内部结构和非均匀烧蚀等对演化过程和解体后碎片运动的影响。本文提出了一种可变形不规则离散元方法,用于仿真分析流星进入大气层时轨迹和姿态变化、烧蚀、解体和爆轰等现象。利用多面体离散元模型可模拟不同结构的异质流星;通过拉伸多面体模型顶点,仿真流星的粗糙外形和烧蚀形变;利用Jones-Wilkins-Lee状态方程计算烧蚀产生的集中爆轰产物的压力.以Chelyabinsk流星事件中发生轨迹偏离的碎片为例,数值仿真集中爆轰产物的膨胀驱动作用对碎片运动方向和轨迹演化的影响.仿真结果表明,碎片弹出轨迹特征和烧蚀剩余质量均在观测值范围内,验证了新方法的有效性.

D-transposition of the great arteries(d-TGA)is surgically repaired with the arterial switch operation(ASO)with excellent results,however short and long-term morbidities still develop including neurocognitive *** significant central sleep apnea is uncommon in non-premature infants,but when present indicates immature autonomic control of respiration likely due to a neurologic *** report the unanticipatedfinding of central sleep apnea in four-term neonates with d-TGA after uncomplicated ASO,with the short-term complication of delayed hospital discharge and long-term concerns regarding this early marker of brain immaturity and its hindrance to normal *** this report,we will review each patient’s clinical course and then examine the literature on pediatric central sleep apnea,neurodevelopmental outcomes after ASO,and the important overlap of these entities in the care of patients going forward.

Academic productivity correlates with industry earnings in foot and ankle fellowship programs in the United States:A retrospective analysis

BACKGrOUND The study investigates the connection between academic productivity and industry earnings in foot and ankle orthopedic surgery *** metrics like the H-index and Open Payments Database(OPD)data,it addresses a gap in understanding the relationship between scholarly achievements and financial outcomes,providing a basis for further exploration in this specialized medical *** To elucidate the trends between academic productivity and industry earnings across foot and ankle orthopedic surgery fellowship programs in the United *** This study is a retrospective analysis of the relationship between academic productivity and industry earnings of foot and ankle orthopedic surgery fellowships at an individual faculty and fellowship *** productivity was defined via H-index and recorded from the Scopus *** earnings were recorded from the *** Forty-eight foot and ankle orthopedic surgery fellowships(100%of fellowships)in the United States with a combined total of 165 physicians(95.9%of physicians)were *** individual physician(n=165)total life-time earnings reported on the OPD website was United States Dollar(USD)451430.30±1851084.89(range:USD 25.16-21269249.85;median:USD 27839.80).Mean physician(n=165)H-index as reported on Scopus is 14.24±12.39(range:0-63;median:11).There was a significant but weak correlation between individual physician H-index and individual physician total life-time earnings(Prman’s rho=0.334)and a significant and moderate positive correlation between combined fellowship H-index and total life-time earnings per fellowship(P=0.004,Spearman’s rho=0.409).CONCLUSION There is a significant and positive correlation between academic productivity and industry earnings at foot and ankle orthopedic surgery fellowships in the United *** observation is true on an individual physician level as well as on a fellowship level.

BACKGrOUND Distal humerus elbow fractures are one of the most common traumatic fractures seen in pediatric patients and present as three main types:Supracondylar(SC),lateral condyle(LC),and medial epicondyle(ME)*** To evaluate the epidemiology of pediatric distal humerus fractures(SC,LC,and ME)from an American insurance claims *** A retrospective review was performed on patients 17 years and younger with the ICD 9 and 10 codes for SC,LC and ME fractures based on the IBM Truven MarketScan®Commercial and IBM Truven MarketScan Medicare Supplemental *** from 2015 to 2020 were queried for treatments,patient age,sex,length of hospitalization,and *** A total of 1133 SC,154 LC,and 124 ME fractures were *** fractures had the highest percentage of operation at 83%,followed by LC(78%)and ME fractures(41%).Male patients were,on average,older than female patients for both SC and ME *** In the insurance claims databases used,SC fractures were the most reported,followed by LC fractures,and finally ME *** was identified to be a factor for how a pediatric distal humerus fractures,with patients with SC and LC fractures being younger than those with ME *** peak age per injury per sex was similar to reported historic central tendencies,despite reported trends for younger physiologic development.

Projected Changes in Extreme Event Indices for Alaska

As climate has warmed in recent decades, Alaska has experienced a variety of high-impact extreme events that include heat waves, wildfires, coastal storms and freezing rain. Because the warming is projected to continue, it is essential to consider future changes when planning adaptation actions and building resilience. In this study, we synthesize information on future changes in extreme events in Alaska from an ensemble of regional climate model simulations performed as part of Arctic-COrDEX (Coordinated regional Climate Downscaling Experiment). A set of 13 extreme event indices, based on those developed by the World Climate research Programme’s Expert Team on Climate Change Detection and Indices (ETCCDI), are evaluated from the Arctic-COrDEX output for Alaska. Of the 13 indices, six pertain to temperature, five to total precipitation, one to wind and one to snow. The results for locations in seven different climate zones of Alaska include large increases (5˚C - 10˚C) in the temperature thresholds for the five hottest and coldest days of the year, and large increases in warm spell duration and decreases in cold spell duration. Changes in the cold day temperature threshold are generally larger than the changes in the hot day temperature threshold, consistent with the projections of a stronger warming in winter than in summer in Alaska yearly maximum 1-day and 5-day precipitation amounts as well as the yearly number of consecutive wet days are projected to increase at all locations. The indices for heavy snow days and high-wind days show mixed changes, although the results indicate increases in heavy snow days at the more northern locations and increases in windy days at coastal locations. The changes in the extreme event indices continue through 2100 under the higher-emission (rCP 8.5) emission scenario, while the changes generally stabilize under the lower-emission (rCP 4.5) scenario. .

Periodic Addition of Glucose Suppressed Cyanobacterial Abundance in Additive Lake Water Samples during the Entire Bloom Season

Previously, we showed that prophylactic addition of glucose to Harsha Lake water samples could inhibit cyanobacteria growth, at least for a short period of time. The current study tested cyanobacterial control with glucose for the entire Harsha Lake bloom season. Water samples (1000 ml) were collected weekly from Harsha Lake during the algal-bloom season starting June 9 and lasting until August 24, 2022. To each of two 7-liter polypropylene containers, 500 ml of Harsha Lake water was added, and the containers were placed in a controlled environment chamber. To one container labeled “Treated,” 0.15 g of glucose was added, and nothing was added to the container labeled “Control.” After that, three 25 ml samples from each container were collected and used for 16S rrNA gene sequencing each week. Then 1000 ml of Harsha Lake water was newly collected each week, with 500 ml added to each container, along with the addition of 0.15 g glucose to the “Treated” container. Sequencing data were used to examine differences in the composition of bacterial communities between Treated and Control containers. Treatment with glucose altered the microbial communities by 1) reducing taxonomic diversity, 2) largely eliminating cyanobacterial taxa, and 3) increasing the relative abundance of subsets of non-cyanobacterial taxa (such as Proteobacteria and Actinobacteriota). These effects were observed across time despite weekly inputs derived directly from Lake water. The addition of glucose to a container receiving weekly additions of Lake water suppressed the cyanobacterial populations during the entire summer bloom season. The glucose appears to stimulate the diversity of certain bacterial taxa at the expense of the cyanobacteria.
