Older adults are significantly imp.cted by natural hazards and disasters that are exacerbated by climate change. Understanding their awareness and p.ep.redness is essential for enhancing disaster resilience. This study investigated the attitudes, actions, and recommendations of older adults regarding natural hazards that p.se risks in their geograp.ic area—sp.cifically floods, wildfires, and/or earthquakes in Canada. Methods for this study included survey and focus group. with older adults(n = 161 and n = 10, resp.ctively) and other high-risk group. from across Canada, that are vulnerable to these natural hazards. The main findings from this study are that current awareness and p.ep.redness among older adults is low, though stronger p.rcep.ions of risks are associated with risks sp.cific to geograp.ic locations where resp.ndents live. Several barriers, such as hazard vulnerability misp.rcep.ions, cost-related reasons, and lack of hazard awareness have resulted in low awareness and p.ep.redness among these p.p.lations. The two main recommendations arising from this research are:(1) imp.ove awareness and p.ep.redness with tailor-made emergency p.ep.redness materials for older adults;and(2) adop. community-based ap.roaches to disaster p.ep.redness through existing community group. to strengthen social connections with a focus on locally sp.cific hazards. The findings from this research can be ap.lied to other hazards, including heatwaves and p.ndemics.

This editorial commented on an article in the World Journal of Gastroenterology titled“Risks of Reactivation of Hep.titis B Virus in Oncological p.tients Using Tyrosine Kinase-Inhibitors:Case Rep.rt and Literature Analysis”by Colap.etro et *** this editorial,we focused on p.oviding a more comp.ehensive exp.oration of hep.titis B virus reactivation(HBVr)associated with the usage of tyrosine kinase inhibitors(TKIs).It includes insights into the mechanisms underlying HBV reactivation,the temp.ral relationship.between TKIs and HBV reactivation,and p.eventive *** aim is to understand the need for nucleos(t)ide analogs(NAT)and serial blood tests for early recognition of reactivation and acute liver injury,along with management *** are considered to be an intermediate(1%-10%)of *** guidelines stip.late that p.tients receiving therap. with high or moderate risks of reactivation or recent cancer diagnosis must have at least tested hep.titis B surface antigen,anti-hep.titis B core antigen(HBc),and anti-hep.titis B surface ***-HBc screening in highly endemic areas means p.op.e with negative tests should be vaccinated against *** or nucleotide analogs(NAs)like entecavir(ETV),tenofovir disop.oxil fumarate(TDF),and tenofovir alafenamide(TAF)form the basis of HBV reactivation p.op.ylaxis and treatment during ***,lamivudine,telbivudine,and adefovir are generally discouraged due to their reduced antiviral efficacy and higher risk of fostering drug-resistant viral ***,these less effective NAs may still be utilized in cases where ETV,TDF,and TAF are not feasible treatment op.ions.

Background:The mobilization and redistribution of severe acute resp.ratory syndrome coronavirus 2(SARS-CoV-2)sp.cific T-cells and neutralizing antibodies(nAbs)during exercise is p.rp.rted to increase immune surveillance and p.otect against severe coronavirus disease 2019(COVID-19).We sought to determine if COVID-19 vaccination would elicit exercise-resp.nsive SARS-CoV-2 T-cells and transiently alter nAb ***:Eighteen healthy p.rticip.nts comp.eted a 20-min bout of graded cycling exercise before and/or after receiving a COVID-19 *** major leukocyte subtyp.s were enumerated before,during,and after exercise by flow cytometry,and immune resp.nses to SARS-CoV-2 were determined using whole blood p.p.ide stimulation assays,T-cell recep.or(TCR)-βsequencing,and SARS-CoV-2 nAb ***:COVID-19 vaccination had no effect on the mobilization or egress of major leukocyte subsets in resp.nse to intensity-controlled graded ***,non-infected p.rticip.nts had a significantly reduced mobilization of CD4+and CD8+naive T-cells,as well as CD4+central memory T-cells,after vaccination(synthetic immunity group.;this was not seen after vaccination in those with p.ior SARS-CoV-2 infection(hybrid immunity group..Acute exercise after vaccination robustly mobilized SARS-CoV-2 sp.cific T-cells to blood in an intensity-dep.ndent *** group. mobilized T-cells that reacted to sp.ke p.otein;however,only the hybrid immunity group.mobilized T-cells that reacted to membrane and nucleocap.id *** increased significantly during exercise only in the hybrid immunity ***:These data indicate that acute exercise mobilizes SARS-CoV-2 sp.cific T-cells that recognize sp.ke p.otein and increases the redistribution of nAbs in individuals with hybrid immunity.

Chronic lymp.ocytic leukemia(CLL) is the most common leukemia in the western world. Desp.te significantadvances in therap. over the last decade CLL remains incurable. Current front-line therap. often consists of chemoimmunotherap.-based regimens, most commonly the fludarabine, cyclop.osp.amide p.us rituximab combination, but rates of relap.e and refractory disease are high among these p.tients. Several key signaling p.thways are now known to mediate the survival and p.oliferation of CLL cells in vivo, the most notable of which are the p.thways mediated by the B-cell recep.or(BCR) and cytokine recep.ors. A better understanding of the p.thogenesis of the disease, the underlying biology of the CLL-cell and the roles of the tumour microenvironment has p.ovided the rationale for trials of a range of novel, more targeted therap.utic agents. In p.rticular, clinical trials of ibrutinib and idelalisib, which target the Brutons tyrosine kinase and the delta isoform of p.osp.oinositol-3 kinase comp.nents of the BCR signaling p.thway resp.ctively, have shown extremely p.omising results. Here we review the current literature on the key signaling p.thways and interactions of CLL cells that mediate the survival and p.oliferation of the leukemic cells. For each we describe the results of the recent clinical trials and in vitro studies of novel therap.utic agents.
