BACKGr.UND： Cor.ical motor.neur.ns, also known as upper.motor.neur.ns, ar. lar.e pr.jection neur.ns whose axons convey signals to lower.motor.neur.ns to contr.l the muscle movements. Degener.tion of cor.ical motor.neur.n axons is implicated in sever.l debilitating disor.er. including her.ditar. spastic par.plegia （HSP）. Since the discover. of the fir.t HSP gene, SPASTthat encodes spastin, over.70 distinct genetic loci associated with HSP have been identified. How the mutations of these functionally diver.e genes r.sult in axonal degener.tion and why cer.ain axons ar. affected in HSP r.main lar.ely unknown. The development of induced plur.potent stem cell （iPSC） technology has pr.vided r.sear.her. an excellent r.sour.e to gener.te patient-specific human neur.ns to model human neur.pathological pr.cesses including axonal defects. METHODS： In this ar.icle, we will fir.t r.view the pathology and pathways affected in the common for.s of liSP subtypes by sear.hing the PubMed database. We will then summar.ze the findings and insights gained fr.m studies using iPSC-based models, and discuss challenges and futur. dir.ctions. r.SULTS： HSPs, a heter.geneous gr.up of genetic neur.degener.tive disor.er., exhibit similar.pathological changes that r.sult fr.m r.tr.gr.de axonal degener.tion of cor.ical motor.neur.ns. r.cently, iPSCs have been gener.ted fr.m sever.l common for.s of HSP including SPG4, SPG3A, and SPG11 patients. Neur.ns der.ved fr.m HSP iPSCs exhibit impair.d neur.te outgr.wth, incr.ased axonal swellings, and r.duced axonal tr.nspor., r.capitulating disease-specific axonal defects. CONCLUSIONS： These patient-der.ved neur.ns offer.a unique tool to study the pathogenic mechanisms and explor. the tr.atments for.r.scuing axonal defects in HSP, as well as other.diseases involving axonopathy.
