The liver is an immunologically tolerant organ and a common metastatic site of multiple cancer *** a role for cancer cell invasion programs has been well characterized,whether and how liver-intrinsic factors drive metastatic spread is incompletely ***,we show that aberrantly activated hepatocyte-intrinsic cell cycle-related kinase(CCRK)signaling in chronic liver diseases is critical for cancer metastasis by reprogramming an immunosuppressive *** an inducible liverspecific transgenic model,we found that CCRK overexpression dramatically increased both B16F10 melanoma and MC38 colorectal cancer(CRC)metastasis to the liver,which was highly infiltrated by polymorphonuclear-myeloid-derived suppressor cells(PMNMDSCs)and lacking natural killer T(NKT)*** of PMN-MDSCs in CCRK transgenic mice restored NKT cell levels and their interferon gamma production and reduced liver metastasis to 2.7% and 0.7%(metastatic tumor weights)in the melanoma and CRC models,***,CCRK activated nuclear factor-kappa B(NF-κB)signaling to increase the PMN-MDSC trafficking chemokine C-X-C motif ligand 1(CXCL1),which was positively correlated with liver-infiltrating PMN-MDSC levels in CCRK transgenic ***,CRC liver metastasis patients exhibited hyperaaivation of hepatic CCRK/NF-κB/CXCL1 signaling,which was associated with accumulation of PMN-MDSCs and paucity of NKT cells compared to healthy liver transplantation *** summary,this study demonstrates that immunosuppressive reprogramming by hepatic CCRK signaling undermines antimetastatic *** findings offer new mechanistic insights and therapeutic targets for liver metastasis intervention.

Is it safe to perform operation for colorectal malignancy in Chinese patients without DVT prophylaxis？ An 8-year experience from a regional hospital in Hong Kong

Background Colorectal surgery was regarded as one of the high risk surgery for post-operative deep vein thrombosis （DVT） and pulmonary embolism. This study aimed at investigating the incidence of venous thromboembolism （VTE） after colorectal surgery for *** Data were collected from the prospective database of colorectal malignancy from 2000 to 2008. A total of 1421 colorectal （open and laparoscopic） operations were performed for the colorectal malignancy without DVT *** Only seven patients （0.5%） developed symptomatic DVT and one of them had complication of pulmonary embolism without mortality. Open operation for colorectal malignancy was identified as possible risk factor of DVT,however, risk factors like operative time, low anterior resection, sex, age etc. were not *** Risk of venous thromboembolism after colorectal operation is low in Chinese of our locality and it might be safe to perform colorectal operation for malignancy without DVT prophylaxis.

Single incision laparoscopic right hemicolectomy： different approaches

Background Single incision laparoscopic colectomy has been performed in recent years, and has been shown to be feasible and safe. This study was to assess the feasibility of single incision laparoscopic right hemicolectomy and to compare the differences in different approaches. Methods This retrospective study included eighteen patients with carcinoma of caecum and ascending colon, undergoing single incision laparoscopic right hemicolectomy. This study also compared single incision laparoscopic right hemicolectomy using different approaches：（1） single incision multiport, （2） single access port and （3） glove port. Results There was no statistical difference in surgical outcomes. Concerning the surgeon＇s satisfaction toward three methods, overcrowding and durability were similar but the single incision multiport was associated with the highest gas-leak and the ＂glove＂ port was associated with poor durability. However, the method of single incision multiport has the lowest average cost of the special trocar or port in each operation. The operative time and blood loss of the operations in this study were comparable to previous publications. Conclusion There was no significant difference between different approaches of single incision laparoscopic right hemicolectomy for colonic cancer in right side colon.

Objective:To describe our experience on lead extraction in Hong ***:Pacing lead extraction is required when there is infection,lead fracture with protrudingfiagments and absence of venous access during lead ***:We performedthe first lead extraction in Hong Kong using the Cook sheath on March *** 1996,Spectranetics laser sheath using Excimer Laser System(Model CVX-300)was *** was performed under general anesthesis(Cook sheath)or conscious sedation(Laserextraction).Results:Twenty patients with a total of 26 leads were *** indicationsfor lead extraction were:*** retention wire of Aceufix lead(15 patients)2,Infectedpacing leads(4 patients).*** access problem(1 patient).9 leads were extracted withthe Cook sheath,the remaining 17 leads by larer-assisted *** were 20 atrial leadsand 6 ventricular leads from Telectrontics,*** and Intermedics,with thelongest irnplanted lead of 15 *** extraction eas achieved in 24/26(93%)of leadsIn one patient,fracture of outer insulation of Accutix at the superior vena cava occurred afierusccessful removal of the lead from the atrium,and this requited venotomy for *** of advancement of locking styler occurred in another patient with an uninfectedvetricular lead which was not extractde,There was no mortality nor any other majorcomplictaion,Re-implantation was performed at the same setting except for the 4 infectedcases Conelmslom:Lead extraction can be perfomned effectively and smfely with,either theCook or Lmser extraction method,the latter can be chieved without general *** extraction may be incteasingly used in the young patients to reduce the qumber ofhardwares inside the beart and for the purpose of schieving venous access,lends that areabandoned at the time of pacernaker change should be capped rather than cut to facilitate leadextraction in the futureA.
