Does immunohistochemical staining have a clinical impact in early gastric cancer conducted endoscopic submucosal dissection?

AIm: To evaluate clinicopathologic parameters and the clinical significance related lymphovascular invasion (LVI) by immunohistochemical staining (IHCS) in endoscopic submucosal dissection (ESD). mETHODS: Between may 2005 and may 2010, a total of 348 lesions from 321 patients (mean age 63 ± 10 years, men 74.6%) with early gastric cancer (EGC) who met indication criteria after ESD were analyzed retrospectively. The 348 lesions were divided into the absolute (n = 100, differentiated mucosal cancer without ulcer ≤ 20 mm) and expanded (n = 248) indica-tion groups after ESD. The 248 lesions were divided into four subgroups according to the expanded ESD indication. The presence of LVI was determined by factor Ⅷ-related antigen and D2-40 assessment. We compared LVI IHCS-negative group with LVI IHCSpositive in each group. RESULTS: LVI by hematoxylin-eosin staining (HES) and IHCS were all negative in the absolute group, while was observed in only the expanded groups. The positive rate of LVI by IHCS was higher than that of LVI by HES (n = 1, 0.4% vs n = 11, 4.4%, P = 0.044). LVI IHCS-positivity was observed when the cancer invaded to the mucosa 3 (m3) or submucosa 1 (Sm1) levels, with a predominance of 63.6% in the subgroup that included only Sm1 cancer (P m3 or Sm1 invasion by the tumor was significantly associated with a higher rate of LVI by IHCS, but no factor was significant in a multivariate analysis. There were no cases of tumor recurrence or metastasis during the median 26 mo follow-up. CONCLUSION: EGCs of the absolute group are immunohistochemically stable. The presence of LVI may be carefully examined by IHCS in an ESD expanded indication group with an invasion depth of m3 or greater.

Changes in Small Intestinal motility and Related Hormones by Acupuncture Stimulation at Zusanli(ST 36) in mice

Objective： To clarify the effects of acupuncture stimulation at Zusanli（ST 36） on the hormonal changes. methods： Eight-week-old male C57BL/6 mice received acupuncture stimulation at acupoint ST 36 or Quchi（LI 11） once a day for 3 or 5 days in the acupuncture-stimulated groups, but not received in the normal group（n=6 in each group）. On day 3 or 5, animals were given 0.1 m L of charcoal orally with a bulbed steel needle, 30 min after the last acupuncture stimulation. Ten minutes later, mice were anesthetized, and the intestinal transit and the concentrations of vasoactive intestinal peptide（VIP）, motilin, ghrelin and gastrin in the serum were measured. Results： Compared to no acupuncture stimulation, acupuncture stimulation at ST 36 for 5 days increased the intestinal transit and down-regulated the concentration of VIP and up-regulated the concentrations of motilin, ghrelin and gastrin（P〈0.05 or 0.01）, whereas acupuncture stimulation at LI 11 did not change them significantly（P〉0.05）. Conclusion： Acupuncture stimulation at ST 36 for 5 days enhances the small intestinal motility and regulates the secretion of hormones related to small intestinal motility.

AIm： To confirm the predictive factors for interferon （IFN）-α and ribavirin combination therapy for chronic hepatitis patients with hepatitis C virus （HCV） genotype lb. mETHODS： HCV RNA from 50 patients infected with HCV genotype lb was studied by cloning and sequencing of interferon sensitivity determining region （ISDR）, PKR- eIF2α phosphorylation homology domain （PePHD）. Patients were treated with IFN-α and ribavirin for 6 mo and grouped by effectiveness of the therapy. A variety of factors were analyzed. RESULTS： Our data showed that age, HCV RNA titer, and ISDR type could be used as the predictive factors for combined IFN-α and ribavirin efficacy. Characteristically, mutations in PePHD appeared only when the combination therapy was effective. Other factors, such as sex and alanine aminotransferase （ALT） level, were not related to its efficacy. Adjusting for age and HCV RNA titer indicated that the ISDR type was the most potent predictive factor. CONCLUSION： HCV RNA ISDR type is an important factor for predicting efficacy of IFN-α and ribavirin combination therapy in Korean patients.

metachronous bile duct cancer nine years after resection of gallbladder cancer

We report a rare case of a 74-year-old man with metachronous gallbladder cancer and bile duct cancer who underwent curative resection twice, with the operations nine years apart. At the age of 65 years, the patient underwent a cholecystectomy and resection of the liver bed for gallbladder cancer. This was a welldifferentiated adenocarcinoma, with negative resection margins （T2NOm0, stage Ⅰ B）. Nine years later, during a follow-up examination, abdominal computed tomography and mRCP showed an enhanced 1.7 cm mass in the hilum that extended to the second branch of the right intrahepatic bile duct. We diagnosed this lesion as a perihilar bile duct cancer, Bismuth type Ⅲ a, and performed bile duct excision, right hepatic Iobectomy and Roux-en-Y hepaticojejunostomy. The histological diagnosis was a well-differentiated adenocarcinoma with one regional lymph node metastasis （TINIm0, stage Ⅱ B）. Twelve months after the second operation, the patient is well, with no signs of recurrence. This case is compared with 11 other cases of metachronous biliary tract cancer published in the world medical literature.

Peroxisome proliferator-activated receptor-delta agonist ameliorated inflammasome activation in nonalcoholic fatty liver disease

AIm: To evaluate the inflammasome activation and the effect of peroxisome proliferator-activated receptors(PPAR)-δ agonist treatment in nonalcoholic fatty liver disease(NAFLD) ***: male C57BL/6J mice were classified according to control or high fat diet(HFD) with or without PPAR-δ agonist(GW) over period of 12 wk [control, HFD, HFD + lipopolysaccharide(LPS), HFD + LPS + GW group]. Hep G2 cells were exposed to palmitic acid(PA) and/or LPS in the absence or presence of ***: HFD caused glucose intolerance and hepatic steatosis. In mice fed an HFD with LPS, caspase-1 and interleukin(IL)-1β in the liver were significantly increased. Treatment with GW ameliorated the steatosis and inhibited overexpression of pro-inflammatory cytokines. In Hep G2 cells, PA and LPS treatment markedly increased m RNA of several nucleotide-binding andoligomerization domain-like receptor family members(NLRP3, NLRP6, and NLRP10), caspase-1 and IL-1β. PA and LPS also exaggerated reactive oxygen species production. All of the above effects of PA and LPS were reduced by GW. GW also enhanced the phosphorylation of AmPK-α.CONCLUSION: PPAR-δ agonist reduces fatty acidinduced inflammation and steatosis by suppressing inflammasome activation. Targeting the inflammasome by the PPAR-δ agonist may have therapeutic implication for NAFLD.

采用凹印机(最大速度500m/min)在PET(聚对苯二甲酸乙二醇酯)和BOPP(双向拉伸聚丙烯)承印物上进行印刷试验,以研究高速凹版印刷下油墨的转移情况和印刷适性。将水性油墨用酒精稀释后,在特定的印刷速度、印刷压力和网穴尺寸等多种印刷设备条件下对柔性承印物进行印刷。采用光学显微镜、UV可见光谱仪和原子力显微镜(AFm)测量印刷图像和油墨转移率。通过对界面张力、黏度以及油墨与承印物之间润湿角的测量与评价,定量分析它们对印刷特性的影响。实验结果表明,影响印刷特性的最重要的因素是油墨黏度、表面张力和界面张力。稀释后的油墨在PET和BOPP薄膜上的铺展系数都小于零,也就是说油墨难以在二者上顺利铺展,因此,扩散可能是由于剪切力导致油墨流动。显微镜或光谱仪测量表明,在各种试验印刷设备条件下,印刷速度不会对PET薄膜上印刷图像的均匀性和色度产生很大影响,而BOPP薄膜上的图像质量则在很大程度上依赖于转速。两者存在差异的主要原因在于BOPP薄膜多孔表面的毛细管堵塞导致油墨的铺展能力降低。此外,网穴尺寸(网点百分比)增大导致色度增大,其原因在于油墨和承印物间的接触面积变大。尤其对于BOPP薄膜来说,其印刷图像的色度会随着网点百分比的增大而线性增大。应该指出,在相同的稀释条件和设备条件下,较之PET薄膜来说,BOPP薄膜上的印刷图像的色度与墨层厚度远远高于PET薄膜。BOPP与PET薄膜在油墨转移率上的较大差异也许可以在一定程度上解释两者在界面张力上的差异,以及BOPP承印物上的吸收现象。

A Comparative Study of Antifatigue Effects of Taurine and Vitamin C on Chronic Fatigue Syndrome

Vitamin C and taurine (TR) are well known as active com.onents for fatigue recovery. However, the mechanism of the anti-fatigue effects of vitamin C and TR is still unclear. Our study was designed to evaluate the anti-fatigue activities of vitamin C and TR in an animal test for fatigue and to compare the activities between vitamin C and TR. materials and methods: Vitamin C, TR or their combination were orally administrated to mice once daily for 15 days, and then metabolic activities such as blood glucose, triglyceride (TG), lactate, and lactate dehydrogenase (LDH) as well as antioxidant activities such as malondialdehyde (mDA) and superoxide dismutase (SOD) were determined (evaluated) after forced swimming test (FST). Results: Compared with the control group, C100, C200, and T50 showed a tendency to decrease mobility in FST. moreover, TG (C100, C200, T200), LDH (C200), lactic acid (C100) and mDA (C50, C100, C200) levels were inhibited by vitamin C and TR. Conclusions: These results suggest that vitamin C and TR have anti-fatigue activities in mice, with vitamin C providing a stronger effect.
