Background As a consequence of the demographic development with increasing proportion of older people,the prevalence of heart failure(HF)is expected to rise with considerable economic and societal ***,knowledge on cardiac structure and function among population-based samples of the exceptional old is *** Population-based study of all persons(no exclusion criteria)living in the western part of Denmark and turning 100 years in the year ***-home face-to-face interviews were conducted,and echocardiography and blood sampling for plasma Brain Natriuretic Peptide(BNP)were offered to those who were able to give *** Out of 303 eligible,238(79%)participated,of which 125(53%)accepted *** ventricular(LV)dysfunction was present in 68(54%)of the participants of whom less than half had HF *** hypertension was present in 31(42%)with no correlation to LV *** well-known association between increased level of BNP and the prevalence of LV dysfunction could not be *** This in-home echocardiographic study shows that more than half of the participants had LV dysfunction,although mostly *** was no association between heart failure symptoms and LV ***,BNP seems to have lost its biomarker potential to rule out heart failure in *** to the latter,and the questionable symptom validity among centenarians,the current criteria to diagnose HF might be less valid in a centenarian population than in younger olds.

Accuracy between clinical and radiological diagnoses compared to surgical orbital biopsies

AIM:To assess the concordance between diagnosing orbital lesions by clinical examination,orbital imaging,and histological evaluation,in order to help guide future research and clinical ***:A retrospective analysis was undertaken at a large regional tertiary referral centre of all surgical orbital biopsies performed over a 5-year period,from 1st January 2015 until 31st December *** and concordance between clinical,radiological and histological diagnoses are reported as percentage sensitivity and positive predictive ***:A total of 128 operations involving 111 patients were ***,sensitivities of 47.7%for clinical and 37.3%for radiological diagnoses were found when compared to the histological gold *** lesions that have characteristic clinical and radiological features had the highest sensitivity at 71.4%and 57.1%,*** conditions showed the lowest sensitivity in both clinical(30.3%)and radiological(18.2%)*** PPV for inflammatory conditions were 47.6%for clinical and 30.0%for radiological ***:Accurate diagnoses are difficult to reach by relying on clinical examination and imaging *** orbital biopsy with histological diagnosis should remain the gold standard approach for definitively identifying orbital *** larger scale prospective studies would help further refine concordance and guide future research avenues.

Two Types of Diurnal Variations in Heavy Rainfall during July over Korea

This study examined the characteristics of the diurnal variations of heavy rainfall(≥110 mm in 12 hours)in Korea and the related atmospheric circulation for July from 1980−*** the analysis period,two dominant pattens of diurnal variation of the heavy rainfall emerged:all-day heavy rainfall(AD)and morning only heavy rainfall(MO)*** the AD-type,the heavy rainfall is caused by abundant moisture content in conjunction with active convection in the morning(0000−1200,LST;LST=UTC+9)and the afternoon hours(1200−2400 LST).These systems are related to the enhanced moisture inflow and upward motion induced by the strengthening of the western North Pacific subtropical high and upper-tropospheric *** the MO-type,heavy rainfall occurs mostly in the morning hours;the associated atmospheric patterns are similar to the *** find that the atmospheric pattern related to severe heavy rainfalls in 2020 corresponds to a typical AD-type and resembles the 1991 heavy-rainfall system in its overall synoptic/mesoscale *** present results imply that extremely heavy rainfall episodes in Korea during the 2020 summer may occur again in the future associated with the recurring atmospheric phenomenon related to the heavy rainfall.

High-resolution multi-scale air pollution system:Evaluation of modelling performance and emission control strategies

A high percentage of the world's population lives in areas where air pollutant concentrations exceed the World Health Organization *** work aims to develop and test,a high-resolution multi-scale air pollution modelling system by integrating a set of adequate *** system is able to provide detailed air pollutant concentrations in urban areas and support air quality management strategies through a better identification of different atmospheric *** also allows furthering the design and assessment of air pollution control measures for a specific *** evaluate its performance and suitability,the system was applied to the Macao Special Administrative Region(SAR),China,one of the most densely populated areas on earth,during a winter period when this area is affected by high levels of Particulate Matter(PM).Although the developed system tends to underestimate the PM concentrations,it revealed a good performance in reproducing the temporal and spatial air pollution *** exceedances of the Chinese air quality standards were calculated and high population exposure to PM pollution was *** tested urban atmospheric emission reduction scenarios have shown air quality improvements,indicating that emission reduction measures at urban level should focus on the domestic ***,it is crucial to implement joint pollution prevention strategies with neighbouring regions to improve the air quality in Macao *** approach developed in this work can support policymakers in defining new strategies to reduce atmospheric pollution in urban areas.

A homogeneous layer of Bi_2O_3-Bi_(14)WO_(24) composite(BWO/Bi_2O_3) thin film was fabricated using a combination of electrodeposition and thermal treatment. The evenly distributed Bi14 WO24 component within the Bi_2O_3 layer was found to be important in stabilising the photoelectrochemical performances of Bi_2O_3 photoanode by promoting the photoelectron transport. The unmodified Bi_2O_3 suffered from severe photocorrosion as proven by X-ray diffraction(XRD) and inductively coupled plasma(ICP) analyses while the composite thin film was active without noticeable activity decay for at least 3 h of illumination. This strategy might be applicable to other photocatalysts with stability issues.

Numerical simulations of cellular detonation diffraction in a stable gaseous mixture

In this paper,the diffraction phenomenon of gaseous cellular detonations emerging from a confined tube into a sudden open space is simulated using the reactive Euler equations with a two-step Arrhenius chemistry *** two-dimensional and axisymmetric configurations are used for modeling cylindrical and spherical expansions,*** chemical parameters are chosen for a stable gaseous explosive mixture in which the cellular detonation structure is highly *** mesh refinement(AMR)is used to resolve the detonation wave structure and its evolution during the *** numerical results show that the critical channel width and critical diameter over the detonation cell size are about 1371 and 2571,*** numerical findings are comparable with the experimental observation and confirm again that the critical channel width and critical diameter differ essentially by a factor close to 2,equal to the geometrical scaling based on front curvature *** unstable mixtures where instabilities manifested in the detonation front structure play a significant role during the transmission,the present numerical results and the observed geometrical scaling provide again evidence that the failure of detonation diffraction in stable mixtures with a regular detonation cellular pattern is dominantly caused by the global curvature due to the wave divergence resulting in the global decoupling of the reaction zone with the expanding shock front.

Composition of common bile duct stones in Chinese patients during and after endoscopic sphincterotomy

AIM： Endoscopic sphincterotomy （ES） is a well-established therapeutic modality for the removal of common bile duct （CBD） stones. Alter ES there are still around 10% of patients that experience recurrent CBD stones. The aim of this study is to investigate the composition of CBD stones before and alter ES and its clinical significance in Chinese ***： From January 1996 to December 2003, 735 patients with CBD stones received ES at Kaohsiung Veterans General Hospital and stone specimens from 266 patients were sent for analysis. Seventy-five patients had recurrent CBD stones and stone specimens from 44 patients were sent for analysis. The composition of the stones was analyzed by infrared （IR） spectrometry and they were classified as cholesterol or bilirubinate stones according to the predominant composition. Clinical data were ***： In the initial 266 stone samples, 217 （82%） were bilirubinate stones, 42 （16%） were cholesterol stones,3 were calcium carbonate stones, 4 were mixed cholesterol and bilirubinate stones. Patients with bilirubinate stones were significantly older than patients with cholesterol stones （66＋13 years vs 56＋17 years, P = 0.001）. In the 44 recurrent stone samples, 38 （86%） were bilirubinate stones, 3 （7%） were cholesterol stones, and 3 were mixed cholesterol and bilirubinate stones. In 27 patients, both initial and recurrent stone specimens can be obtained,23 patients had bilirubinate stones initially and 2 became cholesterol stones in the recurrent attack. Non-small-bowel lesions encountered during double-balloon enteroscopy performed for obscure gastrointestinal bleeding

AIM:To report the incidence of non-small-bowel bleeding pathologies encountered during double-balloon enteroscopy (DBE) procedures and to analyse their ***: A retrospective study of a prospective DBE database conducted in a tertiary-referral center was conducted. A total of 179 patients with obscure gastrointestinal bleeding (OGIB) referred for DBE from June 2004 to November 2008 were analysed looking for the incidence of non-small-bowel lesions (NSBLs; all and newly diagnosed) encountered during ***: There were 228 (150 antegrade and 78 retrograde) DBE procedures performed in 179 patients. The mean number of DBE procedures was 1.27 per patient. The mean age (SD) of the patients was 62 ± 16 years old. There were 94 females (52.5%). The positive yield for a bleeding lesion was 65.9%. Of the 179 patients, 44 (24.6%) had NSBLs (19 of them had dual pathology with small-bowel lesions and NSBLs); 27 (15.1%) had lesions not detected by previous endoscopies. The most common type of missed lesions were vascular ***: A significant proportion of patients (24.6%) had lesions within reach of conventional endoscopy. Careful repeat examination with gastroscopy and colonoscopy might be required.

Tunable rainbow light trapping in ultrathin resonator arrays

Rainbow light trapping in plasmonic devices allows for field enhancement of multiple wavelengths within a single ***,many of these devices lack precise control over spatial and spectral enhancement profiles and cannot provide extremely high localised field *** we present a versatile,analytical design paradigm for rainbow trapping in nanogroove arrays by utilising both the groove-width and groove-length as tuning *** couple this design technique with fabrication through multilayer thin-film deposition and focused ion beam milling,which enables the realisation of unprecedented feature sizes down to 5 nm and corresponding extreme normalised local field enhancements up to *** demonstrate rainbow trapping within the devices through hyperspectral microscopy and show agreement between the experimental results and *** combination of expeditious design and precise fabrication underpins the implementation of these nanogroove arrays for manifold applications in sensing and nanoscale optics.

Trochanter/calcar preserving reconstruction in tumors involving the femoral head and neck

AIM: To evaluate the results of hip reconstruction with extensive excision for tumor confined to the femoral head and neck. METHODS: We designed a resection preserving the greater trochanter and lower portion of calcar femorale, and utilized conventional total hip prosthesis. We retrospectively reviewed 7 patients, who underwent a wide resection and reconstruction using conventional hip prosthesis. There were 3 men and 4 women and their mean age was 42.5 years(22 to 65 years). The histologic diagnosis of each patient was low-grade osteosarcoma, diffuse large B-cell lymphoma, liposclerosing myxofibroma, intraosseous lipoma, chondroblastoma, giant cell tumor and focal intramedullary fibrosis. RESULTS: One patient with lymphoma died due to disease dissemination at 10 mo postoperatively and the remaining 6 patients were followed for a mean of 4.7 years(3 to 6 years). All patients were able to return to their daily activities and no patient had local recurrence. No radiographic signs of loosening, wear, and osteolysis were found at the last follow-up. CONCLUSION: Trochanter/calcar-preserving resectionof the proximal femur and reconstruction using conventional total hip prosthesis, is a satisfactory treatment for tumors confined to the femoral head and neck.
