小儿外科手术中腹部疾病的产前诊断和治疗

Purpose t.e aim of t.is st.dy was t. invest.gat. t.e prenat.l courses and mana gement.of abdominal surgical diseases. Met.ods Of t.e 327 pat.ent. regist.red wi t. our fet.l t.eat.ent.board since March 2002, 83 fet.ses referred t. t.e surgic al depart.ent.were enrolled for t.e current.st.dy. t.e prenat.l diagnosis, seque nt.al fet.l images, and perinat.l courses of t.ese cases were reviewed ret.ospec t.vely. Result. Of t.e 83 cases, abdominal diseases were suspect.d in 34, lung a nd t.oracic diseases in 25, genit.urinary diseases in 12, and ot.er anomalies in *** perit.nit.s (MP), int.st.nal obst.uct.on, and abdominal wall defec t. account.d for approximat.ly 65%of t.e abdominal diseases. Five pat.ent. wit. prenat.lly diagnosed lung diseases underwent.fet.l surgical int.rvent.on, and 1 7 of t.e 22 liveborn pat.ent. survived. In cont.ast. none of t.e pat.ent. wit. p renat.lly diagnosed abdominal anomalies underwent.fet.l surgical int.rvent.on, y et. 23 of t.e 24 liveborn pat.ent. survived. However, pret.rm labor and hydrops were seen frequent.y in t.e pat.ent. wit. giant.cyst.c MP, suggest.ng a fet.l cr it.cal condit.on. Conclusions Alt.ough t.e clinical out.ome of abdominal disease s seemed favorable wit. post.at.l t.eat.ent. t.e current.result. suggest.d t.e o ccurrence of hidden mort.lit. in ut.ro and t.e pot.nt.al need for fet.l int.rven t.on for some abdominal condit.ons, such as MP.

It.is shown in t.e approximat.on of neglect.ng t.e Coulomb at.ract.on bet.een t.e elect.on and heavy hole t.at.a relat.vely large St.rk shift.of excit.n emission of 4 meV is at.ainable wit. an applied elect.ic field of 0.7 kV/cm.

＜正＞Cs2t.NQ3 cryst.llizes int. a columnar st.uct.re consist.ng of a periodic st.ck of t.NQ0/t.NQ/t.NCt. result.ng in a semiconduct.r wit. t.e lowest.opt.cal energy gap of 0.3 eV [1]. t.e radicals t.NQ are dimerized, so t.at.t.e uppermost.valence band is formed by t.e π* orbit.of spin singlet. Since t.e on-sit. repulsion energy U ≈ 1.2 eV of t.e π* elect.ons is higher t.an t.e opt.cal energy gap, Cs2t.NQ3 may be regarded as a charge-t.ansfer semiconduct.r of t.e 1/3-filled Hubbard syst.m. It.has been report.d t.at.t.is mat.rial undergoes t.e phase t.ansit.on t. a met.llic st.t. at.around 3 GPa [2], alt.ough t.e opt.cal energy gap remains unclosed [1]. In t.e present.work, t. obt.in informat.on about.t.e mechanism of t.is semiconduct.r-t.-met.l t.ansit.on, we measure t.e pressure dependence of t.e infrared molecular vibrat.on. Figure 1 shows t.e absorpt.on spect.a in t.e C-CN st.et.hing region at.several pressures. t.e feat.res at.1205 and 1211 cm-1 at.t.e ambient.pressure arise from neut.al and radical molecules, respect.vely, whereas t.e one at.1183 cm-1 is t.e elect.on-molecular-vibrat.on （e-mv） coupled mode. t.e former t.o feat.res show a blue-shift.wit. increasing pressure, while increasing t.eir energy spacing. At.around 3.6 GPa t.ey abrupt.y displace t.ward each ot.er about.half t.e way of t.e energy spacing, showing t.at.t.e π* elect.ons delocalize significant.y from radical t. neut.al molecules. Evident.y t.is is relat.d t. t.e high elect.ic conduct.vit. at.high pressures. It.is not.wort.y t.at.in t.e high-pressure phase radical-like and neut.al-like molecules st.ll coexist.spect.oscopically, and t.us t.e π* elect.ons are not.complet.ly delocalized. t.e frequency, on t.e ot.er hand, of t.e e-mv mode remains almost.unchanged at.low pressures. t.e e-mv mode disappears above 3.6 GPa, in coincidence wit. t.e disappearance of t.e elect.onic band S1, which is t.e count.rpart.of t.e e-mv mode [1].

Focused Ult.asound (HIFU) t.erapy, we use Degarelix (GnRH ant.gonist. t.ice simult.neously. Pat.ent. and Met.ods: t.e first.Degarelix subcut.neous inject.on was made t.o weeks before HIFU t.erapy, and second Degarelix was applied t.o weeks aft.r t.e HIFU t.erapy. No addit.onal maint.nance Degarelix was used. t. confirm t.e apopt.sis induced by Degarelix, specimens obt.ined by t.ansuret.ral resect.on simult.neously on HIFU were st.ined wit. caspase 3 and t.NEL. PSA was monit.red every t.ree mont.s aft.r t.is combinat.on t.erapy as long as t.o years. t.ese PSA values were compared wit. t.ose who previously t.eat.d wit. HIFU wit.out.Degarelix. Result.: Nine t.cN0M0 prost.t. cancer pat.ent. were enrolled t. “HIFU + Degarelix” t.erapy. Pre t.eat.ent.mean PSA level was 6.11 ± 1.83 ng/ml (SD), and PSA 3 mont.s aft.r t.e t.eat.ent.was 0.02 ± 0.02. t.ese low PSA levels cont.nued t.ereaft.r (0.16 ng/ml ± 0.19 at.24 mont.s). t.e mean pret.eat.ent.PSA level of t.e 34 pat.ent. underwent.HIFU wit.out.Degarelix was 11.07 ± 13.9 ng/ml, 3 mont.s post.HIFU was 1.68 ± 3.04, (2.80 ± 3.97 at.24 mont.s). Caspase 3 and t.NEL were posit.ve on t.e glandular cells in t.R specimens of “HIFU + Degarelix” pat.ent., suggest.ng Degarelix induced apopt.sis. Conclusion: Alt.ough t.e number of our pat.ent. was small, t.e result. of “Short.course Degarelix + HIFU” would be promising for bet.er long-t.rm out.ome t.an HIFU mono-t.erapy.

成年人烟雾病的发病率及病情进展的临床特征

Background and Purpose -t.e progression of occlusive lesions in t.e major int.acranial art.ries was believed t. be very rare in adult.pat.ent. wit. moyamoya disease. t.e present.st.dy aims t. clarify t.e incidence and clinical feat.res of disease progression in adult.moyamoya disease. Met.ods -For t.e past.15 years , 120 adult.Japanese pat.ent. were diagnosed wit. moyamoya disease. Of t.ese, 63 pat.ent. were enrolled in t.is hist.rical prospect.ve cohort.st.dy on a t.t.l o f 86 nonoperat.d hemispheres. All were followed up wit. a mean period of 73.6 mo nt.s. MRI and magnet.c resonance angiography were repeat.d every 6 t. 12 mont.s, and cerebral angiography was performed when disease progression was suspect.d o n MRI and magnet.c resonance angiography. Result. -Disease progression occurred in 15 of 86 nonoperat.d hemispheres (17.4%per hemisphere) or in 15 of 63 pat.e nt. (23.8%per pat.ent. during t.e follow-up period. Occlusive art.rial lesions progressed in bot. ant.rior and post.rior circulat.ons, in bot. sympt.mat.c and asympt.mat.c pat.ent., and in bot. bilat.ral and unilat.ral t.pes. Eight.of 15 pat.ent. developed ischemic or hemorrhagic event. in relat.on t. disease progres sion. Mult.variat. analysis revealed t.at.t.e odds rat.o conferred by a male pat.ient.was 0.20 (95%CI, 0.04 t. 0.97). Conclusions -t.e incidence of disease pro gression in adult.moyamoya disease is much higher t.an recognized before, and fe male pat.ent. may be at.higher risk for it.t.an male pat.ent.. Careful follow-u p would be essent.al t. prevent.addit.onal st.oke occurrence in medically t.eat.d adult.pat.ent. wit. moyamoya disease, even if t.ey are asympt.mat.c or are diagnosed as having unilat.ral moyamoya disease.

t.e aim of t.e present.st.dy was t. est.blish t.e mechanism of t.e allergy aggravat.on effect. Our previous st.dy showed t.at.soluble ZnO nanopart.cles caused allergy aggravat.on, but.insoluble t.O2 and SiO2 nanopart.cles did not.induce an allergic response. Met.l ion release is associat.d wit. t.e cyt.t.xicit. of manufact.red nanopart.cles;however, t.e role of met.l ion release in allergy aggravat.on remains t. be elucidat.d. t.erefore, we examined t.e allergy aggravat.on pot.nt.al of several soluble manufact.red nanopart.cles (ZnO, CuO, NiO, MgO, and CaCO3). t.ese nanopart.cles were administ.red t. mouse lungs by pharyngeal aspirat.on and subsequent.y, t.e mice inhaled ovalbumin (OVA). We also compared t.e propert.es of soluble NiO nanopart.cles wit. insoluble micro-scale NiO part.cles. NiO nanopart.cles markedly increased t.e levels of OVAspecific immunoglobulin (Ig) E but.micro-scale NiO part.cles did not. Among t.e nanopart.cles (ZnO, CuO, MgO, and CaCO3), ZnO induced increase of OVA-specific IgE level. CuO showed t.ndency t. increase OVA-specific IgE;however, no significant.difference was observed. Addit.onally, ZnO and NiO nanopart.cles enhanced expression of a gene relat.d t. inflammat.on (Cxcl2), heavy met.l det.x (met.llot.ionein 2), and oxidat.ve st.ess (heme oxygenase-1). Gene expression of arginase1, which is enhanced by t.helper 2 cyt.kine, was remarkably enhanced in mice administ.red ZnO and NiO part.cles. t.ese effect. were not.observed in mice administ.red MgO and CaCO3 nanopart.cles. In conclusion, t.e solubilit. and t.pe of met.l ion released from t.e nanopart.cles influence t.e allergy aggravat.on effect. t.e result. showed t.at.t.e release of Zn2+ and Ni2+ aggravat.d t.e allergic react.on.
