Objective:To explore the serodiagnosis of hydatid cyst in human using different antigens of sheep(hydatid fluid,somatic and Lxcretory/secretory antigens of protoscolexi by ELISA and compares this result with commercial human ELISA ***:one hundred blood samples from patients with history of severe abdominal pain and cosinophilia were *** serum samples were obtained from surgically and pathologically confirmed cystic echinococcosis patients from Mashhad university hospital as positive control and 5 serum samples from infant under one year old as negative *** samples were centrifuged at 3000xg at 20℃for 15 min and sera were stored at-20℃.First,these samples were tested for the presence of antibody by commercial human *** was developed on microplates coated with hydatid fluid,Somatic and Excretory/secretory antigens of protoscolex of ***:The results of this study as analyzed by Kappa test showed that,hydatid fluid antigen could be used as a precise source of detection in indirect ELISA ***:Hydatid fluid in comparison with Excretory-secretory and somatic antigens showed more compatibility agreement in kappa test which can be used for further studies in development of any ELISA test for diagnosis of human hydatidosis.

BACKGrOUND:Endoscopic retrograde cholangiopancrea-tography(ErCP)is widely used to manage post-cholecystectomy bile ***,the best endoscopic intervention remains *** investigated the success of a 7 French double pigtail stent following sphincterotomy in the management of such bile leaks. METHODS:Between July 1998 and June 2008,48 patients were referred for ErCP for presumed post-cholecystectomy bile *** were confirmed at ErCP and managed by a combination of sphincterotomy and stent insertion unless contraindicated. rESULTS:Bile duct cannulation was successful in 44(91.7%)patients.A leak of the cystic duct was demonstrated in 19(43.2%)patients,the duct of Luschka in 11(25.0%),and the common hepatic duct in 5 (11.4%).Complete transection of the common bile duct occurred in 4 *** remaining patients had no cholangiographic evidence of a *** was performed in 34 patients.A 7 French double pigtail plastic stent was placed in all 35 patients with cholangiographic evidence of a bile *** bile leaks were demonstrated at a follow-up of 8-16 weeks and all stents were removed successfully. CONCLUSION:The combination of sphincterotomy and insertion of a 7 French double pigtail stent results in excellent outcomes in the management of post-cholecystectomy bile leaks.

Esophageal carcinoma is a highly lethal cancer associated with high morbidity and mortality. Esophageal squamous cell carcinoma and esophageal adenocarcinoma are the two distinct histological types. There has been significant progress in endoscopic diagnosis and treatment of early stages of cancer using resection and ablation techniques, as shown in several trials in the recent past. Earlier detection of esophageal cancer and advances in treatment modalities have lead to improvement in the 5-year survival from 5% to about 20% in the past decade. Endoscopic eradication therapy is the preferred modality of treatment in cancer limited to mucosal layer of the esophagus as there is very low risk of lymph node metastasis, leading to high cure rates, low risk of recurrence and with few adverse effects. The most common adverse events seen are strictures, bleeding and rarely perforation which can be endoscopically managed. In patients with recurrent advanced disease or invasive tumor, esophagectomy with lymph node dissection remains the mainstay of treatment. There is debate on post-endoscopic surveillance with some studies suggesting closer follow up with upper endoscopy every 6 mo for the first 1-2 years and then annually for the 3 years while others recommending the appropriate action only if symptoms or other abnormalities develop. Overall, the field of endoscopic therapy is still evolving and focus should be placed on careful patient selection using a multidisciplinary approach.

Iron deficiency anemia and iron overload conditions affect more than one billion people *** homeostasis involves the regulation of cells that export iron into the plasma and cells that utilize or store *** cellular iron balance in humans is primarily mediated by the hepcidin-ferroportin *** is the sole cellular iron export protein,and its expression is regulated transcriptionally,post-transcriptionally and ***,a hormone produced by liver cells,post-translationally regulates ferroportin expression on iron exporting cells by binding with ferroportin and promoting its internalization by endocytosis and subsequent degradation by *** of iron homeostasis leading to iron deposition in vital organs is the main cause of death in betathalassemia ***-thalassemia patients show marked hepcidin suppression,ineffective eiythropoiesis,anemia and iron ***-thalassemia is common in the Mediterranean region,Southeast Asia and the Indian subcontinent,and the focus of this review is to provide an update on the factors mediating hepcidin related iron dysregulation in beta-thalassemia *** this process may pave the way for new treatments to ameliorate iron overloading and improve the long term prognosis of these patients.

Thermal injury has been shown to result in suppression of many aspects of immunity. We studied the effect of cimetidine on suppressed cell mediated immunity in burned mice, utilizing a method of quantitating sensitivity to the contact antigen, 2,4-dinitrofluorobenzene. Daily administration of cimetidine resulted in maintenance of normal cell mediated immunity in these otherwise immunosuppressed animals. This effect may be secondary to the antagonism of cimetidine with histamine H2receptors on suppressor cells. Immunorestoration with cimetidine may improve resistance of the burned host to infectious complications.
