Glasgow prognostic score after concurrent chemoradiotherapy is a prognostic factor in advanced head and neck cancer

Objective: This study aims to evaluate the impact and potential prognostic roles of the pre- and post-treatment Glasgow prognostic score (GPS) and the change thereof in patients with advanced head and neck cancer undergoing concurrent chemoradiotherapy (CCRT). Methods: We collected GPS and clinicopathological data of 139 stage III, IVA, and IVB head and neck cancer patients who underwent CCRT between 2008 and 2011. Their GPSs pre- and post-CCRT and the change thereof were analyzed for correlations with recurrence and survival. Results: The GPS changed in 72 (51.8%) patients, with worse scores observed post-CCRT in 65 (90.3%) of the GPS changed patients. Patients in the improved GPS group showed a tendency toward better survival. From the multivariate analysis, the post-CCRT GPS level was an independent prognostic factor in addition to tumor stage. Conclusions: After CCRT, a high GPS was revealed to be an important predictor of survival for advanced head and neck cancer.

Comprehensive Nutrition Intervention with Omega-3 Fatty Acid, Micronutrient, and Probiotic-Enriched Regimens Improves Malnutrition in Patients with Head and Neck Cancers

Malnutrition is a common and unavoidable medical issue in patients with head and neck cancer during and after the treatment period, which deserves an intensive medical attention since nutrition status is often related with patients’ prognosis and treatment outcomes. In review of literature and our recent research, we found that an early and intensive nutrition intervention, including nasogastric tube feeding, minimized body weight loss, reduced treatmentinduced adverse effects, and increased survival benefits. Patients with BMI < 19 kg/m2 and were received the Ethanwell/ Ethanzyme regimen enriched with omega-3 fatty acids, micronutrients, and probiotics showed significantly less body weight loss. Patients with the malnourished status at the time of 3 months after treatment completion showed early recurrence and had significant negative effect on overall survival. In conclusion, comprehensive nutrition intervention should be started as early as the treatment begins and be continued following therapy completion, in order to prevent or overcome malnutrition in patients with head and neck cancer.

Effect of transcutaneous electrical nerve stimulation through acupoints of Pucan (BL 61) and Shenmai (BL 62) on intraocular pressure in patients with glaucoma： a randomized controlled trial

OBJECTIVE： To investigate the effects of transcutaneous electrical nerve stimulation （TENS） through acupoints on the intraocular pressure （lOP） in patients with glaucoma. METHODS： A single-blinded, randomized experimental design study was conducted. Patients from the North of Taipei Ophthalmology outpatient department diagnosed with intraocular hypertension were selected and were further enrolled if they met the research subject criteria. The patients who met all criteria, and who met no exclusion criteria became the participants. Participants were randomly assigned into the treated group, wherein the TENS electrodes （Qing Ming Medical Device Co., Ltd., New Taipei City, China） were applied with direct current （DC） on the acupoints Pucan （BL 61） and Shenmai （BL 62） for 20 min. The control group received no TENS electrode treatments. Measurements of lOP were taken four times using a pneumotonometer （Clinico Co., Ltd., New Taipei City, China）, before, immediately after, 30 min after, and 60 min after TENS electrode treatments. RESULTS： Eighty-two participants completed the experiment, lOP decreased more in the group treated with DC （P 〈 0.01） than in the control *** lOPs were not affected by the age of the patients and their usage of medications. CONCLUSION： TENS of acupoints of Pucan （BL 61） and Shenmai （BL 62） could significantly reduce the lOP in patients with glaucoma.

Advanced gastrointestinal stromal tumor patients with complete response after treatment with imatinib mesylate

AIM： Most gastrointestinal stromal tumors （GISTs） express constitutively activated mutant isoforms of kit kinase or platelet-derived growth factor receptor alpha （PDGFRA）, which are potential therapeutic targets for imatinib mesylate （Glivec）. Partial response occurred in almost two thirds of GIST patients treated with Glivec. However, complete response （CR） after Glivec therapy was sporadically reported. Here we illustrated advanced GIST patients with CR after Glivec treatment. METHODS： Between January 2001 and June 2005, 42 advanced GIST patients were treated with Glivec. Patients were administered 400 mg of Glivec in 100-mg capsules, taken orally daily with food. The response of the tumor to Glivec was evaluated after one month, three months, and every three months thereafter or whenever medical need was indicated. Each tumor of patients was investigated for mutations of kit or PDGFRA. RESULTS： The median follow-up time of the 42 ad-vanced GIST patients treated with Glivec was 16.9 months （range, 1.0- 47.0 months）. Overall, 3 patients had complete response CR （7.1%）, 26 partial response （67.8%）, 5 stationary disease （11.9%）, and 3 progressive disease （11.9%）. The median duration of Glivec administration for the three patients was 36 months （range, 23-36 months）. The median time to CR after Glivec treatment was 20 months （range, 9-26 months）. Deletion and insertion mutations of c-kit exon 11 and insertion mutation of c-kit exon 9 were found in two cases and one case, respectively. CONCLUSION： Complete response （CR） can be achieved in selected advanced GIST patients treated with Glivec. The median time to CR after Glivec treatment was 20 months. Deletion and insertion mutations of kit exon 11 and insertion mutation of kit exon 9 contribute to the genetic features in these selected cases.

AIM： To determine how glucocorticoids （GCs） may affect the growth and chemosensitivity of common carcinoma cells. METHODS： The effect of dexamethasone （DEX） on growth and chemosensitivity was assessed in 14 carcinoma cell lines. The function of GC receptors （GR） was assessed by MMTV reporter assay. Overexpression of GR was done by in vitro transfection and expression of a GR-expressing vector. Immunohistochemical stain of tissues and ceils were done by PA1-511A, an anti-GR monodonal antibody. RESULTS： DEX inhibited cell growth of four （MCF-7, MCF- 7/MXR1, MCF-7/TPT300, and HeLa）, increased cisplatin cytoxicity of one （SiHa）, and decreased dsplatin cytotoxicity of two （H460 and Hep3B） cell lines. The GR content of the seven cell lines affected by DEX was significantly higher than those of the seven cell lines unaffected by DEX （5.2±2.5×10^4 sites/cell vs1.3±1.4×10^4 sites/cell, P= 0.005）. Only two DEX-unresponsive cell lines {NPC-TW01 and NPC- TW04） oontained high GR amounts in the range （1.9-8.1×10^4 sites/cell） of the seven DEX-responsive cell lines. The GR function of NPC-TW01 and NPC-TW04, however, was foundto be impaired. The importance of high cellular amount of GR in mediating DEX susceptibility of the cells was further exemplified by GR dose-dependent drug resistance to cisplatin of AGS, a cell line with low GR content and was unaffected by DEX before transfection of GR-expressing vector. Immunohistochemical studies of human cancer tissues showed that 5 of the 45 （11.1%） breast cancer and 43 of the 85 （50.6%） non-small cell lung cancer had high GR contents at the ranges of the GC-responsive carcinoma cell lines. CONCLUSION： The growth and chemosensitivity of human carcinomas with high GR contents may be affected by GC. However, in light of the heterogeneous and even contradictive effects of GC on these cells, routine examination of GR contents of human carcinoma tissues may not be clinically useful until other markers that help predict the ultimate effect of

Prognostic analysis of patients with pancreatic head adenocarcinoma less than 2cm undergoing resection

AIM:To investigate the differences in clinicopathological features between patients with pancreatic cancer greater or less than 2 cm situated over the pancreatic head and the prognostic factors for survival of patients with pancreatic cancer2 cm)and 36 cases of small(S)-PAC(tumor≤2 cm).We compared the clinicopathological characteristics and prognosis of L-PAC and S-PAC *** clinicopathological characteristics of S-PAC were investigated to clarify the prognosis predictive factors of S-PAC. RESULTS:One hundred and fifty-nine PAC patients, aged 16-93 years(median,59.0 years)with a tumor at the pancreatic head undergoing intentional curative resection were *** S-PAC and L-PAC patients had similar demographic data,clinical features,and tumor markers(a similar positive rate of carcinoembryonic antigen and carbohy- drate antigen 19-9).There were also similar rates of lymph node metastasis,portal vein invasion,stage distribution,tumor differentiation,positive resection margin,surgical morbidity and mortality observedbetween the two *** a follow-up period ranging from 1.0 to 122.7 mo(median,10.9 mo), S-PAC and L-PAC patients had a similar prognosis after resection(P=0.4805).Among the S-PAC patients group,patients with higher albumin level(>3.5 g/dL) had more favorable survival than those with lower albumin levels,which was the only favorable predictive prognostic ***,early-staged(stageⅠ,Ⅱ) S-PAC patients tended to have a more favorable outcome than late-stage(stageⅢ,Ⅳ)S-PAC patients, but this was not statistically significant. Manufacture of a hollow corner cube retroreflector for next generation of lunar laser ranging

Lunar laser ranging has made significant contributions to the study of gravitational physics and the Earth-Moon system. The best results for fundamental gravitational experiments have been achieved using lunar laser ranging data accumulated so far. However, corner cube retroreflector arrays placed on the Moon currently set a limit on the laser-ranging precision, which is approximately several centimeters for a single photon received. To achieve millimeter precision, next generation of lunar laser ranging using a single hollow retroreflector with a large aperture has been proposed. We developed a prototype hollow retroreflector with a 100-mm aperture using silicate bonding together with a new fabrication method. Dihedral angle offsets of 0.5′′, 0.8′′and 1.9′′were realized, which partly come close to meeting the requirements(offset of 0.6′′for each dihedral angle) for lunar laser ranging. Fluctuation of the wavefront is approximately 1.038λ at 633 nm. A thermal cycle test ranging from -40℃ to +75℃ was carried out for 18.5 periods(approximately 5 d). After this test, the dihedral angle offsets were measured to be 0.39′′, 1.00′′and 2.06′′. The results indicate the potential application of our method for manufacturing a hollow retroreflector with a large aperture to realize lunar laser ranging.
