Uterine leiomyoma, a benign tumor, may be treated with hormone therapy, conventional surgical resection and uterine artery embolization (UAE): this paper reports the study on safety after UAE treatment. Pregnancy processes and prognoses of a total of 41 cases from 39 patients (two women were pregnant twice) who succeeded to pregnancy after UAE treatment in our clinic were studied. The average age of patients was 34.5 (29 - 40). Patients included 24 cases of multiple uterine leiomyoma and 14 cases of solitary leiomyoma;the average size of leiomyoma was 388.3 cm3 (15 - 1059 cm3) and the average period between UAE operation and pregnancy was 26.5 months (1 - 120). Causes of pregnancy were 29 cases of natural pregnancy and 12 cases of infertility treatment: 28 cases succeeded in delivery, 7 cases resulted in spontaneous abortion, and 6 cases are ongoing. Premature birth, placental abnormality and low-weight babies were observed in 3, 3, and 1 cases, respectively;the latter 1 case was of triplets. Although the incidence of placental abnormality was a little high, no other serious complications were observed, suggesting that pregnancy after UAE has no problem from the obstetric point of view. In the future, UAE is considered to be worthwhile if carefully done for cases who want to become pregnant.

Background: One-lung ventilation (OLV) is generally adopted for thoracic surgery. The systemic application of a high fraction of inspiratory oxygen (F1O2) and continuous positive airway pressure (CPAP) to the non-ventilated lung is useful for preventing arterial oxygen desaturation. The adverse effects of elevated F1O2 include oxidative lung injury, resorption atelectasis and coronary and peripheral vasoconstriction. It is preferable to avoid hyperoxemia in patients with complications such as chronic obstructive pulmonary disease, idiopathic pneumonia, and bleomycin-treated lungs. We aimed to determine whether the application of 60% O2 CPAP to the non-ventilated lung is sufficient to provide adequate oxygenation with 60% O2 to the ventilated lung. Methods: A total of 70 patients scheduled to receive elective thoracic surgery requiring OLV were recruited. Left double-lumen tubes were applicable in all surgeries. Patients were randomly allocated to one of two groups, to receive either 60% O2 CPAP (60% CPAP group, n = 35), or 100% O2 CPAP (100% CPAP group, n = 35) at a setting of 2 - 3 cmH2O, applied to the non-ventilated lung. Arterial blood gas analyses were obtained at the following stages: RA, spontaneous breathing under room air (RA);TLV, during total lung ventilation (TLV) prior to the initiation of OLV;T5, 5 min after the initiation of OLV;T15, 15 min after the initiation of OLV;T30, 30 min after the initiation of OLV. Results: The PaO2 value in 60% CPAP group vs. 100% CPAP group at each measurement were as follows: RA (mean [standard deviation: SD], 89.7 [8.2] mmHg vs. 85.8 [11.9] mmHg);TLV (277.9 [52.9] mmHg vs. 269.2 [44.0] mmHg);T5 (191.4 [67.9] mmHg vs. 192.3 [66.0] mmHg);T15 (143.2 [67.3] mmHg vs. 154.7 [60.8] mmHg) and T30 (95.6 [32.0] mmHg vs. 112.5 [36.5] mmHg), respectively. The aim of this study was to investigate the relationship among body image perceptions, a desire for thinness, and dieting behavior in young females in Japan. The subjects were 302 Japanese female university students (age 19.9 ± 1.4 years). An anonymous, self-administered questionnaire survey was conducted between July and August 2011. The questionnaire clarified the subjects’ physical status, perception about personal body shape, desired body shape, the time frame when they first thought about body shape dieting, exercise habits, and eating behaviors. The results revealed that many of the underweight or normal weight subjects perceived themselves as obese, and often the reasons for the perception of obesity were comparisons with others or with themselves in the past. The scores of EAT-26 in those who perceived themselves as obese were higher than the scores in those who did not. Those who perceived themselves as obese had a desire to become slim, were concerned about body shape at an early age, were concerned about dieting, had dieting experience, and were willing to increase daily exercise. These results indicated that incorrect perceptions about body shape have connection to a desire for thinness and dieting behavior.

The occurrence of lipodystrophy in patients taking antihuman immunodeficiency virus(HIV) medications is a serious problem as it is irreversible even after drug withdrawal. Although it was first recognized in patients taking proteinase inhibitors, other types of anti-HIV agents can also cause lipodystrophy. In a recent publication by Jones et al entitled "Highly active antiretroviral therapy dysregulates proliferation and differentiation of human pre-adipocytes" in World Journal of Virology, it was reported that simultaneous treatment of human subcutaneous adipocytes with anti-HIV drugs with different mechanisms of action synergistically exerted antiadipogenesis effects in vitro, warning us to take utmost care in every case receiving combination antiretroviral therapy(cART). For elucidation of the molecular basis for cART-related lipodystrophy, multi-faceted approaches should be taken, based on a deeper understanding of the development and organization of adipose tissues.

The Development of S-Equol Diastereoisomer Specific ELISA

Equol is a metabolite of soybean isoflavone, daidzein, and many health benefits are expected. Endogenous equol in urine is S-equol and mostly exists as glucuronate or sulfate conjugate. In this study we preliminary established the simple enzyme-linked immunosorbent assay (ELISA) without deconjugation, then developed the S-equol specific ELISA involves deconjugation showing high stereospecificity to S-equol without using stereospecific antibody. For the simple ELISA, we used a polyclonal antibody that targets the regions not influenced by inhibition by conjugation of glucuronate and sulfate and achieved the correlation coefficient;r = 0.975, but the value was 30 % lower than high performance liquid chromatography (HPLC). Developing upon this we invented the specific ELISA established from S-equol homogeneous combination for the standard and enzyme-labeled antigen to enhance stereospecificity. The correlation with HPLC was favorable: r = 0.986, y = 0.996x – 6. Compared to the previous method using (R,S)-equol combination, cross-reactivity with R-equol was reduced from 65 to 13 %, and that with daidzein from 0.31% to 0.08%, markedly increased in the specificity. This study is expected to be applied for both simple clinical researches, and stereospecific immunoassays in which specific antibody preparation is difficult.

Induction of Oral Tolerance in Neonatal Mice by Transfer of Food Allergens as IgA-Immune Complexes in Breast Milk

＜正＞Although semi-natural grasslands make an important contribution to regional biological diversity in Japan, methods to estimate this contribution have not been investigated. To evaluate the biological diversity of semi-natural grasslands resulting from the combination of the three data sources-plant communities, butterfly communities, and the interaction of both communities-, three areas in the sub-alpine zone in central Japan were surveyed. The number of butterflies and the frequency of interactions with plants were recorded by the transect method within August and September 2002 at the same time when vegetation surveys were carried out. The biological diversity of a plant community as a semi-natural grassland in the Mt. Yashigamine was higher than that of the other areas examined, while the biological diversity of the Mt. Nyuugasa was higher than that of the Mt. Ikeyama （Mori and Okubo 2005）. From the number of butterfly species and individuals and the characteristic species, Mt. Yashigamine was evaluated as having the highest biological diversity, with Mt. Nyuugasa second and Mt. Ikeyama third. From the results of the ER-index value analysis, the three study areas were similarly estimated to represent an environmental condition between the primitive stage and afforested stage. Nevertheless, the ER-index for the afforested stage at Mt. Ikeyama was the highest of the three areas, while the ER-index and Rl-index results from Mt. Ikeyama were lowest. The biological diversity of butterfly communities was the same as that of the plant communities. We postulate that the similarity in the two sets of results is because the plant communities influence the butterfly communities through the supply of feed and habitat. It appears that the Mt. Yashigamine scored the highest biological diversity because it occurs within a larger contiguous area of semi-natural grassland. The results of the frequency of nectar feeding at Mt. Various opinions have been presented on the merits and demerits that breastfeeding gives for the allergic onset of the babies. In this report, we focused on whether food proteins eaten by mother mice and secreted into breast milk as IgA-immune complexes contribute to the allergy prevention through oral tolerance in infants who ingest the milk. BALB/c mice were divided into two groups;E-group fed only egg white proteins and M-group fed only cow’s milk proteins as a dietary protein source. After immunizing M-group infants fed their own mother’s milk with ovalbumin/alum, diarrhea associated with experimental Th2 intestinal inflammation was induced by oral administration of ovalbumin. The diarrhea was dramatically suppressed in E-group infants. Concomitantly, low level of serum anti-ovalbumin- and ovomucoid-IgG1 and IgE, suppression of IL-4 synthesis by spleen cells, and low incidence of anaphylactic death after intravenous injection of ovalbumin were observed preferentially in E-infants. Immune complexes of respective dietary proteins and IgA were found in the breast milk obtained from each group of mother. Oral administration of pseudo immune complex chemically synthesized with ovalbumin and monoclonal mouse IgA in advance effectively suppressed anti-ovalbumin-IgG1 synthesis in adult mice after immunization with ovalbumin. The tolerance induced by the pseudo immune complex of ovalbumin diminished spontaneously while mice did not take egg white proteins. Thus, immune tolerance and then prevention of allergic disorder against dietary proteins were acquired via breastfeeding by mothers feeding the relevant proteins, probably through the immune complexes of dietary proteins and sIgAs secreted into breast milk.

Factors Influencing Behavior of Taking Medicine in Elderly Patients Undergoing Hemodialysis

Purpose: The elderly are often faced with multiple diseases, in particular hemodialysis (HD) which requires many kinds of medication. This study examined the factors influencing the behavior of taking medicine in elderly patients undergoing HD. Subjects and Methods: The subjects comprised 70 outpatients > 65 years undergoing HD (48 male and 22 female). The mean age of the patients was 72.5 ± 4.4 years. The mean duration of dialysis history was 6.1 ± 5.2 years. We performed a questionnaire survey using Medication Assessment Tool, The Kidney Disease Quality of Life (QOL)-Short Form and Acceptance for Dialysis Instrument. Result: Two factors related to poor behavior of taking medicine: dialysis history < 2 years and poor acceptance of dialysis. The multiple regression analysis showed patient satisfaction (β = –0.329, p < 0.01) and symptoms/problems (β = –0.273, p<0.05) as significant independent variables relating to behavior of taking medicine. Conclusion: Poor behavior of taking medicine is related to a short dialysis period, poor acceptance and poor satisfaction of dialysis therapy in elderly patients.
