The Development of S-Equol Diastereoisomer Specific ELISA

Equol is a metabolite of soybean isoflavone, daidzein, and many health benefits are expected. Endogenous equol in urine is S-equol and mostly exists as glucuronate or sulfate conjugate. In this study we preliminary established the simple enzyme-linked immunosorbent assay (ELISA) without deconjugation, then developed the S-equol specific ELISA involves deconjugation showing high stereospecificity to S-equol without using stereospecific antibody. For the simple ELISA, we used a polyclonal antibody that targets the regions not influenced by inhibition by conjugation of glucuronate and sulfate and achieved the correlation coefficient;r = 0.975, but the value was 30 % lower than high performance liquid chromatography (HPLC). Developing upon this we invented the specific ELISA established from S-equol homogeneous combination for the standard and enzyme-labeled antigen to enhance stereospecificity. The correlation with HPLC was favorable: r = 0.986, y = 0.996x – 6. Compared to the previous method using (R,S)-equol combination, cross-reactivity with R-equol was reduced from 65 to 13 %, and that with daidzein from 0.31% to 0.08%, markedly increased in the specificity. This study is expected to be applied for both simple clinical researches, and stereospecific immunoassays in which specific antibody preparation is difficult.

Various opinions have been presented on the merits and demerits that breastfeeding gives for the allergic onset of the babies. In this report, we focused on whether food proteins eaten by mother mice and secreted into breast milk as IgA-immune complexes contribute to the allergy prevention through oral tolerance in infants who ingest the milk. BALB/c mice were divided into two groups;E-group fed only egg white proteins and M-group fed only cow’s milk proteins as a dietary protein source. After immunizing M-group infants fed their own mother’s milk with ovalbumin/alum, diarrhea associated with experimental Th2 intestinal inflammation was induced by oral administration of ovalbumin. The diarrhea was dramatically suppressed in E-group infants. Concomitantly, low level of serum anti-ovalbumin- and ovomucoid-IgG1 and IgE, suppression of IL-4 synthesis by spleen cells, and low incidence of anaphylactic death after intravenous injection of ovalbumin were observed preferentially in E-infants. Immune complexes of respective dietary proteins and IgA were found in the breast milk obtained from each group of mother. Oral administration of pseudo immune complex chemically synthesized with ovalbumin and monoclonal mouse IgA in advance effectively suppressed anti-ovalbumin-IgG1 synthesis in adult mice after immunization with ovalbumin. The tolerance induced by the pseudo immune complex of ovalbumin diminished spontaneously while mice did not take egg white proteins. Thus, immune tolerance and then prevention of allergic disorder against dietary proteins were acquired via breastfeeding by mothers feeding the relevant proteins, probably through the immune complexes of dietary proteins and sIgAs secreted into breast milk.

The Cardiac Function in the Beach Chair Position under General Anesthesia

Background: Shoulder surgery is performed in the beach chair position (BCP). The systemic arterial blood pressure (BP) must be increased to prevent cerebral hypoperfusion. However, it is not clear how the cardiac function is affected when BP increase to maintain cerebral perfusion pressure in anesthetized patients. Methods: An analysis was performed using the data from 13 patients. We prepared a parallel circuit using a FloTrac Sensor transducer and an arterial BP transducer. Following the transfer of the patient to the BCP under general anesthesia, the FloTrac Sensor transducer was placed at the level of the fourth intercostal space, the arterial BP transducer was placed at the external auditory meatus level. We selected two points before surgery (120 s apart), during which the mean arterial BP (mABP) at the level of the brain was stable and at which the values in the supine position and the BCP were within 5 mmHg. Results: While the patients were in the supine position, the mean mABP at the mid-axillary level was 65.7 mmHg. In the BCP, the mean mABP was 66.5 mmHg at the external auditory meatus and 80.7 mmHg at the fourth intercostal space. The cardiac index changed from 2.2 (supine position) to 2.5 l/min/m2 (BCP). The stroke volume index was significantly increased from 35.8 to 42.3 ml/m2 (P = 0.003). The heart rate changed from 63.0 to 58.6 beats/min. The stroke volume variation was significantly decreased from 12.4% to 8.8% (P = 0.024). Conclusion: In order to ensure patient safety, close attention should be paid to the systemic cardiovascular changes that occur when the BP is increased.

Factors Influencing Behavior of Taking Medicine in Elderly Patients Undergoing Hemodialysis

Purpose: The elderly are often faced with multiple diseases, in particular hemodialysis (HD) which requires many kinds of medication. This study examined the factors influencing the behavior of taking medicine in elderly patients undergoing HD. Subjects and Methods: The subjects comprised 70 outpatients > 65 years undergoing HD (48 male and 22 female). The mean age of the patients was 72.5 ± 4.4 years. The mean duration of dialysis history was 6.1 ± 5.2 years. We performed a questionnaire survey using Medication Assessment Tool, The Kidney Disease Quality of Life (QOL)-Short Form and Acceptance for Dialysis Instrument. Result: Two factors related to poor behavior of taking medicine: dialysis history < 2 years and poor acceptance of dialysis. The multiple regression analysis showed patient satisfaction (β = –0.329, p < 0.01) and symptoms/problems (β = –0.273, p<0.05) as significant independent variables relating to behavior of taking medicine. Conclusion: Poor behavior of taking medicine is related to a short dialysis period, poor acceptance and poor satisfaction of dialysis therapy in elderly patients.

BACKGROUND Fistula formation is a severe adverse event related to antiangiogenetic agents such as bevacizumab and inferior mesenteric arteriovenous fistula(IMAVF) is a result of acquired factor, especially colon surgery. However, IMAVF occurs very rarely and there are few reports in patients during chemotherapy. We report a case of a patient who developed IMAVF during treatment with bevacizumab in metastatic colorectal cancer(mCRC) after colon *** SUMMARY An 81-year-old man was diagnosed with descending colon cancer and underwent left hemicolectomy without any complications. He was definitely diagnosed with high-risk stage 2 and received tegafur-uracil plus leucovorin as adjuvant chemotherapy. Three years and 6 mo after the operation, the cancer relapsed with peritoneal dissemination. The patient underwent CyberKnife radiosurgery targeting the recurrent tumor and received chemotherapy with S-1 plus bevacizumab. At 1 year after chemotherapy, he complained of severe diarrhea, which is suspected drug-induced colitis. As diarrhea worsened despite the termination of treatment, he underwent colonoscopy and computed tomography (CT) scans that revealed edematous change from sigmoid to rectosigmoid colon. CT scans also revealed an aneurysm adjacent to the inferior mesenteric vein and multidetector CT angiography showed the IMAVF. Elective angiography confirmed the diagnosis of an IMAVF and it was successfully treated by arterial embolization. The patient resumed chemotherapy with only S-1 6 mo after *** Clinicians should keep in mind the probability of severe diarrhea arose from IMAVF in mCRC patients treated with bevacizumab.

Purpose: To identify factors related to changes in foot status and foot care in patients with diabetes mellitus participating in an exercise program. Methods: The subjects were 21 patients with Type 2 diabetes mellitus (8 males, 13 females). The mean age of the patients was 62.3 ± 12.5 years old. Foot status, physical measurements and daily activities were monitored and several surveys were performed. Results: Changes in foot status were significantly positively correlated with walking at normal speed (r = 0.54, p < 0.05) and fast speed (r = 0.68, p p < 0.05). Foot care showed a significant negative correlation with burden of diet therapy (r = -0.48, p < 0.05) and a significant positive correlation with the self-efficacy of diet therapy (r = 0.65, p < 0.01). Conclusions: Subjects who were fast walkers showed greater changes in foot status. Poor foot care behavior is also related to greater changes in foot status. Regular interventions for foot care are important to improve self-care behavior during an exercise program.

当说话者需要使用多个形容词来描述物体时,并非随机排列形容词的顺序,而是会呈现某种规律。对形容词顺序的认知和交流限制是语言产生领域一个重要的问题。本研究旨在通过探究汉语中形容词的表征和排序过程来进一步探讨这一问题。研究分为两个实验进行。其中,实验1主要探究参照性交流中形容词的语义特征(与交流目的相关的特征)是否影响不同类型的形容词(颜色vs.大小)的表达顺序。实验以42名母语为汉语的大学生为被试,采用对话范式,以参照环境(颜色相关vs.大小相关vs.颜色、大小都相关)为被试内变量,被试描述中的形容词顺序为因变量。实验结果发现,形容词的语义特征影响形容词的顺序:颜色相关时,被试描述中出现"颜色-大小"顺序显著多于"大小-颜色"顺序;大小相关时,"大小-颜色"顺序显著多于"颜色-大小"顺序。此外,通过进一步分析被试描述中"的"数量发现,名词结构会影响形容词的顺序偏好:当描述中带有2个"的"时,存在"大小-颜色"偏好,即语义偏好(内在的特征更接近名词),但参照环境的影响不显著,即无区分性影响;当描述中带有1或0个"的"时,无"大小-颜色"偏好,区分性影响显著,即存在交流偏好(前置促进交流目的达成的特征)。实验2则通过采用启动范式和对话范式来比较被试描述中不同数量"的"的短语结构,进一步探究形容词的语义特征如何影响不同类型形容词(颜色vs.大小)的表达顺序,以及句法结构在其中的作用。实验以44名母语为汉语的大学生为被试,以启动("颜色-大小"顺序启动vs."大小-颜色"顺序启动)和参考环境(颜色相关vs.大小相关)为被试内变量,因变量同样是被试描述中的形容词顺序。结果发现,启动效应显著,区分性影响不显著,"的"对形容词顺序的影响显著。当描述中带有0个"的"时,语义偏好显著,而当描述中带有1个或者2个"的"时,语义偏好不显著。本研究通过上述两个实验首次验证了汉语句法结构对参照性交流中形容词顺序偏好的影响,即线性结构的描述存在语义偏好,层级结构的描述存在交流偏好。

Purpose: Nedaplatin (CDGP) is the one of the platinum-derivatives to further improve the anti-tumor effect and to reduce adverse effects of cisplatin, such as renal toxicity。The present study evaluated the efficacy and adverse events of chemotherapy by superselective intra-arterial infusion of CDGP and 5-Fu combined with concurrent radiotherapy in patients with advanced squamous cell carcinoma of the oral cavity。Methods- Eighteen patients were treated with CDGP plus 5Fu in combination with concurrent irradiation of 40-60Gy。Twelve patients were treated as a preoperative therapy and 6 patients were as a definitive therapy。Eleven patients had stage III carcinomas and 7 patients had stage IV。Results: The overall clinical response rates were 88.9% and histological effects according to the grading system of Shimosato et al。were seen in 83.3%。Adverse events were observed in all patients。Mucositis, leucopenia and thrombocytopenia (>Grade 2 )were seen in 15 (83.3%), 12(66.7%) and 8(44.4%) of 18 patients, respectively。With a follow-up ranging from 8-45 months, 3 years overall survival rates are 69.9% for all patients, and 81.5% as a preoperative therapy, 50.0% as a definitive therapy。Conclusion: Concurrent chemoradiotherapy using CDGP plus 5-Fu was tolerated with good clinical and histological effects, resulting in good local control for advanced oral carcinoma。
