Pulmonary alveolar microlithiasis： an interesting case report with systematic review of Indian literature

Pulmonary alveolar microlithiasis is a rare disease characterized by intra-alveolar presence of microliths. This study reports an interesting case of pulmonary alveolar microlithiasis and provides a systematic review of cases reported from India. A 23-year-old female presented with a history of cough, wheeze, chest pain, and episodic wheeze for five months. Pulmonary function tests demonstrated an obstructive pattern, and chest X- ray showed fine micronodular opacities predominantly involving the middle and lower zones of both lungs. Transbronchial lung biopsy revealed the diagnosis. She responded well to inhaled steroid therapy. A systematic review of literature was performed and identified 73 cases of pulmonary alveolar microlithiasis reported from India. The mean （SD） age of the patients was 28.8 （14.9） years, with an almost equal male：female ratio. Many patients were asymptomatic at presentation. Breathlessness and cough were the most common symptoms, and the disease progressed into respiratory failure associated with cor pulmonale. About one-third of the cases were initially misdiagnosed and treated as pulmonary tuberculosis. Extra-pulmonary manifestations and co- morbidities were also evident in our series. This systematic review helps to determine epidemiological and clinical characteristics of pulmonary alveolar microlithiasis. Further research is needed to elucidate the etiopathogenesis, diagnosis, and therapeutic options, which are beneficial in developing and identifying cost- effective treatment for pulmonary alveolar microlithiasis.

Gamma Radiation Shielding Properties of Steel and Iron Slags

Using steel slag and two types of (soft and hard) iron slags, ten samples were prepared. Different gamma radiation interaction parameters were computed theoretically and measured experimentally at different energies: 60 keV, 136 keV, 662 keV, 1173 keV and 1332 keV in low and medium energy range using narrow transmission geometry. It has been observed that shielding effectiveness of a sample is inversely proportional to Half Value Layer (HVL). The obtained results were compared with Pure Flyash and it was observed that slag is better aggregate than flyash in shielding radiation as well as in construction applications. The results have been presented in the form of tables and graphs with more useful conclusions.

Synthesis and Optical Characterization of Silver Doped Sodium Borate Glasses

Silver doped sodium borate glasses prepared by melt-quenching technique were checked by XRD technique for their amorphous nature. It is observed that the molar volume increases with increasing Ag2O content leading to open struc- ture. Fourier Transform Infrared spectroscopy (FTIR) reveals the formation of BO3 and BO4 groups upon addition of silver oxide as modifier. From the Ultraviolet-Visible (UV-VIS) absorption spectra it is seen that the optical band gap increases with the increase of Ag2O content. Urbach energy is observed between 0.55 - 0.77 eV. The results obtained from molar volume, Fourier Transform Infrared spectroscopy and band gap energy measurements are in agreement with each other and nearly give the similar information about the studied glasses.

Synthesis and Characterization of Cadmium Containing Sodium Borate Glasses

Glass system (80% B2O3 – (20 – y)% Na2CO3 – y% CdO, where y = 3, 6, 9, 12 and 15 mol%) prepared by meltquenching technique and checked by XRD technique reveals the amorphous nature of these glasses. Increases in density from 2.18 to 2.82 (g/cm3) were observed with an increase in CdO content. Ultraviolet-Visible (UV-VIS) absorption spectra of polished parallel surfaces glass samples showed a decrease in the optical band gap from 3.72 to 3.59 eV with increasing CdO content. The results suggest that CdO content in glass samples plays a role as a network modifier. In addition, a correlation between the results from density, UV-VIS and FTIR measurements indicates that change in the atomic structure is due to the formation of BO4 units.

Cryptococcosis is the third most commonly occurring invasive fungal disease in solid organ transplant recipients(SOT).It is caused by encapsulated yeast,Cryptococcus species,predominantly Cryptococcus neoformans and Cryptococcus *** kidney transplant recipients are at the lowest risk of cryptococcosis when compared to other solid organ transplant recipients such as lung,liver or heart,still this opportunistic infection causes significant morbidity and mortality in this subset of *** rates with cryptococcosis range from 10%-25%,while it can be as high as 50%in SOT recipients with central nervous system *** main aim of diagnosis is to find out if there is any involvement of the central nervous system in disseminated disease or whether there is only localized pulmonary involvement as it has implications for both prognostication and *** of cryptococcal antigen(CrAg)in cerebrospinal fluid or plasma is a highly recommended test as it is more sensitive and specific than India ink and fungal *** CrAg lateral flow assay is the single point of care test that can rapidly detect cryptococcal polysaccharide *** of cryptococcosis is challenging in kidney transplant *** from the reduction or optimization of immunosuppression,lipid formulations of amphotericin B are preferred as induction antifungal *** and maintenance are done with fluconazole;carefully monitoring its interactions with calcineurin *** review further discusses in depth the evolving developments in the epidemiology,pathogenesis,diagnostic assays,and management approach of cryptococcosis in kidney transplant recipients.
