目的评价naldemedine单、多次给药在中国健康成人受试者中的安全性、耐受性和药代动力学特征。方法10例中国健康受试者单、多次口服naldemedine后,用液相色谱质谱联用仪检测血浆中naldemedine以及代谢物(nor-naldemedine)的浓度,经Phoenix WinNonlin 8.3.1软件计算药代动力学参数。结果相比于单次给药,多次给药后naldemedine的tmax(单次为1.06 h,多次为1.08 h)、C_(max)(单次为4.16 ng·mL^(-1),多次为4.11 ng·mL^(-1))均无明显变化,AUC0-τ高于单次给药(单次为23.88 ng·h·mL^(-1),多次为28.93 ng·h·mL^(-1)),t_(1/2)稍有延长(单次为9.26 h,多次为12.50 h),多次给药后有轻微蓄积(C_(max)的蓄积比为0.99,AUC的蓄积比为1.22);nor-naldemedine的t_(1/2)(单次为18.10 h,多次为29.30 h)、C_(max)(单次为0.19 ng·mL^(-1),多次为0.38 ng·mL^(-1))和AUC0-τ(单次为3.13 ng·h·mL^(-1),多次为6.02 ng·h·mL^(-1))均明显升高,tmax(单次为3.49 h,多次为3.56 h)无明显变化。代谢物与naldemedine的C_(max)比值和AUC比值明显升高,多次给药后存在蓄积(C_(max)的蓄积比为2.01,AUC的蓄积比为1.95)。所有治疗后出现的不良事件均为轻度。结论中国健康受试者连续多次(第1天单次、第4~13天每天1次)口服naldemedine片0.2 mg后,在体内消除较快,有轻度蓄积,在多次给药阶段第2次给药前达到稳态,且有良好的安全性和耐受性。

EVAPOTRANSPIRATION OF LOW-LYING PRAIRIE WETLAND IN MIDDLE REAChES OF hEIhE RIVER IN NORThWEST ChINA

Low-lying prairie wetland, which has characteristics of both grassland and wetland, has irreplaceable ecological functions in inland river basins of Northwest China. Owing to its small-scale distribution, so far, the observation and research on it are rare. The estimation of evapotranspiration is significant to ecological and environmental construction, scientific management of pasture and protection of wetland. For studying the evapotranspiration （ET） of low-lying prairie wetland in the middle reaches of the heihe River, an inland river, in Northwest China, the automatic weather station in Linze Ecological Experimental Station of Lanzhou University （39°15′ 3″N, 100°03′ 52″ E）, Linze, Gansu Province, was selected as a case study. Based on meteorological data collected, Bowen-Ratio Energy Balance （BREB） method was used to calculate the evapotranspiration （ET） of low-lying prairie wetland. The analysis results showed that in a whole year （September 2003 -August 2004）, the total ET was 611.5mm and mean daily 1.67mm/d. The ET varied with different growing stages. In non-growing stage （NGS）, initial growing stage （IGS）, middle growing stage （MGS） and end growing stage （EGS）, the ET was 0.57, 2.01, 3.82 and 1.49mrrdd, with a percentage of total ET of 18.26%, 9.20%, 61.83% and 10.71% respectively. In March, ET began to increase. But in April, the ET increased most. After that, it increased gradually and got the maximal value in July. From then on, the ET decreased gradually. In September, the ET decreased rapidly. With the ending of growing and the freezing of soil, the ET stopped from the middle of November to February in next year. hourly ET analysis showed that at 8：00 a.m. （during MGS at 7：00 a.m.）, the evapotranspiration began, at 13：00 p.m. got its maximal value and at 19：00 p.m. （during MGS at 20：00 p.m.）, the evapotranspiration stopped. The intensity of ET in sunny day was much larger than that in cloudy day in the same growing stage.

helicobacter heilmannii sensu stricto-related gastric ulcers:A case report

A spiral bacterium (Sh9), morphologically different from helicobacter pylori (h. pylori), was found in a 62-year-old woman’s gastric mucosa. Gastroscopic examination revealed multiple gastric ulcers near the pyloric ring; mapping gastric biopsy showed mild mononuclear infiltration with large lymphoid follicles in the antrum, without corpus atrophy. Urea breath test and h. pylori culture were negative, but Giemsa staining of biopsies revealed tightly coiled bacteria that immunostained with anti-h. pylori antibody. Sequencing of Sh9 16S rRNA and the partial urease A and B subunit genes showed that the former sequence had highest similarity (99%; 1302/1315 bp) to helicobacter heilmannii (h. heilmannii) sensu stricto (h. heilmannii s.s.) BC1 obtained from a bobcat, while the latter sequence confirmed highest similarity (98.3%; 1467/1493 bp) to h. heilmannii s.s. hU2 obtained from a human. The patient was diagnosed with multiple gastric ulcers associated with h. heilmannii s.s. infection. After triple therapy (amoxicillin, clarithromycin, and lansoprazole) with regimen for eradicating h. pylori, gastroscopy showed ulcer improvement and no h. heilmannii s.s. upon biopsy.

Is hepatic arterial infusion chemotherapy effective treatment for advanced hepatocellular carcinoma resistant to transarterial chemoembolization?

AIM:To evaluate the effectiveness of hepatic arterial infusion chemotherapy(hAIC) for advanced hepatocellular carcinoma(hCC) resistant to transarterial chemoembolization(TACE).METhODS:This study was conducted on 42 patients who received hAIC for advanced hCC between 2001and 2010 at our hospital.5-fluorouracil(5-FU) was administered continuously for 24 h from day 1 to day 5 every 2-4 wk via an injection ***-arterial cisplatin or subcutaneous interferon was administered in combination with the *** patients enrolled in this retrospective study were divided into two groups according to whether or not they fulfilled the criteria for resistance to TACE proposed by the Japan Society of hepatology in 2010(written in Japanese);one group of patients who did not fulfill the criteria for TACE resistance(group A,n = 23),and another group who fulfilled the criteria for TACE resistance(group B,n = 19).We compared the outcomes in terms of the response and survival rates between the two ***:Both the response rate and tumor suppression rate following hAIC were significantly superior in group A than in group B(response rate:48% vs 16%,P = 0.028,tumor suppression rate:87% vs 53%,P = 0.014).Furthermore,both the progression-free survival rate and survival time were significantly superior in group A than in group B(3-,6-,12-,and 24-mo = 83%,70%,29% and 20% vs 63%,42%,16% and 0%,respectively,P = 0.040,and 9.8 mo vs 6.2 mo,P = 0.040).A multivariate analysis(Cox proportional hazards regression model) showed that resistance to TACE was an independent predictor of poor survival(P = 0.007).CONCLUSION:hAIC administrating 5-FU was not effective against advanced hCC resistant to *** tools for treatment,i.e.,molecular-targeting agents may be considered for these cases.

To understand rice types that were utilized during postdomestication and in the modern age and the potential of genetic research in aged rice materials,archaeogenetic analysis was conducted for two populations of archaic rice grains from the Mojiaoshan site during the Liangzhu Period in China(2940 to 2840 BC).Sequencing after the PCR amplification of three regions of the chloroplast genome and one region of the nuclear genome showed recovery rates that were comparable to those in previous studies except for one chloroplast genome region,suggesting that the materials used in this work were appropriate for recovering genetic information related to domestication traits by using advanced *** after sequencing in these regions proved the existence of Japonica and Indica chloroplasts in archaic grains from the west trench,which were subsequently classified into eight plastid groups(type I–VIII),and indicated that these rice grains derived from different maternal lineages were stored together in storage houses at the Mojiaohsan *** these plastid groups,type V exhibited the same sequences as two modern Indica accessions that are utilized in basic studies and rice *** was inferred that part of the chloroplast genome of archaic rice has been preserved in modern genetic resources in these two modern Indica accessions,and the results indicated that rice related to their maternal ancestor was present at the Mojiaoshan site during the Liangzhu Period in *** usefulness of archaeogenetic analysis can be demonstrated by our research data as well as previous studies,providing encouragement for the possibility that archaeogenetic analysis can be applied to older rice materials that were utilized in the rice-domesticated period.
