In the present work,the transparent oxyfluoride glass-ceramic samples containing GdF3:RE^3+(RE=Tb,Eu)nanocrystals(nGCs)were fabricated via controlled heat-treatment of precursor xerogels prepared using a sol-gel *** formation of GdF3 nanocrystalline phase from gadolinium(III)trifluoroacetate was verified based on XRD *** average crystal sizes calculated from Scherrer formula were estimated to^10 nm as well as^6 nm for Tb^3+-and Eu^3+-doped samples,*** optical behavior of prepared sol-gel samples was evaluated based on photoluminescence excitation(PLE)and emission spectra(PL)as well as luminescence decay *** samples exhibit the 5D4→7Fj(j=6-3,Tb3+)and the 5D0→7Fj(j=0-4,Eu3+)emission bands recorded within the visible spectral area under excitation at near-UV(393 nm(Eu^3+),351,369,378 nm(Tb^3+))as well as middle-UV illumination(273 nm(Gd3+)).Additionally,based on recorded decay curves,the luminescence lifetimes(τm)for the 5D4(Tb^3+)and the 5D0(Eu3+)excited states were also *** general,recorded luminescence spectra and double-exponential character of decay curves for nGCs indicate a successful migration of Tb^3+and Eu^3+dopant ions from amorphous silicate framework to lowphonon energy GdF3 nanocrystal phase.

Many patients with pancreas cancer present with locally advanced pancreatic cancer(LAPC).The principle tools used for diagnosis and staging of LAPC include endoscopic ultrasound,axial imaging with computed tomography and magnetic resonance imaging,and diagnostic *** definition of resectability has historically been vague,as there is considerable debate and controversy as to the definition of *** the patient with LAPC,there is some level of involvement of the surrounding vascular structures,which include the superior mesenteric artery,celiac axis,hepatic artery,superior mesenteric vein,or portal *** feasible,most surgeons would recommend possible surgical resection for patients with borderline LAPC,with the goal of an R0 *** initially unresectable LAPC,neoadjuvant should be strongly ***,these patients should be offered neoadjuvant therapy,and the tumor should be assessed for possible response and eventual *** efficacy of neoadjuvant therapy with this approach as a bridge to potential curative resection is broad,ranging from 3%-79%.The different modalities of neoadjuvant therapy include sin-gle or multi-agent chemotherapy combined with radiation,chemotherapy alone,and chemotherapy followed by chemotherapy with *** review focuses on patients with LAPC and addresses recent advances and controversies in the field.

Background: Patients with advanced hepatobiliary cancer (HBC) have a dismal prognosis and limited treatment options. Immunotherapy has been considered as a promising treatment, especially for cancers not amenable to surgery. Methods: Between 2004, and 2015, patients diagnosed with hepatocellular carcinoma (HCC), intra- and extrahepatic cholangiocarcinoma and gallbladder cancer (GBC) were identified in the National Cancer Database. Results: Among 249,913 patients with HBC, only 585 (0.2%) patients received immunotherapy. Among patients who received immunotherapy, most patients were diagnosed between 2012 and 2015, had private insurance, as well as an income ≥$46,000 and were treated at an academic facility. The use of immunotherapy among HBC patients varied by diagnosis (HCC, 67.7%;bile duct cancer, 14%). On multivariable analysis, a more recent period of diagnosis (OR 1.80, 95% CI: 1.44-2.25), median income >$46,000 (OR 1.43, 95% CI: 1.11-1.87), and higher tumor stage (stage III, OR 2.22, 95% CI: 1.65-3.01;stage IV, OR 3.24, 95% CI:2.41-4.34) were associated with greater odds of receiving immunotherapy. Conclusions: Overall utilization of immunotherapy in the US among patients with HBC was very low, yet has increased over time. Certain socioeconomic factors were associated with an increased likely of receiving immunotherapy, suggesting disparities in access of patients with lower socioeconomic status.

Background:We sought to assess the overall benefit of laparoscopic versus open hepatectomy for treatment of colorectal liver metastases(CRLMs)using the win ratio,a novel methodological ***:CRLM patients undergoing curative-intent resection in 2001-2018 were identified from an international multi-institutional *** were paired and matched based on age,number and size of lesions,lymph node status and receipt of preoperative *** win ratio was calculated based on margin status,severity of postoperative complications,90-day mortality,time to recurrence,and time to ***:Among 962 patients,the majority underwent open hepatectomy(n=832,86.5%),while a minority underwent laparoscopic hepatectomy(n=130,13.5%).Among matched patient-to-patient pairs,the odds of the patient undergoing laparoscopic resection“winning”were 1.77[WR:1.77,95%confidence interval(CI):1.42-2.34].The win ratio favored laparoscopic hepatectomy independent of low(WR:2.94,95%CI:1.20-6.39),medium(WR:1.56,95%CI:1.16-2.10)or high(WR:7.25,95%CI:1.13-32.0)tumor burden,as well as unilobar(WR:1.71,95%CI:1.25-2.31)or bilobar(WR:4.57,95%CI:2.36-8.64)*** odds of“winning”were particularly pronounced relative to short-term outcomes(i.e.,90-day mortality and severity of postoperative complications)(WR:4.06,95%CI:2.33-7.78).Conclusions:Patients undergoing laparoscopic hepatectomy had 77%increased odds of“winning”.Laparoscopic liver resection should be strongly considered as a preferred approach to resection in CRLM patients.

AIM： Grapefruit-seed extract （GSE） containing flavonoids, possesses antibacterial and antioxidative properties but whether it influences the gastric defense mechanism and gastroprotection against ethanol- and stress-induced gastric lesions remains unknown. METHODS： We compared the effects of GSE on gastric mucosal lesions induced in rats by topical application of 100% ethanol or 3.5 h of water immersion and restraint stress （WRS） with or without （A） inhibition of cyclooxygenase （COX）-1 activity by indomethacin and rofecoxib, the selective COX-2 inhibitor, （B） suppression of NO-synthase with L-NNA （20 mg/kg ip）, and （C） inactivation by capsaicin （125 mg/kg sc） of sensory nerves with or without intragastric （ig） pretreatment with GSE applied 30 min prior to ethanol or WRS. One hour after ethanol and 3.5 h after the end of WRS, the number and area of gastric lesions were measured by planimetry, the gastric blood flow （GBF） was assessed by H2-gas clearance technique and plasma gastrin levels and the gastric mucosal generation of PGE2, superoxide dismutase （SOD） activity and malonyldialdehyde （MDA） concentration, as an index of lipid peroxidation were determined. RESULTS： Ethanol and WRS caused gastric lesions accompanied by the significant fall in the GBF and SOD activity and the rise in the mucosal MDA content. Pretreatment with GSE （8-64 mg/kg i g） dose- dependently attenuated gastric lesions induced by 100% ethanol and WRS; the dose reducing these lesions by 50% （ID50） was 25 and 36 mg/kg, respectively, and this protective effect was similar to that obtained with methyl PGE2 analog （5 μg/kg i g）. GSE significantly raised the GBF, mucosal generation of PGE2, SOD activity and plasma gastrin levels while attenuating IVlDA content. Inhibition of PGE2 generation with indomethacin or rofecoxib and suppression of NO synthase by L-NNA or capsaicin denervation reversed the GSE-induced protection and the accompanying hyperemia. AIM： To determine whether ischemic preconditioning （IP） affects the development of edematous cerulein-induced pancreatitis and to assess the role of cyclooxygenase-1 （COX-l）, COX-2, and heat shock protein 70 （HSP 70） in this process. METHODS： In male Wistar rats, IP was performed by damping of celiac artery （twice for 5 min at 5-min intervals）. Thirty minutes after IP or sham operation, acute pancreatitis was induced by cerulein. Activity of COX-1 or COX-2 was inhibited by resverabol or rofecoxib, respectively （10 rng/kg）. RESULTS： IP significantly reduced pancreatic damage in cerulein-induced pancreatitis as demonstrated by the improvement of pancreas histology, reduction in serum lipase and poly-C ribonuclease activity, and serum concentration of pro-inflammatory interleukin （IL）-1β. Also, IP attenuated the pancreatitis-evoked fall in pancreatic blood flow and pancreatic DNA synthesis. Serum level of anti-inflammatory IL-10 was not affected by IP. Cerulein-induced pancreatitis and IP increased the content of HSP 70 in the pancreas. Maximal increase in HSP 70 was observed when IP was combined with cerulein-induced pancreatitis. Inhibition of COXs, especially COX-2, reduced the protective effect of IP in edematous pancreatitis. CONCLUSION： Our results indicate that IP reducespancreatic damage in cerulein-induced pancreatitis and this effect, at least in part, depends on the activity of COXs and pancreatic production of HSP 70.

High-throughput discovery of fluoride-ion conductors via a decoupled,dynamic,and iterative(DDI)framework

Fluoride–ion batteries are a promising alternative to lithium–ion batteries with higher theoretical capacities and working voltages,but they have experienced limited success due to the poor ionic conductivities of known electrolytes and ***,we report a high-throughput computational screening of 9747 fluoride-containing materials in search of fluoride-ion *** a combination of empirical,lightweight DFT,and nudged elastic band(NEB)calculations,we identified>10 crystal systems with high fluoride *** applied a search strategy where calculations are performed in any order(decoupled),computational resources are reassigned based on need(dynamic),and predictive models are repeatedly updated(iterative).Unlike hierarchical searches,our decoupled,dynamic,and iterative framework(DDI)began by calculating high-quality barrier heights for fluoride-ion mobility in a large and diverse group of *** high-quality dataset provided a benchmark against which a rapid calculation method could be *** accurate method was then used to measure the barrier heights for 6797 fluoride–ion *** final dataset has allowed us to discover many fascinating,high-performance conductors and to derive the design rules that govern their *** materials will accelerate experimental research into fluoride–ion batteries,while the design rules will provide an improved foundation for understanding ionic conduction.
