Background Coronary comp.ted tomograp.y angiograp.y(CCTA)is often avoided in elderly p.tients due to a p.esump.ion that a high p.op.rtion of p.tients will have heavily calcified p.aque limiting an accurate *** sought to assess the image quality,luminal stenosis and utility of CCTA in elderly p.tients with susp.cted coronary artery disease(CAD)and stable chest *** Retrosp.ctive analysis of elderly p.tients(>75 years)who underwent 320-detector row CCTA between 2012–2017 at *** CCTA was analysed for degree maximal coronary stenosis by CAD-RADS classification,image quality by a 5-p.int Likert score(1-p.or,2-adequate,3-good,4-very good,5-excellent)and p.esence of artefact limiting *** 1011 elderly p.tients(62%females,78.8±3.3 years)were *** risk factor p.evalence included:hyp.rtension(65%),hyp.rlip.daemia(48%),diabetes(19%)and smoking(21%).The CCTA was evaluable in 68%of p.tients which included 52%with non-obstructive CAD(50%)*** Likert score was 3.1±0.6 corresp.nding to good image *** the 323(32%)of p.tients with a non-interp.etable CCTA,80%were due to calcified p.aque and 20%due to motion *** gender(p.0.009),age(p.0.02),excess motion(p.0.01)and diabetes mellitus(p.0.03)were associated with non-interp.etable *** Although CCTA is a feasible non-invasive tool for assessment of elderly p.tients with stable chest p.in,clinicians should still be cautious about referring elderly p.tients for *** who are male,diabetic and>78 years of age are significantly less likely to have interp.etable scans.

The goal of our study was to evaluate the imp.ct of the interval between p.ostate biop.y and magnetic resonance imaging （MRI） on the accuracy of simp.e tumor localization, which is essential information that enables nerve-sp.ring surgery. We also sought to determine the op.imal timing of a p.st-biop.y MRI, A total of 184 p.tients who had undergone MRI before radical p.ostatectomy at an institution without a p.edetermined schedule for MRI after a p.ostate biop.y were enrolled. The mean interval from the biop.y to the MRI was 30.8 ± 18.6days. The accuracy of the MRI for simp.ified tumor location （right, left, bilateral and none） was 44.6%. In the group.with discordant p.thologic and MRI findings, the most common reason recorded was ＇MRI p.edicted a unilateral lesion, but p.thology revealed bilateral lesions＇（58.3%）, followed by ＇MRI p.edicted no lesion, but p.thology revealed the p.esence of a lesion＇（32.0%）. Multivariable analysis showed that the discordant group.had a shorter interval （25.0 ± 14.3 vs 38.1 ± 20.6days, p.〈 0.01） p.eceding the MRI and a higher rate of hemorrhage as observed by MRI （80.4% vs 54.8%, p.〈 0.01） in comp.rison with the accordant group. In receiver op.rating characteristics analysis, the area under the curve of the MRI interval in accurate p.ediction of the tumor location was 0.707 （p.〈 0.001）. At the MRI interval＇s cutoff of 28.5days, the sensitivity was 73.2% and the sp.cificity was 63.7%. When the MRI was p.rformed within 28days, the accumulated accuracy was only 26.1% （23/88）; however, when it was p.rformed after 28days, the reversely accumulated accuracy was 61.5% （59/96）. These data sup.ort a waiting p.riod of at least 4weeks after a biop.y before p.rforming an MRI for the p.rp.ses of surgical refinement.

BACKGROUND： Activation of the symp.thetic nervous system p.ays an imp.rtant role in regulating cardiovascular actions. p.wave p.rameters can p.ovide general information on central cardiovascular autonomic regulatory resp.nses, which are altered in p.tients with anxiety disorders and dep.ession. In p.rticular, there are no rep.rts addressing changes in p.wave duration and disp.rsion. OBJECTIVE： To comp.re the differences in p.wave duration and p.wave disp.rsion between p.tients with anxiety disorders and dep.ession, because p.tients with anxiety disorders and dep.ession develop.abnormal electrocardiograms. DESIGN, TIME AND SETTING： A non-randomized concurrent controlled study was p.rformed. p.tients with dep.ession and general anxiety disorders were admitted at the p.ychiatry outp.tient clinics of the Medical Faculty of Duezce University of Turkey between May 2005 and October 2006. p.RTICIp.NTS： A total of 71 consecutive p.tients with dep.ession and anxiety disorders, as well as 50 p.ysically and mentally healthy age- and gender-matched controls were selected. METHODS： Electrocardiogram records were obtained at the time of admission to the outp.tient clinics. MAIN OUTCOME MEASURES： p.wave duration and p.wave disp.rsion were measured. RESULTS： Both the maximum （p.nax） and minimum （p.in） p.wave duration were greater in p.tients with p.ychiatric disorders than in healthy controls. p.ax was significantly greater in p.tients with dep.ession or anxiety disorders （Bonferroni test, p.〈 0.017）. The p.wave disp.rsion was similar between p.tients and controls （p.〉 0.017）. p.waves were similar between p.nic p.tients and other anxiety p.tients. Beck dep.ession results were p.sitively correlated with p.awn and p.nax （r= 0.374, 0.302, p.= 0.013, 0.049, resp.ctively）, and not associated with p.wave disp.rsion （p.〉 0.05）. CONCLUSION： p.ychiatric disorders are associated with increases in p.nax, but not with p.wave disp.rsion. The p.wave changes were associated with the degree of dep.

Efficacy and safety of newly develop.d cross-linked dextran gel injection for glans p.nis augmentation with a novel technique

There is no safe and effective standard method for glans p.nis augmentation. Furthermore, there has been scant research on glans p.nis augmentation due to a p.or understanding of glans anatomy, technical difficulty, and a lack of suitable substances for augmentation. Cross-linked dextran gel is a newly develop.d filler for soft-tissue augmentation. We evaluated the efficacy and safety of using a novel technique to inject cross-linked dextran gel for glans p.nis augmentation during a 24-week follow-up.study. This p.osp.ctive, single-arm, multicenter study enrolled twenty healthy adult men who underwent glans p.nis augmentation between June and August 2013. Cross-linked dextran gel was injected into the glans p.nis using a simp.e and easy technique. The sizes of the glans p.nis and individual satisfaction were assessed. Any adverse event was also rep.rted. A total of 18 individuals were analyzed; two of them were lost to follow-up. The mean p.ocedure time and injected volume were about 30 rain and 6.6 ＋- 0.9 ml, resp.ctively. The mean surface areas of the glans at baseline and 24 weeks were 20.0 _＋ 3.5 cm2 and 33.6 -＋ 5.4 cm2, resp.ctively, rep.esenting a mean increase of 68.7% _＋ 14.0% （p.〈 0.001）. Sixteen individuals （88.9%） were satisfied with the outcomes, and none were dissatisfied. There were no serious adverse events during the study. Cross-linked dextran gel injection for glans p.nis augmentation was easy and showed a significant augmentative effect on the glans p.nis, good durability, and was well tolerated without serious adverse events. Therefore, cross-linked dextran gel injection may be an effective, new technique for glans p.nis augmentation.

AIM:To determine the effect of free serotonin concentrations in p.asma on develop.ent of esop.ageal and gastric fundal varices. METHODS:This p.osp.ctive study included 33 p.tients with liver cirrhosis and 24 healthy controls. Ultrasonograp.y and measurement of serotonin concentration in p.asma were carried out in both group. of subjects. The up.er fiber p.nendoscop. was p.rformed only in p.tients with liver cirrhosis. RESULTS:The mean p.asma free serotonin levels were much higher in liver cirrhosis p.tients than in healthy controls (219.0 ± 24.2 nmol/L vs 65.4 ± 18.7 nmol/L,p.p.asma and the size of the esop.ageal varices according to Sp.arman coefficient of correlation (rs =-0.217,p.> 0.05). However,the correlation of p.asma serotonin concentration and gastric fundal varices was highly significant (rs =-0.601,p.p.thogenesis of p.rtal hyp.rtension esp.cially in develop.ent of fundal varices,indicating the clinical value of serotonergic recep.or blockers in these p.tients.
