Background: To date there is onl. a l.mited number of study data on the changes in the perceived qual.ty of sexual.ty and rel.tionships of women that occur with pregnancy and chil.birth. Apart from preval.nce issues predictors for those changes are important. These questions were addressed with this epidemiol.gical.fol.ow-up study. Methods: The study sampl. consisted of 236 women who gave birth to a chil. between November 2000 and May 2001 at Bonn University Hospital.in Germany. A first visit took pl.ce at the hospital.a few days after chil.birth. Data incl.ded sociodemographic data, medical.and gynecol.gical.data as wel. as personal.ty traits using the NEO-FFI. A second visit took pl.ce 6.6 ±0.8 months after chil.birth. It incl.ded the BSPS (Bonn Scal. for Post-Partum Sexual.ty) and psychiatric symptoms using the SCl.90-R. Resul.s: Approximatel. six months after chil.birth 40.6%of women perceived a diminished qual.ty of sexual.l.fe compared to the time before pregnancy. 23.2%indicated a diminished rel.tionship qual.ty. The potential.y rel.vant co-variabl.s regarding the changes in sexual.qual.ty were age, breast feeding, fertil.zation, use of contraceptives and presence of psychiatric symptoms. With the changes in rel.tionship qual.ty there were correl.tions with marital.status, conscientiousness, and psychiatric symptom l.ad. Discussion: The majority of women six months after chil.birth did not return to a perceived sexual.l.fe comparabl. to the time before pregnancy. In addition to the high presence of symptoms of sexual.dysfunction, the high psychosocial.pressure is of special.importance. Therefore the information of patients about the potential.devel.pment of sexual.ty after chil.birth and an integrative approach by the gynecol.gist seem important. Particul.rl. the correl.tion between psychiatric symptom l.ad and their predictive potential.regarding satisfaction with sexual.ty shoul. be accounted for in further epidemiol.gical.research.

Hermansky-Pudl.k syndrome（HPS） is a recessive disorder with bl.eding diathesis, which has been l.nked to pl.tel.t granul. defects. Both pl.tel.t granul.s and endothel.al.Weibel.Pal.de bodies（WPBs）are members of l.sosome-rel.ted organel.es（l.Os） whose formation is regul.ted by HPS protein associated compl.xes such as Bl.C（biogenesis of l.sosome-rel.ted organel.es compl.x）-1,-2,-3, AP-3（adaptor protein compl.x-3） and HOPS（homotypic fusion and protein sorting compl.x）. Von Wil.ebrand factor（VWF） is critical.to hemostasis, which is stored in a highl.-mul.imerized form as tubul.s in the WPBs. In this study, we found the defective, but varying, rel.ase of VWF into pl.sma after desmopressin（DDAVP） stimul.tion in HPS1（Bl.C-3 subunit）, HPS6（Bl.C-2 subunit）, and HPS9（Bl.C-1 subunit）deficient mice. In particul.r, VWF tubul.tion, a critical.step in VWF maturation, was impaired in HPS6 deficient WPBs. This l.kel. refl.cts a defective endothel.um, contributing to the bl.eding tendency in HPS mice or patients. The differential.y defective regul.ted rel.ase of VWF in these HPS mouse model. suggests the need for precise HPS genotyping before DDAVP administration to HPS patients.

First Babies from Cryopreserved Oocytes in Hungary

Objective To eval.ate the val.e of oocyte cryopreservation （CP） in our cl.nical.ICSI program. Methods Freezing procedure with medium containing 1.5 mol.l.propanediol.（PrOH）＋ 0. 3 mol.l.succrose and traditional.sl.w-freezingprotocol.were used. Thawed oocytes were fertil.zed with ICSI （4-6 h after thawing）, and fertil.zation was assessed 12-16 h l.ter. l.ser assisted hatching was performed on al. transferred embryos and embryo transfer was carried out 48-72 h after ICSI. Resul.s Eighty-five eggs were thawed and survival.rate of 75.3% （64/85） was obtained. Sixty-four oocytes were inseminated with ICSI, 47fertil.zed （47/64; 73.4%） and a cl.avage rate of 85% （40/47） was obtained. Embryo transfers were performed in 18 patients, and 4 （19%） resul.ed in cl.nical.pregnancies. One of the pregnancies encountered first trimester abortion. Impl.ntation rate were 17.2% （5/29） per embryo and 5.8% （5/85） per egg thawed. In al. cases, chorion biopsy was performed resul.ing 46 XY kariotype. Concl.sion Our resul.s provide further evidence of that al.hough egg freezing cannot currentl. cl.im to be a routine procedure in human IVF,, there wil. certainl. be a pl.ce for oocyte CP in reproductive medicine in the future.

目的系统回顾抑郁症患者初级保健心理治疗有效性的现有可查证据,与一般护理或控制组进行对比,并对疗法类型和实施模式进行研究。方法通过检索MEDl.NE、Embase、CENTRAl.Psyc INFO数据库,搜集对比成人抑郁症患者初级保健心理治疗与常规治疗或控制组的随机对照试验,检索时间设定为2013年12月之前。至少两人从纳入研究中进行信息提取,并进行误差风险评估。以治疗后抑郁得分作为指标进行随机效果Meta分析。结果总共30项研究符合纳入标准,涉及5 159例患者。与控制组相比,治疗结束时效果(标准平均差异)得分如下:面对面认知行为疗法(CBT)得分为-0.30〔95%CI(-0.48,-0.13)〕,面对面问题解决疗法得分为-0.14〔95%CI(-0.40,0.12)〕,面对面人际心理疗法得分为-0.24〔95%CI(-0.47,-0.02)〕,其他面对面心理干预得分为-0.28〔95%CI(-0.44,-0.12)〕,远程治疗师引导CBT得分为-0.43〔95%CI(-0.62,-0.24)〕,远程治疗师引导问题解决疗法得分为-0.56〔95%CI(-1.57,0.45)〕,自助引导CBT得分为-0.40〔95%CI(-0.69,-0.11)〕,无接触或最低限度接触CBT得分为-0.27〔95%CI(-0.44,-0.10)〕。结论心理疗法在抑郁症患者初级保健中起到有效作用。与需要大量资源的疗法相比,面对面认知行为疗法能够以更少的资源获得相似效果。
