Aspects of the non-pharmacological treatment of irritable bowel syndrome

Irritable bowel syndrome(IBS) is one of the most commonly diagnosed gastrointestinal conditions. It represents a significant healthcare burden and remains a clinical challenge. Over the years IBS has been described from a variety of different perspectives; from a strict illness of the gastrointestinal tract(medical model) to a more complex multi-symptomatic disorder of the brain-gut axis(biopsychosocial/psychosomatic model). In this article we present aspects of the pathophysiology and the non-pharmacological treatment of IBS based on current knowledge. Effects of conditioned stress and/or traumatic influences on the emotional system(top-down) as well as effects on the intestine through stressors,infection,inflammation,food and dysbiosis(bottom-up) can affect braingut communication and result in dysregulation of the autonomic nervous system(AnS),playing an important role in the pathophysiology of IBS. Conditioned stress together with dysregulation of the autonomic nervous system and the emotional system may involve reactions in which the distress inside the body is not recognized due to low body awareness. This may explain why patients have difficulty identifying their symptoms despite dysfunction in muscle tension,movement patterns,and posture and biochemical functions in addition to gastrointestinal symptoms. IBS shares many features with other idiopathic conditions,such as fibromyalgia,chronic fatigue syndrome and somatoform disorders. The key to effective treatment is a thorough examination,including a gastroenterological examination to exclude other diseases along with an assessment of body awareness by a body-mind therapist. The literature suggests that early interdisciplinary diagnostic cooperation between gastroenterologists and body-mind therapists is necessary. Foveal thickness reduction after anti-vascular endothelial growth factor treatment in chronic diabetic macular edema

AIM：To report foveal thickness reduction in eyes with resolution of macular edema and recovery of a foveal depression after one-year of anti-vascular endothelial growth factor（anti-VEGF） therapy for center-involving diabetic macular edema（DME）.METHODS：Foveal thickness was assessed with optical coherence tomography to determine the central subfield foveal thickness（CSFT） and macular volume in 42 eyes with DME（CSFT〉275 μm）. Evaluations also included measurement of best-corrected visual acuity（BCVA）, and were performed at baseline, and upon foveal depression recovery achieved after 12 monthly intravitreal injections of either 1.5 mg/0.06 mL bevacizumab（n=21） or 0.5 mg/0.05 mL ranibizumab（n=21）. Data was compared to 42 eyes of normally sighted, non-diabetic, healthy individuals with similar age, gender and race ***：Mean baseline BCVA was 0.59±0.04 and 0.32± 0.03 log MAR（P〈0.001） after treatment and resolution of DME, with all, but 3 eyes, showing BCVA improvement. Mean CSFT before treatment was 422.0±20.0 μm, and after treatment, decreased to 241.6±4.6 μm（P〈0.001）, which is significantly thinner than CSFT found in control subjects（272.0±3.4 μm; P〈0.001）. Moreover, in 33/42 DM eyes（79%）, CSTF was thinner than the matched control eye. Macular volume showed comparable results, but with lower differences between groups（control：8.5±0.4 mm^3; DME：8.2±1.0 mm^3; P=0.0267）.COnCLUSIOn：DME eyes show significantly lower foveal thickness than matched controls after DME resolution achieved with one-year anti-VEGF therapy. Further investigation into the reasonsfor this presumable retinal atrophy using fluorescein angiography and functional parameters as well as establishing possible predictors is warranted. This finding should be considered during the treatment of DME.

AIM: To elucidate the differences in somatic, psycho-logical and biochemical pattern between the subtypes of irritable bowel syndrome (IBS). METHODS: Eighty IBS patients, 30 diarrhoea pre-dominant (D-IBS), 16 constipation predominant (C-IBS) and 34 alternating IBS (A-IBS) underwent physi-otherapeutic examinations for dysfunctions in body movements and awareness and were compared to an apparently healthy control group (AHC). All groups an-swered questionnaires for gastrointestinal and psycho-logical symptoms. Biochemical variables were analysed in blood. RESULTS: The D-IBS group showed less body aware-ness, less psychological symptoms, a more normal sense of coherence and psychosocial rating as well as higher C-peptide values. C-IBS had a higher degree of body dysfunction and psychological symptoms, as well as the lowest sense of coherence compared to controls and D-IBS. They also demonstrated the most elevated prolactin levels. A-IBS had the lowest degree of body disturbance, deteriorated quality of life and affected bi-ochemical pattern. All subtypes had higher pain scores compared to controls. In addition they all had signifi -cantly increased triglycerides and elevated morning cortisol levels, however, without statistical signifi cance compared with the ***: IBS subtypes showed different pro-files in body awareness, somatic and psychological symptoms and in biochemical variables. D-IBS differed compared to the other groups by lowered body aware-ness, less psychological symptoms and a higher sense of coherence and elevated C-peptide values. C-IBS and A-IBS subtypes suffered more from depression and anxiety, associated with a lower quality of life. These differences may be important and will be taken into account in our treatment of these patients.

本文采用一种简单、有效的规模化溶液化学策略,在基底(如商用氟掺杂氧化锡透明导电涂层玻璃(FTO))和光活性薄膜(如赤铁矿)之间形成丰富的背接触界面,并用于低成本水氧化反应.高分辨率电子显微镜(扫描电镜、透射电镜、扫描透射电镜)、原子力显微镜、元素成像(电子能量损失谱和能量色散谱)和光电化学研究表明,可通过前驱体溶液的化学成分工程来有效降低机械应力、晶格失配、电子势垒和FTO与赤铁矿在背面接触界面之间的空隙以及FTO与电解液之间的短路和有害反应,进而提升这些低成本光阳极对水氧化反应以及PEC水分解清洁、可持续地生产氢气的整体效率.本研究对通过最小化在介孔电极的背接触界面和晶粒边界上的电子-空穴复合,进而提高电荷收集效率具有重要意义,可提高低成本PEC水裂解装置的整体效率和规模化的能力.

Timing of Proterozoic magmatism in the Sunsas belt, Bolivian Precambrian Shield, SW Amazonian Craton

We present new U–Pb zircon and monazite ages from the Sunsas belt granitic magmatism in Bolivia, SW Amazonian Craton. The geochronological results revealed four major magmatic events recorded along the Sunsas belt domains. The older igneous event formed a granitic basement coeval to the Rio Apa Terrane(1.95–1.85 Ga) in the southern domain. The second magmatic episode is represented by 1.68 Ga granites associated to the Paraguá Terrane(1.69–1.66 Ga) in the northern domain. The 1.37–1.34 Ga granites related to San Ignacio orogeny represent the third and more pervasive magmatic event, recorded throughout the Sunsas belt. Moreover, magmatic ages of ~1.42 Ga revealed that the granitogenesis associated to the Santa Helena orogeny also affected the Sunsas belt, indicating that it was not restricted to the Jauru Terrane. Lastly, the 1.10–1.04 Ga youngest magmatism was developed during the Sunsas orogeny and represents the final magmatic evolution related to Rodinia assembly. Likewise, the 1.95–1.85 and 1.68 Ga inherited zircon cores obtained in the ~1.3 Ga and 1.0 Ga granite samples suggest strong partial melting of the Paleoproterozoic sources. The 1079 ± 14 Ma and 1018 ± 6 Ma monazite crystallization ages can be correlated to the collisional tectono-thermal event of the Sunsas orogeny, associated to reactions of medium-to high-grade metamorphism. Thus, the Sunsas belt was built by heterogeneous 1.95–1.85 Ga and 1.68 Ga crustal fragments that were reworked at 1.37–1.34 Ga and 1.10–1.04 Ga related to orogenic collages. Furthermore, the 1.01 Ga monazite age suggests that granites previously dated by zircon can bear evidence of a younger thermal history. Therefore, the geochronological evolution of the Sunsas belt may have been more complex than previously thought.

ng>ng>ng>ng>In Mali, chest injuries remain a real public health problem and are associated with heavy morbidity and mortality. Faced with a resurgence of urban civil violence and the explosion of road accidents, we decided to conduct this study in order to describe the epidemiological, clinical and therapeutic aspects of thoracic trauma in the emergency department. ng>Method and Material:ng> This is a descriptive prospective study over a period of one year in the emergency department of the CHU Gabriel Touré. Including all patients admitted for thoracic trauma. ng>Analysis and Entry: ng>Data were entered and analyzed on SPSS software version 20.0. The test was significant for a p value ng>Results:ng> We recorded 21,090 appeals in our structure among which 1284 patients were suspected of thoracic trauma. The diagnosis of thoracic trauma was retained in 119 (0.56%) patients. All patients were transported to the emergency room without prehospital medicalization. Clinical presentation was dominated by dyspnea in 54.6% of patients, however pain was the almost constant symptom in conscious victims. Various traumatic mechanisms had caused these lesions of the thorax, of which road traffic accidents represented half of the causes, followed by urban civil violence in 28.6% of patients. Landslides and falls from a great height were responsible for 19.5% of chest injuries. Open chest trauma was the type of lesion found in a third of the cases. This type of injury was exclusively due to blows and injuries during the brawls. Standard chest radiography was performed as the first intention in 60% of patients compared to 10.9% for the pleuropulmonary ultrasound. Thirteen patients required ventilatory assistance after orotracheal intubation. The average length of stay was 65.23 hours.
