Analytical evaluation of steady-state solute distribution in through- diffusion and membrane behavior test under non-perfectly flushing boundary conditions

The through-diffusion and membrane behavior testing procedure using a closed-system apparatus has been widely used for concurrent measurement of diffusion and membrane efficiency coefficients of low-permeability clay-based barrier materials.However,the common assumption of perfectly flushing conditions at the specimen boundaries could induce errors in analyses of the diffusion coefficients and membrane efficiencies.In this study,an innovative pseudo three-dimensional(3D)analytical method was proposed to evaluate solute distribution along the boundary surfaces of the soil-porous disks system,considering the non-perfectly flushing conditions.The results were consistent with numerical models under two scenarios considering different inflow/outflow positions.The proposed model has been demonstrated to be an accurate and reliable method to estimate solute distributions along the bound-aries.The calculated membrane efficiency coefficient and diffusion coefficient based on the proposed analytical method are more accurate,resulting in up to 50%less relative error than the traditional approach that adopts the arithmetic mean value of the influent and effluent concentrations.The retar-dation factor of the clay specimen also can be calculated with a revised cumulative mass approach.Finally,the simulated transient solute transport matched with experimental data from a multi-stage through-diffusion and membrane behavior test,validating the accuracy of the proposed method.

Primary tumor location and survival in colorectal cancer: A retrospective cohort study

BACKGROUND Primary tumor location is a prognostic factor for metastatic colorectal cancer(m CRC).Post hoc analyses of m CRC clinical trials,including FIRE-3,CALGB/SWOG 80405,suggest that primary tumor location is also predictive of survival benefit with cetuximab or bevacizumab in combination with 5-fluorouracil-based chemotherapy.AIm Evaluate prognostic/predictive roles of primary tumor location in real-world m CRC patients treated with cetuximab or bevacizumab plus 5-fluorouracil-based chemotherapy.mETHODS This retrospective cohort study selected patients with KRAS wild-type m CRC who initiated first-line therapy with cetuximab or bevacizumab in combination with 5-fluorouracil/leucovorin/irinotecan(FOLFIRI)or 5-fluorouracil/leucovorin/oxaliplatin(FOLFOX)between January 2013 and April 2017 from the Flatiron Health electronic health record-derived database of de-identified patientlevel data in the United States.Primary tumor location was abstracted from patients’charts.Left-sided primary tumor location(LPTL)was defined as tumors that originated in the splenic flexure,descending colon,sigmoid colon,or rectum;right-sided primary tumor location(RPTL)was defined as tumors that originated from the appendix,cecum,ascending colon,hepatic flexure,or transverse colon.Propensity score matching was used to balance the baseline demographic and clinical characteristics between patients treated with cetuximab and patientstreated with bevacizumab.Kaplan-meier and Cox regression methods were used for survival analyses.RESULTS A total of 1312 patients met the selection criteria.Of 248 cetuximab plus FOLFIRI or FOLFOX patients,164 had LPTL and 84 had RPTL;of 1064 bevacizumab plus FOLFIRI or FOLFOX patients,679 had LPTL and 385 had RPTL.Cetuximab LPTL and RPTL patients were more likely to receive FOLFIRI vs bevacizumab patients(LPTL:64.0%vs 24.3%;RPTL:76.2%vs 24.9%,Pmab RPTL vs bevacizumab RPTL patients(Pmab RPTL patients were more likely to have stage III

metabolic syndrome and the urinary microbiome of patients undergoing percutaneous nephrolithotomy

Objective:To identify possible stone-promoting microbes,we compared the profiles of microbes grown from stones of patients with and without metabolic syndrome(metS).The association between metS and urinary stone disease is well established,but the exact pathophysiologic relationship remains unknown.Recent evidence suggests urinary tract dysbiosis may lead to increased nephrolithiasis risk.methods:At the time of percutaneous nephrolithotomy,bladder urine and stone fragments were collected from patients with and without metS.Both sample types were subjected to expanded quantitative urine culture(EQUC)and 16 S ribosomal RNA gene sequencing.Results:Fifty-seven patients included 12 controls(21.1%)and 45 metS patients(78.9%).Both cohorts were similar with respect to demographics and non-metS comorbidities.No controls had uric acid stone composition.By EQUC,bacteria were detected more frequently in metS stones(42.2%)compared to controls(8.3%)(p=0.041).Bacteria also were more abundant in stones of metS patients compared to controls.To validate our EQUC results,we performed 16 S ribosomal RNA gene sequencing.In 12/16(75.0%)sequence-positive stones,EQUC reliably isolated at least one species of the sequenced genera.Bacteria were detected in both“infectious”and“non-infectious”stone compositions.Conclusion:Bacteria are more common and more abundant in metS stones than control stones.Our findings support a role for bacteria in urinary stone disease for patients with metS regardless of stone composition.

Background:Lacrosse is one of the fastest-growing sports in the United States.Its rules regarding permitted contact differ by sex and age.There are no known studies using a nationally representative data set to analyze lacrosse injury patterns over several years by sex and age in the youth population.methods:A retrospective analysis was performed using data from the National Electronic Injury Surveillance System for youth aged 11-18 years who were treated for lacrosse-related injuries in U.S.emergency departments from 2000 to 2016.Based on our review of the case narratives,we created and coded a new injury-mechanism variable.We generated national estimates from 6406 cases.Results:An estimated 206,274 lacrosse-related injuries to youths aged 11-18 years were treated in U.S.emergency departments from 2000 to 2016.The rate of injuries per 10,000 significantly increased from 1.9 in 2000 to a peak of 5.3 in 2012(pmechanism,body part injured,and diagnosis differed by sex.Boys were 1.62 times(95%confidence interval(95%CI):1.25-2.09)more likely than girls to be injured by player-to-player contact.Girls were 2.21 times(95%CI:1.96-2.49)more likely than boys to have non-contact injuries.Overall,as age increased,the percentage of injuries from lacrosse sticks decreased and player-toplayer contact increased.Conclusion:Despite additional protective regulations in the sport,lacrosse is an important source of injury where we continue to see differences by sex and age.This study supports the continuation,modification,and addition of rules aimed at reducing lacrosse injury risk.

Tuning structural,electrical,dielectric and magnetic properties of mg-Cu-Co ferrites via dysprosium(Dy^(3+))doping

In current research work,Dy3+substituted mg0.5Cu0.25Co0.25Fe2-xDyxO4(0.0≤x≤0.04 with the step interval of 0.01)soft ferrites were synthesized by the sol-gel auto combustion method.The prepared samples were characterized by the techniques using X-ray diffraction(XRD),scanning electron microscopy(SEm),energy dispersive X-ray spectroscopy(EDS),Fourier transform infrared(FTIR)spectroscopy,curre nt-voltage(Ⅰ-Ⅴ)measurement,LCR meter,vibrating sample magnetometer(VSm)and Raman.XRD data reveal that the average crystallite size is 49.71 nm and the lattice constant is 0.83703 nm for sample x=0.03.The non-uniform grain growth was demonstrated by micrographs and impurity-free elemental composition was observed from EDX analysis.The DC resistivity has an increasing and decreasing trend in ferromagnetic and paramagnetic regions with an increase in temperature.moreover,the high resistivity is observed with the order of 1010Ω·cm,and the activation energy is 0.944 eV for samples x=0.03.The dielectric parameters including dielectric constant,dielectric losses,and impedance gradually decrease with the increase in frequency from 8 Hz to 8 mHz.The minimum dielectric loss at high frequency is found for sample x=0.03.The coercivity(Hc)and saturation magnetization(ms)are found in the ranges of 520.82-544.02 Oe and 20.5841-21.1473 emu/g,respectively.These observations confirm that dysprosium(x=0.03)doped mCC-soft ferrites may be applicable in transformer cores,microwave absorbance,and telecommunication devices.

Tuberculosis(TB)continues to disproportionately affect Inuit populations in Canada with some communities having over 300 times higher rate of active TB than Canadian-born,non-Indigenous people.Inuit Tuberculosis Elimination Framework has set the goal of reducing active TB incidence by at least 50%by 2025,aiming to eliminate it by 2030.Whether these goals are achievable with available resources and treatment regimens currently in practice has not been evaluated.We developed an agent-based model of TB transmission to evaluate timelines and milestones attainable in Nunavut,Canada by including case findings,contact-tracing and testing,treatment of latent TB infection(LTBI),and the government investment on housing infrastructure to reduce the average house-hold size.The model was calibrated to ten years of TB incidence data,and simulated for 20 years to project program outcomes.We found that,under a range of plausible scenarios with tracing and testing of 25%e100%of frequent contacts of detected active cases,the goal of 50%reduction in annual incidence by 2025 is not achievable.If active TB cases are identified rapidly within one week of becoming symptomatic,then the annual incidence would reduce below 100 per 100,000 population,with 50%reduction being met between 2025 and 2030.Eliminating TB from Inuit populations would require high rates of contacttracing and would extend beyond 2030.The findings indicate that time-to-identification of active TB is a critical factor determining program effectiveness,suggesting that investment in resources for rapid case detection is fundamental to controlling TB.

Background and purpose We evaluate whether non-haemorrhagic imaging markers(NHIm)(white matter hyperintensity patterns,lacunes and enlarged perivascular spaces(EPVS))can discriminate cerebral amyloid angiopathy(CAA)from hypertensive cerebral small vessel disease(HTN-cSVD)among patients with isolated lobar intracerebral haemorrhage(isolated-LICH).methods In patients with isolated-LICH,four cSVD aetiologic groups were created by incorporating the presence/distribution of NHIm:HTN-cSVD pattern,CAA pattern,mixed NHIm and no NHIm.CAA pattern consisted of patients with any combination of severe centrum semiovale EPVS,lobar lacunes or multiple subcortical spots pattern.HTN-cSVD pattern consisted of any HTN-cSVD markers:severe basal ganglia PVS,deep lacunes or peribasal ganglia white matter hyperintensity pattern.mixed NHIm consisted of at least one imaging marker from either pattern.Our hypothesis was that patients with HTN-cSVD pattern/mixed NHIm would have a higher frequency of left ventricular hypertrophy(LVH),which is associated with HTN-cSVD.Results In 261 patients with isolated-LICH,CAA pattern was diagnosed in 93 patients,HTN-cSVD pattern in 53 patients,mixed NHIm in 19 patients and no NHIm in 96 patients.The frequency of LVH was similar among those with HTN-cSVD pattern and mixed NHIm(50%vs 39%,p=0.418)but was more frequent in HTN-cSVD pattern compared with CAA pattern(50%vs 20%,pmodel,HTN-cSVD pattern(OR:7.38;95%CI 2.84 to 19.20)and mixed NHIm(OR:4.45;95%CI 1.25 to 15.90)were found to be independently associated with LVH.Conclusion Among patients with isolated-LICH,NHIm may help differentiate HTN-cSVD from CAA,using LVH as a marker for HTN-cSVD.

Risk calculators have offered a viable tool for clinicians to stratify patients at risk of prostate cancer(PCa)and to mitigate the low sensitivity and specificity of screening prostate specific antigen(PSA).While initially based on clinical and demographic data,incorporation of multiparametric magnetic resonance imaging(mRI)and the validated prostate imaging reporting and data system suspicion scoring system has standardized and improved risk stratification beyond the use of PSA and patient parameters alone.Biopsy-naïve patients with lower risk profiles for harboring clinically significant PCa are often subjected to uncomfortable,invasive,and potentially unnecessary prostate biopsy procedures.Incorporating risk calculator data into prostate mRI reports can broaden the role of radiologists,improve communication with clinicians primarily managing these patients,and help guide clinical care in directing the screening,detection,and risk stratification of PCa.

Computational materials discovery efforts are enabled by large databases of properties derived from high-throughput density functional theory(DFT),which now contain millions of calculations at the generalized gradient approximation(GGA)level of theory.It is now feasible to carry out high-throughput calculations using more accurate methods,such as meta-GGA DFT;however recomputing an entire database with a higher-fidelity method would not effectively leverage the enormous investment of computational resources embodied in existing(GGA)calculations.Instead,we propose here a general procedure by which higher-fidelity,low-coverage calculations(e.g.,meta-GGA calculations for selected chemical systems)can be combined with lower-fidelity,high-coverage calculations(e.g.,an existing database of GGA calculations)in a robust and scalable manner.We then use legacy PBE(+U)GGA calculations and new r2SCAN meta-GGA calculations from the materials Project database to demonstrate that our scheme improves solid and aqueous phase stability predictions,and discuss practical considerations for its implementation.

Towards predicting biochar impacts on plant-available soil nitrogen content

Biochars can improve soil health but have been widely shown to reduce plant-available nitrogen(N)owing to their high carbon(C)content,which stimulates microbial N-immobilization.However,because biochars contain large amounts of C that are not microbially available,their total elemental C:N ratio does not correspond well with impacts on soil N.We hypothesized that impacts on soil plant-available N would relate to biochar mineralizable-C(C_(min))content,and that C:N ratios of the mineralizable biochar component could provide a means for predicting conditions of net soil N-mineralization or-immobilization.We conducted two laboratory experiments,the first measuring biochar C_(min)from respiration of isotopically labeled barley biochars manufactured at 300,500,and 750℃,and the second characterizing C_(min)by proxy measurements for ten biochars from six feedstocks at several temperatures.For both experiments,soils were incubated with 2%biochar by mass to determine impacts to soil N-mineralization.Contrary to expectation,all the biochars increased soil N-mineralization relative to unamended soils.Also unexpected,higher temperature(500 and 700℃)barley biochars with less C_(min)stimulated more soil decomposition and more soil N-mineralization than a 350℃barley biochar.However,across diverse biochar feedstocks and production methods,none of the biochar characteristics correlated with soil N-mineralization.The finding of improved soil N-mineralization adds complexity to the range of soil N responses that can be expected in response to biochar amendment.Because of the limited ability to predict soil N responses from biochar properties,users should monitor soil N to manage soil fertility.
