A 55-year old male patient was diagnosed with strongy- loides hyper-infection with stool analysis and intestinal biopsy shortly after his chemotherapy for myeloma. He was commenced on albendazole anthelmintic therapy. After initiation of the treatment he suffered life- threatening gastrointestinal (GI) bleeding. Repeated endoscopies showed diffuse multi-focal intestinal bleeding. The patient required huge amounts of red blood cells and plasma transfusions and correction of haemostasis with recombinant activated factor Ⅶ. Abdominal aorto-angiography showed numerous micro- aneurysms (‘berry aneurysms’) in the superior and inferior mesenteric arteries’ territories. While the biopsy taken prior to the treatment with albendazole did not show evidence of vasculitis, the biopsy taken after initiation of therapy revealed leukoclastic aggregations around the vessels. These findings suggest that, in addition to direct destruction of the mucosa, vasculitis could be an important additive factor causing the massive GI bleeding during the anthelmintic treatment. This might result from substances released by the worms that have been killed with anthelmintic therapy. Current guidelines advise steroids to be tapered and stopped in case of systematic parasitic infections as they might reduce immunity and precipitate parasitic hyper-infection. In our opinion, steroid therapy might be of value in the management of strongyloides hyper- infection related vasculitis, in addition to the anthelmintic treatment. Indeed, steroid therapy of vasculitis with other means of supportive care resulted in cessation of the bleeding and recovery of the patient.

AIM To assess whether the surgical apgar score(SAS) is a prognostic tool capable of identifying patients at risk of major complications following lower extremity amputations *** This was a single-center,retrospective observational cohort study conducted between January 2013 and April 2015. All patients who had either a primary transtibial amputation(TTA) or transfemoral amputation(TFA) conducted at our institution during the study period were assessed for inclusion. All TTA patients underwent a standardized one-stage operative procedure(ad modum Persson amputation) performed approximately 10 cm below the knee joint. All TTA procedures were performedwith sagittal flaps. TFA procedures were performed in one stage with amputation approximately 10 cm above the knee joint,performed with anterior/posterior flaps. Trained residents or senior consultants performed the surgical procedures. The SAS is based on intraoperative heart rate,blood pressure and blood loss. Intraoperative parameters of interest were collected by revising electronic health records. The first author of this study calculated the SAS. Data regarding major complications were not revealed to the author until after the calculation of SAS. The SAS results were arranged into four groups(SAS 0-4,SAS 5-6,SAS 7-8 and SAS 9-10). The cohort was then divided into two groups representing low-risk(SAS ≥ 7) and highrisk patients(SAS < 7) using a previously established threshold. The outcome of interest was the occurrence of major complications and death within 30-d of *** A logistic regression model with SAS 9-10 as a reference showed a significant linear association between lower SAS and more postoperative complications [all patients: OR = 2.00(1.33-3.03),P = 0.001]. This effect was pronounced for TFA [OR = 2.61(1.52-4.47),P < 0.001]. Auxin homeostasis is pivotal for normal plant growth and development. The superroot2 （sur2） mutant was initially isolated in a forward genetic screen for auxin overproducers, and SUR2 was suggested to control auxin conju- gation and thereby regulate auxin homeostasis. However, the phenotype was not uniform and could not be described as a pure high auxin phenotype, indicating that knockout of CYP83B1 has multiple effects. Subsequently, SUR2 was identified as CYP83B1, a cytochrome P450 positioned at the metabolic branch point between auxin and indole glucosinolate metabolism. To investigate concomitant global alterations triggered by knockout of CYP83B1 and the countermeasures chosen by the mutant to cope with hormonal and metabolic imbalances, 10-day-old mutant seedlings were characterized with respect to their transcriptome and metabolome profiles. Here, we report a global analysis of the sur2 mutant by the use of a combined transcriptomic and metabolomic approach revealing pronounced effects on several metabolic grids including the intersection between secondary metabolism, cell wall turnover, hormone metabolism, and stress responses. Metabolic and transcriptional cross-talks in sur2 were found to be regulated by complex interactions between both positively and negatively acting transcription factors. The complex phenotype of sur2 may thus not only be assigned to elevated levels of auxin, but also to ethylene and abscisic acid responses as well as drought responses in the absence of a water deficiency. The delicate balance between these signals explains why minute changes in growth conditions may result in the non-uniform phenotype. The large phenotypic variation observed between and within the different surveys may be reconciled by the complex and intricate hormonal balances in sur2 seedlings decoded in this study.

The objective of the present study is a systematic review on the use of VRE (Virtual Reality Exposure) for PTSD (Post-Traumatic Stress Disorder). The following keywords were used: PTSD, Post-Traumatic Stress Disorder, Posttraumatic Stress Disorder, Virtual Reality in these databases: PsycINFO/PsycLIT, PubMed/Medline, EBSCO, ProQuest, ISI Web of Science, and SciELO/Pepsic. The studies included should have their own empirical data and the intervention should consist of exposure to virtual reality. A total of 241 publications were found, of which 28 were selected for this review: 11 case studies, eight uncontrolled case series, and nine randomized controlled trials. The treatment ranged between 3 and 22 VRE sessions (M (Mean) = 9.09, SD (standard deviation) = 1.57). Among the case studies, the results were promising; all studies were able to reduce the PTSD symptomatology. As to the uncontrolled case series, overall they showed promising results. Regarding the randomized clinical trials, there was found a wide variation concerning the VRE intervention as well as the combined and control intervention. The majority of the studies presented significant results, except two which returned modest or non-significant results. The findings of this study suggest that the use of virtual reality as a tool for exposure therapy for PTSD can be an asset.

AIM To investigate the total blood loss(TBL) and the safety with respect to the re-amputation rate after transtibial amputation(TTA) conducted with and without a tourniquet. METHODS The study was a single-centre retrospective cohort study of patients with a primary TTA admitted between January 2013 and April 2015. All patients with a primary TTA were assessed for inclusion if the amputation was performed because of arteriosclerosis or diabetic complications. All patients underwent a standardized TTA procedure that was performed approximately 10 cm below the knee joint and performed with sagittalflaps. The pneumatic tourniquet, when used, was inflated around the femur to a pressure of 100 mmH g above the systolic blood pressure. The number of blood transfusions within the first four postoperative days was recorded. The intraoperative blood loss(OBL), which is defined as the volume of blood lost during surgery, was determined from the suction volume and by the weight difference of the surgical dressings. The trigger for a blood transfusion was set at a decrease in the Hgb level < 9.67 g/dL(6 mmol/L). Transfusions were performed with pooled red blood cells containing 245 m L per portion, which equals 55 g/L of haemoglobin. The TBL during the first four postoperative days was calculated based on the haemoglobin level and the estimated blood volume. The re-amputation rate was evaluated within 30 d. RESULTS Seventy-four out of 86 consecutive patients who underwent TTA within the two-year study period were included in the analysis. Of these, 38 were operated on using a tourniquet and 36 were operated on without using a tourniquet. There were no significant preoperative differences between the groups. The patients in both groups had a postoperative decrease in their Hgb level compared with preoperative baseline values. Large-Scale Fluidization Features from Late Holocene Coseismic Paleoliquefaction in the Willamette River Forearc Valley, Central Cascadia Subduction Zone, Oregon, USA

A search of Willamette River cutbanks was conducted for the presence of late Holocene paleoli-quefaction records in the Willamette forearc valley, located 175 ± 25 km landward from the buried trench in the central Cascadia subduction zone. Eight cutbank sites are reported that show evidence of large-scale fluidization features (≥10 cm width) including clastic sand dikes and intruded sand sills in Holocene overbank mud deposits. The targeted alluvial sequences, and hosted paleoliquefaction records, are of late Holocene age, as based on radiocarbon dating, flood silt thickness (≤4 m thickness), and minimal consolidation of dike sand (~1.5 ± 0.5 kg·cm-2 unconfined compressive strength). Two of the paleoliquefaction sites, which are separated by 150 km distance, overlap in age (175 - 500 yr BP) with the last great megathrust rupture (Mw 8.5 - 9.0) in the Cascadia margin, dated at AD 1700. The scarcity of exposed late Holocene paleoliquefaction sites in the Willamette River cutbanks motivated subsurface searches for thick basal sand deposits and overlying fluidization features, using floodplain geomorphological analyses, ground penetrating radar, and remote pole-camera scans of deep trench walls (3 - 4 m depth). The onset of large-scale fluidization features occurred in overbank mud deposits (2 - 3 m thickness) above unconsolidated sand bodies (≥2 m thickness) with unconfined compressive strengths of ~1.5 ± 0.5 kg·cm-2. We recommend geomorphically-targeted subsurface explorations rather than traditional cutbank searches for evidence of coseismic paleoliquefaction in high-gradient river valley systems.

本研究以泽西奶牛(Bostaurus)牛犊为对象,探讨温、热环境(30℃)下短暂暴露是否影响其体温、心跳和呼吸频率以及细胞和内分泌应激反应(皮质醇、甲状腺激素、牛生长激素及热应激蛋白72)。结果表明:热暴露导致呼吸频率和心率加快,体温显著升高,但幅度较小。在所研究的细胞和内分泌应激反应中,血浆甲状腺激素浓度略下降,肌肉热应激蛋白72的浓度稍增加。因此,尽管夏季饲养动物所经历的较高环境温度(但非极端温度)能影响泽西牛犊的生理,但是泽西牛犊能对温和的暂时性环境温度改变做出适应性反应。我们认为,直肠温度和细胞热应激蛋白72浓度是所研究参数中评价热胁迫降低动物福利的最适指标。
