Background:Air pollution from motor vehicle traffic is a major source of polycyclic aromatic hydrocarbon （PAH） exposure in urban areas. Certain PAHs have been classified as probable human carcinogens by the International Agency of Research on Cancer. Some research indicates that people with low socio- economic status （SES） and other vulnerabilities may be exposed to heavier air pollution and thus PAHs. However, these findings have been inconsistent. Methods:A cross-sectional study was conducted in 2011, using as the study population base a random sample of household telephone numbers within defined areas based on distance of residences to major roads and highways on the island of Montreal. 200 volunteers （107 females and 93 males） ages 20-53, without renal disease or history of cancer, were recruited throuch telephone screening. 116 participants lived within 500 feet of highways or major roads;36 participants lived between 500 and 750 feet from highways or major roads;and 48 participants lived greater than 750 feet away from highways or major roads. Each participant completed a self-administered questionnaire assessing demo-graphics that included ethnicity and socio-economic indicators. ArcMap software and DMTI electronic street files were used to create zones of distance to highways and major roads. Residential traffic density was calculated based on routinely-collected traffic volume data from municipalities on the island of Montreal and from Transport Quebec. Results:Persons in full-time employment tended to live closer to heavy traffic （OR:2.71, 95% CI:1.26-5.85 for full-time work vs. unemployed）. No association was detected between ethnicity or education and level of traffic exposure. Conclusion:We observed gradients in exposure to traffic only for the aspect of SES related to employment, in the direction that full-time employment is associated with higher traffic exposure.

Objective:To review follow-up imaging after equivocal bone scans in men with castration resistant prostate cancer(CRPC)and examine the characteristics of equivocal bone scans that are associated with positive follow-up ***:We identified 639 men from five Veterans Affairs Hospitals with a technetium-99m bone scan after CRPC diagnosis,of whom 99(15%)had equivocal *** with equivocal scans were segregated into“high-risk”and“low-risk”subcategories based upon wording in the bone scan *** follow-up imaging(bone scans,computed tomography[CT],magnetic resonance imaging[MRI],and X-rays)in the 3 months after the equivocal scan were *** were compared between patients with a positive *** follow-up imaging after an equivocal bone ***:Of 99 men with an equivocal bone scan,43(43%)received at least one follow-up imaging test,including 32/82(39%)with low-risk scans and 11/17(65%)with high-risk scans(p=0.052).Of follow-up tests,67%were negative,14%were equivocal,and 19%were *** those who underwent follow-up imaging,3/32(9%)low-risk men had metastases vs.5/11(45%)high-risk men(p=0.015).Conclusion:While 19%of all men who received follow-up imaging had positive follow-up imaging,only 9%of those with a low-risk equivocal bone scan had metastases versus 45%of those with *** preliminary findings,if confirmed in larger studies,suggest follow-up imaging tests for low-risk equivocal scans can be delayed while high-risk equivocal scans should receive follow-up imaging.

Background: Gastroesophageal reflux is a frequent problem after esophageal atr esia (EA) repair. Our aim was to determine the prevalence of esophagitis and Bar rett esophagus more than 10 years after repair of EA. Methods: Ninety-two patie nts treated between 1973 and 1985 were included in this prospective study. A que stionnaire was completed by 86 patients; esophagogastroscopy was performed in 49 patients. Results: Only 36 patients had no complaints at all. Thirty-one patie nts complained of difficulties swallowing solid food; 23 complained of heartburn . Esophagogastroscopy revealed grade 3 esophagitis in 2 patients and a macroscop ic image of Barrett esophagus in 2. Histology showed esophagitis in 30 patients, gastric metaplasia in 3, and no intestinal metaplasia (Barrett esophagus). Conc lusions: For epidemiologic reasons, that is, the short interval of follow-up (1 0 years) and the low compliance of the study group, larger numbers are needed to decide if routine long-term endoscopic screening after repair of EA is necessa ry. For now, it cannot yet be recommended. The prevalence of symptoms of gastroe sophageal reflux disease in this study group is higher than that in the general population, but we found no severe complications of gastroesophageal reflux in t he pediatric age group.

BACKGROUND Aprotinin has recently been associated with adverse outcomes in patients undergoing cardiac *** reviewed our experience with this agent in patients undergoing cardiac surgery at Duke University Medical Center. METHODS We retrieved data on 10,275 consecutive patients undergoing surgical coronary revascularization at Duke between january 1,1996,and December 31,*** fit data to a logistic-regression model predicting each patient’s likelihood of receiving aprotinin on the basis of preoperative characteristics and to models predicting long-term survival(up to 10 years) and decline in renal function,as measured by increases in serum creatinine levels. RESULTS A total of 1343 patients(13.2%) received aprotinin,6776 patients(66.8%) received aminocaproic acid,and 2029 patients(20.0%) received no antifibrinolytic therapy. All patients underwent coronary-artery bypass grafting,and 1181 patients(11.5%) underwent combined coronary-artery bypass grafting and valve *** the risk-adjusted model,survival was worse among patients treated with aprotinin,with a main-effects hazard ratio for death of 1.32(95%confidence interval[CI],1.12 to 1.55) for the comparison with patients receiving no antifibrinolytic therapy(P=0.003) and 1.27(95%CI,1.10 to 1.46) for the comparison with patients receiving aminocaproic acid(P=0.004).As compared with the use of aminocaproic acid or no antifibrinolytic agent,aprotinin use was also associated with a larger risk-adjusted increase in the serum creatinine level(Pjusted incidence of dialysis(P=0.56). CONCLUSIONS Patients who received aprotinin had a higher mortality rate and larger increases in serum creatinine levels than those who received aminocaproic acid or no antifibrinolytic agent.

背景心外手术过程中的高血压常难以控制，并进而影响手术疗效。氯维地平是一种新型的2-氢吡啶L型钙通道阻滞剂，可有效降低动脉压，起效迅速，且半衰期极短。氯维地平治疗围手术期高血压的安全性事件评估研究（evaluation of clevidipine in the perioperative treatment of hypertension assessing safety events trial, ECLIPSE）旨在比较氯维地平（CLV）与硝酸甘油（NTG）、硝普钠（SNP）、尼卡地平（NIC）治疗心外手术患者急性高血压的疗效与安全性。方法我们分析了3项前瞻性、随机、开放性平行对照研究的资料，这些研究由61家医疗中心参与完成，比较了氯维地平与硝酸甘油、硝普钠、尼卡地平在心外手术患者的围手术期应用情况。共选择1964例患者，1512例按临床标准需紧急治疗高血压，符合随机化纳入标准。这些患者按1：1比例随机编入3个平行的对比治疗组。主要结局为30天内的死亡事件、心肌梗死、中风和肾功能不全，次要结局为血压控制的充分性与精确度的评估报告。结果氯维地平组与其他治疗组相比，心肌梗死、中风和肾功能不全的发生率均无显著差异。硝普钠治疗组与氯维地平组相比，死亡例数显著增加（P=0．04）。与硝酸甘油（P=0．0006）和硝普钠（P=0．003）相比，氯维地平能更有效地把血压控制在预定范围内。在控制血压于一个预定范围方面氯维地平与尼卡地平等效，但当预定血压控制范围较窄时，氯维地平较尼卡地平超出范围的幅度更小。结论氯维地平用于治疗心外手术患者的急性高血压安全有效。

Ileal atresia is considered to be an acquired anomaly. It is thought to be caused by a mesenteric vascular accident during fetal life which leads to necrosis of downstream tissues. We present a case in which ileal atresia occurred after intrauterine laser coagulation therapy for a twin-to-twin transfusion syndrome. The laser treatment might have been the etiological factor of the atresia. A second case in which thrombo-embolic calcifications were found in the liver is presented as well.
