Purpose: To present tec.niques and results of surgic.l repositioning of subluxed and disloc.ted c.psular tension rings (c.Rs). Design: Retrospec.ive interventional c.se series. Partic.pants: Eleven patients with a previously implanted c.R in-the-bag for zonularweaknesswho presentedwith c.R-intraoc.lar lens (IOL)-c.psular bag dec.ntration who underwent surgic.l repositioning. Methods: Data from 11 patients who underwent surgic.l repositioning were evaluated retrospec.ively for underlying diagnosis, interval between initial surgery and dec.ntration, surgic.l tec.nique, c.inic.l results, and c.mplic.tions. Main Outc.me Measures: c.psular tension ring-IOL-c.psular bag c.ntration, final best-c.rrec.ed visual ac.ity (Bc.A), and surgic.l c.mplic.tions. Results: Of the 11 patients with c.R dec.ntration, 3 had it early in the postoperative period, and 8 had it late. Mean (±standard deviation) durations from c.tarac. extrac.ion and c.R implantation to surgic.l repositioning were 6.1±7.9 months for those with dec.ntration early and 49.6±15.3 months for late dec.ntrations (overall range, 0.7-74.7). Of the 11 patients, 7 had pseudoexfoliation, and 4 of the 7 had assoc.ated glauc.ma. Nine patients had subluxation of the c.R-IOL-c.psular bag c.mplex, whic. was managed by an anterior segment approac.. A pars plana vitrec.omy and levitation of the c.R was required in 2 patients due to c.mplete disloc.tion of the c.R into the posterior vitreous. Surgic.l tec.niques for repositioning inc.uded single, double, or 3-point sc.eral suture loop fixation of the c.R through the c.psular bag c.mplex (8 eyes); use of the c.psular tension segment (c.S) plac.d within the c.psular bag for sc.eral suture fixation (2); or iris suture fixation of the IOL haptic. (1)-. All patients ac.ieved suc.essful anatomic.l repositioning of the c.R-IOL-c.psular bag c.mplex. Mean preoperative Bc.A improved from 20/100 to 20/40 postoperatively. Postoperative adjunc.ive bevac.zumab versus plac.bo in primary trabec.lec.omy surgery for glauc.ma

AIM: To c.mpare the effec.iveness of postoperative adjunc.ive use of subc.njunc.ival bevac.zumab in altering the outc.me of primary trabec.lec.omy in terms of sustained lowering of intraoc.lar pressure(IOP) and reduc.ion of postoperative bleb vasc.larization and ***: A prospec.ive, one c.nter, randomized, plac.bo-c.ntrol study. Fifty-nine patients(59 eyes) with unc.ntrolled IOP under maximal tolerated medic.l treatment(MTMT) were rec.uited. A primary trabec.lec.omy with mitomyc.n c.MMc. was done and the patients were randomized to either postoperative subc.njunc.ival injec.ion of bevac.zumab(1.25 mg/0.05 mL) or balanc.d salt solution(BSS). Forty-seven patients(47 eyes) c.mpleted at least one year of follow up and were inc.uded in the study. The main outc.me measure was the IOP, and sec.ndary outc.me measures inc.ude bleb morphology, vasc.larization, and fibrosis, as well as the need for glauc.ma medic.tions and 5-fluorourac.l(5-FU) ***: At 1-year follow up, there was no signific.nt differenc. between groups for IOP(P=0.65), bleb morphology(P=0.65), and the need for glauc.ma medic.tions(P=0.65) or 5-FU needling requirements(P=0.11). However, the bevac.zumab group had a higher rate of suc.ess results, lower use of glauc.ma medic.tions after surgery, and optimal bleb aspec. in more patients, but more 5-FU needling proc.dures required. c.Nc.USION: A bigger sample size is needed in order to determine whether the differenc.s found in the bevac.zumab group are statistic.lly signific.nt.
