Investigation of Spatial Risk Factors for RVF Disease Occurrence Using Remote Sensing &GIS—A Case Study: Sinnar State, Sudan

Rift Valley Fever (RVF) is an emerging, mosquito-borne disease with serious economical and negative implications on human and animal health. This study was conducted to verify the factors which influenced the spatial pattern of Rift Valley Fever occurrence and identified the high risk areas for the occurrence of the disease at Sinner State, Sudan. The normalized difference vegetation index (NDVI) derived from Moderate Resolution Imaging Spectroradiometer (MODIS) satellite and rainfall data in addition to the point data of RVF clinical cases in humans were used in this study. In order to identify the RVF high risk areas, remote sensing data and rainfall data were integrated in a GIS with other information including, soil type, water body, DEM (Digital Elevation Model), and animal routes and analyzed using Spatial Analysis tools. The information on clinical cases was used for verification. The Normalized Difference Vegetation Index (NDVI) was used to describe vegetation patterns of the study area by calculating the mean NDVI. The results of the study showed that, RVF risk increased with the increase in vegetation cover (high NDVI values), and increase in rainfall, which both provided suitable conditions for disease vectors breeding and a good indicator for RVF epizootics. The study concluded that, identification of high risk area for RVF disease improved the understanding of the spatial distribution of the disease and helped in locating the areas where disease was likely to be endemic and therefore preparedness measures should be taken. The identification represents the first step of prospective predictions of RVF outbreaks and provides a baseline for improved early warning, control, response planning, and mitigation. Further detailed studies are recommended in this domain.

Aim: To investigate depression, anxiety, PTSD, and insomnia levels among Sudanese citizens after a year has passed since the start of Armed Forces conflict in Sudan. Methods: An online survey was distributed. It is composed of five parts, covering the following areas: 1) sociodemographic data;2) depression assessment;3) generalized anxiety disorder assessment;4) post-traumatic stress disorder assessment;and 5) insomnia assessment. Statistical Package for Social Sciences version 27 was used for data analysis;frequency and percentage were used to describe the qualitative variables. Spearman’s correlation analysis and Chi-square test were used for correlation and association analysis;a P-value equal to or less than 0.05 was considered statistically significant. Results: The study included 283 participants, primarily female (76.0%), with a mean age of 35.26 ± 6.96 years. The majority of participants were married (50.5%) and had a university level of education (50.2%). At the start of the war, 70.3% of participants were inside the war zones, and at the time of data collection, 73.5% were outside Sudan. The length of stay in the war zone ranged from 1 to 300 days, with a mean of 48.59 ± 70.284 days. Additionally, 63.3% of participants stated that they did not receive any form of mental health intervention, and the remaining did not provide an answer. There was a high prevalence of moderate-severe depression (50.5%), moderate anxiety (35.3%), PTSD (56.5%), and sub-threshold insomnia (53.4%) among the participants. Correlation analyses revealed associations between demographic factors such as sex, age, marital status, and education and mental health issues. Notably, female participants showed a higher prevalence of PTSD (53.0%) compared to male participants (67.6%). Research Background: The high prevalence of diabetes in Sudan, estimated at 16%, highlights the importance of effective health education in diabetes management. Diabetes self-management education has been identified as a crucial tool in enhancing the knowledge, attitudes, and abilities necessary for self-management among individuals with diabetes. Aim: To assess the impact of diabetes self-management education on medication adherence and glycemic control in Sudanese adults with type 2 diabetes before and 3 months after the DSME intervention. Method: The study was conducted in Sudan between September 2022 and March 2023, it was an interventional, one-group, pre- and post-test study that aimed to assess the impact of diabetes self-management education (DSME) on medication adherence and diabetes control in Sudanese adults with type 2 diabetes. The research was conducted in primary health care centers in six cities in Sudan and involved 244 participants. The data entry and statistical analysis were conducted using the Statistical Package for Social Sciences version 27.0. A paired t test was used for analysis. Results: The study included 244 participants, 67% of whom were males. The age mean ± SD was 48.6 ± 9.3 years, and 85.3% of participants were married. Age at onset of diabetes mean ± SD was 40.60 ± 7.81 years;44.6% had diabetes for less than 5 years;and 84.1% had a positive family history of diabetes mellitus. The levels of poor, low, and partial adherence to medication decreased by 8.2%, 4%, and 20.6%, respectively, after the intervention. The levels of good and high medication regime adherence increased by 13% and 19.8%, respectively;BMI decreased by 1.1 ± 0.73 kg/m2 (p = 0.005). The fasting blood sugar decreased by 69 ± 32.9 mg/dl (p = 0.049), and the glycated hemoglobin decreased by 1.21 ± 0.28% (p = 0.001). Study of Thermal Reactivity and Kinetics of HMX and Its PBX by Different Methods

Vacuum Stability Test(VST)was used to determine the thermal behavior and kinetic parameters of 1,3,5,7-tetranitro-1,3,5,7-tetrazocine(HMX)and its mixture with hydroxyl-terminated polybutadiene(HTPB)as a binder coded as HMX/*** fitting and isoconversional method were applied to determine the kinetic parameters based on VST *** comparison,non-isothermal thermogravimetry analysis data(TGA)was also used to calculate the kinetic parameters by using Kissinger,OFW(Ozawa,Flynn,and Wall)and KAS(Kissinger-Akahira-Sunose)*** Kinetics and Technology Solution(AKTS)software was also used to determine the decomposition kinetics of the studied *** Scanning Calorimetry(DSC)was employed to determine the decomposition heat flow properties of the studied *** show that the activation energies obtained using VST results is 360.1kJ/mol for pure HMX and 186.9kJ/mol for HMX/*** activation energies obtained by the three different methods using TGA results are in the range of 360-368kJ/mol for pure HMX and 190-206kJ/mol for HMX/*** is concluded that values of kinetic parameters obtained by VST are close to that obtained by the different techniques using TG/DTG *** onset decomposition peak of HMX/HTPB is lower than that of HMX where the HTPB binder has negative effect on the thermal stability of *** results of all the applied techniques prove that HMX/HTPB has lower activation energy and heat release than the pure *** polymeric matrix has negative effect on the kinetic parameters of HMX.

Background: Stroke is a frequent medical problem and a leading cause of death and disability worldwide. Several conditions and lifestyle factors have been associated with stroke. Aim: To evaluate risk factors in stroke patients in Sulaimani city. Results: 110 patients with stroke were included in this study, hypertension was found to be the most common risk factor in current study. Out of 110 cases, 83 (75.5%) were hypertensive. Peak stroke-prone age was (60 - 69) year for male, (70 - 79) year for female. We found a statistically significant relation between level of TSC, LDL with ischemic stroke (r = 0.4047, P Conclusion: Hypertension is the leading risk factor of stroke. It is therefore essential to detect and treat hypertension at its outset. High value of atherogenic index mostly associated with ischemic stroke .while no relation found with haemorhagic stroke.

Background: Radial artery catheterization is a fundamental approach that is used as a procedural access in the different catheterization laboratories so our study is a comparison between distal radial artery approach and convential traditional transradial approach to explore the fesibility and safety of coronary angiography and percutanous coronary. Efficacy and safety of treatment of hepatitis C virus infection in renal transplant recipients

AIM: To assess the efficacy and safety of combinedpegylated interferon and ribavirin therapy in hepatitis C virus (HCV) infection in renal transplant recipients. METHODS: This is a retrospective chart review of post renal transplant patients who were positive for anti-HCV and HCV-RNA, and who have received treatment with combination of pegylated interferon and ribavirin between October 2003 and December 2008. Only patients with stable graft function and absence of evidence of cirrhosis and who received the therapy for continuous 48 wk were included. Nineteen patients (13 male and 6 female) were identified and included. The patient's complete blood count, liver and kidney prof ile, and cal- culated glomerular f iltration rate (GFR) were monitored every 6-8 wk while on treatment. HCV-RNA was tested at 12 wk for early virological response, at 48 wk for end of treatment response (ETR), and then retested at 24, and 48 wk after completion of therapy for sustained virological response (SVR). Liver biopsies were obtained before treatment from all patients and graft kidney biopsies were performed as required. RESULTS: Of the entire cohort, 9 patients (47.4%) showed an ETR and 8 had SVR (42.1%). Of the 8 patients with abnormal alanine aminotransferase (ALT) levels at baseline, 78.9% had their ALT normalized (including the virological non responders). ALT was normal in all responders at the end of therapy and at 24 wk post therapy (100%). Only one patient (5.3%) developed an increase in creatinine and decline in GFR from baseline towards the end of treatment. This patient's kidney biopsy revealed borderline rejection. There was no impact on response by HCV-genotype, initial HCV RNA load, age or sex of the patient or duration post transplant before commencement of therapy. All patients tolerated treatment in the same way as non-transplant with no unusual or increased occurrence of side effects. Background: The diverse modes in geographical locations, environmental factors, genetic and racial characteristics play a complex role in determining the pattern of neurological disorders worldwide. Determining the pattern of neurological disorders enables health policymakers to plan evidently for service, training, and research priorities. Few prevalence studies in neurology were conducted in Sudan. Methods: This is a retrospective hospital-based study that reviewed the medical records of patients who attended a Dedicated Neurology Clinic (DNC) in Omdurman, the national Capital of Sudan, for 24 months, from January 2016 to January 2018. This study aimed to determine the DNC pattern of neurological disorders as a representative subset prevalence in Sudan. Neurologists conducted the medical workup for diagnosis after at least two visits. All patients have ethically consented. Results: The total number of patients was 1050. Only 749 patients (71.3%) fulfilled the inclusion criteria. The mean age was 46.5 ± 1.9, and males were 45.3%. 72% were from the Capital. The presenting symptoms were headache (16.6%), seizures (11.5%), limbs weakness (11.2%), and lower percentages for other neurological symptoms. The commonest diagnoses were Stroke 12.4%, Epilepsy 9.3%, Primary Headache 8.8%, Movement Disorders 7.3%, Peripheral Neuropathy 6%, Dementia 4% Neuroinfections 1.4%, Demyelinating Disorders 2.6%, Spinal Spondylotic Radiculopathy 2.6% and 1.7% for Cerebral Venous Thrombosis. Conclusion: The data from Sudan-DNC showed that the most common neurological disorders descendingly were Stroke, Epilepsy, Headache, Movement Disorders, Peripheral Neuropathy, Dementia, Infections, Demyelinating Disorders, Spinal Spondylotic Radiculopathy, and Cerebral Venous Thrombosis.
