Prevalence and Factors Associated with Intestinal Metaplasia in Chronic Helicobacter pylori Gastritis in a Country with High Endemicity: Ivory Coast Case

Context/Objective: Few studies have been carried out in a country with high endemicity for Helicobacter pylori (H. pylori) infection in Sub-Saharan Africa looking for the association of intestinal metaplasia (IM) with chronic gastritis. We hypothesize that IM is correlated with the intensity of H. pylori infection in a country with high endemicity, Ivory Coast. The objective of this study was to determine the prevalence of intestinal metaplasia in chronic H. pylori gastritis in Ivory Coast. Methods: This was a prospective, cross-sectional, multicenter study, carried out over a period of 5 months, in the reference hospital centers of Abidjan, specialized in Gastroenterology. All patients who had undergone Gastroscopy with biopsies according to the criteria of the Sydney System for the anatomopathological study, those with chronic gastritis and/or H. pylori intestinal metaplasia on histology and in whom all the parameters of the Sydney system classification had been well informed. The quantitative variables were expressed by their means accompanied by their standard deviations and the qualitative variables by their numbers and percentages. Chi-square and Fischer tests were used to look for associations between variables. The significance level was set at 5%. Results: 152 patients were retained. The mean age was 44.9 ± 12.9 years. The prevalence of intestinal metaplasia was 11.8%. In univariate analysis, no significant association was found between clinical and pathological sociodemographic factors (age, sex, ethnicity, educational level, profession) and intestinal metaplasia in chronic Helicobacter pylori gastric cases. In multivariate analysis we found that prolonged use of Proton Pump Inhibitors (PPIs) and a history of Gastroesophageal Reflux Disease (GERD) were significantly associated with the absence of IM. Simulation of the Hydrodynamic Functioning of the Cavally River Using a Coupled 1D-2D Model in the Ity Area (Zouan-Hounien in Côte d’Ivoire)

In recent years, the Cavally River has been subject to multiple activities, including the construction of diversion channels and a bridge that makes it vulnerable to flooding. In order to assess the impact of these hydraulic structures on the river hydrodynamic functioning, a 1D-2D model was realized. The implementation of the 1D-2D model consisted of first running the 1D model, then the 2D model, and finally in coupling them. The 1D-2D model was designed with the 1988 flood hydrograph, a Manning’s coefficient of 0.052 m1/3/s for the minor bed and 0.06 m1/3/s for the major bed. The results of the hydraulic model show that the velocities are almost identical to those of the Cavally in natural operation. The values of the velocities are included between 0.4 m/s and 1.3 m/s at the level of the minor bed of the river and between 0.06 m/s and 0.71 m/s at the level of the floodplains. The average water level for flood propagation is 262.37 ± 0.44 m before construction of the structures and 262.23 ± 0.85 m after construction of the structures. The 0.41 m reduction in water level due to the diversion canal and bridge is negligible compared to the total fluctuations of the Cavally River, which vary from 6 to 7 m over the year.

Simulation of the Dynamics of Land Cover and Land Use in the Lobo River Watershed Upstream of Nibéhibé(Center-West of Côte d’Ivoire)

From medium-resolution satellite images (Landsat TM, ETM+ and OLI), the spatial dynamics of land cover and land use are highlighted. The objective of this study is to quantify the evolution of land use in the watershed of the Lobo River upstream of Nibéhibé between 1986 and 2019 in order to analyze the impacts of human activities on the landscape. The study method was based, on the one hand, on the processing of satellite images, for the analysis of the dynamics of land use and, on the other hand, on the CA-Markov model, for the prediction of land use by 2050. It emerged from this study that the land use maps produced made it possible to highlight the spatio-temporal dynamics of land use on the basin. For the period from 1986 to 2019, there is a decrease in the area of forests in favor of built-bare ground and crops and fallows. A land use scenario for the years 2019 and 2050 was simulated with an accuracy of 87.11%. The regressive trend in forests seems to continue in the future with current land use practices.

Epidemiological, Diagnostic, Therapeutic and Progressive Aspects of Arterial Hypertension in Children at the University Hospital Center of Bouaké (Côte d’Ivoire)

Introduction: Arterial hypertension (AH) in children is under-diagnosed and often has a poor prognosis. The aim of this study was to describe the epidemiological, diagnostic, therapeutic and evolutionary aspects of hypertension in children at the University Hospital of Bouaké, with a view to improving the prognosis. Methods: This was a cross-sectional, analytical study carried out in the paediatrics department of Bouaké University Hospital. It concerned the medical records of children aged 4 to 15 hospitalised from 1 January 2017 to 31 December 2020 for hypertension. Diagnosis was based on the simplified blood pressure guidelines of the Expert Consensus of the French Society of Hypertension. The variables studied were epidemiological, diagnostic, therapeutic and evolutionary. Quantitative variables were compared at the significance level p ≤ 0.05. Results: The hospital incidence of hypertension was 0.32% (69/21,642). The sex ratio was 0.72. 97.1% of the children were over five years of age. Oedema (49.3%) and breathing difficulties (20.2%) were the main reasons for consultation. Hypertension was classified as threatening (56.5%), confirmed (31.9%) and borderline (11.6%). The cause was renal in 66.7%, dominated by impure nephrotic syndrome (24.6%). Treatment for hypertension consisted mainly of a diuretic (79.7%) and a calcium channel blocker (47.8%). Outcome was favourable in 50.7% of cases. Mortality was 20.3%. No factor was significantly associated with death. Conclusion: Hypertension in children at Bouaké University Hospital is serious. The aetiology is mainly renal. Early diagnosis and management are key to improving prognosis.

Climate Impact on the Productivity of Sugarcane Varieties in Ferke1 Industrial Plantations

This study assesses the climate impact on the productivity of five sugarcane varieties (R579, SP711406, M2593/92, M1400/86, and SP701006) in the industrial plantations of Ferké 1 sugar complex. It is a contribution to research efforts aimed at increasing the productivity of sugarcane varieties in the sugar fields. Also to support agricultural development and guarantee the income of planters. The sugarcane production data are from 2013 to 2017. Climatological data are measured and calculated continuously daily at the production site. In addition, the CMIP-5 (Coupled Model Intercomparison Project) climate database at 1º × 1º horizontal resolution was used for the predictability of crop yields of the 5 sugarcane varieties in the near future (2021-2050) and far future (2056-2075) to improve the quality of climate services to producers. The statistical methodological approach by multiple linear regression associated with the significativity test shows important and significant coefficients of determination (R2 > 0.90) between the yields of sugarcane varieties with certain climatic parameters such as minimum and maximum temperatures, insolation, global solar radiation, and potential evapotranspiration. The impact of rainfall has not been directly evaluated because the linear models do not explicitly show sensitivities to this parameter and the total water requirements for the plot are completely assured by irrigation. The future climate projections analyzed only from extreme thermal parameters (Tmax and Tmin) highlight their strong sensitivities with yields from an idealized model. In this model, we have assumed that the water supply needed by sugarcane is always met by irrigation on different plots. Moreover, linear models do not evolve fast enough in time and changes due to external environmental constraints are not too important at the plot scale. This study introduces a novel image capture and lighting techniques using a cutting-edge hybrid MEMS scanner system designed for compact microscopic *** scanner comprises a tapered optical fiber waveguide and innovative aerosol-jet printed PZT(lead zirconate titanate)bimorph push-pull actuators on a stainless-steel substrate,effectively addressing issues that are commonly associated with PZT on silicon substrates such as fracture and layer *** leveraging nonlinear vibration,the scanner achieves a spiral scan pattern from a single signal input,in addition to the expected two-dimensional scanning and target illumination from two phase-shifted *** capability is further enhanced by a novel process to taper the optical fiber,which reduces illumination scattering and tunes the fiber to the resonant frequencies of the *** precisely tapered tip enables large fields of view while maintaining independent 2-axis scanning through one-degree-of-freedom *** validation showcases the successful generation of a spiral scan pattern with a 60μm diameter scan area and a 10 Hz frame rate,effectively reconstructing scanned images of 5μm lines,cross patterns(15μm in length with a 5μm gap),and structures of a Psychodidae wing.

Major Limb Amputation Secondary to Locally Advanced Cutaneous Squamous Cell Carcinoma in Tropical Environments: Determinants

Objective: To identify the determinants of secondary limb amputation in advanced squamous cell carcinoma. Material and Methods: This was a retrospective study carried out in the Orthopedic Traumatology Department of the Bouaké University Hospital in Côte d’Ivoire from January 2013 to December 2016. It involved ten patients with locally advanced skin squamous cell carcinoma, confirmed by histology and having amputated limbs. The parameters studied were: demographic data (gender, age), socio-economic conditions (occupation, place of residence), the risk factors involved, the use of topical self-medication and the use of traditional medicine, antecedents and comorbidities factors, lifestyle, clinical aspects (seat, size), extension assessment (X-ray), anatomo-pathological examination data, duration of evolution of squamous cell carcinoma, the function of the affected limb, the treatment performed, the evolutionary modalities and the equipment. Results: Between 2013 and 2016, ten patients underwent either upper or lower limb amputations following locally advanced squamous cell carcinoma. The average age was 34.1 years (19 - 64 years). There were 7 men (70%) and 3 women (30%) and all our patients were black (sub-Saharan Africa). The majority of our patients were farmers (70%). The risk factors for squamous cell carcinoma were neglected chronic wounds. All our patients recognized during the interrogation a large use of the topicals of traditional medicine for the treatment of the initial cutaneous lesions. In terms of lifestyle, five (50%) were smokers and alcoholics. The mean time to progression of squamous cell carcinoma was 5.9 years. Lesions were localized preferentially to the pelvic limbs six (60%) cases and four (40%) to the limbs chest. The lesion was diagnosed late in all our patients. The size of the tumor in all our patients was greater than 5 cm and had a deep invasion (nerves, vessels and bone). The proximal ipsilateral ganglionic invasion was constant.

Influence of Climate on Sugarcane Yield in Côte d’Ivoire: Case of the Ferkessédougou Region

This study aims to understand the current climatic trends and explain the possible losses of agricultural yields. To achieve this objective, this work characterized the evolution of extreme temperature indices in the sugar complexes of Ferké 1 and Ferké 2, two stations located in the northern part of C?te d'Ivoire. The onset and cessation dates of the rainy season and the length of the rainy season were investigated. The agricultural and climatic data were obtained from each sugar complex. The period of study ranges from 2002 to 2019 in Ferké 1 and Ferké 2. The results show significant upward trends in extreme temperature indices. The analysis of sugarcane yield associated with the different climatic parameters shows no significant results in general. However, on the Ferkessédougou sugar complexes, the results highlight that maximum and minimum temperatures could be the variables that influence most yield production. The maximum temperature with coefficients of 1.60 and 0.77 at Ferké 1 and Ferké 2 respectively seems to contribute to an increase in yield while the minimum temperature with coefficients of -0.98 and -0.22 at Ferké 1 and Ferké 2 respectively could lead to a loss in yield. The results obtained with the Single Linear Regression (SLR) and the Multiple Linear Regression (MLR) models also highlight the strong influence of minimum and maximum temperatures.
