Peri-Operative Morbidity and Mortality of Elderly Patients in Traumatology-Orthopedics in Bouaké

Introduction: Elderly subjects have physiological and clinical characteristics that make surgical management more complex and have a major impact on their prognosis. The primary objective of this study was to assess peri-operative hospital morbidity and mortality in the elderly. Secondary objectives were to identify factors predictive of death. Methods: This was a retrospective observational descriptive and analytical study carried out over 02 years, based on hospitalized patient records. It concerned patients aged 65 or over who had undergone surgery or for whom an indication for surgery had been given, all pathologies included. A total of 138 files were collected. The mean age was 74.15 years. Diabetes (n = 52;37.68%) and arterial hypertension (n = 38;27.54%) were the most common comorbidities. The reasons for hospitalization were mostly degenerative (n = 56;40.58%) and traumatic (n = 49;35.51%) pathologies. Preoperative delay, intraoperative and postoperative complications, hospital stay, evolution (mode of discharge), time of occurrence and presumed cause of death were studied. Univariate (p Results: The mean preoperative delay was 5.53 days (1 - 23). Intraoperative complications were mainly hemodynamic instability (n = 26, 18.8%). Postoperatively, the main complications were anemia (n = 62;48.44%) and glycemic imbalance (n = 28;21.86%). Average hospital stay was 13.4 days. Age (>75 years), the existence of comorbidities, degenerative pathologies, ASA score > 2, delay in treatment > 3 days, the occurrence of complications and the blood transfusion requirement were the independent predictors of the occurrence of death in the multifactorial analysis. Conclusion: Peri-operative morbidity in the elderly was influenced by age (>75 years), the existence of comorbidities, degenerative pathologies, ASA score > 2, time to treatment > 3 days, the occurrence of complications and the blood transfusion requirement.

Etiologies and Prognostic Factors of Dyspnea in Infants at the University Hospital Center (CHU) of Bouaké (Ivory Coast)

Identify the epidemiological characteristics, etiologies and evolutionary aspects of dyspnea in infants. This was a retrospective study of infants hospitalized for dyspnea from January 1 to December 31, 2020. The parameters studied were sex, age, origin, vaccination status, existence of underlying pathology. Underlying, the diagnosis and the evolutionary modalities. Data analysis and processing were possible using Word, Excel and EPI info version 7 software. We retained 152 infants. The sex ratio was 1.34 and the median age was 4 months. Vaccines according to expanded immunization program (EPI) were up to date in 76.32%. The main antecedents with risk identified were malnutrition, hypotrophy at birth, interventricular communication. The pathologies observed were low acute respiratory diseases in 90.79%, ENT diseases in 04.60% and cardiac diseases in 03.95%. The median length of hospitalization was 4 days. Infants who died accounted for 15.13%. The median age of infants who died was 4 months. The median time to onset of death was 1.63 days. The risk factors for death were age < 6 months (p = 0.003;CI [1.27;9.33]), outdated vaccines (p = 0.012;CI [1.18;5.17]), history with risk (p = 0.031;CI [1.02;4.54]). Dyspnea in infants remains a concern in our service. Reducing mortality involves developing procedures for the management of lower respiratory ailments, continuous staff training and strengthening the technical platform.

Bacteriological Profile of Infections Encountered in a Pneumology Service in a Moderate-Income Country [Pneumology Department of CHU Cocody (Côte d’Ivoire)]

Introduction: Infections represent a real public health problem aggravating the morbidity and mortality of hospitalized patients. Methods: This was a retrospective study with descriptive purposes over a period of 05 years, in the Pneumology Department of the University Hospital of Cocody. Results: The average age in our population was 42 years. We observed a male predo- minance of 64.5%, with a sex ratio of 1.8. Medical history was dominated by HIV infection (23.5%), followed by tuberculosis (15.6%). Concerning lifestyle, smoking was found in 38% of cases. Symptoms progressed chronically in 80% of cases. An infectious syndrome was found in 75% of cases. Microbial culture was positive in 42% of cases. Pseudomonas aeruginosa and Klebsiella pneumoniae were encountered in 26.4% of cases each, followed by Echerichia coli in 10.4% of cases. Klebsiella pneumoniae in the first three years topped the list, but gradually. Pseudomonas aeruginosa maintained its leadership over the last three years. Pseudomonas aeruginosa isolates expressed a resistance rate of 9.8% to ceftazidime and 8.1% to imipenem;to aztreonam (36%), ticarcillin (33.3%) and levofloxacin. These strains were susceptible to fosfomycin (100%), mero- penem (96.6%) and amikacin (96%). For isolated strains of Entero- bacteria- ceae, resistance was observed about ticarcillin (83.3%) and amoxicillin clavula- nic acid (71.2%). Streptococcaceae showed resistance to tetracycline (69.2%) and erythromycin (50%). Over the years there has been an increase in re- sistance to amoxicillin-clavulanic acid ceftriaxone. The death rate was 14%. Conclusion: The bacterial profile of infections is dominated by germs respon- sible for nosocomial infection with significant mortality.

Knowledge, Attitudes and Practices of Mothers Regarding Exclusive Breastfeeding in Rural Areas of Brobo (Côte d’Ivoire)

Introduction:In C?te d’Ivoire, despite awareness raising among mothers by health authorities and health care providers, the exclusive breastfeeding rate is still low, 23%. This rate seems even lower in rural areas. The objective of the study was to evaluate the knowledge, attitudes and practices of mothers regarding exclusive breastfeeding for the reduction of malnutrition in rural areas. Methods: This was a descriptive and analytical cross-sectional study conducted in Brobo from July to September 2020. It included consenting mothers of infants aged 0 to 24 months. Mothers’ knowledge, attitudes and practice were assessed by an anonymous structured questionnaire with a performance threshold set at 75%. Factors associated with exclusive breastfeeding were investigated by the Chi-square test at the 5% level of significance. Results: The average age of the 183 mothers was 25 years (extremes 13 and 49 years). They were primigravida (33%) and had a low socioeconomic level (84%). The mothers knew the definition of exclusive breastfeeding (33%). They knew that breastfeeding promoted ideal infant growth (64%) and protected against diarrhea and infection (18%). They also knew that breastfeeding reduced the mother’s risk of cancer (16%) and strengthened the mother-infant bond (7%). The mothers interviewed stated that they had put the baby to the breast within the first hour after delivery in 2% and 63% of them gave the baby colostrum. Seasonal Variation of Nutrient Pollution and Suspended Solids in the Drainage Network of Okpara Basin in Parakou(North-East of Benin)

This work presents mainly, the seasonal variation of pollution by nutrients and suspended solids thresholds between the old urban core and the peripheral areas of the city of Parakou on the Okpara basin. The city of Parakou has been experiencing strong demographic and socio-economic growth for about two decades. A total of sixty wastewater samples from twelve sampling sites were collected in 2016 according to the AFNOR standards, between the months of March and April in the dry season, then August, September and October in the rainy season. The descriptive statistic of data (minima-maxima) indicates variations of the parameters as follows: The temperature varies between 27.05 and 30.82, the water is a highly variable pH in the dry season (5.84 < pH < 9.14;mean = 7.17) and wet (6.28 < pH < 9.38;mean = 7.45). The reduction potential is lower in rainy season (-89.50 5 respectively reach the highest ratio (55 mg/L O2 and 232.08 mg/LO2) in the dry season than in the wet season (12.00 mg/L O2 and 202.40 mg/L O2). Nitrates have reached a maximum of 12.60 mg/L in the rainy season against 5.40 mg/L in the dry season. For nitrite, yielded 3.34 mg/L against 0.45 mg/L for conversely, phosphates are at 6.45 mg/L in the dry season against 2.32 mg/L in the rainy season. The mean concentrations of suspended solids are between 40 mg/L in the rainy season and 0.52 mg/L in the dry season. From the physico-chemical characterization and the chi-square independent test, this work shows mainly two results: On the one hand, apart from phosphates, concentrations of nitrates, nitrites and suspended solids on the Okpara basin in Parakou, are higher in the rainy season than in dry season. This can be explained by the variability of soil leaching and thus shows a relative link between the quality of surface water and the hydrogeomorphic basin conditions shown by the independence test (0.456 < CC < 0.856). Introduction: Unsuppressed viral load (VL) in immunocompromised children on antiretroviral therapy (ART) increases the risk of child morbidity and death. The aim of the study was to identify factors associated with unsuppressed viral load in children on ART for the improvement of prognosis. Patients and Methods: this is a retrospective, descriptive and analytical study carried out from July 2015 to December 2019 in the 28 pediatric HIV/AIDS treatment centers supervised by the NGO IRAA in the region of Gbêkê. It Included children from 0 to 15 years who were HIV positive, on ART for at least 6 months with at least one viral load. The variables studied were socio-demographic, diagnostic and evolutionary. Introduction: The harmonious development of the intestinal microbiota during the first 1000 days of life promotes the child’s immediate and future good health. Parkia biglobosa is a much-loved and over-exploited African savannah species for its socio-economic importance. Knowing and taking into account its architectural unit, which is the basis for diagnosing phenology, productivity and tree health, could provide a new perspective on its sustainable management. The aim of this study is to establish the architectural development in Parkia biglobosa by retrospective analysis. To achieve this objective, 390 individuals of all sizes ranging from seedlings to senescent trees were observed and analysed under various soil and climatic conditions in Côte d’Ivoire. The results showed that Parkia biglobosa is a light plant but shading tolerant. It is a mixed vegetative axis plant, the stem is orthotropic* in its proximal part and plagiotropic* (collapsing) in its distal part in young stage. The tree then transitions to an adult and old stage into a tree with a plagiotropic* axis in the proximal and distal parts, the trunk is built up by superimposing collapsed relay axes that gradually straighten, branching is sympodial*, growth is defined and sexuality is terminal and lateral. The ontogeny takes place in three phases: initiation of development and establishment of the crown (young), then flowering and establishment of the architectural unity (adult) and finally the death of secondary axes in the crown, duplication of the architecture by a series of partial and total reiterations (old). The level of organisation is 5: the phytomere, the module or growth unit, the axis, the architectural unit and the reiterated complex.
