Emergency Neonatal Obstetric Cares at Cocody University Hospital: Overview of Instrumental Extraction

Objectives: The aim of this study was to show an overview of situation of instrumental extraction at the maternity of Cocody University Hospital by determining availability and assessing the maternal and fetal prognosis. Patients and method: it was a retrospective study with a descriptive purpose on 6 months from January 1 to June 31, 2015. It concerned all patients admitted in the expulsive phase of labor delivery room and with an indication of fetal instrumental extraction and the state of their newborns at birth. Results: We saved 2288 vaginal deliveries;including 28 instrumental deliveries on 104 indications of instrumental extraction is an implementation rate of 26.9% (16 by vacuum extractor and 12 forceps). Among patients with indication but without instrumental extraction (n = 76), there is 42.3% vaginal delivery (n = 44) and 30.8% of cesarean section (n = 32). 44 have given birth vaginally (42.3%) and 32 by caesarean section (30.8%). It’s young patient (28 years), nulliparous (42.3%). Average time between instrumental extraction indication and the delivery of the baby was 58 min in the case of instrumental extraction and 1 hour 41 minutes in the case of spontaneous delivery in anticipation of the c-section. Motherhood had 3 instrumental extractors (2 vacuum extractors and 1 forceps) recycled after each use. The Apgar score was good in 85.7% in children born by instrumental extraction and bad in 54.5% in children born vaginally without instrumental extraction. We found 20 stillborn in intra partum occurred only in children born vaginally without instrumental extraction. Twelve (12) cases of bleeding of the issue by uterine atony (27.3%) were recorded in patients pregnant without instrumental extraction. No maternal deaths were observed. Conclusion: The realization of instrumental extraction rate remains low at the maternity of the UH-C. In the event, the fetal prognosis was better.

Malignant melanomas or lymphoma of the skin are malignant tumors of the skin and/or the mucous membranes whose uterine metastases are rare. The secondary uterine localizations, although rare, must be evoked in front of a pelvic tumoral syndrome, or diffuse metastases and a personal past history of melanoma, even after a long time of remission. In our observation, the evolution of the tumor in the pelvis extended to the muscular structures of the uterus and the sigmoid colon of a 72 year old patient, what made it an exceptional case. The diagnosis of these secondary localizations is a diagnosis of elimination, almost always post-operative, made on the histopathological and immunocytochemical study of the surgical specimen, supported by cytogenetics, even molecular biology. The treatment is based on chemotherapy.

Oil was extracted from seeds of Jatropha Curcas,in high yields(up to 40% by weight).The extracted Jatropha oil was converted in a laboratory reactor to biodiesel by *** of Jatropha oil and Jatropha biodiesel by GC/MS and GC/SIMDIS showed that Jatropha oil could be readily converted to a biodiesel product through NaOH catalyzed *** resulting biodiesel has desirable properties such as high cetane number and low flash point,which are major improvements over the properties of commercial biodiesel fuels.

Breast cancer is a major public health problem both in developed countries, where it represents the first female cancer, and in developing countries, where its incidence is increasing. Purpose: The authors report the results of a study whose objective was to describe the epidemiological and pathological features of women’s breast cancers in Ivory Coast. Material and Methods: This is a retrospective descriptive study on breast cancer histologically confirmed and made from the records of pathology laboratories of the University Hospital in Abidjan. The study period was 24 years (1984-2007). The parameters studied were: frequency, age, risk factors, the macroscopic and histological aspects. Results: Women’s breast cancer was the second cancer among women (13.69%) after cervical cancer and represented 6.51% of cancers. The average age was 45.21 years ranging from 10 years to 85 years. Subjects from 35 to 44 years were the most affected (30.7%) and 8% of patients were under 30 years (n=61). Multiparity (53.55%) and lower socioeconomic level (79.63%) were associated. At the macroscopic level nodular aspects represented 47.62% of cases (n=301). Carcinomas were 92% (n=710) of histological types with 94.36% (n=692) of invasive carcinomas. Sarcomas (5.7%), lymphoma (2.1%) and Paget’s disease (0.3%) were also observed. With regard to prognostic, the grading of Scarff-Bloom and Richardson showed 26.75% (n=156) grade I, 60.64% (n=354) grade II and 12.52% (n=73%) grade III. Conclusion: In consideration of prognosis of advanced forms, an effort should be made for diagnosis and early treatment of women’s breast cancer in Ivory Coast.

Indices of Water Quality and Pesticide Monitoring Survey in Lake of Western Cote d’Ivoire and Its Human Risk Assessment

The aims of the work were to study the current quality of the water in Lake DOHOU used for drinking water supply through several physical, chemical analyses and using water quality indices (WQI). In addition, the question was whether the populations are at risk after drinking water of lake following a reduction of 50% and 75% in the median and maximum values of pesticides. Thus, the results of the pesticide monitoring program were incorporated into probabilistic human health risk assessment exercises. Water samples were collected over a period of one year. Pesticides were subjected to solid phase extraction and then analyzed using gas chromatography coupled to tandem mass spectroscopy. The other parameters were measured according to conventional methods. The results showed that the most frequently detected pesticides were aldicarb (79%), simazine (79%) and monolinuron (44%). They also showed the mean values of concentrations exceeding 1.5 μg/L. Metoxuron detected at a frequency of 29% showed the maximum average concentration (13.46 μg/L). Nearly 98% of the sampling points had at least one substance with an average concentration above the quality standard (0.1 μg/L) and 80% did not meet the total concentration standards. Cumulative risk quotient estimates after 50% or 75% abatement for frequently identified pesticides were greater than unity when extreme?values for adults and children were considered. To determine the suitability of water for aquatic life, drinking water consumption and drinking water production, the water quality index (WQI), the heavy metal pollution (HPI) and the heavy metal evaluation (HEI) were calculated. The poor quality of the water was mainly related to pesticides, organic matter and microbiological parameters. Most of the nutrients and metals studied were often below the standards of drinking water and aquatic life. The synergistic study of Acacia Senegal (arabic gum) and Anacardium occidentale (cashew gum) combination in corrosion inhibition of mild steel was carried out to determine the inhibition efficiency of their combination. The gums were purified using standard methods and the blend prepared using the method as described by Femi – Oyewo et al. The blend was then used for corrosion study at two temperatures (303 K and 333 K) and the corrosion rates calculated. The adsorption process was found to satisfy the Langmuir isotherm and hence the sorption study reveals that the interaction is a physical one since the free energy of adsorption (ΔG^(o) ads kJ/mol) is given as (−16.47 and −15.61) kJ/mol at temperature T = 303 K and 333K accordingly. The GCMS technique was used to characterize the blend, five chemical compounds and their area percentage [6-O-(1-L-maloyl)-alpha-,beta-D-Glcp (veracylglucan A) (22.11%), 6-Octadecanoic acid (55.76%), Methylhexadecanoate (3.97%), 1-Pentadecane carboxylic acid (10.78%) and 9, 12-Octadecadienoic acid (7.38%)] were identified and subjected to computation corrosion study, the binding energy for the constituents of the blend on Fe surface were computed using Accelrys Material Studio and the mean values were found to be −327.097 kJ/mol, −446.943 kJ/mol, −602.826 kJ/mol, −608.625 kJ/mol and −693.749 kJ/mol respectively.

Chlorfenapyr, an insecticidal pyrrole, was applied to concrete arenas at concentrations of 1. 1, 0.825, 0.55, and 0.275 g of active ingredient [AI]/m^2. Adult Tribolium castaneum （Herbst）, the red flour beetle, were exposed for 2, 4, or 8 h at each concentration, then removed and held either with or without food （wheat flour） for 7 days. Survival was assessed when the beetles were removed fi＇om the exposure arenas and daily during the post- exposure period. In the presence of food, survival was high regardless of concentration and the day on which post-treatment survival was assessed, but survival did decrease as the exposure period increased from 4 to 8 h. When the beetles were not given food after exposure, survival at each concentration and exposure period declined during the 1-week post- exposure assessments. This pattern of decrease could be described by linear and non-linear equations. Results show the presence of food material greatly compromised effectiveness of the insecticide, and emphasize the importance of cleaning and sanitation in conjunction with insecticide treatments.

A series of field trials were conducted in a commercial food storage facility to evaluate exposure of stored-product insects to aerosol formulations of synergized pyrethrins and the insect growth regulator methoprene. When adults of Tribolium castaneum （Herbst）, the red flour beetle, and Tribolium confusum （Jacqueline DuVal）, the confused flour beetle were exposed with and without a food source to synergized pyrethrin aerosol, there was no difference in adult mortality with respect to availability of food at either 7 or 14 days after exposure （P≥0.05）. However, mortality was lower in T. confusum （40.4% and 79.3% with flour at 7 and 14 days, 38.9% and 84.8% without flour at 7 and 14 days） compared to T. castaneum （96.5% and 99.8% with flour at 7 and 14 days, 91.0% and 98.7% without flour at 7 and 14 days）. Few late-stage larvae and pupae of either species exposed to the pyrethrin aerosol emerged as adults. In tests with methoprene aerosol, adult emergence of exposed 3- and 4-week-old larvae of T. confusum was less than 2%. Only 0.3% of 4-week-old larvae of T. castaneum exposed in open and obstructed areas emerged as adults. Emergence of adults from eggs of Plodia interpunctella （Hubner）, the Indianmeal moth, embedded in culture media and exposed to the methoprene aerosol was 13.2%±3.5%. Results show that the aerosols evaluated in our study could give effective control of some of the major storedproduct insect pests in commercial food storage facilities, and may offer an alternative to fumigation.
