Treatment of Neglected Elbow Dislocations by Z-lengthening of the Triceps Brachii

Introduction: Neglected elbow dislocations are much more common in developing and underdeveloped countries. The main reason for the delay in diagnosis is that patients initially seek treatment from traditional healers. These will immobilize the elbow in extension, which will lead to the retraction of the triceps muscle and collateral ligaments. Patients and Method: We carried out a prospective study over five years (2014-2019). The study involved patients with unreduced elbow dislocation older than 21 days. All patients with a dislocated fracture and those whose x-ray showed joint fusion were not included in the study. We collected 26 patients, 20 men and 6 women with an average age of 27.5 years (17 and 45 years). The average consultation time was 8.9 months (3 - 13). The approach was posterior about 8 cm above the olecranon and 2 cm below the olecranon, the ulnar nerve was identified and isolated. The triceps tendon was cut in a Z from its myotendin junction to the beak of the olecranon, and then lengthened. Results: The patients were reviewed after a mean follow-up of 24 months (12 - 36). The Mayo Clinic’s average elbow performance index was 77.1 (60 - 85), with 19 good and 6 average results. The mean flexion was 114°(100°- 135°) and the mean extension deficit was 31.15°(10°- 55°). The average range of motion was 82.5°(60°- 120°). We did not find a correlation between the duration of the lesion and functional recovery (p > 0.05). The Wilcoxon test showed that surgery significantly improved the overall range of motion of the elbow (p = 0.001). Discussion: Based on this study, open reduction with tendon lengthening should remain a treatment option regardless of the age and chronicity of the dislocation.

Ankle Arthrodesis in the Treatment of Neglected Ankle Fractures

Introduction: Neglected ankle fractures are common in our regions. Patients first consult traditional healers and it is when the signs persist that they consult the surgeon for a swollen and painful ankle. Initially we always performed anatomical repairs with osteosynthesis, but the majority of patients still complained of pain and disability. We wondered whether a primary arthrodesis would not be indicated in old ankle fractures to avoid disabling pain for patients. We therefore performed a series of arthrodesis on neglected post-traumatic ankles. The objective of our study was to describe the results of arthrodesis on an old post-traumatic ankle and to propose a therapeutic indication in the face of any neglected ankle fracture. Material and Method: This was a prospective study over 8 years between December 2012 and November 2020, involving 36 cases of neglected ankle fractures treated by arthrodesis using Meary’s technique. We used AOFAS (American Orthopedic Foot and Ankle Score) preoperatively and postoperatively for an objective assessment. Results: The average follow-up was 4.2 years and our results were satisfactory (94.4%) with an average AOFAS score ranging from 27.8 preoperatively to 76.7 postoperatively. The fusion rate was 97.2%. Discussion: Arthrodesis appears here as an important alternative in the treatment of neglected ankle fractures with significant trophic disorders.

Potential Impact of Climate Change on the Sediment Fluxes of a Watershed in West Africa: Cas of the Aghien lagoon, Côte d’Ivoire

The semi-distributed SWAT (Soil and Water Assessment Tools) model was used in this study to model the sediment yield in the watershed of the Aghien lagoon with an area of 365 km2, located in the north of the district of Abidjan (South-East from C?te d’Ivoire). A sensitivity and uncertainty analysis, as well as calibration of the SWAT model, was conducted using the Sequential Uncertainty Adjustment Procedure (SUFI-2) which is one of the programs interfaced with SWAT in the SWAT-Cup package (SWAT-Calibration-Uncertainty Programs). Five parameters of the SWAT model were found to be more sensitive to sediment fluxes. These have been modified (calibration) sparingly in order to improve the reproduction of observed sediments data. Two measures were used to assess the uncertainty analysis of the model: P-factor and R-factor. The R2 and Nash-Sutcliffe (NS) coefficients of determination were used to assess the quality of the calibration. The P-factor obtained is 0.58 and the R-factor is 2.28. The NS and R2 coefficients in calibration over the period from June 2014 to January 2015 are 0.51 and 0.86 respectively. These values indicate correct consideration of uncertainties by the model and satisfactory calibration of the SWAT model for solid fluxes. This study focused on the transfer of experimental results of anaerobic digestion of liquid waste from an attiéké (steamed cassava semolina) factory to a 6 m3 pilot digester. The experimental digester and the pilot were powered as follows: lw + U + C (liquid waste + urine + cow dung). To the results, the experimental digester mesophilic with a progressive elimination of COD. Also, the nitrogen concentrations in the experimental reactor had little removal with alkaline pH. As for the biogas product in this digester, a volume of 3.6 m3 was obtained with a positive flammability test. The transition from the laboratory scale to the semi-industrial scale retains the results of purification and fuel biogas production of the experimental digester.

Qualitative Assessment and Typology of the Water Resource Used for the Production of Drinking Water in Duékoué, Western Côte d’Ivoire

The objective of this study was to evaluate the typology of water through physicochemical and bacteriological characterization with the aim of highlighting its vulnerability as a drinking water resource. The methodology adopted was to evaluate the level of pollution by monitoring the spatiotemporal variation of the water pollution indicator levels of lake Dohou. The sampling frequency is monthly for one year, from November 2017 to October 2018, in order to obtain a fairly representative image of water quality and its seasonal evolution in eight (8) well-defined stations. The parameters were determined using the standard methods defined by the French Association for Standardization (AFNOR). This study shows that the waters are acidic, with an average pH of 5.81 and a low electrical conductivity of between 42.67 ± 4.30 and 59.62 ± 21.84 μ***-1. At all stations, seasonal mean water transparencies are low (lso noted that 99.7% of the water samples collected had total nitrogen (TN) levels above the limit of 4 mg.l-1. All of the lake’s waters have non-compliant BOD5 (2.l-1) for raw water intended for the production of drinking water. Sites D6 and D7 have COD/BOD5 ratio greater than 3, which indicates the presence of non-biodegradable organic matter in these areas. Total coliforms, Escherichia coli and Enterococci were present in 100% (28/28) water samples at concentrations ranging from 2300 to 173,000 CFU/100 ml, from 100 to 1650 CFU/100 ml and from 20 to 1140 CFU 1/100 ml respectively. For Salmonella pathogens, they were detected in 50% of the dry season samples and in 100% of the rainy season samples. This almost permanent presence of this pathogenic germ denotes a poor quality of water with reference to this parameter. Hydrochemical Characterization of Groundwater in the Sub-Prefecture of Kokumbo (Ivory Coast)

In the Kokumbo sub-prefecture, groundwater extraction related to gold panning remains a major concern, not to mention the deterioration of its quality. Among the work carried out on water resources, no scientific interest has been shown in groundwater to characterise it. The objective of this study is to contribute to the knowledge of its physico-chemical quality. In situ measurements and physico-chemical analyses using an inductively coupled plasma optical emission spectrometer (ICP-OES) were carried out on five (5) human-powered pump (HPP) water samples and six (6) well water samples collected during low and high water seasons. The results show that the physico-chemical quality of the water, which is sometimes turbid, is satisfactory in terms of the mineralization of the borehole and well water, and the pH of the boreholes, while the temperatures of the two waters and the pH of the wells do not comply with WHO standards. The levels of major ions are recommended for consumption. The waters are classified as predominantly bicarbonate-calcium and magnesium (73%) in the dry season and in the flood season, with an equal split between bicarbonate-calcium and magnesium (45.5%) and chloride-calcium and magnesium (45.5%). The elimination of materials responsible for the turbidity of certain waters by managers or populations is essential for drinking water use. The risk linked to this element means that these turbid waters are not recommended for drinking water.

Assessment of the Quality of Sediments and Agricultural Soils: Case of the Ity-Floleu Area in the Prefecture of Zouan-Hounien, Western Côte d’Ivoire

The objective of this study is to assess the level of metal contamination of sediments and agricultural soils in the Ity-Floleu zone. The concentrations of trace elements (Fe, Mn, As, Cd, Cr, Cu, Hg, Ni, Pb and Zn) were measured in different seasons over two successive years. The sediment pollution index made it possible to note that the sediments and agricultural soils of the various stations studied are highly polluted in all seasons. The calculation of the geoaccumulation index indicates that surface water sediments most often experience extreme or moderate pollution in As, Cd, Cu, Hg and Zn in the dry or rainy season except in Pb in some cases. We observed that the sediments of the Cavally river present a serious pollution due to extreme anthropic activities carried out along the river. Over the entire region, the results of the potential ecological risk index (RI) indicate that all the sediments and agricultural soils analysed present a moderate ecological risk in terms of Pb and Zn in certain cases and an ecological risk is observed low bound to other metals in all seasons. This metallic pollution generated by human activities in this region can have consequences for the environment and biodiversity.

Evaluation of Pollution Due to Gold Mining at Ity-Floleu Area in Sub-Prefecture of Zouan-Hounien, Western Côte d’Ivoire

This study aims to evaluate the mineral pollution of a gold mining environment in the sub-Prefecture of Zouan-Hounien (C?;te d’Ivoire). Samples were taken from surface water and flesh and skin samples of the Cavally River fish species over a period of two successive years. Concentrations in major elements, trace elements (As, Cd, Cr, Cu, Hg, Ni, Pb and Zn), nutrients () and physico-chemical parameters in situ were measured. The results obtained show that the surface waters have very low pH with an average of 5.70. For nutrient salts, the deterioration of water quality in the study area is mainly related to alterations by nitrates, phosphates and sulphates. The concentrations of Fe, Cu, Hg and Pb are above the existing standards (international, FAO/WHO) for the majority of the stations, thus causing the deterioration of the surface water quality. Therefore, their use for agricultural irrigation could have negative impact on the health of the population and on biodiversity. Regarding the bioindicating species, the results obtained after analysis of samples showed that the fish (carp and machoiron) have Fe, Cu, Hg and Pb concentrations higher than the existing standards (international, FAO/WHO) exposing consumers to high health risks. This study showed good correlation between the high values of surface water turbidity and the proximity of gold washing sites. The use of the principal component analysis (PCA) of the physicochemical dataset revealed comparable behaviors for certain minerals within the groups (Fe, K, Mg, Mn and Na) and (As, Cd, Cu, Ni, Pb, Zn and Ca) in water. The Ascending Hierarchical Classification (AHC) analysis has defined three different classes of surface water in the study area. The presence of mineral trace elements in these receiving environments is of natural origin but would be strongly accentuated by human activities.

COVID-19 in Bouake (Côte D’Ivoire): Comparative Study of the Epidemiological, Clinical, Therapeutic, Evolutionary and Prognostic Aspects in Younger and Elderly Patients

Introduction: The clinical manifestations of many diseases vary with age, and older people often do not show typical symptoms of the disease. The present study aims to compare the epidemiological, clinical, therapeutic and evolutionary aspects of elderly patients with COVID-19 compared to young adult patients and to identify risk factors for mortality. Patients and Methods: This was a retrospective single-center analytical study conducted from January 27, 2021 to January 27, 2022 at the COVID-19 Care Center of the Infectious and Tropical Diseases Department of the Bouaké University Teaching Hospital. The study population consisted of all patients aged at least 18 years seen in consultation and/or hospitalized with a positive COVID-19 RT-PCR. The patients were divided into two groups: younger patients (Results: Of a total of 779 patients, 644 (82.7%) were young. The median age of all patients was 41 years (IQR 32 - 54, extreme 18 and 96). Of all the patients 38.5% had a comorbidity. Compared to younger patients, chronic heart disease (61.7% vs 21.5%;p Conclusion: Elderly people with COVID-19 have a different clinical presentation from younger, characterized by more atypical symptoms. Mortality risk factors are age, dyspnoea, impaired consciousness and fever. It is therefore necessary to act on its various factors to improve the prognosis of COVID-19 in this age group.

A systematic study was carried out to assess the level of contamination with fluorides and heavy metals in the drinking water of the city of Daloa as well as the risks to the health of consumers. The waters of 11.11% of the sites sampled exceeded the fluoride limit for drinking water with a contamination index (CI) greater than 0. All the waters recorded concentrations of cadmium (Cd), copper (Cu), iron (Fe), manganese (Mn) and lead (Pb) above the recommended values with CI > 0. However, 22.22% of the sites recorded concentrations below the standard for zinc (Zn) with IC lth showed that the chronic daily intake (CDI) of fluorine and metals was less than 1 (CDI lts and children except for Zn where the CDI > 1 for children in 22.22% of drinking water studied. HQs have an average of less than 1 for fluorine and greater than 1 for all metals. Moreover, the danger indices have values greater than 1. The incremental lifetime cancer risk (IlCR) and the total IlCR are above the recommended values. These results showed that the drinking water sampled is of poor quality due to higher levels of heavy metals, which can constitute a danger to human health. long-term use of one of these poor quality waters can lead to cancer in consumers. It is therefore necessary to treat this water in order to eliminate the metals before using it for drinking. This study can help decision-makers and competent authorities in charge of water management.
