Comparative Study of Gut Microbiota in Breast Cancer Patients versus Controls in Abidjan, Côte d’Ivoire

Context: The gut microbiota represents a complex ecosystem encompassing all unicellular microorganisms residing in the digestive tract, primarily bacteria, fungi, archaea, and even viruses. The relationship between the host and the microbiota is symbiotic: bacteria benefit from a stable environment, while the host gains numerous capabilities in terms of digestion, metabolism, nutrition, and immunity. However, numerous studies suggest that the gut microbiota plays a crucial role in various non-communicable diseases, including obesity, chronic inflammatory bowel diseases, allergic and immune disorders, behavioral disorders, and even certain cancers. The objective of our study was to characterize the gut microbiota of a group of breast cancer patients by comparing it to that of control subjects in Côte d’Ivoire, using a metagenomic approach. Method: A case-control study was conducted from May 2020 to September 2023. A total of 85 women (39 cases and 46 controls) were recruited, and stool samples were collected from both breast cancer patients and healthy women. Among these, ten (10) samples from patients and ten (10) samples from healthy women were randomly selected for the study of the gut microbiota. The gut microbiota was characterized by sequencing the V4 region of 16S rRNA using metagenomic NGS technology, and bioinformatic analysis was performed using the mothur pipeline. Results: In women with breast cancer, we observed a reduction in the relative abundance of the phyla Firmicutes and Bacteroidetes, as well as an increase in the phyla Actinobacteria and Verrucomicrobia. Additionally, their microbiota exhibited lower Chao1 and Sobs diversities compared to the control women (p < 0.05). Molecular variance analysis (AMOVA) revealed a significant difference between the case and control groups (p < 0.001). Production of Yam Tubers Using Seed Tubers from Vitroplants Regenerated from Aerial Stems in the Yam Species Dioscorea alata (L.) and Dioscorea cayenensis-rotundata (L. & P.) in Côte d’Ivoire

The aim of this study is to produce large quantities of yam tubers in the field from seed tubers derived from vitroplants regenerated from aerial stem cuttings and farmers’ tuber seeds. Seed tubers from ten yam cultivars, including five from Dioscorea alata (cDa083, cDa053, cDa115, cDa150 and cDa266) and five from Dioscorea cayenensis-rotundata (cDr015, cDr027, cDr150, cDr206 and cDr148) with distinct agro-morphological characteristics, were used as material. A completely randomized block design with 3 replications was used. In each block, all cultivars and seed types were represented. When put into cultivation, the tubers produced by the vitroplants all germinated and gave 100% healthy plants, compared with 86% to 100% healthy plants for the seed tubers produced by the farmers. Yields per hectare ranged from 10 to 25 tonnes for seed tubers produced by in vitro plants, and from 4 to 9 tons for seed tubers produced by farmers. Seed tubers from vitroplants can therefore be recommended to farmers as a solution to the problem of seed tuber unavailability.

Outcomes of Hemodialysis Patients in Public Centres in Abidjan from September 2018 to October 2021: Data from the Renal Registry of Cote d’Ivoire

Background: Hemodialysis is the most widely used renal replacement therapy in developing countries such as Cote d’Ivoire. Objective: To study the outcome of chronic hemodialysis patients in public centres in Abidjan using data from the Renal Registry of Cote d’Ivoire (2RCI). Method: This was a prospective analytical cohort study of End Stage Kidney Disease (ESKD) patients undergoing iterative hemodialysis in the network of public hemodialysis centres in Abidjan and who had been registered in the 2RCI register by September 2018. Results: Our study involved 363 cases out of 379 patients registered in the 2RCI registry database in 2018. The mean age of the patients was 47.3 ± 12.1 years, with a male predominance (sex ratio 1.97). In 70.24% of cases, they had no health insurance. The average duration of dialysis for our patients was 6.94 ± 4.1 years, with extremes of 2 and 28 years. This duration was less than 5 years in 44.3% of cases, between 6 and 10 years in 42.9% and more than 10 years in 12.6% of cases. The comorbidities found were hypertension in 95.6% of cases, diabetes in 7.8% and HIV in 4.6%. Anemia was found in 68% of cases and stroke in 8.6%. During the study period, 161 deaths were observed, representing a mortality rate of 44.35%. Acute lung oedema (29.8%), stroke (6.8%) and other cardiovascular diseases (19.3%) were the main causes of death. In multivariate analysis, factors such as age ≥ 65 years (HR = 3.66;CI 95% = 1.55 - 8.67;p = 0.003), “married/coupled” status (HR = 2.02;CI 95% = 1.24 - 3.31;p = 0.005) and normal weight at the start of dialysis (OR = 9.59, CI95% = 4.19 - 21.95;p = 0.001) were associated with the risk of death. Conclusion: Hemodialysis is performed in Abidjan on young patients. The mortality rate after three years of dialysis is very high. Hence the need to pursue the policy of decentralizing public centres and optimizing access to dialysis in terms of quality and quantity in order to improve patient survival.

Edible Ectomycorrhizal Mushrooms Russula spp. of Côte d’Ivoire: Total Phenolic Content, HPLC-Profiles of Phenolic Compounds and Organic Acids, Antioxidant Activities

The aim of this study was to investigate the phenolic compounds content, HPLC-profiles of phenolic compounds and organic acids, and also antioxidant activities via the ability to scavenge DPPH radical of three wild edible mushrooms belonging to Russula genus and being collected in center of Côte d’Ivoire. Total phenolic compounds, flavonoids and tannins contents of methanolic extracts were assessed by colorimetric assays. So, the obtained values of these chemical parameters ranged from 394.05 to 513.50 mg/100 g DW, 94.50 to 139.95 mg/100 g DW and from 124.20 to 165 ± 0.54 mg/100 g DW, respectively. Otherwise, HPLC-profiles of the methanolic extracts revealed that quercetin, salicylic acid and tannin ol were the main phenolic compounds in R. delica whereas R. lepida contained gallic acid, catechin and protocatechuic acid as main phenolic compounds. Besides, it showed that the phenolic compounds such as salicylic acid, tannin ol and catechin were observed in R. mustelina. As for HPLC-profiles of organic acid, the fumaric and malic acid were recorded as the main organic acids in the three species of wild edible mushrooms. However, citric acid content was found to be highest in R. lepida. The methanolic extracts of the three mushrooms exhibited high DPPH radical scavenging activities ranging from 74.92% to 58.92%. These wild edible mushrooms could be considered a potential supply source of adequate natural antioxidant for local population.

Looking of finding an alternative to the use of chemical fertilizers to increase yields of cassava (Manihot esculenta), trials were carried out in the Lamto zone in central Côte d’Ivoire. Thus, the effects of compost and ash from cocoa shell and NPK were tested on the agronomic parameters of cassava. The trial was conducted for two years with four varieties of cassava: Yacé, Alleda agba, Six mois and Bonoua, grown on elementary plots treated with one of these fertilizers. The experimental design was complete randomized blocks with three replicates. Analysis of the results showed that the Six mois variety gave the longest stems (131, 67 cm) with the cocoa shell compost. The Bonoua variety, on the other hand, gave the largest diameter of the stem base (21.56 mm), a higher number of leaves (77.30) and a large wingspan (136.

Infantile Hemangiomas: Epidemio-Clinical Profile and Therapeutic Difficulties in Côte d’Ivoire

Introduction :Infantile haemangiomas are the most common benign vascular tumours of infants. In Western countries, they have been the subject of several studies. However, in sub-Saharan Africa, few works are devoted to them. We initiated this work to contribute to a better knowledge of infantile haemangiomas in our work context. Material and Methods: This is a cross-sectional descriptive and analytical study carried out in the Dermatology Department of C.H.U. Treichville from November 2014 to December 2016;All children with infantile haemangiomas have been taken into account, in the study. Results: We counted 42 cases of haemangiomas on 4600 paediatric dermatoses, a prevalence of 0.9%. The sex ratio was 0.35. The age of our patients ranged from 2 days to 5 years. 76.2% of patients consulted after the age of 2 months. 26.9% of patients were born premature. 54% of the haemangiomas started with an erythematous macula. 52.4% had a size between 1 and 3 cm. Superficial haemangiomas (52.38%) and mixed haemangiomas (42.85%) were the most common clinical forms. The ulcerated forms represented 9.5%. The lesions were preferentially located in the cervico-facial region (71.4%) and resulted in aesthetic prejudice in 47% of cases. 3 patients (7.14%) were treated with propranolol. 37 patients (88.1%) used self-medication using poultice. Conclusion: Infantile haemangiomas are infrequent in Cote d’Ivoire. They sit mainly in the cervico-facial area. The ulcerated forms are not negligible and they sometimes cause a significant aesthetic prejudice. Their management is sometimes difficult because of the unavailability of propranolol.

To determine the occurrence of Enterobacter sakazakii and other Enerobateriaceae in commercial powdered infant formula (PIF), 185 packages of PIF from different manufacturers, supermarkets and drug-stores in Abidjan were analyzed. Ten g of sample was homogenized in 90 ml of buffered peptone water (PBW, Biorad, Paris) for further studies. Enterobacteriaceae (coliforms) were enumerated according to French Association of Standardization methods. E. sakazakii was detected according to Kandhai’s method. Bacteria were identified using API20 system. Thirty-eight samples (20.5%) were positive for Enterobacteriaceae. Twenty-four samples (13%) yielded Enterobacter sakazakii. Other Enterobacteriaceae isolated included Pantoea spp. 21 (11.5%), Klebsiella pneumoniae subsp. Pneumonia 8 (4.3%), Citrobacter diversus 1 (0.5%), Citrobacter freundii 1 (0.5%), Enterobacter cloacae 1 (0.5%), Salmonella reading 1 (0.5%), Serratia ficara 1 (0.5%) Serratia odorifera 1 (0.5%). This study is the first report to describe the contamination of PIF from Abidjan with E. sakazakii and several other Enterobacteriaceae that could be opportunistic pathogens. Therefore, well-controlled studies need to be conducted to assess the extent of risk associated with contaminated PIF for infants in Abidjan.

Problem of Obstetrical Evacuations: About 630 Cases Collected at the Maternity of Bouake University Hospital

Objective: To improve the care of evacuees at the maternity of the University Hospital Center of Bouaké (CHUB). Methodology: This is a cross-sectional and descriptive study over a period of three months that covered 630 cases collected at the Maternity of the university Hospital of Bouaké. Results: Obstetric evacuation accounted for 42.5% of admissions to the delivery room. Patients under 20 years and over 34 years of age respectively represented 21% and 13.5% of the total. 62.7% of evacuees were not educated and 84.9% had low economic level. Nulliparous and multiparous women accounted for 54.9% of the patients and 88% had mean prenatal follow up. The evacuations were decided by midwives (91.3%), without previous adapted treatment (79.4%), with a badly filled evacuation card (49.7%) and no partograph. The taxi was the most used means of transportation (75.2%) and most evacuees took less than one hour to access the referral center (61.4%). The reasons for evacuation are mainly dominated by mechanical obstructions (34.8%);on admission the evacuees with no real reference reason were 243 (38.6%) and the diagnosis was inconsistent in 43% of cases. The majority of evacuated women delivered vaginally (69.4%). Most newborns had a satisfactory state at the 5th minute of life (79.4%) and we noted 54 cases (08.3%) of neonatal deaths. 7.8% of evacuees had a complication dominated by postpartum anemia (51.1%);we had lamented 17 cases (2.7%) of maternal deaths among evacuees, attributable to delivery haemorrhage (47.1%) and eclampsia (23.5%). Conclusion: A better organization of the reference and an equipment of the peripheral health structures would improve the prognosis of the evacuees.

Perimeters of Protection Demarcation for New Boreholes of Bonoua Aquifer (Southeastern, Côte d’Ivoire) through Hydrogeological Modeling

Located in the southeastern of Côte d’Ivoire, Bonoua aquifer contains an enormous groundwater potential. The new boreholes have been realized in this region to support the water supply of the populations. Moreover, in this region, agriculture is the main activity, whose pesticides used to guarantee the productivity, constitute a threat for groundwater. Thus, this study was undertaken in order to design a hydrogeological model able to simulate the piezometry of the Bonoua aquifer and determine the perimeters of protection for the new boreholes. The model, carried out under the Visual Modflow interface, was designed in steady-state mode and calibrated manually from the piezometry of the year 2000. The value of the standardized root mean squared residual of 4.86% (<10%) allows to say that the calibration obtained is considered satisfactory. Also, the correlation coefficient between observed and simulated heads of 0.95 permits to confirm a good calibration of the model. Perimeters of protection values were determined by simulating the paths of the virtual particles of water from the calibrated model. The simulations show that generally, the groundwater of Bonoua aquifer flows from north to south and the piezometric levels obtained vary between 65 m in the north and 5 m in the south. Upstream radius of the closed perimeters of protection calculated from the model varies from 172 to 482 m. However, those of distant perimeters of protection vary from 2877 to 6441 m.

Hydrochemical Characterization of Groundwater in the Sub-Prefecture of Kokumbo (Ivory Coast)

In the Kokumbo sub-prefecture, groundwater extraction related to gold panning remains a major concern, not to mention the deterioration of its quality. Among the work carried out on water resources, no scientific interest has been shown in groundwater to characterise it. The objective of this study is to contribute to the knowledge of its physico-chemical quality. In situ measurements and physico-chemical analyses using an inductively coupled plasma optical emission spectrometer (ICP-OES) were carried out on five (5) human-powered pump (HPP) water samples and six (6) well water samples collected during low and high water seasons. The results show that the physico-chemical quality of the water, which is sometimes turbid, is satisfactory in terms of the mineralization of the borehole and well water, and the pH of the boreholes, while the temperatures of the two waters and the pH of the wells do not comply with WHO standards. The levels of major ions are recommended for consumption. The waters are classified as predominantly bicarbonate-calcium and magnesium (73%) in the dry season and in the flood season, with an equal split between bicarbonate-calcium and magnesium (45.5%) and chloride-calcium and magnesium (45.5%). The elimination of materials responsible for the turbidity of certain waters by managers or populations is essential for drinking water use. The risk linked to this element means that these turbid waters are not recommended for drinking water.
