Anesthesiological Management of Eclampsia in Tropical Africa: Experience of the University Hospital Center of Bouake

Introduction: Eclampsia is a common cause of maternal mortality in developing countries. The objective of this study is to evaluate the practice of anesthesia for eclampsia in tropical Africa. Patients and Methods: Retrospective descriptive study on the parturients who benefited from eclampsia anesthesia in the operating theaters of the gynecology and obstetrics department, the University Hospital Center (CHU) of Bouake over a two-year period (January 2015 to December 2016). The parameters studied were: anesthetic risk assessment, anesthetic management, immediate anesthetic and post-anesthetic accidents and incidents. Results: Out of a total of 3831 emergency cesarean sections, 65 were for eclampsia, or 1.69% of obstetrical surgical activity. The mean age of the patients was 20.30 ± 3.34 years (range: 14 years and 39 years). Prenatal consultation was not performed in 70% of cases and 85% of patients were primiparous. Patients classified as ASA IIIU accounted for 75% of the population and in 62% of cases, the Glasgow score was between 9 and 12. Induction was achieved with thiopental in 98% of patients and vecuronium was the only muscle relaxant used. The most common fetal complications were hypotrophy (50%), prematurity (30%) and respiratory distress (40%). Postoperative anesthetic complications were agitation (70%) and wake delay (15%). Maternal mortality was 5% and the neonatal death rate was around 7.69%. The elements of poor maternal prognosis were the ASA IVU class (P = 0.015) and the Aldrete awakening score between 3 and 5 awakening (P = 0.001). Conclusion: Anesthesia for eclampsia at CHU of Bouake is difficult. It needs to be improved thanks to better equipment of the health structures.

Practical Artificial Ventilation in an African Tropical Environment: Experience of the Intensive Care Unit of the University Hospital of Treichville

Objective: To describe the practice of artificial ventilation (VA) in a resuscitation unit of a developing country with a view to its improvement. Patients and Methods: Prospective study for descriptive and analytical purposes, carried out in the intensive care unit of the University and Hospital Center of Treichville (Ivory Coast) from April 2009 to June 2010. All the patients having benefited from a artificial ventilation for a duration greater than 6 hours were included in this study. The studied parameters were: Socio-demographic (age, sex), diagnostic, therapeutic (indications, duration and complications of artificial ventilation), evolutionary. Results: Out of a total of 204 admissions during the study period, 81 patients received artificial ventilation, an incidence of artificial ventilation in the order of 39.7%. There were 49 men and 32 women. The ventilated patients had an average age of 43.9 years (range: 4 years and 85 years). Pathologies requiring artificial ventilation were neurological (46%) and traumatic (28%). stroke was the leading medical condition (65%) while polytrauma was the major traumatic condition (65%). The most commonly used ventilatory modes were controlled volume ventilation (52.4%) and assisted ventilation (34.9%). The mean duration of artificial ventilation was 5.98 ± 3.73 days (range: 1 day and 21 days). The nosocomial pneumonia acquired under mechanical ventilation (PAVM) constituted 27% of the complications observed under artificial ventilation. The average length of ICU stay for all ventilated patients was 9.85 +/- 7.51 days (range: 1 day and 31 days). The lethality in our series was 80%. Patient age was the only prognostic factor associated with death (P = 0.003). Conclusion: The practice of artificial ventilation is still difficult in Ivory Coast and is at the origin of many complications such as nosocomial pneumonia acquired under mechanical ventilation which complicate the life threatening of the patients.

Evaluation of the Effect of Four Levels of Shade on the Growth and Development of Desmodium adscendens with a View to Its Domestication as a Cover Crop in Côte d’Ivoire

Desmodium adscendens is a plant in the Fabaceae family that is very important to man, both agronomically and medicinally. It is an important cover crop that contributes to improving soil fertility, reducing the risks associated with crop pests and weeds. However, very little research has been carried out into its domestication for use as a cover crop in banana plantations in Côte d’Ivoire. The general objective of this study was to evaluate, under semi-controlled conditions, the effect of different levels of shade on the growth and development of Desmodium adscendens. The studies were carried out on the experimental site of the UP-Plant Physiology Laboratory. Four shading levels were evaluated: 0 (NO), 20% (N1), 40% (N2) and 60% (N3) light levels. The Desmodium adscendens cuttings used were collected from a 3-month nursery. For each treatment, 10 tubs containing sterilized soil were used, and 50 cuttings were planted. After planting, growth and development parameters were assessed weekly on 10 plants. In addition, yield and fresh and dry mass were measured. Finally, the reproductive cycle of Desmodium adscendens was determined. The results showed that plant growth and development were greatest in plants grown under the 40% shade level, followed by the 20% and 60% shade levels. In contrast, plants in full light showed poor growth and development. The 40% shade level was the most favorable for growing Desmodium adscendens cuttings, with a cycle length of 67 days. Growing Desmodium adscendens under shade with 20% and 40% light levels could be recommended to farmers.

Rain-Flow Modeling Using a Multi-Layer Artificial Neural Network on the Watershed of the Cavally River(Cote d’Ivoire)

Water resources management is nowadays a significant stake for the world. However, missing or bad quality of the hydro-climatic historical data available of the studied area makes sometimes hydrological studies difficult. Generally, conceptual rain-flow models are designed to bring an appropriate answer with the correction of gaps and prediction of the flows. Historical hydro-climatic data of the Ity station, located on Cavally River, contain gaps which must be bridged. This study aims to establish a rainfall-runoff model through artificial neural networks for filling the gaps into the flow data series of the hydrometric station of Ity on the watershed of Cavally River. A multi-layer perceptron of feed forwards with two entries (monthly average rain and evapotranspiration) and an exit (flows) was established with flow evapotranspiration data. Comparison of the criteria of performance of the various architectures of the neural network model showed that architecture 2-3-1 gives best results. This architecture provides Nash coefficients of 75.79% and correlation linear coefficient of 95.64% for the calibration and Nash coefficients of 73.32% and correlation linear coefficient of 98.33% for the validation. The correlations between simulated flows and observed flows are strong. The correlation coefficients are 83.89% and 83.08% respectively for the calibration and validation.

A Comparison of Hydrothermal Aging, SO₂ and Propene Poisoning Effects on NH₃-SCR over Cu-ZSM-5 and Cu-SAPO-34 Catalysts

This study was aimed to investigate the effects of hydrothermal aging, propene and SO2 poisoning on the ammonia-selective catalytic reduction (NH3-SCR) performance of both Cu-SAPO-34 and Cu-ZSM-5. The catalytic activities of fresh, aged and poisoned samples were tested in ammonia-selective catalytic reduction (NH3-SCR) of NOx conditions. The XRD, TG and N2-desorption results showed that the structures of the Cu-SAPO-34 and Cu-ZSM-5 remained intact after 750˚C hydrothermally aged, SO2 and propene poisoned. After hydrothermal aging at 750˚C for 12 h, the NO reduction performance of Cu-ZSM-5 was significantly reduced at lower temperatures, while that of Cu-SAPO-34 was less affected. Moreover, Cu-SAPO-34 catalyst showed high NO conversion with SO2 or propene compared to Cu-ZSM-5. However, Cu-ZSM-5 showed a larger drop in catalytic activity with SO2 or propene compared to Cu-SAPO-34 catalyst. The H2-TPR results showed that Cu2 ions could be reduced to Cu and Cu0 for Cu-ZSM-5, while no significant transformation of copper species was observed for Cu-SAPO-34. Meanwhile, the UV-vis DRS results showed that CuO species were formed in Cu-ZSM-5, while little changes were observed for the Cu-SAPO-34. Cu-SAPO-34 showed high sulfur and hydrocarbon poison resistance compared to Cu-ZSM-5. In summary, Cu-SAPO-34 with small-pore zeolite showed higher hydrothermal stability and better hydrocarbon and sulfur poison resistant than Cu-ZSM-5 with medium-pore.

The urban environment is confronted with recurring calamities including floods, landslides and forced escapes of populations during periods of rain. In Côte d’Ivoire, the municipalities of Abobo and Attécoubé suffered enormous material and human damage following the torrential rains of 2014. These urbanized areas are built on large plateaus bordered by thalwegs where 61% of these areas have no drainage network, thus facing a lack of sanitation. Moreover, topographical, rainfall, anthropogenic and technical conditions are factors explaining the occurrence of natural disasters and their consequences in the said communes. The objective of this article is to assess the socio-economic impact of the occurrence of hazards. It focuses on the identification of risk sectors in the municipalities of Abobo and Attécoubé and the material, human and economic consequences that result from rain accidents.

Infantile Hemangiomas: Epidemio-Clinical Profile and Therapeutic Difficulties in Côte d’Ivoire

Introduction :Infantile haemangiomas are the most common benign vascular tumours of infants. In Western countries, they have been the subject of several studies. However, in sub-Saharan Africa, few works are devoted to them. We initiated this work to contribute to a better knowledge of infantile haemangiomas in our work context. Material and Methods: This is a cross-sectional descriptive and analytical study carried out in the Dermatology Department of C.H.U. Treichville from November 2014 to December 2016;All children with infantile haemangiomas have been taken into account, in the study. Results: We counted 42 cases of haemangiomas on 4600 paediatric dermatoses, a prevalence of 0.9%. The sex ratio was 0.35. The age of our patients ranged from 2 days to 5 years. 76.2% of patients consulted after the age of 2 months. 26.9% of patients were born premature. 54% of the haemangiomas started with an erythematous macula. 52.4% had a size between 1 and 3 cm. Superficial haemangiomas (52.38%) and mixed haemangiomas (42.85%) were the most common clinical forms. The ulcerated forms represented 9.5%. The lesions were preferentially located in the cervico-facial region (71.4%) and resulted in aesthetic prejudice in 47% of cases. 3 patients (7.14%) were treated with propranolol. 37 patients (88.1%) used self-medication using poultice. Conclusion: Infantile haemangiomas are infrequent in Cote d’Ivoire. They sit mainly in the cervico-facial area. The ulcerated forms are not negligible and they sometimes cause a significant aesthetic prejudice. Their management is sometimes difficult because of the unavailability of propranolol.

Characterization of the Origin of Pollutants in Groundwater from Biostatistical Tests:The Case of Catchment Ehania,South-Eastern Cote d’Ivoire

The threat of agro-industrial exploitations on the quality of groundwater resources has become a major concern for the world’s population, mainly in agricultural areas. To solve this problem, several methods such as vulnerability to pollutants and statistics methods, have been adopted to protect these groundwaters. In this study, statistical tests were used to determine the likely source of pollutants in the groundwater resources. First, we made a comparative analysis and then analyzed pollutants parameters using Kohonen self-organizing map (SOM) which allows basic on the similarity to help out any correlation between the physic-chemical parameters. To validate these results, a land use map was developed to identify the sources of pollutants in production functions of different activities taking place on the catchment area. The results indicate very high levels of certain pollutants such as NO3-, Cl- and SO42- with generally higher than World Health Organization (WHO) standards values. These high levels observed could be attributed to the generally free nature of aquifers alteration and sedimentary formations in areas of high agro-industrial production. The analysis of the data from SOM method reveals a strong correlation between pollutants (NO3-, SO42-,Cl- ) and conductivity showing the likely origin of these surface pollutants,an origin that is linked to the existence of larges agro-industrials exploitations. These results are reinforced by those of the overlay map of the sampling sites on the land use map, which shows a strong coincidence between high values of pollutants and production areas of the pollutants that are agro-industrial exploitations. These areas of coincidence are observed in the north where found banana’s industrials plantations that are less visible and mostly in the southwest part where is one of the largest oil palm plantations in the world (36,000 hectares).

Production of High-Purity Silica Sand from Ivorian Sedimentary Basin by Attrition without Acid Leaching Process for Windows Glass Making

To produce high-purity silica sand usable for glass making, the present study was carried out. The objective of this work was to increase the silicon dioxide (SiO2) content to at least 99% using a simple process without chemical input. The raw sand samples were taken from the Ivorian sedimentary basin, from Maféré and Assinie areas, Côte d’Ivoire. Wet sieving and attrition technique were used for the purification process. The results from the energy dispersive spectrometer (EDS) analyses of the raw and treated samples show a significant increase of silica content and a significant reduction of impurities. The silica content (SiO2) of the sand of Maféré increases from 98.73% ± 0.15% to 99.92% ± 0.05%. And the sand of Assinie increased from 98.82% ± 0.67% in the raw samples to 99.44% ± 0.27% after treatment. The rate of iron oxide and alumina is reduced in these sands. Moreover, the sand of Maféré contains 53.2% of grains of size lower than 500 microns and that of Assinie contains 29.30%. Regarding the chemical composition of these purified sands, they meet the standard BS2975s, the American Ceramic Society and the National Bureau of standards for window glass making.

Socio-Cultural Representations and Therapeutic Itineraries of Parents of Autistic Children Followed in the INSP’s Children’s Mental Health Services

Pervasive developmental disorders (PDD) remain little known to populations in developing countries. In black Africa their social representations remain strongly influenced by local belief systems. The general objective of this study was to understand the perceptions and representations of Ivorian parents vis-à-vis PDD. This was a mixed (qualitative and quantitative) prospective cross-sectional study with a descriptive aim that involved a sample of 49 parents. The sampling was of the qualitative type by multiple cases with reasoned choice by saturation. Our results showed that male parents were mostly aged between 40 - 49 years (48.98%) with a higher level of education (67.34%) while mothers were mostly aged between 30 - 39 (61.22%) and a higher level (30.61%). Autistic children were negatively perceived by their parents: either as a source of psychological suffering (82.85%), or as mysterious children who sacrificed their parents (44.66%), or as “bobo” children (mute children in common Ivorian language) (16.66%) or like rude children (13.34%). The supposed origin of the disorder according to the parents was mystical-religious (60.94%);natural (25%);hereditary (6.25%). In 6.25% of cases, PDD were assumed to be of unknown or iatrogenic origin attributable to vaccination (1.56%). 75.51% of parents said that in addition to conventional medical therapies, they also used traditional therapies. The use of this therapeutic alternative would be linked to the perceptions and beliefs that feed the socio-cultural representations of our respondents.
