Introduction:In C?te d’Ivoire, despite awareness raising among mothers by health authorities and health care providers, the exclusive breastfeeding rate is still low, 23%. This rate seems even lower in rural areas. The objective of the study was to evaluate the knowledge, attitudes and practices of mothers regarding exclusive breastfeeding for the reduction of malnutrition in rural areas. Methods: This was a descriptive and analytical cross-sectional study conducted in Brobo from July to September 2020. It included consenting mothers of infants aged 0 to 24 months. Mothers’ knowledge, attitudes and practice were assessed by an anonymous structured questionnaire with a performance threshold set at 75%. Factors associated with exclusive breastfeeding were investigated by the Chi-square test at the 5% level of significance. Results: The average age of the 183 mothers was 25 years (extremes 13 and 49 years). They were primigravida (33%) and had a low socioeconomic level (84%). The mothers knew the definition of exclusive breastfeeding (33%). They knew that breastfeeding promoted ideal infant growth (64%) and protected against diarrhea and infection (18%). They also knew that breastfeeding reduced the mother’s risk of cancer (16%) and strengthened the mother-infant bond (7%). The mothers interviewed stated that they had put the baby to the breast within the first hour after delivery in 2% and 63% of them gave the baby colostrum. In the particular context of climate change in côte d’Ivoire and the vulnerability of farmers to its effects, one of the major issues is how these changes could impact cocoa yields of cocoa production areas. Thus, the objective of this study is to sustainably increase the resilience of all cocoa farming stakeholders to the impacts of climate change. The study was carried out in the central and southern areas of côte d’Ivoire with a focus on eleven localities that have many communities of cocoa producers and a humid climate. The rainfall and temperature observation data using come from the cRU, they cover the historical period from 1971 to 2000 at 0.5o × 0.5o horizontal scale. As for the RcP4.5 and RcP8.5 climate scenarios, they come from the cORDEX database and cover the 2021-2050 period. The methodology is based on the calculation of climatic indices sensitive to cocoa cultivation which are the number of consecutive dry days (cDD), the number of consecutive wet days (cWD), the amount of rain during the rainy season and the maximum temperature above 33℃. The results show that for all the localities studied, indices such as cDD and cWD could experience an increase. In addition, the total amount of rain during the long rainy season (April to June) is calculated on the basis of the threshold of 700 mm representing the minimum annual precipitation during the rainy season necessary for good growth of the cocoa tree. It reveals that for the two scenarios the cumulative rainfall will all be greater than 700 mm. Regarding temperatures, the central and southern areas could have a low number of hot days (temperature greater than or equal to 33℃ which is the tolerable threshold for cocoa cultivation). The eleven localities, therefore, remain favorable areas for cocoa cultivation in terms of climatic conditions based on temperature and rainfall, despite the regional dimension of the effects of climate change and the associated constraints.

Factors Associated with the Unsuppressed Viral Load of children on Antiretroviral Therapy Followed Up in the GbêkêRegion (côte d’Ivoire)

Introduction: Unsuppressed viral load (VL) in immunocompromised children on antiretroviral therapy (ART) increases the risk of child morbidity and death. The aim of the study was to identify factors associated with unsuppressed viral load in children on ART for the improvement of prognosis. Patients and Methods: this is a retrospective, descriptive and analytical study carried out from July 2015 to December 2019 in the 28 pediatric HIV/AIDS treatment centers supervised by the NGO IRAA in the region of Gbêkê. It Included children from 0 to 15 years who were HIV positive, on ART for at least 6 months with at least one viral load. The variables studied were socio-demographic, diagnostic and evolutionary. Data analysis was descriptive and analytical with a significance level of p Results: out of 329 children included, 118 (62 boys, 53 girls) had a non-suppressed VL, i.e. a prevalence of 36%. The mean age at diagnosis was 61 months. The mother was a small trader (36.4%), illiterate (45.8%). Parkia biglobosa is a much-loved and over-exploited African savannah species for its socio-economic importance. Knowing and taking into account its architectural unit, which is the basis for diagnosing phenology, productivity and tree health, could provide a new perspective on its sustainable management. The aim of this study is to establish the architectural development in Parkia biglobosa by retrospective analysis. To achieve this objective, 390 individuals of all sizes ranging from seedlings to senescent trees were observed and analysed under various soil and climatic conditions in côte d’Ivoire. The results showed that Parkia biglobosa is a light plant but shading tolerant. It is a mixed vegetative axis plant, the stem is orthotropic* in its proximal part and plagiotropic* (collapsing) in its distal part in young stage. The tree then transitions to an adult and old stage into a tree with a plagiotropic* axis in the proximal and distal parts, the trunk is built up by superimposing collapsed relay axes that gradually straighten, branching is sympodial*, growth is defined and sexuality is terminal and lateral. The ontogeny takes place in three phases: initiation of development and establishment of the crown (young), then flowering and establishment of the architectural unity (adult) and finally the death of secondary axes in the crown, duplication of the architecture by a series of partial and total reiterations (old). The level of organisation is 5: the phytomere, the module or growth unit, the axis, the architectural unit and the reiterated complex. Appropriateness of colonoscopy in cocody teaching hospital center in 2010: A prospective study using criteria established by the European panel on the appropriateness of gastrointestinal endoscopy (EPAGE)

Aims: 1) To assess the feasibility of EPAGE criteria in clinical practice;2) To assess appropriateness colonoscopy using EPAGE criteria;3) To compare colonoscopy appropriateness and endoscopic lesions. Method: Hundred thirtynine consecutive patients explored by colonoscopy were included. The appropriateness of colonoscopy was evaluated by EPAGE criteria. Results: EPAGE criteria were applicable among 127 patients (91% of the cases). colonoscopies were appropriate, uncertain and inappropriate in respectively 40%;27% and 24% of the cases. Rate of abnormal colonoscopies was significantly different between the 3 groups (p = 0.03). cancers were diagnosed exclusively in the groups with appropriate and uncertain colonoscopies (11% and 5%). colonic adenomas were observed in the three groups (appropriate (7%), uncertain (5%) and inappropriate (6%)). Subjects with uncertain indication were older (58.25 years;p = 0.035). conclusion: EPAGE criteria are feasible in clinical practice in cocody’s teaching hospital center. colonoscopies are generally appropriate in our hospital. However, within sight of the significant number of normal colonoscopies and discovered lesions in uncertain indication, colonoscopy cannot concern exclusively calculation of a score of appropriateness.

Background: Some national and international strategies for the detection and prevention of emerging infectious diseases have been established across sectors. The capacity to carry out these tasks varies from country to country, and that remains largely undervalued. The Pasteur Institute of c?te d’Ivoire has created and implemented the capacity of national reference centers to fight against emerging and other infectious diseases. Objective: Show on the one hand the strategies used to develop the National Reference centers and the IPcI and on the other hand the results obtained by performing these strategies. Method: Datas collection by documentary analysis (published scientific articles and grey literature) was done on Google Scholar, PUBMED and institutional reference documents. The documentary research was carried out to have a better understanding of strategies used to create and develop the NRcs in microbiology of communicable diseases. Results: Seven integrated strategies were launched: 1-training and workforce development;2-investigation of epidemics or public health events;3-strengthening laboratory epidemiological research;4-strengthening surveillance systems;5-improving communication with partners and stakeholders;6-building national and international collaborations;and 7-strengthening technical and technological platforms. In two decades, the number of researchers has risen from 10 in 2004 to ninety (90) in 2021, with 12 senior researchers and 32 junior researchers. A number of health service staff had attended a qualifying training course, 27 investigations into outbreaks and other public health events had been carried out, 18 short-term research projects had been launched, major surveillance programs and epidemiological research efforts on vector-borne, food-borne and nosocomial infections had begun, and several scientific manuscripts had been published or were edited in the writting press. conclusion: The Ivorian experience shows that, with concerted eff

Prevalence of HBs Ag among blood donors in Transfusion center, Abidjan (Ivory coast)

Aim: To study the prevalence of HBsAg among persons without risk factors for hepatitis B. Patients and Methods: cross-sectional study (28 January to 31 December 2010) about 4310 new volunteers blood donors consecutively recruited. Among all, a risk factor for hepatitis B was investigated (standardized questionnaire and clinical examination). Blood of all blood donors without risk factors of hepatitis B (n = 3823) were systematically collected for HBsAg, hepatitis c antibody, syphilis serology and human immunodeficiency virus serology. The blood was analyzed with a third generation ELISA (Abbott Murex). Results: HBsAg was positive in 179 persons (4.68% [95% cI 4% - 5%]). It was associated with hepatitis c antibodies (0.28% (n = 11)), syphilis antibodies (0.68% (n = 26)) or anti-human immunodeficiency virus antibodies (0.2% (n = 8)). In multivariate analysis, age 30 to 39 years (1.545 [95% cI 1.074 to 2.222]), male sex (2.426 [95% cI 1.550 to 3.799]) and positive syphilis serology (14.344 [95% cI 5.161 to 39.865]) were associated with presence of HBsAg. conclusion: The prevalence of HBsAg among adult without risk factors for hepatitis B was not negligible. Also, it is essential to maintain routine screening before any vaccination against hepatitis B in adult. Our study is not for an immunization against hepatitis B only targeting risk populations in adult.

conjunctival squamous cell carcinoma is a common malignant tumor in adults. It affects the peri-limbic conjunctiva. This is a neoplasia that is increasing in tropical Africa because it occurs in subjects with prolonged exposure to ultraviolet rays, in patients immunocompromised to the human immunodeficiency virus and HPV infection. computed tomography allows one to make a diagnosis. We report the case of 57 patients in whom the diagnosis was made radiologically and confirmed by histology.

The duodenal metastases of a malignant melanoma are rare. We report a case at a 86-year-old, hypertense patient, without tumoral history, who was hospitalized in our department for a change of the general state and an anaemia ferriprive without found home (foyer) of display. In the clinical examination, we had found anemia syndrome, nodules under cutaneous about 2 cm in diameter at the level of the thorax, of the belly, the members and of the lower lip partners in a painless left adenopathy axillary about 3 cm in diameter and an absence of méléna in the rectal touch. The gastroscopy had highlighted polypoides ulcéro-bourgeonnantes hurts with a blackish pigmentation by places at the level of the bulb and of the second duodenum. In the histology of biopsie fragments, we noted a fuso-cellular proliferation and epithelioid with deposits of melanin. The positivity of antibodies anti-protein S100 and anti HMB45 in the immunohistochemical study allowed confirming the diagnosis. The gastroduodenal hurts, persons suffering from a liver complaint and lungworts in the scanner testify of a terminal metastatic scattering. The death arose two months later, in a picture of circulatory collapse further to a high digestive bleeding of big abundance.
