Within the framework of disaster risk management, this article proposes an interdisciplinary method for the analysis of multiple natural hazards, including climate change’s influences, in the context of cultural heritage. A taxonomy of natural hazards applicable to cultural heritage was developed based on the existing theoretical and conceptual frameworks. Sudden-onset hazards, such as earthquakes and floods, and slow-onset hazards, such as wetting–drying cycles and biological contamination, were incorporated into the hazard assessment procedure. Future alteration of conditions due to climate change, such as change in heat waves’ duration, was also taken into account. The proposed hazard assessment framework was applied to the case of the Historic Centre of Rethymno, a city on the northern coast of the island of Crete in Greece,to identify, analyze, and prioritize the hazards that have the potential to cause damage to the center’s historic structures. The assessment procedure includes climate model projections, GIS spatial modeling and mapping, and finally a hazard analysis matrix to enable the sharing of a better understanding of multiple hazards with the *** results can facilitate decision making by providing the vulnerability and risk analysis with the nature and spatial distribution of the significant hazards within the study area and its setting.

AIM To evaluate the utility of fecal calprotectin(FC) in predicting relapse and endoscopic activity during follow-up in an inflammatory bowel disease(IBD) *** All FC measurements that were obtained during a 3-year period from patients with inflammatory bowel disease in clinical remission were identified. Data regarding the short-term(6 mo) course of the disease were extracted from the medical files. Exclusion criteria were defined as:(1) An established flare of the disease at the time of FC measurement,(2) Loss to follow up within 6 mo from baseline FC measurement, and,(3) Insufficient data on file. Statistical analysis was performed to evaluate whether baseline FC measurement could predict the short term clinical relapse and/or the presence of mucosal *** We included 149 [Crohn's disease(CD) = 113, Ulcerative colitis(UC) = 36, male = 77] IBD patients in our study. Within the determined 6-month period post-FC measurement, 47(31.5%) had a disease flare. Among 76 patients who underwent endoscopy, 39(51.3%) had mucosal healing. Baseline FC concentrations were significantly higher in those who had clinical relapse compared to those who remained in remission during follow up(481.0 μg/g, 286.0-600.0 vs 89.0, 36.0-180.8, P < 0.001). The significant predictive value of baseline median with IQR FC for clinical relapse was confirmed by multivariate Cox analysis [HR for 100μg/g: 1.75(95%CI: 1.28-2.39), P = 0.001]. Furthermore, lower FC baseline values significantly correlated to the presence of mucosal healing in endoscopy(69.0 μg/g, 30.0-128.0 vs 481.0, 278.0-600.0, in those with mucosal inflammation, median with IQR, P < 0.001). We were able to extract cut-off values for FC concentration with a high sensitivity and specificity for predicting clinical relapse(261 μg/g with AUC = 0.901, sensitivity 87.2%, specificity 85.3%, P < 0.001) or mucosal healing(174 μg/g with AUC = 0.956, sensitivity 91.9%, specificity 87.2%, P < 0.001). Chronic infection with hepatitis C virus(HCV) is a leading cause of liver-related morbidity and mortality worldwide and predisposes to liver fibrosis and endstage liver complications. Liver fibrosis is the excessive accumulation of extracellular matrix proteins, including collagen, and is considered as a wound healing response to chronic liver injury. Its staging is critical for the management and prognosis of chronic hepatitis C(CHC) patients, whose number is expected to rise over the next decades, posing a major health care challenge. This review provides a brief update on HCV epidemiology, summarizes basic mechanistic concepts of HCV-dependent liver fibrogenesis, and discusses meth-ods for assessment of liver fibrosis that are routinely used in clinical practice. Liver biopsy was until recently considered as the gold standard to diagnose and stage liver fibrosis. However, its invasiveness and drawbacks led to the development of non-invasive methods, which include serum biomarkers, transient elastography and combination algorithms. Clinical studies with CHC patients demonstrated that non-invasive methods are in most cases accurate for diagnosis and for monitoring liver disease complications. Moreover, they have a high prognostic value and are cost-effective. Non-invasive methods for assessment of liver fibrosis are gradually being incorporated into new guidelines and are becoming standard of care, which significantly reduces the need for liver biopsy.

AIM:To compare diverse endoscopic interventions in the management of occluded uncovered self-expanding metal stents(SEMSs) that had been placed for palliative treatment of unresectable malignant biliary ***:A retrospective review was undertaken in 4 tertiary endoscopic centers to determine optimal management of different types of occluded *** technical success of performed treatment in occluded SEMSs,the patency of the stent,the need for re-intervention and the financial costs of each treatment were ***:Fifty four patients were included in the analysis;21 received Hanaro,19 Wallstent and 14 *** the relief of obstruction,a plastic stent was inserted in 24 patients,a second SEMS in 25 and mechanical cleaning was performed in 5 *** overall median second patency rates between second SEMSs and plastic stents did not differ(133 d for SEMSs vs 106 d for plastic stents;P = 0.856).Similarly,no difference was found between the overall survival of SEMS and plastic stent groups,and no procedure-related complications *** cost analysis showed that successive plastic stenting was a cost-saving strategy at least in ***:Insertion of uncovered SEMSs or plastic stents is a safe and effective treatment for occluded uncovered SEMSs;insertion of plastic stents appears to be the most cost-effective strategy.

The discovery of diagnostic and prognostic biomarkers for neurodegenerative diseases represents an unmet clinical challenge. For example, the diagnosis of Parkinson's disease (PD) relies mainly on the presence of clinical symptoms. Therefore, the identification and use of novel PD biomarkers would allow the application of disease-modifying treatments at the very early stages of neurodegeneration. The presynaptic protein, α-synuclein, has been genetically and biochemically linked with PD pathogenesis and has been considered as a potential biomarker for the diagnosis of PD and the related synucleinopathies. The vast majority of studies have assessed the measurement of α-synuclein, alone or in combination with other biomarkers in the cerebrospinal fluid (CSF), since it is the biofluid that most closely reflects the pathophysiology of the brain. The diagnostic value of the monomericα-synuclein but also the oligomeric, the phosphorylated and the aggregated forms of the protein has been evaluated using a variety of immunoassays. The results have so far been reproducible but the assays used are still lacking the required diagnostic accuracy. Recent reports have shown that Protein misfolding cyclic amplification is a technique that has the potential to detect α-synuclein seeds in samples of CSF with high sensitivity and across different synucleinopathies. In an effort to increase the source of biomarker for PD and related synucleinopathies,α-synuclein has also been measured in neuronal exosomes, small vesicles of endosomal origin that are secreted from neurons into the CSF or the periphery. The potential diagnostic value of exosomes stems from the notion that exosomes carry a disease-specific repertoire of marker proteins. Therefore, the assessment of exosome-associated α-synuclein species may also open up new avenues for disease diagnosis in different synucleinopathies.

Diffusive modelling of glioma evolution: a review

Gliomas, the most aggressive form of brain cancer, are known for their widespread invasion into the tissue near the tumor lesion. Exponential models, which have been widely used in other types of cancers, cannot be used for the simulation of tumor growth, due to the diffusive behavior of glioma. Diffusive models that have been proposed in the last two decades seem to better approximate the expansion of gliomas. This paper covers the history of glioma diffusive modelling, starting from the simplified initial model in 90s and describing how this have been enriched to take into account heterogenous brain tissue, anisotropic migration of glioma cells and adjustable proliferation rates. Especially, adjustable proliferation rates are very important for modelling therapy plans and personalising therapy to different patients.

The present study investigates the force transfer mechanisms for open hoop fiber reinforced plastic (FRP) strips attached at reinforced concrete (R/C) beams with or without anchorage. These open hoop FRP strips are utilized in R/C beams that are in need of shear capacity upgrade. This type of retrofitting is necessary for R/C structures designed with less stringent seismic loading conditions than those currently required. For this purpose special unit beam concrete specimens were fabricated and were used to attach open hoop carbon (CFRP) or steel (SFRP) FRP strips with or without anchoring. A novel loading arrangement was utilized to apply the necessary forces to these unit beam specimens together with instrumentation capable of capturing the behaviour of these specimens up to failure. Studying in this way the transfer of forces from the open hoop FRP strips, it could be demonstrated that when this type of retrofitting was accompanied with a properly designed anchoring device, a significant increase in the bearing capacity of the tested specimens was observed. Moreover, the observed failure was that of the fracture of the FRP strips for all such specimens. The highest degree of FRP material exploitation was achieved in the specimen that utilizes a patented anchoring device together with two layers of SFRP strips. Debonding of the FRP strips or failure of the anchoring device results, as was to be expected, in relatively unsatisfactory FRP material exploitation.

Dyspnea(or breathlessness) is a symptom describing a perceived experience of breathing discomfort. Children's awareness of dyspnea is variable and there is only a poor correlation between the objective respiratory distress measurements and the subjectively awareness of dyspnea. Those who do not perceive dyspnea may not be motivated to comply with their daily prophylactic treatment. Since dyspnea is the main symptom of asthma, and disease management is based largely on the description of symptoms between clinic visits, unreliable symptom report may mislead decision-making for longterm treatment of asthma. Thus, therapeutic decisions should not be taken solely on patients' perception and description of dyspnea.

In this paper, we carry out QoE (Quality of Experience) assessment of fairness between players in a networked balloon bursting game with olfactory and haptic senses. We also make a comparison of the fairness among different types of networked games with olfactory and haptic senses in the assessment. Then, we clarify the differences owing to usage of olfactory and haptic senses among the games. As a result, we illustrate that the fairness is largely affected by the delay difference, and we also demonstrate that the allowable range depends on the type of games, type of senses which are employed in the games, and the play methods.
