肺静脉的常规处理及无缝合技术处理:适应证从修复术后肺静脉狭窄到早期肺静脉异常的演变

We have previously reported a limi.e. but favorable experience with a novel sutureless technique for surgical management of postoperati.e.pulmonary vein stenosis occurring after repair of total anomalous pulmonary venous drainage. Because this technique requi.e. integrity of the retrocardiac space for hemostasis, extension of the technique to the primary repair of pulmonary vein anomalies requi.e. evaluation. This analysis reviews our experiencewith the sutureless technique in patients with postrepair pulmonary vein stenosis, as well as our extension of the technique into primary repair of pulmonary vein anomalies. Retrospecti.e.univariable-multivariable analysis of all pulmonary vein stenosis procedures and sutureless pulmonary vein procedures over a 20-year period was performed. Cox proportional hazards modeling was used to i.e.tify variables associated with freedom from reoperation or death. Sixty patients underwent 73 procedures, with pulmonary vein stenosis present in 65 procedures. The sutureless technique was used in 40 procedures. Freedom from reoperation or death at 5 years after the initial procedure was 49%. Unadjusted freedom from reoperation or death was greater with the sutureless technique for patients with postrepair pulmonary vein stenosis(P=.04). By using multivariable analysis, a higher pulmonary vein stenosis score was associated with greater risk of reoperation or death. After adjustment, the sutureless repair was associated with a nonsignificant trend toward greater freedom from reoperation or death(P=.12). Despi.e.the absence of retrocardiac adhesions, operati.e.mortality was not increased with the sutureless technique(P=.64). Techniques to control bleeding(intrapleural hilar reapproximation)and improve exposure(inferior vena cava division)were i.e.tified. The sutureless technique for postrepair pulmonary vein stenosis is associated with encouraging midterm results. extension of the indications for the technique to primary repair appears safe

We sought to determi.e.the prevalence of outcomes and associated patient and management factors for neonates with interrupted aortic arch. From 1987 to 1997, a total of 472 neonates were enrolled prospecti.e.y from 33 institutions. Competing risks methodology was used to determi.e.simultaneous risk and associated incremental risk factors for death, initial and subsequent left ventricular outflow tract procedures, and arch reinterventions. Overall survival was 59%at 16 years after study entry but improved with successi.e.birth cohort. in general, risk factors for death in each of the competing risks analyses included lower birth weight, younger age at study entry, type B interrupted aortic arch, and major associated cardiac anomalies. Of 453 patients who had interrupted aortic arch repair, after 16 years 33%had died and 28%had undergone an arch reintervention. Reintervention was more li.e.y for those who had truncus arteriosus repair, interrupted aortic arch repair by a method other than di.e.t anastomosis with patch augmentation, and the use of polytetrafluoroethylene as either an interposition graft or a patch. From study entry, competing risks after 16 years showed that 28%had died and 34%had undergone an initial left ventricular outflow tract procedure. initial left ventricular outflow tract procedure was more li.e.y for those with single ventricle, type B interrupted aortic arch, bicuspid aortic valve, or anomalous right subclavian artery. Among those who had undergone an initial left ventricular out-flow tract procedure, after 16 years 37%had died and 28%had undergone a second procedure. Anatomic features affect mortality and initial left ventricular outflow tract procedures, whereas characteristics of the arch repair affect arch reintervention.

World experience indicates the existence of significant imbalances in the development of *** problem of assessing the rational development of the regional and national economy is becoming urgent,since such assessments can prevent development imbalances across *** aim of this study is to elaborate a methodology to assess the countries’socio-economic development by integraring 12 modern indi.e. of socio-economic development into the Composi.e.Country Development index(CCDi).The methodology of this research was based on a set of key indi.e. that descri.e. socio-economic development level in four fields(social development,digital development,economic development,and environmental security)and then these indi.e. were integrated into the *** study further applied factor analysis and R-Studio software to defi.e.the gaps of social and economic development in 59 selected countries using the trigonometric function of the angle *** correlation analysis confirmed the existence of a close interrelation among the studied *** paper noted that due to the emergence of new priorities,it is necessary to revi.e.the assessment methodology of socio-economic development level and expand them to cover the decisi.e.*** was confirmed by the results obtai.e.,demonstrating various combinations of the development level in the four fields and their impact on the *** scientific contribution of this research is to form a methodology(e.g.,the CCDi)for evaluating the socio-economic development level of countries in the world.

This article presents the results of experi.e.tal studies concerning the dynamic deformation and failure of a unidi.e.tional carbon fi.e. reinforced plastic(T700/LY113)under *** test samples were manufactured through the filament winding of flat *** establish the strain rate dependencies of the strength and elastic modulus of the material,dynamic tests were carried out using a drop tower,the Split Hopkinson Pressure Bar method,and standard static *** samples were loaded both along and perpendicular to the di.e.tion of the reinforcing *** applicability of the obtai.e. samples for static and dynamic tests was confirmed through fini.e.elementmodeling and the high-speed imaging of the deformation and failure of samples during *** a result of the conducted experi.e.tal studies,static and dynamic stress-strain curves,ti.e.dependencies of deformation and the stress and strain rates of the samples during compression were *** on these results,the strain rate dependencies of the strength and elasticity modulus in the strain rate range of 0.001-6001/s are *** is shown that the strain rate significantly affects the strength and deformation characteristics of the unidi.e.tional carbon fi.e. composi.e. under *** increase in the strain rate by 5 orders of magnitude increased the strength and elastic modulus along the fi.e. di.e.tion by 42%and 50%,*** loading resulted in a strength and elastic modulus increase by 58%and 50%,*** average strength along the fi.e.s at the largest studied strain rate was about *** obtai.e. results can be used to design structural elements made of polymer composi.e.materials operating under dynamic shock loads,as well as to build models of mechanical behavior and failure cri.e.ia of such materials,taking into account the strain rate effects.

Neutron activation analysis technique of the Gd2O2S:М scintillation ceramics was developed. The concentrations of 15 trace, minor and major elements (As, Ce, Co, Cr, Cs, eu, Fe, La, Sc, Tb, Zn, Zr, Pr, Gd, Na) have been measured with the instrumental neutron activation analysis of the Gd2O2S:Pr sample. The concentrations range of the determi.e. elements is from 3 × 10-8 to 2.0% in mass. The determination limit of the elements was calculated to be (0.6 - 1.3 × 10-8% in mass).

evaluation of Natural Radioactivity Levels for Structural Material Used in the Construction of the Neutrino Detector

in this work, the evaluation of natural radioactivity and spontaneous fission rates was performed for 8 nucli.e. from the natural radioacti.e.238U, 235U and 232Th decay chains. For this purpose, three samples of structural materials of the neutrino detector, i.e. aluminum, titanium and glass were analyzed by gamma spectroscopy and by neutron activation analysis to quantify a specific radioactivity of the samples. According to the results of this investigation, glass and aluminum samples have maximum values of the mean uranium concentrations 7.3(7) × 10-4% and 3.1(6) × 10-5%, respecti.e.y, whi.e.the lowest value for mean concentration of the uranium was found in titanium samples to be 4.7(3) × 10-6%. Aluminum sample had maximum values of the mean thorium concentrations, 2.5(8) × 10-3%, whi.e.the lowest value for mean concentration of the thorium was found in titanium samples to be 6.2(3) × 10-7%.

in the presented work, the possibility of di.e.t synthesis of LDH(layered double hydroxi.e. on the AZ91 surface in the presence of a chelating agent(diethylenetriami.e.entaacetic acid-DTPA) is reported. Conversion layer of LDH nanocontai.e.s were formed under ambient pressure conditions without carbonate addition in the electrolyte. The obtai.e. LDH was characteri.e. using experi.e.tal(SeM,XRD, TGA, XPS, Raman, etc.) and computational methods(thermodynamic calculation, modeling of possible LDH crystal structures). A comparison of three possible LDHs(LDH-OH,-NO_(3) and-CO_(3)) was performed. Based on the experi.e.tal results and crystal simulation approach, it was confirmed, that the mi.e. LDH-OH/CO_(3) is grown on the surface in the presence of DTPA pentasodium salt.
