Intentionally unilateral prostatic artery embolization:Patient selection,technique and potential benefits

BACKGROUND Prostatic artery embolization(PAE)is a promising but also technically demanding interventional radiologic treatment for symptomatic benign prostatic *** technical challenges in PAE are associated with the complex anatomy of prostatic arteries(PAs)and with the systematic attempts to catheterize the PAs of both pelvic *** procedure times and high radiation doses are often the result of these attempts and are considered significant disadvantages of *** authors hypothesized that,in selected patients,these disadvantages could be mitigated by intentionally embolizing PAs of only one pelvic *** To describe the authors’approach for intentionally unilateral PAE(IU-PAE)and its potential *** This was a single-center retrospective study of patients treated with IU-PAE during a period of 2 ***-PAE was applied in patients with opacification of more than half of the contralateral prostatic lobe after angiography of the ipsilateral PA(subgroup A),or with markedly asymmetric prostatic enlargement,with the dominant prostatic lobe occupying at least two thirds of the entire gland(subgroup B).All patients treated with IU-PAE also fulfilled at least one of the following criteria:Severe tortuosity or severe atheromatosis of the pelvic arteries,non-visualization,or visualization of a tiny(<1 mm)contralateral PA on preprocedural computed tomographic *** contrast-enhanced ultrasonography(iCEUS)was applied to monitor prostatic ***-PAE patients were compared to a control group treated with bilateral *** IU-PAE was performed in a total 13 patients(subgroup A,n=7;subgroup B,n=6).Dose-area product,fluoroscopy time and operation time in the IU-PAE group(9767.8μGy∙m^(2),30.3 minutes,64.0 minutes,respectively)were significantly shorter(45.4%,35.9%,45.8%respectively,P<0.01)compared to the control *** and imaging outcomes did not differ significantly between the IU-PAE group and the control

Prostatic artery embolization(PAE)has gained acceptance as a minimally invasive,safe and effective treatment of symptomatic benign prostatic *** imaging is an indispensable part of post-interventional evaluation of PAE and serves both clinical and investigational *** this context,ultrasonography(US)has a central and multifaceted ***-scale US is routinely utilized for measurement of significant outcome parameters(prostatic volume,intra-vesical prostatic protrusion and post-void residual volume)before and after *** of these parameters may become more obvious onemonth post-PAE,or ***-enhanced US(CEUS)with intravenous administration of a second-generation echo-enhancer can demonstrate prostatic infarcts(as enhancement defects)immediately post-PAE and monitor their resolution over *** volume of prostatic infarcts can also be measured and compared to prostatic *** infarction is a definite sign of the local efficacy of PAE and a predictor of prostate shrinkage and(at least in some patients)of clinical *** can also be performed intraoperatively in the angio-suite,for on-site evaluation of the ischemic effect;a variation of this technique,with intraarterial(instead of intravenous)administration of diluted echo enhancer,can also be applied intraoperatively,to map the embolized territory and to prevent non-target *** experience with USelastographic techniques(shear-wave and strain elastography)has shown that they can detect and quantify the improvement of tissue elasticity post-PAE,thus providing new insights into the therapeutic mechanisms of this *** utilization of high-end equipment,experience and standardized imaging protocols,US could be the primary modality for imaging evaluation of PAE.

Associations among benign prostate hypertrophy, atypical adenomatous hyperplasia and latent carcinoma of the prostate

Aim： To investigate the frequency of atypical adenomatous hyperplasia （AAH） and its associations with benign prostate hypertrophy （BPH） and latent histological carcinoma of the prostate （LPC） in autopsy material. Methods： Two hundred and twelve prostate specimens obtained from autopsy material were subjected to whole mount analysis in an attempt to investigate the associations among BPH, AAH and LPC. Results： Most histological carcinomas and AAH lesions were found in enlarged prostates with intense hypertrophy. No statistically significant relation was found between BPH and the main characteristics of LPC, such as tumor volume, histological differentiation and biological behavior. Our data regarding multi-focal tumors showed a tendency for multi-focal carcinomas to develop in larger prostates, and a tendency of AAH lesions to develop in larger prostates. No statistically significant relation was found between AAH and LPC. Conclusion： There seems not any causative aetiopathogenetical or topographical relation between AAH lesions and prostate adenocarcinoma. AAH lesion seems to be a well-defined mimicker of prostatic adenocarcinoma, and the reported association of AAH with prostatic carcinoma could probably be an epiphenomenon.

Hepatocellular carcinoma treated with transarterial chemoembolization:Evaluation with parametric contrast-enhanced ultrasonography

AIM: To evaluate the response of hepatocellular carcinoma (HCC) to transarterial chemoembolization (TACE) using a simplified protocol of parametric contrast-enhanced ultrasound (pCEUS). METHODS: Eighteen patients with HCC (18 target tumors, diameter: 2.8-12 cm) were evaluated before, and 20 d after TACE. The distribution and morphology of TACE-induced necrosis in these tumors precluded accurate evaluation by visual assessment or by simple measurements. For pCEUS, a 4.8 mL bolus of SonoVue (Bracco, Milan, Italy) was intravenously administered and analysis of tumor perfusion during the initial phase of enhancement (0-30 s post injection) was performed with dedicated software (Qontrast, Bracco, Milan, Italy). Time-intensity curves were plotted and three parameters were calculated: peak intensity (PI, in percentage %), time to peak (TTP in seconds, s) and area under the curve during wash-in (AUC-WI, in arbitrary units, a.u). Magnetic resonance imaging was the standard imaging modality for post-treatment evaluation. Changes in tumor size were recorded and response was assessed according to response evaluation criteria in solid tumors criteria. RESULTS: A statistically significant decrease in PI and AUC-WI was observed in the treated tumors post TACE; PIpre: 21.5% ± 8.7% (mean ± SD), PIpost: 12.7% ± 6.7%, P < 0.001, AUC-WI pre: 17493 ± 9563 a.u, AUC-WI post: 9585 ± 5494 a.u, P < 0.001. A slight increase in TTP was noted post TACE, but this was not statistically significant; TTP pre: 13.1 ± 4.3 s, TTP post: 13.6 ± 4.2 s , P = 0.058). The changes in the aforementioned parameters were not accompanied by significant tumor shrinkage. CONCLUSION: pCEUS, even when limited to the study of the arterial phase of tumoral enhancement, can detect and quantify early perfusional changes in HCC post TACE.

The effect of Panton Valentin Leukocidin (PVL) on the airway smooth muscle viability

Methicillin-resistant Staphylococcus aureus (MRSA) is recognized as a worldwide pathogen, and the incidence of community-acquired infections (CA-MRSA) is increased. A virulence factor has been found in most CA-MRSA infections, the Panton-Valentin leukocidin (PVL), which causes polymorphonuclear leukocytes lysis and acute uncontrolled inflammation and tissue injury. In this study we investigated the effect of bacterial supernatant of PVL positive or negative strains on airway smooth muscle obtained from rabbit trachea. MRSA that carry the PVL-gene, confirmed by PCR, is cultured on GP agar and colonies were transferred into casein casein yeast extract medium. The culture supernatants were removed after centrifugation and the presence of PVL was confirmed using an immunochromatographic test. Rabbit tracheal ASMC were isolated and incubated with PVL positive or negative bacterial supernatant (1:20 - 1:2000) for 1 - 3 days. The effect of PVL on the ASMC morphology or viability was estimated using microscope observations or indirect immunofluores- cence with anti-Smooth muscle α-actin antibody and Dapi for DNA staining, and Trypan blue staining, respectively. ASMC incubated with PVL exhibit increased cell size, granular cytoplasm, and ruptured nuclei. Furthermore, PVL reduces cell number mainly in ASMC incubated in the presence of 10% FBS, therefore actively proliferating cells. These results show that apart from the known effect of PVL on immune cells and inflammation process, PVL has a direct toxic effect on airway smooth muscle cells.

Recurrence of Poorly Differenciated Cervical Cancer by Single Splenic Metastasis: Case Report and Literature Review

Background: The incidence of cervical cancer in Belgium is 11.1 per 100,000. With the introduction of cervical cytology screening and more recently anti-HPV vaccination, this rate has been decreasing for almost 20 years. Despite this, some patients are missed by the screening and prevention system and cervical cancer is still diagnosed at an advanced stage. Recurrences by splenic metastases are rare and are most often found at autopsy. Case Study: We describe the case of a 41-year-old caucasian patient with a single splenic recurrence after radiotherapy, chemotherapy, brachytherapy, and surgery for a poorly differentiated adenocarcinoma of the cervix grade 3 at an initial stage IIB according to FIGO. This recurrence happens 3 years after the initial treatment. After monitoring this asymptomatic lesion, the size increase results in laparoscopic splenectomy. Histology demonstrates a splenic metastasis recurrence of adenocarcinoma of endocervical origin. Conclusion: The spleen is a rare metastatic site in cervical cancer. Splenectomy followed by chemotherapy is the therapy most often found in the literature, which is however poor in this regard.

Design and Construction of a Forest Village in Greece

The aim of this paper is the study for the construction of a forest village in the area of Souvardo in Kalavrita in the prefecture of Achaia. This paper presents the history and purpose of forest villages, as well as the laws that govern them. The methodology of the paper is analyzed and the particular characteristics of the study area are examined with the accompaniment of the general characteristics related to the formation of the space. Further, aerial photographs are listed with the help of Google Earth program, giving emphasis to the general plan of the study area. The reception building and the host installation of the forest village are described, as well as the building and topographic plans are designed with the use of AutoCAD and Photoshop programs. Furthermore, the cost of the development project is analyzed. Finally, the conclusions of the study and recommendations to the visitor of the area about sights, natural landscapes and archaeological sites axe given. Criteria for intervention and promotion of tourism and proposals for tourism development in the area, such as ideas for the development of cultural tourism and ecotourism in the region, are suggested.

Role of hydrogen sulphide in airways

The toxicity of hydrogen sulfide(H2S) has been known for a long time, as it is prevalent in the atmosphere. However accumulative data suggest that H2 S is also endogenously produced in mammals, including man, and is the third important gas signaling molecule, besides nitric oxide and carbon monoxide. H2 S can be produced via non enzymatic pathways, but is mainly synthesizedfrom L-cysteine by the enzymes cystathionine-γ-lyase, cystathionine-β-synthetase, cysteine amino transferase and 3-mercaptopyruvate sulfurtransferase(3MTS). The formation of H2 S from D-cysteine via the enzyme D-amino acid oxidase and 3MTS has also been described. Endogenous H2 S not only participates in the regulation of physiological functions of the respiratory system, but also seems to contribute to the pathophysiology of airway diseases such as chronic obstructive pulmonary disease, asthma and pulmonary fibrosis, as well as in inflammation, suggesting its possible use as a biomarker for these diseases. This review summarizes the different implications of hydrogen sulfide in the physiology of airways and the pathophysiology of airway diseases.
